We exploit a universe dataset of state school students in England with linked test score records to document the evolution of attainment through school for different ethnic groups. The analysis yields a number of striking findings. First, we show that, controlling for personal characteristics, all minority groups make greater progress than white students over secondary schooling. Second, much of this improvement occurs in the high-stakes exams at the end of compulsory schooling. Third, we show that for most ethnic groups, this gain is pervasive, happening in almost all schools in which these students are found. We address some of the usual factors invoked to explain attainment gaps: poverty, language, school quality, and teacher influence. We conclude that our findings are more consistent with the importance of factors like aspirations and attitudes.

Purpose: The purpose of this review is to chronicle the extent to which the Pinkston and colleagues model is utilized in single-parent training group (SPG) interventions in the home environment for children aged 5 to 12 or preadolescent school-aged children. Methods: Several databases were searched electronically and independent full reviews were conducted with N = 7 studies that met the inclusion criteria. Results: We identified the outcome measures of relevant studies of SPG interventions across five content themes: (1) rate of positive parent attention, (2) change in rate of negative parent attention, (3) change in rate of child compliance, (4) change in rate of child chore completion, and (5) change in nature of parent–child dyad. A review of the research provided empirical evidence that SPG interventions are effective in increasing positive parent–child social interactions, child compliance, and differential attention that correlates to the modification of problematic and oppositional child behaviors. Conclusions: The studies herein provide guidance and suggestions for future interventions utilizing SPG approaches. [ABSTRACT FROM AUTHOR]

Background: In the Global Burden of Disease Study 2013 (GBD 2013), knowledge about health and its determinants has been integrated into a comparable framework to inform health policy. Outputs of this analysis are relevant to current policy questions in England and elsewhere, particularly on health inequalities. We use GBD 2013 data on mortality and causes of death, and disease and injury incidence and prevalence to analyse the burden of disease and injury in England as a whole, in English regions, and within each English region by deprivation quintile. We also assess disease and injury burden in England attributable to potentially preventable risk factors. England and the English regions are compared with the remaining constituent countries of the UK and with comparable countries in the European Union (EU) and beyond.Methods: We extracted data from the GBD 2013 to compare mortality, causes of death, years of life lost (YLLs), years lived with a disability (YLDs), and disability-adjusted life-years (DALYs) in England, the UK, and 18 other countries (the first 15 EU members [apart from the UK] and Australia, Canada, Norway, and the USA [EU15+]). We extended elements of the analysis to English regions, and subregional areas defined by deprivation quintile (deprivation areas). We used data split by the nine English regions (corresponding to the European boundaries of the Nomenclature for Territorial Statistics level 1 [NUTS 1] regions), and by quintile groups within each English region according to deprivation, thereby making 45 regional deprivation areas. Deprivation quintiles were defined by area of residence ranked at national level by Index of Multiple Deprivation score, 2010. Burden due to various risk factors is described for England using new GBD methodology to estimate independent and overlapping attributable risk for five tiers of behavioural, metabolic, and environmental risk factors. We present results for 306 causes and 2337 sequelae, and 79 risks or risk clusters.Findings: Between 1990 and 2013, life expectancy from birth in England increased by 5·4 years (95% uncertainty interval 5·0-5·8) from 75·9 years (75·9-76·0) to 81·3 years (80·9-81·7); gains were greater for men than for women. Rates of age-standardised YLLs reduced by 41·1% (38·3-43·6), whereas DALYs were reduced by 23·8% (20·9-27·1), and YLDs by 1·4% (0·1-2·8). For these measures, England ranked better than the UK and the EU15+ means. Between 1990 and 2013, the range in life expectancy among 45 regional deprivation areas remained 8·2 years for men and decreased from 7·2 years in 1990 to 6·9 years in 2013 for women. In 2013, the leading cause of YLLs was ischaemic heart disease, and the leading cause of DALYs was low back and neck pain. Known risk factors accounted for 39·6% (37·7-41·7) of DALYs; leading behavioural risk factors were suboptimal diet (10·8% [9·1-12·7]) and tobacco (10·7% [9·4-12·0]).Interpretation: Health in England is improving although substantial opportunities exist for further reductions in the burden of preventable disease. The gap in mortality rates between men and women has reduced, but marked health inequalities between the least deprived and most deprived areas remain. Declines in mortality have not been matched by similar declines in morbidity, resulting in people living longer with diseases. Health policies must therefore address the causes of ill health as well as those of premature mortality. Systematic action locally and nationally is needed to reduce risk exposures, support healthy behaviours, alleviate the severity of chronic disabling disorders, and mitigate the effects of socioeconomic deprivation.Funding: Bill & Melinda Gates Foundation and Public Health England. Objective: To investigate the utility of an adapted version of a racial respect scale originally developed for African American youth. We modified the scales for gender-specific use with adult African Americans, and added a subscale to assess racial respect related to romantic partnerships. The current study seeks to investigate the relationship between the reliability and construct validity of the racial respect scale and a measure encompassing 4 theorized adaptive and maladaptive identity types, the Perceptions of African American Experience (PAAX). The study authors also investigate the hypothesized relationship between racial respect and health outcomes and mental health adjustment. Method: The newly developed African American Respect Scale-Women (AARS-W) and African American Respect Scale-Men (AARS-M) are psychometrically tested with samples of adult African Americans (N = 278) recruited in 3 U.S. cities: Portland, Oregon; St. Louis, Missouri; and Washington, D.C., which were selected to represent the diversity in East Coast, Midwest, and West Coast populations. Results: Findings from the current study provide evidence that racial respect predicts prosocial adaptive identity and is associated with participation in prosocial activities and positive health outcomes while the lack of it predicts maladaptive identity and poor mental health adjustment as well as the lack of access to the social determinants of health such as adequate education and employment opportunities. Conclusions: Social workers working with adult African Americans will find the results from the current study helpful in their professional practice. [ABSTRACT FROM AUTHOR]

- Abstract
The Global Burden of Disease, Injuries, and Risk Factor study 2013 (GBD 2013) is the first of a series of annual updates of the GBD. Risk factor quantification, particularly of modifiable risk factors, can help to identify emerging threats to population health and opportunities for prevention. The GBD 2013 provides a timely opportunity to update the comparative risk assessment with new data for exposure, relative risks, and evidence on the appropriate counterfactual risk distribution.

- Abstract
The Global Burden of Disease Study 2013 (GBD 2013) aims to bring together all available epidemiological data using a coherent measurement framework, standardised estimation methods, and transparent data sources to enable comparisons of health loss over time and across causes, age–sex groups, and countries. The GBD can be used to generate summary measures such as disability-adjusted life-years (DALYs) and healthy life expectancy (HALE) that make possible comparative assessments of broad epidemiological patterns across countries and time. These summary measures can also be used to quantify the component of variation in epidemiology that is related to sociodemographic development.

This article pioneers a study into the use of the tantalum pentoxide nanoceramics as novel candidates for dose enhancement radiotherapy. It is revealed that a significant induced dose enhancement on radioresistant cancer cells expose to tantalum pentoxide nanoparticles and irradiated with 10 MV. In this study, in vitro experiments are performed. The radiobiological endpoint is clonogenic survival. We exposed 9L gliosarcoma cells to the nanoparticles at 50-500 μg mL
−1 range and observed concentration-dependent toxicity. Irradiation of the exposed and unexposed cells with 10 MV X-ray photons reveals a sensitization enhancement ratio of 1.33. The associated cell survival curves demonstrate a significant change in shape, indicative of increased lethality of the local radiation environment. We postulate that this enhancement is primarily due to secondary electrons produced from photoelectric interaction and pair production, with backscattering on nanoparticle aggregates leading to increased radiobiological effectiveness. Up-to-date evidence about levels and trends in disease and injury incidence, prevalence, and years lived with disability (YLDs) is an essential input into global, regional, and national health policies. In the Global Burden of Disease Study 2013 (GBD 2013), we estimated these quantities for acute and chronic diseases and injuries for 188 countries between 1990 and 2013.

186. The role of health protection teams in reducing health inequities: findings from a qualitative study.

Introduction: The UK Health Security Agency's (UKHSA) Health Protection Teams (HPTs) provide specialist public health advice and operational support to NHS, local authorities and other agencies in England. The development of a three-year UKHSA Health Equity strategy creates a unique opportunity for HPTs to reduce health inequities within their work. Aims: This study aimed to understand current health equity activities and structures within HPTs, and to propose future HPT-led health equity activities. Methods: Between November 2021 - March 2022, HPT staff from the nine UKHSA regions were invited to participate in a semi-structured interview or focus group. Results: Twenty-seven participants covering all nine UKHSA regions took part in a total of 18 interviews and two focus groups. There was enthusiasm to address health inequity, and many reported this as their motivation for working in public health. All HPTs routinely engaged in health equity work including, variously: liaising with other organisations; advocacy in case and outbreak management meetings; developing regional HPT health equity action plans; and targeting under-served populations in day-to-day work. HPT staff discussed the challenge of splitting their time between reacting to health protection incidents (e.g., COVID as the main priority at the time) and pro-active work (e.g., programmes to reduce risk from external hazards for vulnerable populations). Although COVID had raised awareness of health inequities, knowledge of health equity among the professionally diverse workforce appeared variable. Limited evidence about effective interventions, and lack of clarity about future ways of working with other organisations were also shared as barriers to tackling health inequities. Conclusion: HPTs welcomed the development of UKHSA's health equity strategy, and through this study identified opportunities where HPTs can influence, support and lead on tackling health inequities. This includes embedding health equity into HPTs' acute response activities, stakeholder working, and staff management. This study also identified a need for health equity training for HPTs to improve knowledge and skills, utilising evidence-based approaches to health equity. Finally, we have identified areas where HPTs can lead, for example using brief advice interventions and through developing resources, such as standard operating procedures that focus on vulnerable populations. This article highlights institutional inequalities and cultural insensitivities that contribute to African American underutilization of mental health care service systems. The paper discusses how African Americans encounter barriers to accessing mental health systems and the issues and challenges to achieving culturally relevant assessments and diagnostics, treatment modalities, evaluations, and research. A cultural competency framework is used to address critical service delivery and service implementation issues that have potentially negative effects on therapeutic treatments for African Americans. Finally, recommendations for modification and enhancement are made in order to achieve culturally sensitive and responsive mental health services for African Americans. [ABSTRACT FROM AUTHOR]

The authors say PHE has advanced the debate about sugar and the clear need for a sugary drinks tax.

The investigation of family participation in the changing children's mental health care system is not a commonly explored theme. The study in this article documents the efforts of family members involved with statewide family networks (SFNs) seeking legislative and systems change. Data from the National Evaluation of Statewide Family Networks demonstrate the contributions made and the processes facilitated by these families. These data highlight the legislative, policy development, systems change, and funding strategies used by SFNs to enhance the children's mental health care system. As expected of a radical structuralist organizations, SFNs seek fundamental changes. Their collective social action is part of an emerging, orchestrated effort by affected professionals and families of children with serious emotional and behavioral disorders to establish better service systems that are responsive to children, flexible. seamless, and sufficient in resources. The results of this study provide empirical evidence of the hard work and contributions made by family members in the science of enhanced collaboration between families and professionals, and improved service systems overall. Also highlighted are implications for future study on how the contributions of family members can effect changes in key areas of children's mental health through participation in SFNs. This article examines the extent to which statewide family organizations are committed to cultural competence through the involvement of culturally diverse families at all levels: as clients, volunteers, staff and board members. The data show that families who participate in and govern statewide family organizations seek to enhance participation of culturally diverse families in all aspects of operational and governance activities. Implications for future studies, which would measure the extent to which these organizations are achieving a balance bet ween culturally diverse and white families, are highlighted. Objectives: Despite significant investment in social prescribing in England over the last decade, we still do not know if it works, or how models of social prescribing fit within wider health and care policy and practice. This study explores current service delivery structures and assesses the feasibility of a national evaluation of the link worker model. Methods: Semi-structured interviews were conducted between May and September 2020, with 25 key informants from across social prescribing services in England. Participants included link workers, voluntary, community and social enterprise staff, and those involved in policy and decision-making for social prescribing services. Interview and workshop transcripts were analysed thematically, adopting a framework approach. Results: We found differences in how services are provided, including by individual link workers, and between organisations and regions. Standards, referral pathways, reporting, and monitoring structures differ or are lacking in voluntary services as compared to clinical services. People can self-refer to a link worker or be referred by a third party, but the lack of standardised processes generated confusion in both public and professional perceptions of the link worker model. We identified challenges in determining the appropriate outcomes and outcome measures needed to assess the impact of the link worker model. Conclusions: The current varied service delivery structures in England poses major challenges for a national impact evaluation. Any future rigorous evaluation needs to be underpinned with national standardised outcomes and process measures which promote uniform data collection. [ABSTRACT FROM AUTHOR]

Background: PRIMEtime CE is a multistate life table model that can directly compare the cost effectiveness of public health interventions affecting diet and physical activity levels, helping to inform decisions about how to spend finite resources. This paper estimates the costs and health outcomes in England of two scenarios: reformulating salt and expanding subsidised access to leisure centres. The results are used to help validate PRIMEtime CE, following the steps outlined in the Assessment of the Validation Status of Health-Economic decision models (AdViSHE) tool.Methods: The PRIMEtime CE model estimates the difference in quality adjusted life years (QALYs) and difference in NHS and social care costs of modelled interventions compared with doing nothing. The salt reformulation scenario models how salt consumption would change if food producers met the 2017 UK Food Standards Agency salt reformulation targets. The leisure centre scenario models change in physical activity levels if the Birmingham Be Active scheme (where swimming pools and gym access is free to residents during defined periods) was rolled out across England. The AdViSHE tool was developed by health economic modellers and divides model validation into five parts: validation of the conceptual model, input data validation, validation of computerised model, operational validation, and other validation techniques. PRIMEtime CE is discussed in relation to each part.Results: Salt reformulation was dominant compared with doing nothing, and had a 10-year return on investment of £1.44 (£0.50 to £2.94) for every £1 spent. By contrast, over 10 years the Be Active expansion would cost £727,000 (£514,000 to £1,064,000) per QALY. PRIMEtime CE has good face validity of its conceptual model and has robust input data. Cross-validation produces mixed results and shows the impact of model scope, input parameters, and model structure on cost-per-QALY estimates.Conclusions: This paper illustrates how PRIMEtime CE can be used to compare the cost-effectiveness of two different public health measures affecting diet and physical activity levels. The AdViSHE tool helps to validate PRIMEtime CE, identifies some of the key drivers of model estimates, and highlights the challenges of externally validating public health economic models against independent data.
