164. Admission Factors Predicting Family Medicine Specialty Choice: A Literature Review and Exploratory Study among Students in the Rural Medical Scholars Program

Purpose: The Rural Medical Scholars Program (RMSP) was created to increase production of rural family physicians in Alabama. Literature review reveals reasons medical students choose careers in family medicine, and these reasons can be categorized into domains that medical schools can address through admission, curriculum, and structural interventions. We examine whether admission factors can predict family medicine specialty choice among students recruited from rural Alabama. Methods: We developed a questionnaire to study the ability of admission factors to predict family medicine specialty choice among Rural Medical Scholars (RMS). Eighty RMS graduates were surveyed by mail and 64 (80%) responded. Findings: Student characteristics of humanitarian outlook with commitment to rural or underserved populations, family medicine decision or intention made before or at medical school admission, and community influence were positive associations with RMS choosing family medicine residencies; shadowing in an urban hospital was a negative association. Conclusions: Statements of interest, intentions, plans, and decisions regarding family medicine should be elicited at the time of RMSP admission interview. Strong attachment to home community and commitment to serving and living in a rural area are also important. Students whose introduction to medicine was informed through shadowing or observing in urban hospitals should be considered less likely to become family physicians. Larger sample size studies are needed to assess the role of gender, race, marital status, size of rural town, and MCAT score of candidates in affecting residency choices of students selected for this rural medical education track.

- Abstract
This article presents a framework for understanding learning experience as an object for instructional design--as an object for design as well as research and understanding. Compared to traditional behavioral objectives or discrete cognitive skills, the object of experience is more holistic, requiring simultaneous attention to cognition, behavior, and affect--even agency and identity. An emphasis on learner experience necessarily puts learners front and center of design concerns. But experience involves more than the learner: it is a "transactional" construct involving a person's encounters with their world over time. The framework presents selected individual and situational qualities, as well as temporal dimensions that affect the nature and level of experience. A concluding section considers how a focus on learning experiences might guide professional practice, opening the door for transformative learning and deeper forms of learning impacts. A transformative learning experience (TLE) is an especially meaningful encounter that leaves a lasting impact on a person's sense of competence or place in the world. The construct of TLE is presented in transactional terms as an irreducible interplay between learner and the world (including the natural world, artifacts, and other people). Indicators of TLE are grouped under the following clusters: personal meaning, competence, and relationships. Adding to conventional discourse about instructional design, the authors discuss moral, aesthetic, and mythological/archetypal considerations for creating learning encounters that could become transformative for learners. Trinidad's modern coastal depositional systems are for the first time documented by (traditional) descriptive and (modern) semiquantitative and quantitative classification methods. The processes controlling the morphology and distribution of these systems are also investigated. Google Earth™ satellite images, together with published basinal processes data (e.g., significant wave height, surface littoral currents, and tidal cyclicity) are used to describe system morphology, map depositional elements, and to determine the processes responsible for system distribution, respectively. Coastal depositional systems along Trinidad's east, south, and west coasts comprise deltas, estuaries, tidal-inlet complexes, strandplains, and tidal flats. Their distribution is controlled by the wave regime, although they are also influenced by tidal and fluvial processes. The wave regime changes from wave-dominated along the open east coast, to mixed-energy wave-dominated along the semi-sheltered south coast, and to mixed-energy tide-dominated along the sheltered west coast. Distribution of depositional systems related to this regime are such that the wave-dominated east coast sees the development of wave-dominated estuaries. The south coast has a series of wave-dominated, tide- and river-influenced deltas. The sheltered west coast is the most morphologically diverse with river- and wave-dominated deltas, strandplains, and tidal flats. The application of semiquantitative and quantitative classification methods on modern systems has showed their competence in classifying mixed-influence systems from their morphology and sedimentary record. Quantitative classification using depositional elements lends an appreciation to the morphological signatures of mixed-influence systems. Quantitative classification for sedimentary sections gives an appreciation of the preservation of mixed-influence physical processes. The combination of both allows the correlation of the impact of physical properties on morphology and sedimentary record (i.e., possible disparities between morphology and processes). The combination of semiquantitative and quantitative methods may also hold the key to unlocking the mixed-processes classification of depositional elements.

AECT has recently (yet again!) redefined our field, reverting back to the use of the term "educational technology." We believe this recent change is problematic for a number of reasons, but primarily because of the weak rationale offered for the change. This change affects how external audiences view our profession and is likely to confuse practitioners in corporate and higher education settings in particular. We offer a review of job postings, program titles, and listserv discussions to support our case. The labels we use to define ourselves are critically important--and we hope to see a stronger case made for changes for our foundational definitions in the future.

Aim Previous studies reported conflicting evidence on the effects of obesity on outcomes after gastrointestinal surgery. The aims of this study were to explore the relationship of obesity with major postoperative complications in an international cohort and to present a meta-analysis of all available prospective data.Methods This prospective, multicentre study included adults undergoing both elective and emergency gastrointestinal resection, reversal of stoma or formation of stoma. The primary end-point was 30-day major complications (Clavien-Dindo Grades III-V). A systematic search was undertaken for studies assessing the relationship between obesity and major complications after gastrointestinal surgery. Individual patient meta-analysis was used to analyse pooled results.Results This study included 2519 patients across 127 centres, of whom 560 (22.2%) were obese. Unadjusted major complication rates were lower in obese vs normal weight patients (13.0% vs 16.2%, respectively), but this did not reach statistical significance (P = 0.863) on multivariate analysis for patients having surgery for either malignant or benign conditions. Individual patient meta-analysis demonstrated that obese patients undergoing surgery for malignancy were at increased risk of major complications (OR 2.10, 95% CI 1.49-2.96, P Conclusions In our international data, obesity was not found to be associated with major complications following gastrointestinal surgery. Meta-analysis of available prospective data made a novel finding of obesity being associated with different outcomes depending on whether patients were undergoing surgery for benign or malignant disease.

The contributions to this issue share a focus on design of e-learning environments. Instructional designers stand at very early stages of knowledge in this area, but with great potential for growth and progress. This commentary offers an activity-based perspective on e-learning environments, resulting in a flexible stance toward instructional strategies, artifact design, emergent activity, and learning outcomes. This same flexibility should be evident as we in the field appropriate theories from other disciplines and develop some of our own. I support the contributors' call for more design research specifically addressing challenging problems of practice encountered by design practitioners.

Category: Sports. Introduction/Purpose: Large segmental Achilles tendon defects present a difficult problem to treating surgeons. Multiple procedures have been utilized to reconstruct these defects, but no studies have evaluated the comparative tensile strength of the various repair methods. Our institution has recently described the use of a dual semitendinosus allograft for Achilles reconstruction. Advantages of this procedure include eliminating donor site morbidity and providing an increased surface area for healing and tendon incorporation. Our study investigated the tensile strength of this novel technique as compared to a standard myofascial turndown procedure. We hypothesized that no differences in biomechanical properties would be found between dual semitendinosus reconstruction and Achilles myofascial turndown reconstruction. Methods: An 8-cm segmental Achilles defect was created in both specimens of nine matched pair, cadaveric lower extremities. The specimens in each pair were randomly assigned to undergo allograft or turndown reconstruction. The myofacial turndown was secured distally with modified Kessler sutures of 0 braided polyester sutures through bone tunnels in the calcaneus and proximally with multiple interrupted figure-of-8 polyester braid sutures. Semitendinosus grafts were anchored proximally with a Pulvertaft weave and then distally through two bone tunnels within the calcaneus and sutured together around the posterior heel with similar polyester sutures. The foot was disarticulated through the subtalar joint and the Achilles was dissected free of excess soft tissues. The constructs were mounted onto a load frame and differential variable reluctance transducers were applied to the construct. Specimens were preconditioned and then loaded axially. Tensile forces were recorded at 10 mm of displacement and at failure Results: Semitendinosus allograft failure occurred via calcaneal bone bridge fracture in 8 of 9 specimens, and all myofascial turndowns failed through suture pullout through the fascial tissue at its insertion. None of the specimens failed at the MTS grip sites or via de novo tendon substance rupture. Average ultimate tensile strength of the semitendinosus allograft reconstruction was 290.9± 83.2 N compared to that of the turndown repair which was 140.7± 43.5 N. At 10 mm of displacement, average tensile strength of the allograft repair was 156.9 ± 29.7 N versus the turndown repair at 101.2 ± 20.0 N. Strength differences between the two repairs were significant at both failure and 10 mm displacement (p < 0.001). Conclusion: Dual semitendinosus allograft reconstruction demonstrates superior tensile strength properties when compared to myofascial turndown in a cadaveric model of large Achilles tendon defects.

INTRODUCTION There is little biomechanical evidence to support the traditional guideline that 45° represents acceptable proximal humerus deformity. We evaluated glenohumeral contact pressure (GP) and area, subacromial contact pressure, and joint abduction to assess biomechanical changes with different proximal humerus deformities. METHODS Fifteen fresh-frozen cadaver shoulders were used. Intact specimens were tested on a custom dynamic shoulder frame. Subsequently, a surgical neck fracture was made in each specimen and fixed using a custom dual hinge plate for fixation of 15°, 30°, and 45° deformities in varus, valgus, antecurvatum, retrocurvatum, and combined varus-antecurvatum and valgus-retrocurvatum. Specimens were then retested. RESULTS Compared with the intact state, GP was markedly lower with all levels of varus and varus-antecurvatum deformity. Valgus and combined valgus-retrocurvatum deformity of 45° led to notable increases in GP compared with the intact state. Varus deformities of 30° and 45° caused significant increases in subacromial pressures and limited abduction markedly from 60° to 54.2° and 44.6° (P < 0.001). DISCUSSION Varus and antecurvatum proximal humerus deformities as small as 15° were associated with notable alterations in glenohumeral joint mechanics. With valgus and retrocurvatum deformity, statistically significant joint alterations occurred only at higher deformity levels.

The intertidal coastline of Ceredigion, Wales, comprises a patchwork of unstable sand and cobble beaches, and stable bedrock areas and boulder-fields. The last two shoreline types support rock-pools with growths of the red alga Corallina officinalis, the thalli of which are a popular substrate for calcareous epiphytes. Replicate samples of C. officinalis (four per site) were taken from (a) three bedrock sites (Ceinewydd, Aberystwyth Victoria Rocks and Castle Rocks) and (b) three boulder-fields (Llanon, Aberaeron lower shore (Aberaeron LS), Llanina) on the lower shore. The middle shore boulder field at Aberaeron (Aberaeron MS) was also sampled. These replicates were examined for calcareous meiofauna (63–2000 μm) not previously examined as a community: spirorbids, foraminifera, gastropods, bryozoans, ostracods and ophiuroids. These were assigned to sessile and vagile modes of life. The sessile association overwhelmingly dominated bedrock coastlines and the Aberaeron MS, while the vagile association was at its most abundant on the Corallina from lower shore, stable boulder-fields. Gastropods were almost entirely limited to Corallina on boulder-fields. We hypothesize that the boulders induce low-energy turbulence among breaking waves, allowing the less firmly attached vagile meiofauna to dominate on C. officinalis in rock-pools in lower shore boulder-fields. The small attachment area of sessile organisms allows them to settle bedrock sites in greater densities than do vagile organisms at boulder-field sites, which are presumed to require larger foraging areas.

Mercury is found in Arctic marine mammals that are important in the diet of northern Indigenous peoples. The objectives of the present long‐term study, spanning a 45‐yr period, were to 1) investigate the temporal trends of total mercury (THg; muscle and liver) and selenium (Se; liver) in ringed seals (Pusa hispida) from different regions of the Canadian Arctic; and 2) examine possible relationships with age, diet, and climate parameters such as air temperature, precipitation, climatic indices, and ice‐coverage. Ringed seals were collected by hunters in northern communities in the Beaufort Sea, Central Arctic, Eastern Baffin Island, Hudson Bay, and Ungava/Nunatsiavut regions (Canada) between 1972 and 2017. Mercury levels did not change through time in seal liver, but THg levels in muscle decreased in seals from Hudson Bay (−0.91%/yr) and Ungava/Nunatsiavut (−1.30%/yr). Carbon stable isotope values in seal muscle decreased significantly through time in 4 regions. Selenium‐to‐THg ratios were found to be >1 for all years and regions. Variation partitioning analyses across regions indicated that THg trends in seals were mostly explained by age (7.3–21.7%), climate parameters (3.5–12.5%), and diet (up to 9%); climate indices (i.e., Arctic and North Atlantic Oscillations, Pacific/North American pattern) explained the majority of the climate portion. The THg levels had a positive relationship with Arctic Oscillation for multiple regions. Associations of THg with air temperature, total precipitation, and sea‐ice coverage, as well as with North Atlantic Oscillation and Pacific/North American pattern were found to vary with tissue type and geographical area. Background: Debate exists on the optimum fixation construct for large avulsion fractures of the fifth metatarsal base. We compared the biomechanical strength of 2 headless compression screws vs a hook plate for fixation of these fractures. Methods: Large avulsion fractures were simulated on 10 matched pairs of fresh-frozen cadaveric specimens. Specimens were assigned to receive two 2.5-mm headless compression screws or an anatomic fifth metatarsal hook plate, then cyclically loaded through the plantar fascia and metatarsal base. Specimens underwent 100 cycles at 50%, 75%, and 100% physiological load for a total of 300 cycles. Results: The hook plate group demonstrated a significantly higher number of cycles to failure compared with the screw group (270.7 ± 66.0 [range 100-300] cycles vs 178.6 ± 95.7 [range 24-300] cycles, respectively; P = .039). Seven of 10 hook plate specimens remained intact at the maximum 300 cycles compared with 2 of 10 screw specimens. Nine of 10 plate specimens survived at least 1 cycle at 100% physiologic load compared with 5 of 10 screw specimens. Conclusion: A hook plate construct was biomechanically superior to a headless compression screw construct for fixation of large avulsion fractures of the fifth metatarsal base. Clinical Relevance: Whether using hook plates or headless compression screws, surgeons should consider protecting patient weight-bearing after fixation of fifth metatarsal base large avulsion fracture until bony union has occurred.

The novel coronavirus disease 2019 (COVID‐19) is a highly infectious and rapidly spreading disease. There are limited published data on the epidemiology and outcomes of COVID‐19 infection among organ transplant recipients. After initial flulike symptoms, progression to an inflammatory phase may occur, characterized by cytokine release rapidly leading to respiratory and multiorgan failure. We report the clinical course and management of a liver transplant recipient on hemodialysis, who presented with COVID‐19 pneumonia, and despite completing a 5‐day course of hydroxychloroquine, later developed marked inflammatory manifestations with rapid improvement after administration of off‐label, single‐dose tocilizumab. We also highlight the role of lung ultrasonography in early diagnosis of the inflammatory phase of COVID‐19. Future investigation of the effects of immunomodulators among transplant recipients with COVID‐19 infection will be important.

Ecological theory suggests that demographic responses by populations to environmental change vary depending on whether individuals inhabit central or peripheral regions within the species’ geographic range. Here, we tested this prediction by comparing a population of ringed seals Pusa hispida located at high latitudes as part of their core range (core) with a population located at the southern extremity of their range (peripheral). First, we compared the two regions’ environmental trends in timing of sea-ice breakup and freeze-up, open-water duration and the North Atlantic Oscillation (NAO). We found that the core region shifted to progressively warmer conditions in the early 1990s; whereas, in the peripheral region, the warming trend shifted in 1999 to one with no warming trend but high inter-annual variability. Next, we examined how body condition, inferred from blubber depth, responded to temporal changes in sea-ice and climatic variables – variables that have been shown to influence ringed seal demography. Core seals displayed minimal seasonal changes in body condition; whereas peripheral seals displayed a 20–60% amplitude seasonal change in body condition with a phase shift to earlier initiation of fat accumulation and loss. Finally, we tested for interannual differences and found that both core and peripheral seals responded similarly with decreased body condition following more positive NAO. Environmental variables influenced body condition in opposite directions between the two regions with core seals declining in body condition with later spring breakup and shorter open-water duration, whereas peripheral seals showed opposite relationships. Seals living at the core likely benefit from an evolved match between adaptation and environmental variation resulting in dampened seasonal and interannual fluctuations in body condition. Knowledge of how different populations respond to environmental change depending on geographic location within a species range can assist in anticipating population specific responses to climate warming.

Accounts of killer whale (Orcinus orca) predation on marine mammals in the Canadian Arctic are relatively uncommon. Although second-hand reports of killer whale predation events in the Arctic are more common in recent years, these observations are generally poorly documented and the outcome of attacks are often unknown. On 12 August 2016, a floating bowhead whale (Balaena mysticetus) carcass was found off-shore in Cumberland Sound, Nunavut — presumably predated by killer whales that were sighted in the area. Inspection of the carcass revealed injuries consistent with published accounts of killer whale predation on large whales and observations of killer whale predation on bowheads described in Inuit traditional knowledge. The bowhead was male, 6.1 m long in good nutritional condition and estimated between 14 and 16 months old. As a recently weaned yearling, this whale would have been highly vulnerable to killer whale predation. With decreasing summer sea ice making some areas of the Arctic more accessible, the incursion and presence of killer whales in the Arctic is expected to increase. A better understanding of Arctic killer whale predation pressure is needed to predict the potential impact they will have on the eastern Canada–west Greenland bowhead population as well as on other marine mammal prey.

Background: Aspirin and mechanical compression devices are approved means of venous thromboembolism (VTE) prophylaxis after total joint arthroplasty. Prior studies of mechanical compression pumps after joint arthroplasty have been limited to the inpatient setting. The purpose of this study was to evaluate outpatient compliance and utilization factors in a rural population after elective hip or knee arthroplasty. Methods: Utilization for portable pneumatic compression pumps after joint arthroplasty was prospectively recorded (hours). Compliance was defined as the recommended 20 hours per day. A questionnaire 2 weeks postoperatively assessed factors that may contribute to noncompliance. Patients were followed up for 90 days postoperatively to record VTE events. Results: Data were collected for 115 joint arthroplasty patients (50 hips, 65 knees). Postdischarge day one had the highest average usage at 13.2 hours/day (66.0%, range 0%-100%), but this number fell to 4.8 hours/day (24.0, range 0%-100%) by day 14. Patient compliance (>20 hours use/day) was highest on postdischarge day one at 40 patients (34.7%). By postdischarge day 14, patient compliance fell to 17 patients (14.8%). Difficulty using the pumps (P = .027) and pump-associated heat (P = .009) were significantly associated with patient compliance. A deep vein thrombosis and nonfatal pulmonary embolism were recorded in 2 separate patients. Conclusions: This study demonstrated poor outpatient compliance with portable pneumatic compression devices. Poor compliance was related to pump heat and difficulty with pump use.
