The careless activity of drivers in logistics transportation is a primary reason inside the vehicle during road accidents. This research aims to reduce the number of accidents caused by a failure of the driver in logistics transportation by incorporating an autonomous system. We propose a convolutional neural network -based architecture to recognize and classify different positions which cause road accidents. The proposed system is evaluated with the State Farm Distracted Driver Database, which included examples illustrating ten different driving positions like reaching behind and talking to the passenger, making up, safe driving, talking on the phone, clothing, checking right/left hand, right/left hand, and running the radio. The proposed approach has also been tested against recent algorithms and evaluated. Background/aim: The purpose of this study was to determine effect of age, sex, affected extremity, disability severity, treatment type, cerebrovascular accident (CVA) etiology, number of treatment sessions, and CVA duration on the functional improvement of the stroke patients who participated in a physical medicine and rehabilitation program. Materials and methods: The research sample consisted of 322 stroke patients. Clinical and demographic features including age, sex, affected extremity, disability severity, treatment type, CVA etiology, number of treatment sessions, and CVA duration were recorded. Functional status was evaluated retrospectively by using the functional independence measure (FIM) at admission and discharge. Results: It was detected that discharge FIM score of the patients exhibited an increase of significance level (p < 0.05). It was found that age, number of treatment sessions, CVA duration and FIM admission score were determinative parameters in FIM gain level (p < 0.05) while sex, affected extremity, and CVA etiology were not effective in FIM gain level (p > 0.05). Conclusion: Results show that functional improvement after rehabilitation was better in the younger ages, shorter CVA durations and moderate functional disturbances. Introduction: Vestibular migraine (VM) is a complex disease characterized by recurrent episodes of migraine associated with vertigo attacks that are observed in 1–3% of the general population. Given its high prevalence and the impact on the health system, it is important to characterize these patients, in order to offer an accurate diagnosis and a proper treatment. As the diagnosis of VM is based on clinical features, the study of potential biomarkers has gained more interest in the last years, to improve the precision in the diagnosis of this disease. The aim of this review is to summarize the main tests available for the diagnosis of VM, including the accuracy of biomarkers for the diagnosis of VM. Areas covered: This review summarizes the main information on VM, including all diagnosis records published in the field in the last 10 years, and focusing on candidate biomarkers for the diagnosis of VM patients. Expert opinion: There is a limited knowledge in the pathophysiology of VM. The search of biomarkers for diagnosis of VM is needed to improve the precision in the diagnosis promoting clinical and translational research. Implementation of industrial wastes such as bottom ash in ground improvement can be cost-effective and environment-friendly. Ground improvement is an effective method of mitigation to improve problematic soils including soft kaolin clay soils as the problematic soils always expose to the severe settlements, low shear strength, immoderate plasticity, greater compressibility, dispersivity, bulging, erodibility, and susceptibility to climatic variables. Several studies conducted on the granular column using the bottom ash column. However, only a few studies have reported findings coherent with the statistical analysis. In this study, the lateral load capacity of bottom ash column-kaolin clay has been conducted. Coherently, the reinforced kaolin clay samples were tested via particle size distribution, Atterberg limit test, relative density, compaction test, permeability test, unconfined compression test, and unconsolidated undrained triaxial test with the single and group of encapsulated bottom ash columns with the geotextile encasement and a prediction model was developed. The effect of a number of columns, column diameter, column height, area replacement ratio, height penetration ratio, height-diameter column ratio, volume replacement ratio, and confining pressures on the shear strength of the single and group of encapsulated bottom ash columns have been investigated. The findings showed the effectiveness of using the bottom ash columns at various number of column, column diameter, column height, area replacement ratio, height penetration ratio, height-diameter column ratio, volume replacement ratio, and confining pressures can enhance the shear strength of the soil up to 77.00% at the optimal utilization of single encapsulated bottom ash column of 10-mm diameter and 80-mm height. In this study, the influence of artificial aging at various temperatures and annealing on the mechanical characteristics of the Al-6.2Zn2.3Mg2.3Cu alloy composites bearing various amounts of TiB2 particulates was investigated. Initially, the composites were produced with the cold pressing aid at 316MPa and subsequently sintering at 590°C. The composites were subjected to artificial aging at 100, 120, and 140°C for 100 hours and the annealing. The microstructural observations were done using the SEM, EDX, and XRD analyses to clarify the materials' phases. Next, the hardness and three-point bending tests of the investigated materials were done. All samples reached their highest hardness values after the artificial aging at 120°C for about 11 h. Besides, the bending strength level of Al-6.2Zn2.3Mg2.3Cu/TiB2p composites was found to be lower than that of the Al-6.2Zn2.3Mg2.3Cu alloy. [ABSTRACT FROM AUTHOR]

Objective: The study aimed to compare state and trait anxiety, blood pressure, and heart rate of patients with and without coronary angiography experience who undergo coronary angiography intervention. Methodology: This study is cross-sectional comparative research. A total of 160 patients, including 80 patients undergo for the first time coronary angiography and 80 patients with at least one experience of coronary angiography were included in the sample of the study. In the study, data were collected using 'Patient Information Form' and 'State-Trait Anxiety Inventory', blood pressure measurement device, and pulse oximeter. Results: The mean of state anxiety scores of the patients with and without CA experience were 39.35±5.31 and 39.98±4.04, respectively; (p=0.395). The mean of trait anxiety scores of the patients with and without CA experience were 44.73±6.84 and 44.51±6.05, respectively (p=0.826). There was no statistically significant difference observed in state and trait anxiety between two groups. Before the CA procedure, the systolic and diastolic blood pressure average of the patients with CA experience were statistically higher than those without CA experience (p<0.05). Conclusion: The anxiety levels of the patients before the CA were observed to be similar and moderate, regardless of their CA experience. The diagnosis of Familial Mediterranean fever (FMF) based on clinical findings supported by genetic mutation. Recently, the new Eurofever/PRINTO classification criteria including genetic analysis were established. The aim of this study is to evaluate the performance of the new criteria in FMF patients with a single exon 10 mutation in childhood. The study group consisted of FMF patients who had a single exon 10 mutation in a referral center in Turkey. Patients with periodic fever, aphthous stomatitis, pharyngitis and adenitis (PFAPA) syndrome were included as a control group. The medical charts of all patients were reviewed retrospectively. A total of 106 FMF patients (59 boys) were enrolled in the study group. The median age at first symptom was 5; the median age at diagnosis was 7 years. The mean follow-up was 33 ± 35.4 months. Majority of the patients (n = 58, 54.7%) had heterozygous M694V, 16 (15%) patients had M694V/E148Q and 13 (13.8%) patients had heterozygous M680I mutation. The sensitivity of the Yalcinkaya-Ozen criteria was 98.1% and it was 97.1% for the Eurofever/PRINTO classification criteria. The specificity of the new Eurofever/PRINTO classification criteria was 96.7% and it was 74.1% for the Yalcinkaya-Ozen criteria. The new Eurofever/PRINTO classification criteria have a good sensitivity as the Yalcinkaya-Ozen criteria in patients with a single exon 10 mutation. Additionaly, the new criteria have better specificity. It should be useful to apply the clinical only criteria where the carrier rate is high. [ABSTRACT FROM AUTHOR]

In the mid-2010s NATO allies were facing the resurgence of a Russian threat at their borders, as well as terrorist actions in Europe and the MENA region. This evolving security environment provoked heated talks both within and outside NATO on its adaptation, often depicted as being the sign of irreconcilable disagreements. Conversely, this article argues that the "NATO 360-degree" concept adopted during the Warsaw Summit shows cohesion between the allies thanks to the Alliance's decision-making process. As a security community, member states were incentivised to find common grounds despite their diverging interests, which resulted in this new concept encompassing the "arc of insecurity". Its subsequent implementation also confirms the cohesion hypothesis, despite its obvious refocusing towards the East and collective defence. This article will first present the diverging threat perceptions within the Alliance. It will then focus on the implementation of the "NATO 360-degree" concept, promoted during the Warsaw summit, to finish with an initial assessment of the changes at work. [ABSTRACT FROM AUTHOR]

Children with Coronavirus disease 2019 (COVID-19) are being reported to have manifestations of hyperinflammatory states and/or Kawasaki-like disease. In this study, we investigated children with typical and atypical Kawasaki disease (KD) likely to be associated with COVID-19. We have reported four children with Kawasaki-like disease probably associated with COVID-19. The clinical features were consistent with incomplete KD in three patients. SARS-CoV-2 RT-PCR was positive in one and the serology was positive in one patient with negative RT-PCR. Corticosteroids, anakinra, intravenous immunoglobulin (IVIG), and acetylsalicylic acid were used in the treatment. Three patients recovered after the treatment while one patient died. The literature review revealed 36 articles describing 320 children with Kawasaki-like disease associated with COVID-19. SARS-CoV-2 RT-PCR was negative in 120 (65.5%) of 183 patients while the serology was positive in 130 (83.8%) of 155 patients. The therapeutic options have included IVIG, acetylsalicylic acid, tocilizumab, anakinra, enoxaparin, and methylprednisolone. Pediatric COVID-19 cases may present with atypical/incomplete Kawasaki-like disease. Thus, pediatricians need to be aware of such atypical presentations resembling KD for early diagnosis of COVID-19. This study aim to detect the groundwater pollution connected to multiple effect such as the interaction of mineral/ore-water, mining destruction and agricultural activities in Kaman (Kırçehir). The study area has recently abondoned iron, antimony, gold and floride mine areas and so, there are a lot of mine piles. Mine waste is a rich source of hazardous trace elements to the environment. In many areas expecially abandoned sulphidic mine wastes may cause to produce acid mine drainage. Although no acid mine drainage is observed in the study area, the region needs to be reworked for this purpose in the following years. The electrical conductivity, pH and Eh values of the waters range between 126-1994 μS/cm; 7.28-8.58; 222- 649 mV respectively. In the rainy season, the pH values of the waters decreased due to the effect of slight acidic precipitation and in parallel with this decrease, EC, TDS, Ca, Na, SO4, HCO3, As, Sb, U concentrations of some waters increase due to the increase of solubility of elements. As, Sb, F, U, NO3, NH4 concentrations some of water exceed the maximum limit values given in the Turkish and World Health Organization Standards. Purpose: Some reports have described arterial hypertension (AH) in patients treated by serotonin reuptake inhibitor (SRI) or serotonin-norepinephrine reuptake inhibitor (SNRI) antidepressants. The mechanism remains discussed, some authors suggesting a role of SERT (SERotonin Transporter) inhibition whereas others discussing NET (NorEpinephrine Transporter) involvement. The present study used the pharmacoepidemiological-pharmacodynamic (PE-PD) method to investigate the role of these transporters in SRI- and SNRI-induced AH. Methods: The study involved two successive approaches: first, a PE study (disproportionality analysis) investigating in VigiBase®, the World Health Organization Individual Case safety Report (ICSR) database, the relationships between exposure to SRI AND SNRI, and reports of AH. The primary analysis compared patients receiving one SRI (or one SNRI) with non-users. Secondary analyses were performed according to the pharmacological classes. Results are expressed as reporting odds ratios (ROR) with 95% CI and information component (IC), an indicator for disproportionate Bayesian reporting. Second, we performed a PD study using linear regression analyses to explore the association between the AH signal and binding affinities for NET and SERT (expressed as their pKi ratio) of SRIs and SNRIs. Results: A significant ROR value was found for each individual SRI (except fluvoxamine) and each individual SNRI. ROR values were also significant for SRIs and SNRIs in general with higher values for SNRIs than for SRIs. Similar trends were found using IC. A significant correlation was found between the signal of AH and the NET/SERT pKi ratio (y = 6.57x – 2.55, R2 = 0.68, Pearson coefficient correlation = 0.82). Conclusion: The present study found a positive association between the NET/SERT pKi ratio and the occurrence of arterial hypertension with SRI and SNRI antidepressants. These results are important for the selection of antidepressants in hypertensive and/or at risk depressive patients as well as for future development of antidepressants devoid of hypertensive effect. Spatial orientation is achieved by integrating visual, vestibular and proprioceptive cues. Individuals that rely strongly upon visual cues to facilitate spatial orientation are termed visually dependent. Heightened visual reliance commonly occurs in patients following vestibular dysfunction and can influence clinical outcome. Additionally, psychological factors, including anxiety, are associated with poorer clinical outcome following vestibular dysfunction. Given that visual dependency measures are affected by psychological and contextual influences, such as time pressure, we investigated the interaction between time pressure and anxiety upon visual dependency in healthy controls and vestibular migraine patients. Visual dependency was assessed using a "Rod and Disk" task at baseline and under time pressure (3 s to complete the task). Non‐situational (trait) and situational (state) anxiety levels were quantified using the Spielberg State‐Trait Anxiety Inventory. We calculated the change in visual dependency (VD) [∆VD = VDtime pressure − VDbaseline] and correlated it with participants' trait anxiety scores. We observed a significant negative correlation between trait anxiety and the change in VD (R2 =.393, p <.001) in healthy controls and a positive correlation in dizzy patients (R2 =.317, p <.001). That is, healthy individuals that were more anxious became less visually dependent under time pressure (i.e., more accurate), whereas less anxious individuals became more visually dependent. The reverse was observed in vestibular migraine patients. Our results illustrate that anxiety can differentially modulate task performance during spatial orientation judgements under time pressure in healthy individuals and dizzy patients. It has recently become increasingly popular for small entrepreneurs in Dagestan to introduce elements of Islamic economy into their everyday economic practices. In my paper, I take a closer look at everyday life and the ways of conducting business among small entrepreneurs in Makhachkala, the capital of the Republic of Dagestan in the Russian Federation. In order to scrutinize the relationship between everyday religious observance, space and economic practices within a broader socio-political context, I introduce the term 'halal landscape'. 'Halal landscapes' emerge through the gradual infusion of Islam into the sphere of economic activities, where they form 'Islam-inspired' social spaces, in which economic and moral dimensions are interwoven with formal and informal norms and regulations, and where social life – the area of interaction between human and non-human actors – has its unique materiality and temporality. In my paper, I look into the halal landscapes of Dagestani entrepreneurs in Makhachkala and demonstrate the analytical potential of the term to study the social and cultural nature of Islam-inspired economic practices. Important elements of these halal landscapes include the avoidance of deception and usury, promotion of honesty and observance in the workplace, payment of zakat, as well as thorough knowledge about these issues. My analysis is based on the results of multi-temporal fieldwork conducted in Makhachkala in 2017–2019 as well as on earlier field observations gathered during regular visits in 2004–2016. Both Generation X and Y currently work at organizations and companies together. Responding to expectations of both these Generations with the same tools may satisfy one group but upset the other. Thus, if factors related to work stress and the job satisfaction are different for Generation X and Y as suggested by the theory, these differences will have to define being Generation X or Y. Based on this inference, the aim of the study is to research whether or not the level of the work stress and job satisfaction of Generation X and Y can accurately classify these two Generations. The study is a quantitative research. The scope of the research is the Head Office and Istanbul branches of Participation Banks. Three scales were used in the collection of data, namely being Generation X or Y, work stress and job satisfaction. The job satisfaction scale was the Minnesota Satisfaction Questionnaire created by Weiss et al., (1967). Being Generation X or Y and the work stress scale were created by benefiting from literature. Logistic Regression method was used in the analysis of the data. SPSS software was used for data analysis. A total of 392 survey data were analyzed to reach the findings. Research hypotheses are "perceived job satisfaction has a significant impact on the prediction of being designated as Generation X or Y" and "Perceived job stress has a significant impact on the prediction of being designated as Generation X or Y" According to the results of the research, perception levels of work stress and job satisfaction cannot predict to be Generation X or Y significantly. There is no relationship between job satisfaction and work stress factors with being Generation X and Y. These factors affect the X and Y generations similarly. [ABSTRACT FROM AUTHOR]

Objectives: Investigation of the association between epicardial adipose tissue thickness (EATT) and P-wave dispersion (Pd), QT dispersion (QTd), corrected QT dispersion (QTcd) and Tp-e interval in children with Type 1 Diabetes Mellitus (T1DM) was aimed. Methods: Forty-one children with T1DM and 41 age- and gender-matched healthy children were included in the study. Demographical characteristics of all cases were examined. In echocardiography; in addition to conventional echocardiographic measurements, end-systolic EATT was measured from right ventricular free wall. In electrocardiogram; Pd, QTd, QTcd and Tp-e interval durations, as well as Tp-e/QT and Tp-e/QTc ratios were calculated. Correlation values between EATT and electrocardiographic parameters were also noted. Results: Mean age of the patient group was determined to be 12.43 ± 3.04 years and that of the control group was determined to be 12.08 ± 2.56 years. There was no significant difference between the groups in regard to age, gender, body weight, height and body mass index. In the patient group; EATT, Pd, QTd, QTcd and Tp-e interval were determined to be significantly higher compared to the control group. In the patient group, no significant correlation was determined between EATT and Pd, QTd, QTcd and Tp-e. However, when both patient and control groups were evaluated together, a statistically significant positive correlation was determined between EATT and Pd, QTd, QTcd and Tp-e. Conclusions: In children with T1DM, an increase in epicardial adipose tissue thickness and in risk of cardiac arrhythmias has been demonstrated. Familial Mediterranean fever (FMF) is a hereditary auto-inflammatory disease with accompanying findings of amyloidosis and vasculitis. M694V is one of the most common mutations associated with amyloidosis. This study compared the macular optical coherence tomography angiography measurements in FMF patients who were genetically verified to carry the M694V mutation of the MEFV gene to those in healthy controls. The vessel densities (VDs) of superficial (SVP) and deep vascular plexus (DVP) of the retina, and choriocapillaris, foveal avascular zone (FAZ) perimetry, foveal VD 300µ around the FAZ (FD-300), acirculatory index (AI) and non-flow area were measured with optical coherence tomography angiography (OCT-A). The FMF and control groups were matched for age and gender. Compound heterozygous pathogenic variants were excluded. Thirty-eight FMF patients with M694V mutations (28 heterozygous and 10 homozygous) and 40 healthy controls were included. The two groups were similar with the regard to age and gender (P=0.88 and P=0.49, respectively). None of the investigated parameters, including the vessel densities of the SVP and DVP, and choriocapillaris, FAZ perimetry, FD-300, AI, and non-flow area showed a statistically significant difference between the FMF and control groups. The article examines how the epistemologies of Islam and modern science are reconciled in the writings of the contemporary Turkish Sunni Islamic scholar Fethullah Gülen (b. 1938), one of the once most influential yet vastly controversial religious leaders in contemporary Turkey. Through a close reading of his texts on science, the article analyzes how Gülen defines the scientific practice as an ethical act of reading the universe with heart and mind, and as a path in which one can fully grasp the magnificence of God. Framing with references to Sufism, Gülen also explains science as a domain where Islamic ethics can be practiced. The article shows that Gülen's project of Islamization of science is not limited to the domain of knowledge, but rather comprises a comprehensive cultural transformation led by a new epistemic approach to contest the secular hegemony in the country. Therefore, Gülen's politics of Islam at large should be understood as a project of transforming the episteme, or the hegemonic regime of knowledge, in society, from secular to Islamic. However, the article also notes, somehow paradoxically, in his harmonization of the Sufi notions with secular science, Gülen cannot escape from being captured by the very epistemology of the modern science. Aim: Increased intimal thickness in coronary arteries, extensive calcification, and atheromatous plaque that does not cause luminal irregularities in a significant portion of the patients with coronary slow flow (CSF). Arterial stiffness is an indicator for atherosclerosis. We aimed to investigate the relation between coronary slow flow phenomenon (CSFP) and arterial stiffness. Method: Total of 73 patients were included in the study, and a control group was formed with 64 individuals. Aortic stiffness index β (ASIβ) and pulse wave velocity (PWV) were used as the determinant of arterial stiffness in all analyses. Result: Pulse wave velocity values were significantly higher in the coronary slow flow group than the control group (P <.001). PWV, aortic stiffness index β (ASIβ) values were found to be significantly higher in the CSF group. ASIβ value was 3.4 ± 1.0 in CSF patients and 2.2 ± 0.6 in the control group (P <.001). Receiver operating characteristic curve (ROC) analysis showed that PWV predicted coronary slow flow with 97% sensitivity and 90% specificity for 7.15 cutoff value. And aortic stiffness index was found to predict coronary slow flow with 83% sensitivity and 75% specificity for 2.63 cutoff value. Conclusion: Our findings prove that coronary slow flow phenomenon should be considered a subgroup of coronary artery diseases and that increased PWV is an indicator of CSFP. [ABSTRACT FROM AUTHOR]

Objectives: Neuroinflammation represents one of the two major pathological components of multiple sclerosis (MS). The aim of our study was to find the role of the late pro-inflammatory cytokine HMGB1 (high mobility group box) in MS pathogenesis.Subjects and Methods: A total of 165 patients from three MS centers in Slovakia were enrolled in the study. Patients underwent a complex clinical investigation and their plasma levels of HMGB1 were analyzed by a sandwich ELISA test.Results: MS patients had 4.5 times higher plasma level of HMGB1 (median, 13.529 ng/mL; IQR = 2.330-113.45) than healthy controls (median, 2.999 ng/mL; IQR = 1.686-9.844; P < 0.0001). The concentrations of HMGB1 were significantly associated with increased number of affected areas diagnosed by MRI (P < 0.0001) (the median for one affected area, 4.205 ng/mL; median for five affected areas, 17.843 ng/mL; P < 0.05). Patients with at least one active lesion in any of the affected areas in the brain had significantly higher plasma levels of HMGB1 (median, 20.118 ng/mL; IQR, 3.693-100.12) than those without any active lesion (median, 16.695 ng/mL; IQR, 3.255-113.45; P < 0.0235). We found also a very highly significant association of HMGB1 plasma levels with clinical condition expressed as EDSS (expanded disability status scale) (P < 0.0001); patients with higher EDSS had higher levels of HMGB1 (EDSS ≤ 2.5, 11.648 ng/mL vs. EDSS ≥ 3, 17.549 ng/mL; P = 0.0115).Conclusion: Our results suggest chronic low-grade inflammation in MS patients that correlates with clinical conditions of MS patients, and for HMGB1 as a possible target molecule in future therapy. The case of Prof. İştar Gözaydın is one of the most visible and tragicomic examples for academics who have been victimized in Turkey by the Justice and Development Party (AKP) government. Gözaydın was not the first one and perhaps will not be the last because the authoritarian mindset that encapsulates the academics and scholars started long before the foundation of AKP, despite the fact that it was deepened and broadened by it. This article aims to explain the intense recrimination of academics by a repressive and hegemonic political power in Turkey in the second decade of the 2000s. Treatment of vestibular migraine currently lacks a firm scientific basis, as high quality randomized controlled trials are not available. Therefore, recommendations are largely borrowed from the migraine sphere. The first therapeutic step is explanation and reassurance. Many patients do not need pharmacological treatment, as attacks may be infrequent and tolerable. Acute attacks can be ameliorated in some patients with antiemetic drugs such as diphenhydramine, meclizine, and metoclopramide. Frequent attacks may warrant pharmacological prophylaxis with metoprolol, amitriptyline, topiramate, valproic acid, or flunarizine. Nonpharmacological measures including regular exercise, relaxation techniques, stress management, and biofeedback may be similarly effective and can be combined with a pharmacological approach. Objective To define the characteristics of children expressing the FMF phenotype under colchicine until it was ceased and to compare the clinical features of patients requiring colchicine again with the patients who did not need colchicine. Methods Sixty-four of 1786 children with FMF in whom colchicine was stopped by the physician or patients/parents were enrolled. These patients were grouped as children who were in need of colchicine due to attacks and/or elevated acute phase reactants after cessation of colchicine (group 1) and children in whom colchicine was not necessary and not restarted (group 2). Results Colchicine was stopped in 59.4% by the physician and in 40.6% by the patient/parents. It was ceased at a median of 10.6 years of age (range 2.1–20.5) and attack- and inflammation-free periods of 18.2 months (range 6–148). The median follow-up of 64 patients after colchicine cessation was 37.4 months (range 6.4–154.7). It was restarted in 17 patients due to attacks (n = 11) or elevated acute phase reactants (n = 6). The age at cessation of the colchicine was lower (P = 0.04) and the duration of colchicine treatment until its cessation was shorter (P = 0.007) in group 1 compared with group 2. Conclusion Life-long colchicine treatment may not be required in all FMF patients. There are no current guidelines to determine in which patients it is safe to stop colchicine. We found that younger age during cessation and shorter duration of colchicine treatment lead to a higher risk of needing to restart colchicine. [ABSTRACT FROM AUTHOR]

Objective: Familial Mediterranean fever (FMF) is a common, inherited, autosomal recessive inflammatory disease in children. The diagnosis of FMF is based on clinical features and positive family history supported with genetic testing. This study aimed to determine the frequency and distribution of Mediterranean fever (MEFV) gene alterations of a city in Northern Anatolia. Materials and Methods: We evaluated MEFV gene mutations in 374 children preliminary diagnosed as FMF by a commercial kit based on real-time polymerase chain reaction technique in a one-year period, and screened 12 mutations. Results: At least one mutation was detected in 213 patients (57%) and 38 genotypes with 1 1 distinct mutations.A total of 137 (64. 3%) of mutation-positive children were heterozygous, 45 (21. 1%) were compound heterozygous, and 2 (0.9%) were complex heterozygous; and 14 (6.4%) patients were homozygous, 6 (2.8%) were compound homozygous, and 3 (1.4%) were complex homozygous. With a frequency of 50.1%, R202Q was the most common mutation. Also, R202Q/M694V was the most common compound heterozygous genotype. In 43 alleles, R202Q-M694V mutations were found to be in linkage disequilibrium. In our cohort, M694V E148Q, V726A, and M680I (G/C) were other common mutations; whereas F479L, A744S, K695R, P369S, M694I, and R76IH were the rare mutations. None of our patients had M680I (G/A) mutation. Conclusion: We determined the most common MEFV alteration prevalence in children of our region for the first time. Objective: To explore the frequency of MEFV gene mutations in children with Henoch–Schönlein purpura who had no prior familial Mediterranean fever diagnosis and to evaluate the association of MEFV mutations with the clinical and laboratory features of Henoch–Schönlein purpura. Methods: Data of 1120 patients diagnosed with Henoch–Schönlein purpura were reviewed retrospectively. The spectrum and degree of organ involvement and acute phase reactant levels were documented for each patient. Blood for MEFV gene mutation analysis was obtained either at the time of the Henoch–Schönlein purpura diagnosis or during follow-up visits. Pathological specimens of patients who underwent biopsy (renal/skin) were evaluated with special consideration for immunofluorescent examinations. Results: Two hundred and thirty-eight (21.3%) patients were found to have one of the MEFV mutations in which exon 10 mutations were the most common (16.7%). Abdominal pain, joint involvement, scrotal involvement, and relapse were more frequent, and acute-phase reactant levels were significantly high in patients with MEFV mutations. More severe characteristics were observed in the presence of homozygous exon 10 mutations. There was no significant association between exon 2 variants and clinical course of Henoch–Schönlein purpura. Patients carrying MEFV mutations did not have significantly higher levels of IgA deposits in the biopsy materials. Conclusion: Henoch–Schönlein purpura in patients with homozygous exon 10 MEFV mutations seems to be more severe than that in patients carrying other mutations. In patients with exon 10 MEFV mutations, Henoch–Schönlein purpura might be considered as an associated presentation of familial Mediterranean fever rather than a separate clinical entity.
