Abstract

Prader-Willi Syndrome (PWS) is a multisystemic genetic disease characterized by hypothalamic hypogonadism, mental retardation and compulsive hyperphagia associated with early and severe obesity. Complications of overweight, such as type-2 diabetes Mellitus, dyslipidemia and diffuse atheromatosis are common. We report a 15 years old morbid obese male with PWS, with a body mass index of 57.7 kg/m², refractory to weight-lowering treatments. He underwent preoperative evaluation and treatment by a multidisciplinary team, and subjected to a 95% gastrectomy, leaving a 50 ml remnant pouch and a long limb (120 cm) Y-Roux gastro-jejuno anastomosis. There were no surgical complications, oral feeding was initiated at the 5th day with an hypocaloric diet. During the first postoperative year, the patient lost 70 kg, achieving a body mass index of 30 kg/m². Surgical treatment can become a therapeutic choice for obesity in PWS patients (Rev Méd Chile 2003; 131: 427-31).

A également fait l'objet d'une présentation dans : INRA Mensuel, 105-106, 2000/07, pp 17-18 Diffusion du document : INRA Unité de Recherches sur l'Economie des Qualifications Agro-alimentaires 8 Avenue René Laennec 72000 Le Mans (FRA); National audience; Une large enquête scientifique a été réalisée auprès de 1 045 personnes, au printemps 99, afin de comprendre la perception qu'a le citoyen des organismes génétiquement modifiés (OGM) : 80% des gens interrogés disent qu'un produit OGM est un produit déqualifié, 60% sont inquiets, mais cherchent à faire la part des choses. Quand les OGM sont arrivés sur le marché, ils ont bénéficiés d'un a priori favorable, simplement parce qu'ils sont le résultat de la science et que la science est, en général, perçue positivement. Puis, il y a eu rapidement une attitude de blocage. En l'absence de vraie réponse de la science et de la société, le public dubitatif est devenu sensible aux discours militants hostiles et l'inquiétude s'est accrue. Quand on élimine les croyances à priori, il ne reste pas grand chose. Chacun se raccroche à des attitudes conventionnelles ou s'abrite derrière un scepticisme général. Finalement, la logique de déqualification des OGM s'impose chez tous les protagonistes : les consommateurs frustrés dans leur désir de comprendre et de plus en plus dubitatifs à propos de la science et de la technologie, les scientifiques et les opérateurs économiques déçus des attitudes de rejets. Il convient donc actuellement de recréer les conditions d'un vrai dialogue "science société". Cela suppose de disposer d'un laps de temps suffisant pendant lequel le consommateur doit trouver sur le marché des produits qu'il estime normal d'y trouver, donc sans OGM, sans indication spéciale, cette dernière étant réservée aux produits "suspects" et sans variation de prix perceptible. Il faut donc rapidement créer une filière de végétaux garantis non OGM pour la constitution d'un véritable marché.

Background Implantable defibrillators are the most effective means to prevent sudden death in patients with malignant ventricular tachyarrhythmias. The availability of this type of devices is limited in Chile, due to their high price. Aim To report the first patients treated with implantable defibrillators in our hospital. Patients and methods Nine males and one female aged 13 to 65 years old are reported. Three presented with ventricular fibrillation (presenting out of the hospital in three) and the rest had ventricular tachycardia resistant to drugs or radiofrequency ablation. Results All implants were performed using intracardiac electrodes. The generator was implanted in the pectoral region in nine and in the abdomen in one. A successful defibrillation was obtained with less than 15 J in four patients, with 20 J in three and with 24 J in three. There were no complications during the procedure. After a 12 months follow up, four patients have been treated by the implantable device. One of these subjects had a ventricular fibrillation in two occasions. One patient died of a bronchopneumonia two years after the implant. Conclusions Implantable defibrillators are an effective therapy for the treatment of malignant ventricular arrhythmias with a high risk of sudden death.
