PURPOSEAlthough financial hardship in childhood cancer contributes to poor outcomes, no standardized tool to assess its impact exists. Existing methods are heterogeneous and designed using high-income country (HIC), adult perspectives. This project aimed to construct a stakeholder-informed conceptual framework of financial hardship in childhood cancer with global relevancy.METHODSGroup concept mapping, a participatory mixed-methods approach, was used. Participants were parents or caregivers of a child with cancer, childhood cancer survivors, and clinical or nonclinical support personnel, fluent in English, Spanish, or Portuguese. A representative panel established a comprehensive list of relevant items. Participants individually sorted these items into concepts and then rated each item for impact using a four-point Likert scale. Multidimensional scaling and hierarchical cluster analysis identified concepts. Descriptive statistics were calculated for impact ratings.RESULTSOne fourth (21/80) of participants were parents/caregivers or childhood cancer survivors. Participants worked in clinical (44/80), charity/volunteer (27/80), and other nonclinical (13/80) roles. Of the 41 represented countries, 78.0% (32) were low- and middle-income countries (LMICs). Conceptual themes spanned six distinct clusters: medical, nonmedical, assistance and support, treatment impact, family impact, and caregiver impact. These were distinct in composition compared with an existing framework for adult oncology. Caregiver impact (mean, 3.39) and treatment impact (mean, 3.29) were the highest rated clusters, and impact ratings were higher in LMICs compared with HICs.CONCLUSIONWe developed a framework for financial hardship in childhood cancer that reflects the voices of stakeholders, including parents and caregivers, from diverse global contexts. The findings lay a foundation for the development and validation of tools to systematically assess financial hardship in families of children with cancer and inform effective policies and interventions.

In this work, methanol extract of Murraya koeginii stem has been studied as an ecofriendly inhibitor against mild steel corrosion in 1 M hydrochloric acid medium. Ultraviolet–visible and Fourier transform infrared spectroscopic tools were employed to characterize the extract. Corrosion inhibition efficacy of the extract against mild steel was studied in terms of weight loss method and electrochemical measurement method. Weight loss measurement shows the inhibition efficiency of 94.95 % for the first 6 h of immersion time and maximum efficiency of 96.39 % for 24 h immersion time using 1000 ppm inhibitor solution. Electrochemical measurement shows that for the same concentration of inhibitor, inhibition efficiency is 81.54 % in as-immersed condition whereas it is 85.76 % in 1 h immersed condition. Free energy of the adsorption reflected the physisorption followed by chemisorption. Similarly, thermodynamic parameters revealed the feasibility and spontaneity of the adsorption. Obtained results showed that the methanol extract of Murraya koeginii stem can be a potential corrosion inhibitor.

PURPOSEStigma is an understudied barrier to health care acceptance in pediatric oncology. We sought to explore the stigma experience, including its impact on cancer treatment decision making, and identify strategies to mitigate stigma for patients with osteosarcoma and retinoblastoma in Guatemala, Jordan, and Zimbabwe.METHODSParticipants included caregivers, adolescent patients (age 12-19 years), and health care clinicians. A semistructured interview guide based on The Health Stigma and Discrimination Framework (HSDF) was adapted for use at each site. Interviews were conducted in English, Spanish, Arabic, or Shona, audio-recorded, translated, and transcribed. Thematic analysis focused on stigma practices, experiences, outcomes, drivers, mitigators, and interventions.RESULTSWe conducted 56 interviews (28 caregivers, 19 health care clinicians, nine patients; 20 in Guatemala, 21 in Jordan, 15 in Zimbabwe). Major themes were organized into categories used to adapt the HSDF to global pediatric cancer care. Themes were described similarly across all sites, ages, and diagnoses, with specific cultural nuances noted. Pediatric cancer stigma was depicted as an isolating and emotional experience beginning at diagnosis and including internalized and associative stigma. Stigma affected decision making and contributed to negative outcomes including delayed diagnosis, treatment abandonment, regret, and psychosocial fragility. Overcoming stigma led to positive outcomes including resilience, treatment adherence, pride, and advocacy. Identified stigma drivers and mitigators were linked to potential interventions.CONCLUSIONParticipants describe a shared stigma experience that transcends geography, cultural context, age, and diagnosis. Stigma manifestations have the potential to impact medical decision making and affect long-term psychological outcomes. Stigma assessment tools and interventions aimed at stigma mitigation including educational initiatives and support groups specific to pediatric cancer should be the focus of future research.

PURPOSEOne determinant of the paucity of data on childhood cancer in low- and middle-income countries is the lack of capacity to register these cases. Combining expertise of the Global Initiative for Cancer Registry Development (GICR) and St Jude Global, we developed a ChildGICR educational program to promote data production.METHODSWe first conducted a needs assessment to identify priority educational topics. Then, we designed the ChildGICR Masterclass, in which individuals with the potential to lead pediatric cancer registration were supported to prepare standard educational material. The outcomes were evaluated using qualitative and quantitative measures.RESULTSOn the basis of indications by 38 GICR collaborators, we identified seven topics relevant to childhood cancer: burden description, registration principles, tumor classification, tumor staging, data quality control, data analysis, and data use. The ChildGICR Masterclass was held online in 2021 over 12 weeks. The 22 nominated participants created presentations in working groups and during live sessions. They also designed future training courses tailored to the needs of their region. Nineteen participants viewed the course experience as excellent, and 20 would continue engagement in the ChildGICR training activities. The developed material was 89% useful according to the faculty of the three online end courses, taught to 88 participants in 16 countries in 2022 and 2023. Among the 75 responding participants, 72 agreed that the learning objectives were attained and 60 were keen to engage in childhood cancer registration activities.CONCLUSIONThe ChildGICR Masterclass participants laid the foundation for a network of trainers. Knowledge dissemination in childhood cancer registration is the first necessary step toward evidence-based cancer control. The ChildGICR Masterclass can serve as a model to design, plan, and implement educational programs for health care professionals.

Introduction: Partially controlled acute pain after abdominal surgery is associated with a variety of unwanted postoperative consequences, like respiratory complications, delirium, myocardial ischaemia, prolonged hospital stays, and chronic pain later on. A good postoperative recovery depends greatly on a proper analgesic regimen. While epidural analgesia has been used to provide postsurgical abdominal pain relief, peripheral nerve blockade is a good alternative. Aim: To compare the analgesic efficacy of Transverse Abdominis Plane (TAP) block and Epidural block in patients undergoing nephrectomy. Materials and Methods: In this single-blinded parallel-group randomised controlled study was conducted in the Department of Urology at the Institute f Postgraduate Medical Education and Research in Kolkata, West Bengal, India from November 2021 to October 2022. A total of 78 patients (18-65 years) with American Society of Anaesthesiologists (ASA) Grade-I and II were randomly assigned to Group-EA (Epidural) and Group-TA (TAP Block). Group-EA received 11 mL of 0.125% bupivacaine, and Group-TA received 11 ml of 0.125% bupivacaine, both given at an 8-hour interval for 24 hours. The primary outcome was to compare postoperative pain using the Visual Analogue Scale (VAS) score at 1, 8, 12, and 24 hours after surgery. Secondary outcome measures included assessing motor function using the Bromage Score, monitoring haemodynamic parameters (pulse rate, mean arterial pressure), and evaluating Postoperative Nausea and Vomiting (PONV). A p-value 0.05). The pain scores between the EA and TA groups at 1, 8, 12, and 24 hours (0.10 vs. 0.08, 0.74 vs. 0.82, 1.00 vs. 1.31, 1.80 vs. 1.92) showed no significant difference at the specified times. However, the comparison of the mean Bromage scores in the EA and TA groups at the same time intervals (1 vs. 1.15, 1.08 vs. 2.51, 1.97 vs. 3.21, 1.97 vs. 4.74) revealed a significantly higher value in the TAP block group compared to the Epidural block group. The Epidural group had significantly lower blood pressure and pulse rate but experienced more PONV (15.4 vs. 2.6) compared to the TAP block group. Conclusion: Epidural block resulted in hypotension, bradycardia, shivering, and PONV as side-effects, which were negligible in the TAP block group. However, postoperative analgesia was quite comparable between the two groups. Postoperative pain, as assessed by VAS score, changed significantly in the TAP block group (intragroup p-value=0.0063), whereas it remained constant in the epidural group (intragroup p-value 0.094), and the difference between the two groups was statistically insignificant. The occurrence of hypotension, bradycardia, and PONV was significant in the epidural group, whereas postoperative mobility was better in the TAP group.

Shared sanitation services continue to expand in India's growing cities, yet, these facilities are blighted by issues including lack of privacy, overflow of raw sewage, and notably, a lack of water supply. Access to water in shared toilets for menstruating women and girls to wash themselves, their reusable menstrual products, their stained clothes, and sanitation infrastructure is a critical but all too-often neglected aspect of menstrual hygiene management (MHM). Drawing on the existing literature and data from Maharashtra, this commentary article uses the example of urban India to highlight the ‘need of the hour’, the provision of water in shared sanitation facilities in the global South for MHM as and when required. Evidence-based water supply norms in shared sanitation for MHM, from menarche to the perimenopause, are key, to highlight the need for water as an ‘MHM plus’ requirement for women and girls reliant on shared facilities. Connecting shared toilets to piped water is a long-term process, but the provision of water in buckets is a start. Hygiene promotion can create awareness of the importance of water for sustainable MHM. Monitoring and evaluation can determine the impact of supplying water in shared toilets on MHM. HIGHLIGHTS Water supply is needed for menstrual hygiene management (MHM).; The supply of water for MHM is neglected in shared toilets.; Providing water in shared toilets can improve MHM and reproductive health, support women and girls dependent on community and public toilets, encourage sustainable MHM, and reduce menstrual waste.; Evidence-based water supply norms for MHM should support policymaking to provide water supply in shared sanitation services.;

Abstract Background A 7.8 R scale earthquake hit Nepal in April 2015 and caused about 9000 deaths along with damage to infrastructure, including the water and sewage system. Bhaktapur was one of the highly affected districts. A typhoid vaccination campaign (pre-emptive) was carried out among children who were living in the temporary shelters in this district. The assessment of vaccine effectiveness after a pre-emptive typhoid vaccine campaign following an earthquake has previously not been attempted in Nepal. Objective To describe the pre-emptive typhoid Vi capsular polysaccharide vaccination campaign and an evaluation of the vaccine effectiveness. Methods We conducted a pre-emptive typhoid Vi capsular polysaccharide vaccination campaign among children between 2 and 15 years of age dwelling in 23 temporary shelters in Bhaktapur district after the earthquake. Surveillance of clinical typhoid was carried out from 2014 to 2017 in Siddhi Memorial Hospital, the only hospital for children in the district. We calculated vaccine effectiveness using a case–control study design (clinical typhoid as cases and chest x-ray confirmed pneumonia as controls). Results Three thousand nine hundred sixteen children of age 2–15 years residing in the 23 temporary shelters in Bhaktapur received the typhoid Vi capsular polysaccharide vaccine between July and December 2015. 2193 children of age 2–15 years were admitted to the hospital during the study period and 260 (11.9%) were diagnosed with clinical typhoid. The numbers of children admitted with clinical typhoid decreased over the study period (105 in 2014 and 47 in 2017; P = 0.001). Overall vaccine effectiveness was calculated at 52% (95% CI −46 to 85%), and it was 87% (95% CI −25 to 99) among children less than 5 years of age. Conclusions We successfully conducted a pre-emptive vaccination campaign against typhoid after the 2015 Nepal earthquake. The pre-emptive vaccination campaign appeared to be more effective among children less than 5 years of age. Further studies are needed to assess the effectiveness of pre-emptive use of typhoid vaccines in the emergency situations. We highlight the challenges of calculating vaccine effectiveness of a typhoid vaccine in an emergency setting.

Background: Greater palatine foramen (GPF) transmits important neurovascular structures which may get injured during different operative procedures such as intraoral maxillary nerve block, cleft palate repair, and operative procedures involving maxillary sinus and maxillary molars. Hence, proper knowledge about the GPF is very essential. The aim of this study was to study the shape of GPF and its location in relation to molar tooth, measure the diameters of GPF, and measure the distances of GPF from different landmarks of palate. Methodology: A dry bone-based study was conducted on dry human skulls of Eastern Indian population using digital Vernier caliper. At first location of GPF in relation to maxillary molars was noted, followed by anteroposterior (AP) and transverse diameter of GPF and distance of GPF from incisive fossa, posterior border of hard palate (PBHP) and interpalatine suture was measured. Results: In 69.64% of cases, GPF was present medial to the maxillary molar tooth and most common shape is oval type which is found in 44.64% of cases. AP diameter was almost same in both the sides which is 5.13 mm in average and transverse diameter is 2.91 mm. The mean distance of GPF from incisive fossa, PBHP, and interpalatine suture (IPS) is as follows in sequence, 37.24 mm, 3.21 mm, and 13.68 mm. Conclusion: The findings from the present study can be used as a guide for locating GPF during different operative procedures such as palatal reconstruction in cleft palate, maxillary surgeries, and intraoral maxillary nerve block. GPF is located about 3 mm in front of PBHP, most commonly medial to the third maxillary molar, about 15 mm lateral to IPS in the present study among Eastern Indian population.

Abstract Understanding the efficacy of SARS-CoV-2 vaccination in people on immunosuppressive drugs, including those with rheumatoid arthritis (RA), is critical for their protection. Vaccine induced protection requires antibodies, CD4+ T cells, and CD8+ T cells, but it is unclear if these are equally affected by immunomodulatory drugs. Here, we determined how humoral and cellular SARS-CoV-2 vaccination responses differed between people with RA and controls, and which drug classes impacted these responses. Blood was collected from participants with RA on immunomodulatory drugs and controls after their second, third, and fourth SARS-CoV-2 vaccinations. Receptor binding domain (RBD)-specific antibodies were quantified by ELISA. Spike-specific memory T cells were quantitated using flow cytometry. Linear mixed models assessed the impact of age, sex, and immunomodulatory drug classes on SARS-CoV-2 vaccination responses. Compared to non-RA controls (n = 35), participants with RA on immunomodulatory drugs (n = 62) had lower anti-RBD IgG and spike-specific CD4+ T cell levels, but no deficits in spike-specific CD8+ T cells, following SARS-CoV-2 vaccination. Use of costimulation inhibitors was associated with lower humoral responses. JAK inhibitors were associated with fewer spike-specific CD4+ T cells. Participants with RA on immunomodulatory drugs mounted weaker responses to SARS-CoV-2 vaccination, with different drug classes impacting the cellular and humoral compartments.

Introduction: Timely diagnosis of patients affected by an emerging infectious disease plays a crucial role in treating patients and avoiding disease spread. In prior research, we developed an approach by using machine learning (ML) algorithms to predict serious acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection based on clinical features of patients visiting an emergency department (ED) during the early coronavirus 2019 (COVID-19) pandemic. In this study, we aimed to externally validate this approach within a distinct ED population. Methods: To create our training/validation cohort (model development) we collected data retrospectively from suspected COVID-19 patients at a US ED from February 23–May 12, 2020. Another dataset was collected as an external validation (testing) cohort from an ED in another country from May 12–June 15, 2021. Clinical features including patient demographics and triage information were used to train and test the models. The primary outcome was the confirmed diagnosis of COVID-19, defined as a positive reverse transcription polymerase chain reaction test result for SARS-CoV-2. We employed three different ML algorithms, including gradient boosting, random forest, and extra trees classifiers, to construct the predictive model. The predictive performances were evaluated with the area under the receiver operating characteristic curve (AUC) in the testing cohort. Results: In total, 580 and 946 ED patients were included in the training and testing cohorts, respectively. Of them, 98 (16.9%) and 180 (19.0%) were diagnosed with COVID-19. All the constructed ML models showed acceptable discrimination, as indicated by the AUC. Among them, random forest (0.785, 95% confidence interval [CI] 0.747–0.822) performed better than gradient boosting (0.774, 95% CI 0.739–0.811) and extra trees classifier (0.72, 95% CI 0.677–0.762). There was no significant difference between the constructed models. Conclusion: Our study validates the use of ML for predicting COVID-19 in the ED and demonstrates its potential for predicting emerging infectious diseases based on models built by clinical features with temporal and spatial heterogeneity. This approach holds promise for scenarios where effective diagnostic tools for an emerging infectious disease may be lacking in the future.

The poor translatability between preclinical and clinical drug trials has limited pro-cognitive therapeutic development. Future pro-cognitive drug trials should use translatable cross-species cognitive tasks with biomarkers (1) relevant to specific cognitive constructs, and (2) sensitive to drug treatment. Here, we used a difficulty-modulated variant of a cross-species cognitive control task with simultaneous electroencephalography (EEG) to identify neurophysiological biomarkers sensitive to the pro-cognitive effects of dextroamphetamine (d-amp) (10 or 20 mg) in healthy adults (n = 23), in a randomized, placebo-controlled, counterbalanced, double blind, within-subject study, conducted across three test days each separated by one week. D-amp boosted d-prime, sped reaction time, and increased frontal P3a amplitude to non-target correct rejections independent of task difficulty. Task difficulty did however, moderate d-amp effects on EEG during target performance. D-amp suppressed frontal theta power during easy target responses which negatively correlated with drug-induced improvement in hit rate while d-amp-induced changes in P3b amplitude during hard target trials strongly correlated with drug-induced improvement in hit rate. In summary, d-amp affected both behavioral and neurophysiological measures of cognitive control elements. Under low-demand, d-amp diminished cognitive control by suppressing theta, yet under high-demand it boosted control in concert with higher P3b amplitudes. These findings thus appear to reflect a gain-sharpening effect of d-amp: during high-demand processes were boosted while during low-demand processes were neglected. Future studies will use these neurophysiological measures of cognitive control as biomarkers to predict d-amp sensitivity in people with cognitive control deficits, including schizophrenia.

The bench-to-bedside development of pro-cognitive therapeutics for psychiatric disorders has been mired by translational failures. This is, in part, due to the absence of pharmacologically sensitive cognitive biomarkers common to humans and rodents. Here, we describe a cross-species translational marker of reward processing that is sensitive to the aminergic agonist, d-amphetamine. Motivated by human electroencephalographic (EEG) findings, we recently reported that frontal midline delta-band power is an electrophysiological biomarker of reward surprise in humans and in mice. In the current series of experiments, we determined the impact of parametric doses of d-amphetamine on this reward-related EEG response from humans (n = 23) and mice (n = 28) performing a probabilistic learning task. In humans, d-amphetamine (placebo, 10 mg, 20 mg) boosted the Reward Positivity event-related potential (ERP) component as well as the spectral delta-band representations of this signal. In mice, d-amphetamine (placebo, 0.1 mg/kg, 0.3 mg/kg, 1.0 mg/kg) boosted both reward and punishment ERP features, yet there was no modulation of spectral activities. In sum, the present results confirm the role of dopamine in the generation of the Reward Positivity in humans, and pave the way toward a pharmacologically valid biomarker of reward sensitivity across species.
