Home care and telemedicine are crucial for physical and mental health. Although there is a lot of information on these topics, it is scattered across various sources, making it difficult to identify key contributions and authors. This study conducts a scientometric analysis to consolidate the most relevant information. The methodology is divided into two parts: first, a scientometric mapping that analyzes scientific production by country, journal, and author; second, the identification of prominent contributions using the Tree of Science (ToS) tool. The goal is to identify trends and support decision-making in the health sector by providing guidelines based on the most relevant research.

Background and Aims: One major limitation of the spinal block remains the inability to extend the duration of the block intraoperatively unless planned before with spinal or epidural catheters and/or intrathecal additives. This study was designed to compare the effects of intravenous dexmedetomidine versus low-dose ketamine–dexmedetomidine combination infusion on spinal anaesthesia in lower limb orthopaedic surgeries. Methods: This randomised study was conducted in 60 patients scheduled for unilateral lower limb surgeries under spinal anaesthesia. Patients were randomised into Group D (n = 30) (0.5 µg/kg of intravenous (IV) dexmedetomidine bolus followed by maintenance infusion at 0.5 µg/kg/h) and Group LKD (n = 30) (IV bolus of 0.5 µg/kg of dexmedetomidine and 0.2 mg/kg of ketamine, followed by maintenance infusions of dexmedetomidine and ketamine at 0.5 µg/kg/h and 0.2 mg/kg/h, respectively). Ramsay Sedation Scale score of 3–4 was maintained. The t-test or the Wilcoxon–Mann–Whitney U test was used to compare the parameters between groups. Results: The mean sacral segment 1 (S1) regression time was 390.3 [standard deviation (SD):84.38] [95% confidence interval (CI): 360.13, 420.53] versus 393.23 (SD: 93.01) (95% CI: 363.04, 423.43) min in Group D versus Group LKD respectively ((P = 0.701). The number of episodes of hypotension was significantly higher in Group D (19 patients) compared to Group LKD (nine patients) (P = 0.001). Pre- and postoperative stress markers (24 h) and the incidence of postoperative nausea and shivering were comparable between the two groups (P > 0.05). Tramadol requirement in the postoperative period was significantly less in Group LKD compared to Group D (P = 0.003). Conclusion: The duration of S1 regression was similar between group dexmedetomidine (Group D) and group low-dose ketamine and dexmedetomidine (Group LKD).

This paper focuses on introducing and investigating the class of generalized $ n $-fractional polynomial $ s $-type convex functions within the framework of fractional calculus. Relationships between the novel class of functions and other kinds of convex functions are given. New integral inequalities of Hermite-Hadamard and Ostrowski-type are established for our novel generalized class of convex functions. Using some identities and fractional operators, new refinements of Ostrowski-type inequalities are presented for generalized $ n $-fractional polynomial $ s $-type convex functions. Some special cases of the newly obtained results are discussed. It has been presented that, under some certain conditions, the class of generalized $ n $-fractional polynomial $ s $-type convex functions reduces to a novel class of convex functions. It is interesting that, our results for particular cases recaptures the Riemann-Liouville fractional integral inequalities and quadrature rules. By extending these particular types of inequalities, the objective is to unveil fresh mathematical perspectives, attributes, and connections that can enhance the evolution of more resilient mathematical methodologies. This study aids in the progression of mathematical instruments across diverse scientific fields.

Introduction: Acute disseminated encephalomyelitis (ADEM) is an acute autoimmune demyelinating disease of the central nervous system that typically follows an acute viral infection or post-vaccination. It is more common in children than in adults and is characterized by widespread demyelination of the white matter of the brain and spinal cord. ADEM typically presents as a monophasic illness. Multiphasic ADEM has been an emerging topic with case reports highlighting the importance of the need for further investigation and literature on this topic. Multiphasic ADEM in adults is a rare presentation with an atypical course that can cause a delay in diagnosis leading to disability in patients. Case Presentation: Multiple case reports have shown the presence of multiphasic ADEM without an established cause, emphasizing the importance of additional insight and data in this context. This is a unique case of multiphasic ADEM in a 59-year-old male with a temporal gap of 26 years with an idiopathic cause. He was suffering from many neurologic symptoms involving his upper and lower extremities and level of consciousness, and after he was diagnosed with ADEM, he was commenced on steroids and achieved a marked improvement in his clinical status. Conclusion: Rarely, ADEM can present with recurrence or multiple phases, which requires prompt diagnosis and treatment. These phases may or may not be preceded by an identifiable trigger. Most patients respond well to steroids. However, further studies are required to achieve a good understanding and guidelines when dealing with such cases.

Abstract UV sensing 3D printed optical fiber hydrogels provide a flexible and precise method of remotely of detecting exposure to UV radiations. The optical fibers were created using digital light processing 3D printing technique with hydrogel composites, including micro-sized photochromic dyes (pink, blue and their combination). When exposed to ultraviolet (UV) radiation, these dyes exhibited specific absorption characteristics, resulting in significant decreases in both reflection and transmittance mode spectra at 560 nm, 620 nm, and 590 nm. Optical fibers of lengths 1, 2, and 3 cm were manufactured in two orientations: vertical and horizontal. Scanning electron microscopy, Fourier transform infrared spectroscopy, and X-ray diffraction were utilized to characterize the printed fiber probes. The optical performance of the fibers was tested using customized measurement setups. The reflection and transmission of the printed fibers reduced as the length increased due to optical losses. Reflection and transmisson loss of 20–40% can be observed when the length is increased from 1 to 3 cm. The maximum loss in reflection is observed for pink fiber in the presence of UV irradiation. Also, the type of powder used impacted the response and retraction time, whereas the mixed fiber showed the highest response time of 12–20 s under various conditions. The pink dye added fiber probes shows quick response to UV radiation. An increase in the response time is observed with increasing fiber length. The impact of printing orientation on the transmission and reflectance mode operations of optical fibers was assessed. In addition, the stability of the fiber probes are assesed using a green laser having wavelength 532 nm. This work comprehensively examines the optical properties, manufacturing procedures, and sensing capacities of UV-sensitive photochromic optical fiber sensors.

Introduction: Chronic myeloid leukemia (CML) is a myeloproliferative disease caused by the reciprocal translocation of chromosomes 9 and 22, which leads to a chimeric gene product known as BCR-ABL. Some studies have shown a higher incidence of secondary malignancies than the one seen in the general population in patients with CML. Hepatocellular carcinoma (HCC) is rarely reported in association with CML and/or CML-related treatment. Case Presentation: We describe a case of a patient with no history of liver disease and CML on imatinib for 10 years, who presented with worsening right upper quadrant abdominal pain. Imaging revealed a large hepatic mass highly suspicious of malignancy that later was confirmed to be HCC after biopsy. The patient was bracketed with advanced stage HCC (BCLC stage C) and given her advanced age and poor performance status; palliative care was offered. Conclusion: Patients with CML have a common association with secondary malignancies. This is the first case report based on our extensive review of available literature that HCC was diagnosed in a patient with CML on treatment with imatinib without any clear or usual underlying cause.
