Baixada Santista's region is world famous for its high levels of pollution, which are related to industrial, port and urban activities. Around the industrial pole of Cubatao city, high nutrient concentrations are found such as nitrate >90 moll-1 N-NO-3 and phosphate >24 moll-1 P-PO-34. A clear gradient of dilution from the estuary's interior to its mouth is present along the natural channels, but does not reach low concentrations for all elements in the Santos Bay. Artificial urban drainage channels present accentuated differences of the nutrient concentrations inside and outside each lock. The inner channel waters, show low average values of salinity (1.33 PSU) and oxygen (~2.0 mll-1); pH under 8; high averages of nutrient concentrations (ammonium~40 moll-1, nitrate ~42, phosphate ~2.96 moll-1, silicate ~119 moll-1); high total and faecal coliform averages (3566x 103/100 and 1637x 103/100 ml, respectively). Outside of the channels, on the beaches, there were high nutrient concentrations at some points and accentuatedtotal and faecal coliform averages (91x 103/100 and 28x 103/100 ml, respectively). The high TC and FC contents outside of the locks show the inefficiency of the actions to prevent pollution on the beaches. These facts represent risks to local biota and human health. Foraminifera could be used as indicators to evaluate the impacts of oil spills because the crude oil causes several disturbances on the development and reproduction of the species. However, little is known about the relationship between mangrove species and this pollutant. Foraminiferal assemblages were studied in 22 surface sediment samples collected from a mangrove in Guanabara Bay (Brazil) that was severely impacted by an oil spill six years earlier. The foraminiferal responses to the environmental stress helped to determine the most degraded areas. Elevated concentrations of aromatic compounds and potentially bioavailable heavy metals, with contamination levels up to nine times higher compared to the pre-industrial period in the Guanabara Bay. The dominance of anaerobic bacteria, and high esterase enzyme activity (ESTE) suggest that the bacterial community is metabolizing the hydrocarbons in the sediments. Despite these stressors, density and diversity of living foraminifera are comparable to values observed in less impacted Brazilian mangroves. Species distribution patterns reveal an environmental gradient across the mangrove with numerous species increasing their relative abundance towards the areas topographically lower and with higher levels of pollutants and ESTE. Specifically, Caronia exilis, Tiphotrocha comprimata, Ammobaculites exiguus, Textularia paranaguaensis, Ammotium cassis, Ammobaculites dilatatus, Polyssaccamina hipohalina, Siphotrochammina lobata, Ammonia tepida, Ammotium morenoi, Miliammina fusca, Entzia macrescens, Trochammina squamata and Paratrochammina clossi are inferred as pollution-tolerant taxa. Background: Lung transplant (LUTX) candidates have subclinical right ventricular (RV) dysfunction, which has not yet been assessed by speckle-tracking echocardiography (STE)-derived RV free-wall longitudinal strain (RVFWLS). To evaluate the prevalence of RV dysfunction by RVFWLS and its relationship with conventional RV echocardiographic indexes in LUTX candidates., Methods: In a single-center prospective observational cohort study, from January 2021 to March 2023 consecutive LUTX candidates underwent cardiac catheterization, radionuclide ventriculography, standard and STE. The diagnostic accuracy of RV ejection fraction by ventriculography (RVEF), tricuspid annular plane excursion (TAPSE), fractional area change (FAC), tricuspid peak annulus systolic velocity (S') versus RVFWS were computed., Results: Thirty-four patients (female, 41%) with a mean age of 48 [36-59] years old enlisted for pulmonary fibrosis (35%) and cystic fibrosis (30%) were included. At cardiac catheterization, only 7 (23%) had pulmonary hypertension. Around 15-25% presented right heart enlargement. Tricuspid regurgitation was present in 20 (60%) of the patients. Median RVFWLS was -20.1% [-22.5%--17%], being impaired (> -20%) in 16 (47%) of the patients. RVFWLS identified the highest percentage (47%) of RV dysfunction, compared to TAPSE (32%), S' (27%), FAC (26%), and ventriculography (15%), which had very low sensitivity for detecting RV dysfunction compared to RVFWLS., Conclusions: In patients enlisted for LUTX, RV dysfunction assessed by STE-derived RVFWLS is highly prevalent. STE can detect RV dysfunction better than standard two-dimensional echocardiography and ventriculography. The timely administration of the effective therapy is an important factor for the favourable outcome of patients with sepsis. In this study, we evaluated a new rapid antimicrobial susceptibility testing (AST) method, the Vitek®Reveal ™ system, to determine the antibiotic susceptibility of 44 Gram-negative bacteria randomly isolated from blood. The results show a mean turnaround time of 5.42 h. The overall agreement with the reference method was >90%, except for piperacillin/tazobactam and cefepime (90.4%). The study, therefore, suggests that the assay decreases the time for obtaining AST with the potential to have a positive impact on patient care. However, further studies are needed to extend and confirm these preliminary findings, particularly the assay performance for some drugs and eventually for Gram-positive micro-organisms.

Background  The keystone design perforator island flap (KDPIF) was first described by Behan in 2003 as a pliable flap, suitable for all body areas thanks to its curvilinear shape, which made it apt to fit mostly all cutaneous defects. Materials and Methods  In this article, we aim to share our experience using KDPIF for the reconstruction of different defects. A retrospective analysis of patients who underwent cutaneous excision after trauma or tumor resection and reconstruction with a KDPIF at our department from 2020 to 2023 was performed. Patients' characteristics, indications, and operative and postoperative details were analyzed. Results  There was no complete flap loss. One patient experienced partial flap loss without exposure to major structures and needed skin grafting. Research on the use of sex toys has been primarily performed from a medical perspective, while there is still limited research from a psychosocial perspective. To bridge this gap, in this study we examined whether some psychosocial variables might be linked to sex toy ownership in a sample of 3960 Italian (cisgender men and women) sex toy buyers. More specifically, we investigated the association between gender identities and ideologies and the variety and types of sex toys owned. Based on the data, we detected two dimensions underlying the ownership of sex toys: (1) orientation to owning kinky sex toys and (2) orientation to owning clit-oriented sex toys. Results showed that benevolent sexism and gender system justification were negatively correlated with owning clit-oriented toys. Moreover, strongly gender-identified participants owned a small variety of different toys and preferred toys that were designed to stimulate the vagina or clitoris over less commonly-used toys. No significant correlation between feminist identification and sex toy type owned was found when gender identification was taken into account. These results suggest that the owning of sex toys might be associated with traditional gender ideology and the strength of gender identification., (© 2024. The beneficial effects of high-fat low-carbohydrate (HFLC) diets on glucose metabolism have been questioned and their effects on liver metabolism are not totally clear. The aim of this work was to investigate the effects of an HFLC diet under different energy conditions on glucose homeostasis, fatty liver development, and hepatic gluconeogenesis using the isolated perfused rat liver. HFLC diet (79% fat, 19% protein, and 2% carbohydrates in Kcal%) was administered to rats for 4 weeks under three conditions: ad libitum (hypercaloric), isocaloric, and hypocaloric (energy reduction of 20%). Fasting blood glucose levels and total fat in the liver were higher in all HFLC diet rats. Oral glucose tolerance was impaired in isocaloric and hypercaloric groups, although insulin sensitivity was not altered. HFLC diet also caused marked liver metabolic alterations: higher gluconeogenesis rate from lactate and a reduced capacity to metabolize alanine, the latter effect being more intense in the hypocaloric condition. Thus, even when HFLC diets are used for weight loss, our data imply that they can potentially cause harmful consequences for the liver., Competing Interests: There are no competing interests.

The causes of cardiac arrest are extremely heterogeneous. Among these, both hypokalemia and hypocalcemia are known reversible factors that can lead to cardiac arrest. In this report, we present a unique case report of a patient with previously undiagnosed coeliac disease who experienced cardiac arrest due a combination of hypokalemia and hypocalcemia resulting from malabsorption. A 66-year-old male presented to the emergency department with symptoms of malaise, weakness, weight loss, and persistent diarrhea. The patient exhibited characteristic signs of hypokalemia and hypocalcemia, including fasciculations, weakness, and swelling. An electrocardiogram showed a normal rhythm, and blood tests confirmed the electrolyte imbalances. Despite initial treatment, the patient experienced sudden cardiac arrest. Prompt resuscitation efforts were successful in restoring spontaneous circulation. However, recurrent episodes of ventricular arrhythmias and cardiac arrest occurred. Large doses of intravenous potassium chloride, in conjunction with magnesium, were needed prior to restore electrolyte balance. The concomitant severe hypocalcemia required caution calcium supplementation, to avoid further decreases in serum potassium levels. Appropriate ion replacements ultimately led to successful resuscitation with good functional recovery. During the hospital stay, the patient was diagnosed with coeliac disease. This case is noteworthy for its uniqueness, as there are no documented instances in the scientific literature linking cardiac arrest directly to coeliac disease. It is important to emphasize the need for investigating potential reversible causes of cardiac arrest, such as hypokalemia and hypocalcemia, and implementing appropriate interventions to address these factors.

Introduction: The postoperative hemodynamic management after lung transplant (LUTX) is guided by limited evidence. We aimed to describe and evaluate risk factors and outcomes of postoperative vasoactive support of LUTX recipients., Methods: In a single-center retrospective analysis of consecutive adult LUTX, two cohorts were identified: (1) patients needing prolonged vasoactive support (>12 h from ICU admission) (VASO+); (2) or not (VASO-). Postoperative hemodynamic characteristics were thoroughly analyzed. Risk factors and outcomes of VASO+ versus VASO- cohorts were assessed by multivariate logistic regression and propensity score matching., Results: One hundred and thirty-eight patients were included (86 (62%) VASO+ versus 52 (38%) VASO-). Vasopressors (epinephrine, norepinephrine, dopamine) were used in the first postoperative days (vasoactive inotropic score at 12 h: 6 [4-12]), while inodilators (dobutamine, levosimendan) later. Length of vasoactive support was 3 [2-4] days. Independent predictors of vasoactive use were: LUTX indication different from cystic fibrosis (p = .003), higher Oto score (p = .020), longer cold ischemia time (p = .031), but not preoperative cardiac catheterization. VASO+ patients showed concomitant hemodynamic and graft impairment, with longer mechanical ventilation (p = .010), higher primary graft dysfunction (PGD) grade at 72 h (PGD grade > 0 65% vs. 31%, p = .004, OR 4.2 [1.54-11.2]), longer ICU (p < .001) and hospital stay (p = .013). Levosimendan as a second-line inodilator appeared safe., Conclusions: Vasoactive support is frequently necessary after LUTX, especially in recipients of grafts of lesser quality. Postoperative hemodynamic dysfunction requiring vasopressor support and graft dysfunction may represent a clinical continuum with immediate and long-term consequences. Background: Catheter-related thrombosis (CRT) incidence, rate, and risk factors vary in literature due to differences in populations, catheters, diagnostic methods, and statistical approaches. The aim of this single-center, prospective, observational study was to assess incidence, incidence rate (IR), cumulative incidence, and risk factors by means of IR ratio (IRR) of asymptomatic CRT in a non-oncologic Intensive Care Unit (ICU) population. CRT development was assessed daily by means of ultrasound screening. The proportions of patients and catheters developing CRT and CRT incidence rates, expressed as the number of events per catheter-days (cd), were calculated. Kalbfleisch and Prentice's method was used to estimate the cumulative incidence of CRTs. Univariate and multivariable Poisson regression models were fitted to calculate IRR in risk factors analysis., Results: Fifty (25%, 95% CI 19-31) out of 203 included patients, and 52 (14%, 95% CI 11-18) out of 375 catheters inserted developed CRT [IR 17.7 (13.5-23.2) CRTs/1000*cd], after 5 [3-10] days from insertion. Forty-six CRTs (88%) were partial thrombosis. All CRTs remained asymptomatic. Obesity and ECMO support were patient-related protective factors [IRR 0.24 (0.10-0.60), p = 0.002 and 0.05 (0.01-0.50), p = 0.011, respectively]. The internal jugular vein had higher CRT IR than other sites [20.1 vs. 5.9 CRTs/1000*cd, IRR 4.22 (1.22-14.63), p = 0.023]. Pulmonary artery catheter and left-side cannulation were catheter-related risk factors [IRR 4.24 (2.00-9.00), p < 0.001 vs. central venous catheters; IRR 2.69 (1.45-4.98), p = 0.002 vs. right cannulation, respectively]. No statistically significant effect of the number of simultaneously inserted catheters [IRR 1.11 (0.64-1.94), p = 0.708] and of the catheterization length [IRR 1.09 (0.97-1.22), p = 0.155] was detected. The ICU length of stay was longer in CRT patients (20 [15-31] vs. 6 [4-14] days, p < 0.001), while no difference in mortality was observed., Conclusions: CRTs are frequent but rarely symptomatic. This study investigated the effect of the victim's gender and the perpetrator's attractiveness on the observer's blame on the male and female victims of coercive sexual contact. Two hundred and ninety-six participants (184 females) were enrolled in an experiment in which the victim's gender and the offender's attractiveness were manipulated using vignettes depicting cross-gender sexual assault. Participants rated emotions that the victims experienced in being assaulted and attributed victim blame. The results indicate that the male victim was blamed more than the female victim, especially when the female perpetrator was described as attractive. The female victim was perceived as having experienced more negative emotions and fewer positive emotions than the male victim. This research analyzed the effect of professional, organizational and care-unit identifications on both healthcare professionals' quality of professional life and mental health. This research was done in a local hospital in a region of northern Italy which was one of the first regions to be impacted by the first wave of the pandemic. Using a cross-sectional research, a web-based questionnaire was sent to the healthcare professionals. Professional quality of life, professional identifications as well as emotional maladjustment in terms of stress, anxiety and depression were measured. Results indicated that professional and care unit identification were positively linked to increased compassion satisfaction and reduced burnout. Professional identification was negatively associated with secondary traumatic stress as well, while care unit identification was positively associated with vicarious trauma. The negative dimension of the professional quality of life had positive relations with emotional maladjustment. Professional and care unit identifications appeared to have an indirect effect via professional quality of life on maladjustment.

The present study aimed to investigate the role of motivational process and coping resources in health professionals during the COVID-19 emergency examining the role of Care Unit Identification and safety climate perception as resources that can help nurses to cope with stressors. A cross-sectional research design was used and 218 nurses completed a self-report questionnaire measuring: Perception of safety, Care Unit identification, Work Engagement, Psychological Distress, and Burnout. Results revealed that Work Engagement was significantly related with Burnout ( b  = -0.209, 95%CI [-0.309; -0.109]) and Distress ( b  = -0.355, 95%CI [-0.529; -0.18]) especially when the Care Unit identification is high ( b  = -0.303, 95%CI [-0.448; -0.157] and b  = -0.523, 95%CI [-0.772; -0.275], respectively). The safety perception was positively related to Work Engagement ( b  = 0.315, 95%CI [0.198; 0.433]) and had an indirect effect on psychological Distress ( b  = -0.112, 95%CI [-0.181; -0.042]) and Burnout ( b  = -0.066, 95%CI [-0.105; -0.027]). High levels of both Care Unit identification and perception of safety, along with personal work engagement, appear to protect nurses from burnout and psychological distress. Background and Aim: Perceived COVID-19-related stigmatizations have a strong impact on healthcare workers' wellbeing and quality of professional life, decreasing satisfaction and increasing fatigue. This work aims to investigate the role of professional identification in moderating the impact of COVID-19-related stigma on quality of professional life in a sample of healthcare professionals working in hospital., Methods: A cross-sectional design in which a web-based questionnaire was sent to professionals was used to collect answers from 174 participants, most of whom women and nurses., Results: Perceived stigma was negatively related with compassion satisfaction and positively related with an increase in both burnout and secondary traumatic stress. Professional identification had a positive correlation with satisfaction and a negative correlation with burnout, but this was not directly related with secondary traumatic stress. Importantly, stigma and identification interacted so that stigma decreased compassion satisfaction only when identification was low, and increased secondary traumatic stress only when identification was high. No interaction effect appeared with respect to burnout., Conclusions: Experience of stigmatization has the potential to decrease the quality of professional life of healthcare professionals. Professional identification seems to help professional to maintain higher level of compassion satisfaction and reduced burnout. Do superordinate in-group bias as well as temporal and social comparisons offer standalone explanations for system justification? We addressed this question using the latest World Value Survey (7th Wave), combining the responses of 55,721 participants from 40 different nations. Results from a random slope multilevel model showed that superordinate (national) identification, temporal comparison (i.e., the outcomes of an individual relative to those of his/her parents at different time points), and social comparison (based on income levels) were independent and positive predictors of system justification. Specifically, system justification increased when national identification was high, when income increased (i.e., the socioeconomic comparison was positive), and when the outcomes of citizens improved relative to the outcomes of their parents at relevant time points (i.e., the temporal comparison was positive). Incidentally, we also observed an interaction between national identification and temporal comparison (but not with social comparison), indicating that positive temporal comparison seemed to have a reduced effect (but still significant) for highly identified citizens. The present dataset was collected to evaluate the environmental stressors on a lacustrine basin in the Eastern Alps of glacial origin that has been affected in recent years by natural and anthropogenic events such as the construction of a hydroelectric power plant and a series of strong earthquakes during 1976-1977. We collected sediment cores in different sites from the lake margins to the depocenter and performed a multiproxy analysis of sediment sample to highlight lake stratigraphy and major changes occurring at a decadal scale (Polonia et al., [1]). The integrated analyses of sedimentological, geochemical, isotopic, mineralogical and micropaleontological analyses aimed at reconstructing changes in sediment composition and define the triggering mechanisms of altered environmental conditions. Three studies have tested the hypothesis that intermediate-status groups are more oriented to ally with outgroups when their social position is under threat. In study 1, participants believed that their ingroup was intermediate in status and social stratification was manipulated as either stable or status-detrimental unstable. Results indicated that participants were more likely to seek alliances a) with a high-status group and b) when social stratification was status-detrimental unstable. Study 2 showed that participants were more likely to seek alliances with a lower status group when social stratification was status-detrimental unstable rather than stable, while they were supportive of policies helping disadvantaged groups regardless of the stability of social stratification. Study 3 showed that when social stratification was status-detrimental unstable, intermediate-status group members were more oriented to ally with a low-status group, equally supportive of policies helping disadvantaged groups, but less oriented to supplying direct help to a low-status group., Competing Interests: The authors have declared that no competing interests exist.

Background: Early diagnosis of acute kidney injury (AKI) is a major challenge in the intensive care unit (ICU). The AKIpredictor is a set of machine-learning-based prediction models for AKI using routinely collected patient information, and accessible online. In order to evaluate its clinical value, the AKIpredictor was compared to physicians' predictions., Methods: Prospective observational study in five ICUs of a tertiary academic center. Critically ill adults without end-stage renal disease or AKI upon admission were considered for enrollment. Using structured questionnaires, physicians were asked upon admission, on the first morning, and after 24 h to predict the development of AKI stages 2 or 3 (AKI-23) during the first week of ICU stay. Discrimination, calibration, and net benefit of physicians' predictions were compared against the ones by the AKIpredictor., Results: Two hundred fifty-two patients were included, 30 (12%) developed AKI-23. In the cohort of patients with predictions by physicians and AKIpredictor, the performance of physicians and AKIpredictor were respectively upon ICU admission, area under the receiver operating characteristic curve (AUROC) 0.80 [0.69-0.92] versus 0.75 [0.62-0.88] (n = 120, P = 0.25) with net benefit in ranges 0-26% versus 0-74%; on the first morning, AUROC 0.94 [0.89-0.98] versus 0.89 [0.82-0.97] (n = 187, P = 0.27) with main net benefit in ranges 0-10% versus 0-48%; after 24 h, AUROC 0.95 [0.89-1.00] versus 0.89 [0.79-0.99] (n = 89, P = 0.09) with main net benefit in ranges 0-67% versus 0-50%., Conclusions: The machine-learning-based AKIpredictor achieved similar discriminative performance as physicians for prediction of AKI-23, and higher net benefit overall, because physicians overestimated the risk of AKI. This suggests an added value of the systematic risk stratification by the AKIpredictor to physicians' predictions, in particular to select high-risk patients or reduce false positives in studies evaluating new and potentially harmful therapies. A semifield study to assess the effects of iprodione on honeybees at label use rates was conducted on a bloom mustard crop. The present study followed the Organisation for Economic Co-operation and Development guideline 75 tunnel test and consisted of 3 groups: the iprodione-treated group, the untreated control group, and the toxic reference item group. In addition to the tunnels used for biological assessments, a tunnel was set up in the treatment and control groups to determine the level of residues in flowers, nectar, and pollen. The major endpoints to assess the effects of the application of iprodione were mortality, flight intensity, behavior, condition of the colonies, and development of the brood. Residue analysis showed that honeybees were exposed to significant residues of iprodione. However, no adverse effects were observed on overall mortality, flight intensity, behavior, or brood development of honeybees compared to control. It is concluded that iprodione does not adversely affect the health of honeybees when applied in agriculture at commercially relevant rates in a worst-case exposure scenario. Quantifying the relative contribution of multiple isolation barriers to gene flow between recently diverged species is essential for understanding speciation processes. In parapatric populations, local adaptation is thought to be a major contributor to the evolution of reproductive isolation. However, extrinsic postzygotic barriers assessed in reciprocal transplant experiments are often neglected in empirical assessments of multiple isolation barriers. We analyzed multiple isolation barriers between two closely related species of the plant genus Dianthus, a genus characterized by the most rapid species diversification in plants reported so far. Although D. carthusianorum L. and D. sylvestris Wulf. can easily be hybridized in crossing experiments, natural hybrids are rare. We found that in parapatry, pollinator-mediated prezygotic reproductive isolation barriers are important for both D. carthusianorum (0.761) and D. sylvestris (0.468). In contrast to D. carthusianorum, high hybrid viability in D. sylvestris (-0.491) was counteracted by strong extrinsic postzygotic isolation (0.900). Our study highlights the importance of including reciprocal transplant experiments for documenting extrinsic postzygotic isolation and demonstrates clearly divergent strategies and hence asymmetric pre- and postzygotic reproductive isolation between closely related species. It also suggests that pollinator-mediated and ecological isolation could have interacted in synergistic ways, further stimulating rapid speciation in Dianthus., (© 2018 The Author(s). This paper analyses the distribution of benthic foraminifera in a sector of the inner continental shelf of Santa Catarina State (Brazil), which comprises a Marine Protected Area (MPA). Species indicators of continental input suggest that waters under the influence of continental drainage can eventually reach the southwestern part of the reserve, which might jeopardise the ecosystems of this MPA due to the transport of contaminants related to human activities. Species known to be indicators of high marine benthic productivity were more abundant below 30 m, and were associated with areas under the stronger influence of nutrient-enriched water mass. The high density of foraminifera and the low dominance of species found in most samples inside the reserve might be evidence for the positive effects of the prohibition of bottom trawling, ensuring a higher ecological equilibrium of benthic communities. Polycistronic microRNA (miRNA) clusters are a common feature of vertebrate genomes. The coordinated expression of miRNAs belonging to different seed families from a single transcriptional unit suggests functional cooperation, but this hypothesis has not been experimentally tested. Here we report the characterization of an allelic series of genetically engineered mice harboring selective targeted deletions of individual components of the miR-17 ∼ 92 cluster. Our results demonstrate the coexistence of functional cooperation and specialization among members of this cluster, identify a previously undescribed function for the miR-17 seed family in controlling axial patterning in vertebrates and show that loss of miR-19 selectively impairs Myc-driven tumorigenesis in two models of human cancer. By integrating phenotypic analysis and gene expression profiling, we provide a genome-wide view of how the components of a polycistronic miRNA cluster affect gene expression in vivo. The reagents and data sets reported here will accelerate exploration of the complex biological functions of this important miRNA cluster.

Backgraund: Pain is the primary reason for admission to the Emergency Department (ED). However, the management of pain in this setting is often inadequate because of opiophagia, fear of excessive sedation, and fear of compromising an adequate clinical assessment., Methods: An intersociety consensus conference was held in 2010 on the assessment and treatment of pain in the emergency setting. This report is the Italian Intersociety recommendations on pain management in the emergency department setting., Results: The list of level A recommendations includes: 1) use of IV acetaminophen for opioid sparing properties and reduction of opioid related adverse events; 2) ketamine-midazolam combination preferred over fentanyl-midazolam fentanyl-propofol in pediatric patients; 3) boluses of ketamine IV (particularly in the population under the age of 2 years and over the age of 13) can lead to impairment of the upper airways, including the onset of laryngospasm, requiring specific expertise and skills for administration; 4) the use of ketamine increases the potential risk of psychomotor agitation, which can happen in up to 30% of adult patients (this peculiar side effect can be significantly reduced by concomitant systemic use of benzodiazepines); 5) for shoulder dislocations and fractures of the upper limbs, the performance of brachial plexus block reduces the time spent in ED compared to sedation; 6) pain relief and the use of opioids in patients with acute abdominal pain do not increase the risk of error in the diagnostic and therapeutic pathway in adults; 7) in newborns, the administration of sucrose reduces behavioural responses to blood sampling from a heel puncture; 8) in newborns, breastfeeding or formula feeding during the procedure reduces the measures of distress; 9) in pediatric patients, non-pharmacological techniques such as distraction, hypnosis and cognitive-behavioural interventions reduce procedural pain caused by the use of needles; 10) in pediatric patients, preventive application of eutectic mixtures of prilocaine and lidocaine allows arterial and venous samples to be taken in optimum conditions; 11) in pediatric patients, the combination of hypnotics (midazolam) and N2O is effective for procedural pain, but may be accompanied by loss of consciousness., Conclusion: The diagnostic-therapeutic pathway of pain management in emergency should be implemented, through further interdisciplinary trials, in order to improve the EBM level of specific guidelines.

Chromosomal rearrangements have a central role in the pathogenesis of human cancers and often result in the expression of therapeutically actionable gene fusions. A recently discovered example is a fusion between the genes echinoderm microtubule-associated protein like 4 (EML4) and anaplastic lymphoma kinase (ALK), generated by an inversion on the short arm of chromosome 2: inv(2)(p21p23). The EML4-ALK oncogene is detected in a subset of human non-small cell lung cancers (NSCLC) and is clinically relevant because it confers sensitivity to ALK inhibitors. Despite their importance, modelling such genetic events in mice has proven challenging and requires complex manipulation of the germ line. Here we describe an efficient method to induce specific chromosomal rearrangements in vivo using viral-mediated delivery of the CRISPR/Cas9 system to somatic cells of adult animals. We apply it to generate a mouse model of Eml4-Alk-driven lung cancer. The resulting tumours invariably harbour the Eml4-Alk inversion, express the Eml4-Alk fusion gene, display histopathological and molecular features typical of ALK(+) human NSCLCs, and respond to treatment with ALK inhibitors. The general strategy described here substantially expands our ability to model human cancers in mice and potentially in other organisms.

Objectives: The aim of this study was to compare bright blood high spatial resolution (HR) coronary magnetic resonance angiography (MRA) with low spatial resolution (LR) bright blood coronary MRA at 7 T., Materials and Methods: Twenty-four healthy volunteers underwent navigator-gated 3-dimensional imaging of the right coronary artery at 7 T using 2 sequences: HR bright blood and LR bright blood. Image postprocessing involved newly developed multiplanar reformatting to straighten the right coronary artery. Image quality was determined by vessel edge sharpness, signal-to-noise ratio, contrast-to-noise ratio, visible vessel length, and vessel diameter., Results: Vessel edge sharpness was statistically significantly higher in HR as compared with LR (0.57 ± 0.1 vs 0.46 ± 0.06; P < 0.001), at the cost of lower signal-to-noise ratio (HR, 32.9 ± 11.0 vs LR, 112.5 ± 48.9; P < 0.001) and contrast-to-noise ratio (HR, 17.9 ± 7.4 vs LR, 50.5 ± 26.1; P < 0.001). Visible vessel length and vessel diameter were similar for both sequences (P > 0.05)., Conclusions: High spatial resolution bright blood coronary MRA at 7 T is feasible and improves vessel edge sharpness as compared with LR bright blood imaging.

Pain in terminally ill patients with cancer can be often hard to manage, due to the unpredictable kinetics of drugs caused by progressive kidney and liver dysfunction. Plasma concentrations of active metabolites-also a cause of dangerous side effects--could be difficult to estimate. This case report holds the idea that buprenorphine, a partial agonist of m-receptors, even at high dosage, may be effective and safe to use in terminally ill patients with significant liver and kidney impairment.

The authors evaluated the performance of the MycArray™ Yeast ID (Myconostica Ltd, UK) assay in the identification of a total of 88 yeast isolates recovered in culture as compared to that obtained through routine methods. The turn-around time for species identification directly from cultures by the MycArray was 6 hours, much quicker than classical methods and all yeasts were correctly identified. In two cases a double identification including Saccharomyces cerevisiae was noted, but it was not confirmed by culture. The results show that MycArray Yeast ID can be a potential tool for rapid detection and identification of Candida species.

Overwhelming experimental evidence accumulated over the past decade indicates that microRNAs (miRNAs) are key regulators of gene expression in animals and plants and play important roles in development, homeostasis, and disease. The miR-17-92 family of miRNA clusters is composed of 3 related, highly conserved, polycistronic miRNA genes that collectively encode for a total of 15 miRNAs. We discuss recent studies demonstrating that these miRNAs are essential for vertebrate development and homeostasis. We also show how their mutation or deregulation contributes to the pathogenesis of a variety of human diseases, including cancer and congenital developmental defects. Finally, we discuss the current evidence suggesting how the different miRNAs encoded by these 3 clusters can functionally cooperate to fine-tune signaling and developmental pathways.

Background: Strategies to restore sinus rhythm in patients with atrial fibrillation (AF) lasting less than 48 h with haemodynamic stability remain controversial. The aim of this study was to test the hypothesis that electrical cardioversion (EC) would be more effective and safer in converting acute AF to sinus rhythm, compared with intravenous propafenone treatment., Methods: In the emergency department (ED) of Valduce Hospital, a single-centre randomised trial was conducted to compare EC with pharmacological cardioversion (PC) to restore the sinus rhythm in selected patients with acute AF. A total of 247 patients was enrolled (121 in the EC group and 126 in the PC group)., Results: EC was more successful than PC in restoring sinus rhythm. Successful cardioversion was achieved in 108 out of 121 patients in the EC group (89.3%) and 93 out of 126 patients in the PC group (73.8%) (HR in the EC group, 0.34; 95% CI 0.17 to 0.68; p=0.02). The time patients spent in the ED undergoing treatment was significantly lower in the EC group compared with the PC group (median (range), 180 (120-900) vs 420 (120-1400) min; p<0.001)., Conclusions: EC was more effective in patients with acute AF and resulted in a shorter length of stay in the ED than PC. Adverse events were small in number and transient in both groups of patients. Clinical trials registration number NCT00933634.

Background: Diabetic patients have increased interstitial myocardial fibrosis on histological examination. Magnetic resonance imaging (MRI) T(1) mapping is a previously validated imaging technique that can quantify the burden of global and regional interstitial fibrosis. However, the association between MRI T(1) mapping and subtle left ventricular (LV) dysfunction in diabetic patients is unknown., Methods and Results: Fifty diabetic patients with normal LV ejection fraction (EF) and no underlying coronary artery disease or regional macroscopic scar on MRI delayed enhancement were prospectively recruited. Diabetic patients were compared with 19 healthy controls who were frequency matched in age, sex and body mass index. There were no significant differences in mean LV end-diastolic volume index, end-systolic volume index and LVEF between diabetic patients and healthy controls. Diabetic patients had significantly shorter global contrast-enhanced myocardial T(1) time (425±72 ms vs. 504±34 ms, P<0.001). There was no correlation between global contrast-enhanced myocardial T(1) time and LVEF (r=0.14, P=0.32) in the diabetic patients. However, there was good correlation between global contrast-enhanced myocardial T(1) time and global longitudinal strain (r=-0.73, P<0.001). Global contrast-enhanced myocardial T(1) time was the strongest independent determinant of global longitudinal strain on multivariate analysis (standardized β=-0.626, P<0.001). Similarly, there was good correlation between global contrast-enhanced myocardial T(1) time and septal E' (r=0.54, P<0.001). Global contrast-enhanced myocardial T(1) time was also the strongest independent determinant of septal E' (standardized β=0.432, P<0.001)., Conclusions: A shorter global contrast-enhanced myocardial T(1) time was associated with more impaired longitudinal myocardial systolic and diastolic function in diabetic patients.

MicroRNAs belonging to the miR-34 family have been proposed as critical modulators of the p53 pathway and potential tumor suppressors in human cancers. To formally test these hypotheses, we have generated mice carrying targeted deletion of all three members of this microRNA family. We show that complete inactivation of miR-34 function is compatible with normal development in mice. Surprisingly, p53 function appears to be intact in miR-34-deficient cells and tissues. Although loss of miR-34 expression leads to a slight increase in cellular proliferation in vitro, it does not impair p53-induced cell cycle arrest or apoptosis. Furthermore, in contrast to p53-deficient mice, miR-34-deficient animals do not display increased susceptibility to spontaneous, irradiation-induced, or c-Myc-initiated tumorigenesis. We also show that expression of members of the miR-34 family is particularly high in the testes, lungs, and brains of mice and that it is largely p53-independent in these tissues. These findings indicate that miR-34 plays a redundant function in the p53 pathway and suggest additional p53-independent functions for this family of miRNAs., Competing Interests: The authors have declared that no competing interests exist.

The BlrB protein from Rhodobacter sphaeroides is a small 136 amino acid photoreceptor belonging to the BLUF family of blue light receptors. It contains merely the conserved BLUF fold responsible for binding the flavin pigment and a short C-terminal extension of unknown function. We investigated the primary photoreactions of BlrB by picosecond fluorescence and transient absorption spectroscopy. After excitation of the flavin the fluorescence decays in an H/D isotope independent manner with time constants of 21 and 390 ps, indicating a BLUF characteristic heterogeneous excited state quenched by electron transfer. By transient absorption spectroscopy, we observed a rapid relaxation of a vibrationally hot excited state within 6 ps upon excitation at 400 nm. The relaxed excited state evolves biexponentially with 18 ps (27%) and 216 ps (73%) into the signaling state spectrum indicated by a growing absorptive feature at 492 nm. Additionally, a broad triplet feature is observed as residual absorbance at a delay of 5 ns, which we attribute to derive from a significant fraction of free flavin in the sample. The photochemistry of BlrB is similar to other small BLUF proteins in respect to the fast formation of the photoproduct but does not resolve any further intermediates. We compare the photoreaction with other BLUF proteins on the basis of available spectroscopic data and crystal structures. An arginine close to the C2═O carbonyl of the flavin is likely to be a key determinant for the fast electron transfer in BlrB. Additionally, the orientation of the electron-donating tyrosine in respect to the flavin might play a role in the so far unique kinetic separation of the semiquinonic intermediates in Slr1694.
