152. Deceased Kidney Donor Biomarkers: Relationship between Delayed Kidney Function and Graft Function Three Years after Transplantation

With an increasing number of marginal donors, additional methods for the evaluation of cadaveric kidney quality are required. This study aimed to evaluate pretransplant deceased donor serum (s) and urine (u) biomarkers, including neutrophil gelatinase-associated lipocalin (NGAL), kidney injury molecule-1 (KIM-1), interleukin-18, and C-X-C motif chemokine 10 (CXCL10) for predicting early and late graft function. In total, 43 deceased kidney donors and 76 corresponding recipients were enrolled. Delayed graft function (DGF) occurred in 27.6% of cases. sIL-18, sKIM-1, uNGAL, and uKIM-1 were predictors of DGF. A model incorporating sIL-18, uKIM-1, and clinical factors was developed to predict DGF (AUROC 0.863). Univariate analysis showed a negative association between uKIM and graft eGFR at 6, 12, 24, and 36 months, but this was not confirmed in the multivariate analysis. In conclusion, we report a superior performance of donor biomarkers for predicting DGF and later graft function over serum creatinine. Higher levels of donor sIL-18 and uKIM in conjunction with expanded-criteria donors and longer cold ischemia times predicted DGF. With no renal tubular damage in zero-time donor biopsies, higher pretransplant urine and serum NGAL levels were associated with better allograft function one year after transplantation, and sNGAL with graft function three years after transplantation.

The paper investigates the improvement of methods of time-based prioritization of public transport buses on city routes, taking into account pedestrian traffic indicators. A method of non-stop passing the stop-lines at signalized intersections and pedestrian crosswalks by buses in places of high generation of pedestrian traffic has been developed. A study was conducted at three signalized intersections and pedestrian crosswalks in Lviv city in places of high generation of pedestrian flows with dedicated lanes for public transport. Pedestrian speed of movement while crossing the roadway in such locations was studied, and the value of the speed of movement of 15% of the pedestrian flow, which potentially will not have time to cross the roadway during the time of the activated permissive signal, was found. The time parameters of extending the restrictive signal for traffic flows for the safe and unobstructed passage of pedestrian flows at the traffic light object are defined. The distance at which public transport should be located from the signalized facility to extend the restrictive signal to public transport, is also determined.

Signalized pedestrian crosswalks on urban streets are essential for ensuring the safe crossing of the roadway by pedestrians and controlling the movement of vehicles. These crosswalks allow pedestrians to cross the roadway by the set rules, providing safety and comfort to all road users. Pedestrian safety is critical to improving mobility in cities. About 15% of traffic accidents occur at signalized crosswalks. Existing traffic signal control systems are mainly aimed at road traffic, which often results in significant delays and insufficient time for pedestrians to cross. It leads to risky behavior, such as not obeying traffic rules. Current pedestrian detection systems at signalized intersections mainly consist of pedestrian controllers called “buttons”. Limitations include the inability to notify a pedestrian that he, or she has been detected and the inability to dynamically extend crossing times if pedestrians do not cross the road in time. Intelligent transport systems play an essential role in improving mobility and safety and provide innovative methods to connect pedestrians, vehicles and infrastructure. Most of the research on smart and connected technologies focuses on vehicles, but there is a need to use these technologies to study pedestrian behavior, as pedestrians are the most vulnerable participants in the transportation system. Therefore, the transport system should take into account all pedestrians and satisfy all needs. Therefore, this article aims to provide information on improving traffic signal control system at pedestrian crosswalks to make them more pedestrian-friendly without significant disruption to traffic flow. The study was carried out by modeling a pedestrian crosswalk for different combinations of pedestrians and traffic flow. This paper presents modeling results and provides recommendations for the introduction of a rational control mode at pedestrian crosswalks.

In most games, an artificial pathfinding intelligence is required for traversing the fastest discovery. It is essential for many video games, particularly Role Playing Games (RPGs). The algorithm pathfindings implemented in this game are A* and Dijkstra Algorithms. This study aims to test an artificial intelligence system for discovering routes using the A* and Dijkstra algorithms based on RPG Maker MV. The result showed that from the time obtained, in the experiment on eight nodes using the Pathfinding mechanism of A* algorithm has faster result in discovering the nearest route with the time 08:15:23 with format (mm:ss: ms) whereas Dijkstra Algorithm has a 34:47:43 time result. The time record needed represents the distance between the search nodes. It indicates that the multiple weighting in the impassable nodes caused the cost calculation process becomes faster and more efficient.

165. Minimization of public transport delays at arterial streets with coordinated motion

Research results, using which the method of minimization of public transport delay is improved at intersections with the system of coordinated motion control, are given in this paper. Such transport research was carried out with simultaneous application of field measurements of the study of traffic flow indicators and computer simulation in PTV VISSIM to check the level of efficiency of coordinated control and the reliability of the results. The essence of the method is that it reduces the delay in traffic per user of the transport system during his movement through a signalized section of the road network. The effectiveness of this method is achieved under condition of significant intensity of public transport, which is provided with spatial priority in the form of the allocated lane. Invariability of the number of lanes in the area where coordination takes place, and a high level of transit (above 70%) of straight traffic flows are compulsory indicators and parameters. The result is achieved with such phases in the direction of coordinated control, the share of the permissive signal in which is more than 45% of the cycle duration with a duration limit of 90-125 s. With such parameters, the starting delays of the general traffic flow at the stop-lines are minimized, and the maximum values of the saturation flow are achieved. In addition, a sufficient width of the time lane is established for the passage of signalized areas by public transport. There is still some delay in public transport in such a control system, but it is connected with delays at bus stops. The introduction of such systems of coordinated traffic control is recommended on the arterial streets of citywide importance of controlled motion with a distance between adjacent stop-lines of not more than 800 m. This restriction allows avoiding the dissipation of groups of vehicles.

CONTEXT: Ultrasound marking of saphenous veins before vein harvesting facilitates the surgical procedure but is time-consuming. AIM: We evaluated the time benefit of saphenous veins marking by the AccuVein® AV400 system (AccuVein Inc., NY, USA) coupled with ultrasound, over ultrasound alone. SETTINGS AND DESIGN: Multi-center open-label randomized interventional study in two parallel groups. SUBJECTS AND METHODS: After randomization, saphenous veins markings were performed using either AccuVein® coupled with ultrasound or ultrasound alone. One-leg marking time, vascular physician postexamination satisfaction rating, and number of single-use felt markers used were recorded. STATISTICAL ANALYSES: The continuous variables were compared using the Fisher test, whereas Likert scale scores were compared using the nonparametric Mann−Whitney test. The learning effect was evaluated by linear and nonlinear regression models. RESULTS: The average duration of saphenous veins marking (459 ± 184 vs 555 ± 226 s, P = 0.116) and the number of felt markers used were not significantly different between the two techniques. Vascular physician satisfaction was significantly lower with than without the AccuVein® associated to ultrasound. CONCLUSIONS: AccuVein® examination coupled with ultrasound as part of the marking process does not provide a significant benefit in terms of time, satisfaction, or need for markers.

BackgroundHigh-Grade Serous Ovarian Carcinoma (HGSOC) is the most prevalent and lethal subtype of ovarian cancer, but has a paucity of clinically-actionable biomarkers due to high degrees of multi-level heterogeneity. Radiogenomics markers have the potential to improve prediction of patient outcome and treatment response, but require accurate multimodal spatial registration between radiological imaging and histopathological tissue samples. Previously published co-registration work has not taken into account the anatomical, biological and clinical diversity of ovarian tumours.MethodsIn this work, we developed a research pathway and an automated computational pipeline to produce lesion-specific three-dimensional (3D) printed moulds based on preoperative cross-sectional CT or MRI of pelvic lesions. Moulds were designed to allow tumour slicing in the anatomical axial plane to facilitate detailed spatial correlation of imaging and tissue-derived data. Code and design adaptations were made following each pilot case through an iterative refinement process.ResultsFive patients with confirmed or suspected HGSOC who underwent debulking surgery between April and December 2021 were included in this prospective study. Tumour moulds were designed and 3D-printed for seven pelvic lesions, covering a range of tumour volumes (7 to 133 cm3) and compositions (cystic and solid proportions). The pilot cases informed innovations to improve specimen and subsequent slice orientation, through the use of 3D-printed tumour replicas and incorporation of a slice orientation slit in the mould design, respectively. The overall research pathway was compatible with implementation within the clinically determined timeframe and treatment pathway for each case, involving multidisciplinary clinical professionals from Radiology, Surgery, Oncology and Histopathology Departments.ConclusionsWe developed and refined a computational pipeline that can model lesion-specific 3D-printed moulds from preoperative imaging for a variety of pelvic tumours. This framework can be used to guide comprehensive multi-sampling of tumour resection specimens.

IntroductionPseudomyxoma peritonei (PMP) is a rare malignant disease characterized by a massive multifocal accumulation of mucin within the peritoneal cavity. The current treatment option is based on complete cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy. However, the recurrence is frequent with subsequent progression and death. To date, most of the studies published in PMP are related to histological and genomic analyses. Thus, the need for further studies unveiling the underlying PMP molecular mechanisms is urgent. In this regard, hypoxia and oxidative stress have been extensively related to tumoral pathologies, although their contribution to PMP has not been elucidated.MethodsIn this manuscript, we have evaluated, for the first time, the intratumoral real-time oxygen microtension (pO2mt) in the tumor (soft and hard mucin) and surrounding healthy tissue from five PMP patients during surgery. In addition, we measured hypoxia (Hypoxia Inducible Factor-1a; HIF-1α) and oxidative stress (catalase; CAT) markers in soft and hard mucin from the same five PMP patient samples and in five control samples.ResultsThe results showed low intratumoral oxygen levels, which were associated with increased HIF-1α protein levels, suggesting the presence of a hypoxic environment in these tumors. We also found a significant reduction in CAT activity levels in soft and hard mucin compared with healthy tissue samples.DiscussionIn conclusion, our study provides the first evidence of low intratumoral oxygen levels in PMP patients associated with hypoxia and oxidative stress markers. However, further investigation is required to understand the potential role of oxidative stress in PMP in order to find new therapeutic strategies.

In this paper, we study the performance of a TCP connection over cognitive radio networks. In these networks, the network may not always be available for transmission. Also, the packets can be lost due to wireless channel impairments. We evaluate the throughput and packet retransmission timeout probability of a secondary TCP connection over an ON/OFF channel. We first assume that the ON and OFF time durations are exponential and later extend it to more general distributions. We then consider multiple TCP connections over the ON/OFF channel. We validate our theoretical models and the approximations made therein via ns2 simulations.

198. The Role of Pre- and Post-Transplant Hydration Status in Kidney Graft Recovery and One-Year Function

Background and Objectives: Early improvements to graft function are crucial for good outcomes in kidney transplantation (kTx). Various factors can influence early graft function. This study aimed to evaluate the pre- and post-transplant hydration statuses of kTx recipients using bioimpedance analysis (BIA) and lung ultrasonography (LUS) and to investigate the hydration status’ relationship with the function of the transplanted kidney during the first year after transplantation. Materials and Methods: This observational prospective cohort study included deceased kidney recipients transplanted in the Hospital of the Lithuanian University of Health Sciences between September 2016 and January 2023. BIA and LUS were performed before transplantation, on days 3 and 7, and at discharge. Data on recipient and donor clinical characteristics were collected. Graft function was evaluated according to the serum creatinine reduction ratio and the need for dialysis. Hydration status was evaluated by calculating B-lines (BL) on LUS and the ratio of extracellular/total body water on BIA. Results: Ninety-eight kTx recipients were included in the study. Patients with immediate graft function (IGF) were compared to those with slow or delayed graft function (SGF + DGF). Recipients in the SGF + DGF group had a higher sum of BL on LUS before transplantation. After transplantation in early postoperative follow-up, both groups showed hyperhydration as determined by BIA and LUS. After one year, recipients with no BL before transplantation had better graft function than those with BL. Logistic regression analysis showed that having more than one BL in LUS was associated with a 2.5 times higher risk of SGF or DGF after transplantation. Conclusions: This study found that lung congestion detected by LUS before kTx was associated with slower graft recovery and worse kidney function after 1 year. Meanwhile, the hyperhydration status detected by BIA analysis did not correlate with the function of the transplanted kidney.

The paper presents the results of the research concerning chemical composition of acacia honey from the differents region. Analisys concerning the mineral content were made in 14-19 march 2008 in the molecular and atomical spectroscopy laboratoriy of the Faculty of Agro-Food Tehnology and Processing from USABMV Timisoara, through spectroscopy with atomic absorbability in flame with the spectrometer. Contr AA 300 with continous surce. There were anailzted 3 honey samples made in 2007 from Domasnea and Farliug (Caras-Severin) and Bala (Mehedinti). The ash contain in Na, K, Ca, Mg, Cu, Zn, Mn, Fe, Pb, Co, Cr was determined.

In rabbits breeding, the amelioration processes have a high importance because they are aimed to continuously increase the productions concomitantly with the decrease of specific consumption and unit costs. Cross-breeding of two genetically distinguished breeds can produce the heterozis effect, meaning an increase of the possibility that allow a higher productivity. 53 young rabbits were used as biological material, 3 interlinear simple female hybrids of New Zealand White (NZ) being the maternal line and 3 interlinear simple male hybrids of Grand Chinchilla (CH) form the paternal line. The 53 young rabbits were raised in identical environmental conditions so that their genetic potential to determinate the phonotypical expression. The rabbits weighing was done daily, at the same time, in equal foraging and drinking conditions. The rabbits were weaned at 30 days old, and until 80 days old were raised for meat. The two breeds used in the crossbreeding chart, respectively New Zealand White as maternal line and Grand Chinchilla as paternal line, have a good combinative characteristic, and on their hybrids is manifested the heterozis effect. In all the experimental, the hybrids from NZ x CH cross-breeding registered a corporal dynamic higher then the parental breeds.
