Abstract

The uranyl(vi) complex UO2Cl(L) of the redox-active diimino-dipyrrin anion, L– is reported and its reactions with inner- and outer-sphere reductants studied., The uranyl(vi) complex UO2Cl(L) of the redox-active, acyclic diimino-dipyrrin anion, L– is reported and its reaction with inner- and outer-sphere reductants studied. Voltammetric, EPR-spectroscopic and X-ray crystallographic studies show that chemical reduction by the outer-sphere reagent CoCp2 initially reduces the ligand to a dipyrrin radical, and imply that a second equivalent of CoCp2 reduces the U(vi) centre to form U(v). Cyclic voltammetry indicates that further outer-sphere reduction to form the putative U(iv) trianion only occurs at strongly cathodic potentials. The initial reduction of the dipyrrin ligand is supported by emission spectra, X-ray crystallography, and DFT; the latter also shows that these outer-sphere reactions are exergonic and proceed through sequential, one-electron steps. Reduction by the inner-sphere reductant [TiCp2Cl]2 is also likely to result in ligand reduction in the first instance but, in contrast to the outer-sphere case, reduction of the uranium centre becomes much more favoured, allowing the formation of a crystallographically characterised, doubly-titanated U(iv) complex. In the case of inner-sphere reduction only, ligand-to-metal electron-transfer is thermodynamically driven by coordination of Lewis-acidic Ti(iv) to the uranyl oxo, and is energetically preferable over the disproportionation of U(v). Overall, the involvement of the redox-active dipyrrin ligand in the reduction chemistry of UO2Cl(L) is inherent to both inner- and outer-sphere reduction mechanisms, providing a new route to accessing a variety of U(vi), U(v), and U(iv) complexes.

Abstract

Waste electrical and electronic equipment (WEEE) such as mobile phones contains a plethora of metals of which gold is by far the most valuable. Herein a simple primary amide is described that achieves the selective separation of gold from a mixture of metals typically found in mobile phones by extraction into toluene from an aqueous HCl solution; unlike current processes, reverse phase transfer is achieved simply using water. Phase transfer occurs by dynamic assembly of protonated and neutral amides with [AuCl4]− ions through hydrogen bonding in the organic phase, as shown by EXAFS, mass spectrometry measurements, and computational calculations, and supported by distribution coefficient analysis. The fundamental chemical understanding gained herein should be integral to the development of metal-recovery processes, in particular through the use of dynamic assembly processes to build complexity from simplicity.

Abstract

Several titanium(m) complexes incorporating the chelating diamidoamine ligand [(Me3SiNCH2CH2)2NSiMe3]2-[NN2] are described. The reaction between Li2[NN2] and TiCl3(THF)3 (THF = tetrahydrofuran) resulted in the formation of the dimeric titanium(III) chloride {TiCl[NN2]}2 1, from which the monomeric titanium(III) alkyl Ti{CH(SiMe3)2}[NN2] 2 could be synthesized via a salt metathesis route. The alkyl complex 2 was found to react with hydrogen gas to form the thermally stable, dimeric, diamagnetic titanium(III) hydride {TiH[NN2]}2 3; density functional calculations on a simplified model system of 3 indicated the presence of a weak Ti-Ti s- interaction. The solid-state structures of 1, 2, and 3 are described.

Abstract

A dramatic difference in the ability of the reducing An(III) center in AnCp3 (An=U, Np, Pu; Cp=C5 H5 ) to oxo-bind and reduce the uranyl(VI) dication in the complex [(UO2 )(THF)(H2 L)] (L="Pacman" Schiff-base polypyrrolic macrocycle), is found and explained. These are the first selective functionalizations of the uranyl oxo by another actinide cation. At-first contradictory electronic structural data are explained by combining theory and experiment. Complete one-electron transfer from Cp3 U forms the U(IV) -uranyl(V) compound that behaves as a U(V) -localized single molecule magnet below 4 K. The extent of reduction by the Cp3 Np group upon oxo-coordination is much less, with a Np(III) -uranyl(VI) dative bond assigned. Solution NMR and NIR spectroscopy suggest Np(IV) U(V) but single-crystal X-ray diffraction and SQUID magnetometry suggest a Np(III) -U(VI) assignment. DFT-calculated Hirshfeld charge and spin density analyses suggest half an electron has transferred, and these explain the strongly shifted NMR spectra by spin density contributions at the hydrogen nuclei. The Pu(III) -U(VI) interaction is too weak to be observed in THF solvent, in agreement with calculated predictions.

Abstract

The computationally- and experimentally-determined molecular structures of a bis-uranyl(VI) complex of an expanded Schiff-base polypyrrolic macrocycle [(UO2)2(L)] are in close agreement only if the pyridine in the fifth equatorial donor site on the uranium is included in the calculations. The relativistic density functional theory (DFT) calculations presented here are augmented from those on previously reported simpler frameworks, and demonstrate that other augmentations, such as the incorporation of condensed-phase media and the changes in the peripheral groups of the ligand, have only a slight effect. Synthetic routes to pure samples of the bis- and mono-uranyl(VI) complexes have been developed using pyridine and arene solvents, respectively, allowing the experimental determination of the molecular structures by X-ray single crystal diffraction; these agree well with the calculated structures. A comprehensive set of calculations have been performed on a series of actinyl AnO2n+ complexes of this macrocyclic ligand. These include both bis- and mono-actinyl adducts for the metals U, Np and Pu, and formal oxidation states VI and V. The reduction potentials of the complexes for U, Np, and Pu, incorporating both solvation and spin-orbit coupling considerations, show the order Np > Pu > U. The agreement between experimental and computed data for U is excellent, suggesting that at this level of computation predictions made about the significantly more radiotoxic Np and Pu molecules should be accurate. A particularly unusual structure of the mononuclear plutonyl(V) complex was predicted by quantum chemical calculations, in which a twist in the macrocycle allows one of the two endo-oxo groups to form a hydrogen bond to one pyrrole group of the opposite side of the macrocycle, in accordance with this member of the set containing the most Lewis basic oxo groups.

Abstract

Measurements of the anisotropic flow coefficients v2{Ψ2},v3{Ψ3},v4{Ψ4}, and v4{Ψ2} for identified particles (π±,K±, and p+p) at midrapidity, obtained relative to the event planes Ψm at forward rapidities in Au + Au collisions at sNN=200GeV, are presented as a function of collision centrality and particle transverse momenta pT. The vn coefficients show characteristic patterns consistent with hydrodynamical expansion of the matter produced in the collisions. For each harmonic n, a modified valence quark-number Nq scaling [plotting vn{Ψm}/(Nq)n/2 versus transverse kinetic energies (KET)/Nq] is observed to yield a single curve for all the measured particle species for a broad range of KET. A simultaneous blast-wave model fit to the observed vn{Ψm}(pT) coefficients and published particle spectra identifies radial flow anisotropies ρn{Ψm} and spatial eccentricities sn{Ψm} at freeze-out. These are generally smaller than the initial-state participant-plane geometric eccentricities n{ΨmPP} as also observed in the final eccentricity from quantum interferometry measurements with respect to the event plane. © 2016 American Physical Society. Abstract

Jet production rates are measured in p+p and d+Au collisions at sqrt[s_{NN}]=200 GeV recorded in 2008 with the PHENIX detector at the Relativistic Heavy Ion Collider. Jets are reconstructed using the R=0.3 anti-k_{t} algorithm from energy deposits in the electromagnetic calorimeter and charged tracks in multiwire proportional chambers, and the jet transverse momentum (p_{T}) spectra are corrected for the detector response. Abstract

Hydrophobic imidazolium-based ionic liquids (IL) act as catalysts for the epoxidation of unfunctionalized olefins in water using hydrogen peroxide as oxidant. Although the catalysts are insoluble in both the substrate and in water, surprisingly, they are very well soluble in aqueous H2O2 solution, owing to perrhenate-H2O2 interactions. Even more remarkably, the presence of the catalyst also boosts the solubility of substrate in water. This effect is crucially dependent on the cation design. Hence, the imidazolium perrhenates enable both the transfer of hydrophobic substrate into the aqueous phase, and serve as actual catalysts, which is unprecedented. At the end of the reaction and in absence of H2O2 the IL catalyst forms a third phase next to the lipophilic product and water and can easily be recycled.

Abstract

Measurements of the fractional momentum loss $({S}_{\mathrm{loss}}\ensuremath{\equiv}\ensuremath{\delta}{p}_{T}/{p}_{T})$ of high-transverse-momentum-identified hadrons in heavy-ion collisions are presented. Using ${\ensuremath{\pi}}^{0}$ in $\mathrm{Au}+\mathrm{Au}$ and $\mathrm{Cu}+\mathrm{Cu}$ collisions at $\sqrt{{s}_{{}_{NN}}}=62.4$ and 200 GeV measured by the PHENIX experiment at the Relativistic Heavy Ion Collider and and charged hadrons in $\mathrm{Pb}+\mathrm{Pb}$ collisions measured by the ALICE experiment at the Large Hadron Collider, we studied the scaling properties of ${S}_{\mathrm{loss}}$ as a function of a number of variables: the number of participants, ${N}_{\mathrm{part}}$, the number of quark participants, ${N}_{\mathrm{qp}}$, the charged-particle density, $d{N}_{\mathrm{ch}}/d\ensuremath{\eta}$, and the Bjorken energy density times the equilibration time, ${\ensuremath{\varepsilon}}_{\mathrm{Bj}}{\ensuremath{\tau}}_{0}$. We find that the ${p}_{T}$, where ${S}_{\mathrm{loss}}$ has its maximum, varies both with centrality and collision energy. Above the maximum, ${S}_{\mathrm{loss}}$ tends to follow a power-law function with all four scaling variables. The data at $\sqrt{{s}_{{}_{NN}}}=200$ GeV and 2.76 TeV, for sufficiently high particle densities, have a common scaling of ${S}_{\mathrm{loss}}$ with $d{N}_{\mathrm{ch}}/d\ensuremath{\eta}$ and ${\ensuremath{\varepsilon}}_{\mathrm{Bj}}{\ensuremath{\tau}}_{0}$, lending insight into the physics of parton energy loss.

Abstract

Measurements of midrapidity charged particle multiplicity distributions, $dN_{\rm ch}/d\eta$, and midrapidity transverse-energy distributions, $dE_T/d\eta$, are presented for a variety of collision systems and energies. Included are distributions for Au$+$Au collisions at $\sqrt{s_{_{NN}}}=200$, 130, 62.4, 39, 27, 19.6, 14.5, and 7.7 GeV, Cu$+$Cu collisions at $\sqrt{s_{_{NN}}}=200$ and 62.4 GeV, Cu$+$Au collisions at $\sqrt{s_{_{NN}}}=200$ GeV, U$+$U collisions at $\sqrt{s_{_{NN}}}=193$ GeV, $d$$+$Au collisions at $\sqrt{s_{_{NN}}}=200$ GeV, $^{3}$He$+$Au collisions at $\sqrt{s_{_{NN}}}=200$ GeV, and $p$$+$$p$ collisions at $\sqrt{s_{_{NN}}}=200$ GeV. Centrality-dependent distributions at midrapidity are presented in terms of the number of nucleon participants, $N_{\rm part}$, and the number of constituent quark participants, $N_{q{\rm p}}$. For all $A$$+$$A$ collisions down to $\sqrt{s_{_{NN}}}=7.7$ GeV, it is observed that the midrapidity data are better described by scaling with $N_{q{\rm p}}$ than scaling with $N_{\rm part}$. Also presented are estimates of the Bjorken energy density, $\varepsilon_{\rm BJ}$, and the ratio of $dE_T/d\eta$ to $dN_{\rm ch}/d\eta$, the latter of which is seen to be constant as a function of centrality for all systems.

Abstract

Alkyl-substituted phenolic pyrazoles such as 4-methyl-2-[5-(n-octyl)-1H-pyrazol-3-yl]phenol (L2H) are shown to function as Cu-extractants, having similar strength and selectivity over Fe(iii) to 5-nonylsalicylaldoxime which is a component of the commercially used ACORGA® solvent extraction reagents. Substitution in the phenol ring of the new extractants has a major effect on their strength, e.g. 2-nitro-4-methyl-6-[5-(2,4,4-trimethylpentyl)-1H-pyrazol-3-yl]phenol (L4H) which has a nitro group ortho to the phenolic hydroxyl group unit and has an extraction distribution coefficient for Cu nearly three orders of magnitude higher than its unsubstituted analogue 4-methyl-6-[5-(2,4,4-trimethylpentyl)-1H-pyrazol-3-yl]phenol (L8H). X-ray structure determinations and density functional theory (DFT) calculations confirm that inter-ligand hydrogen bonding between the pyrazole NH group and the phenolate oxygen atom stabilise the Cu-complexes, giving pseudomacrocyclic structures. Electron-accepting groups ortho to the phenol oxygen atoms buttress the inter-ligand H-bonding, enhancing extractant strength but the effectiveness of this is very dependent on steric factors. The correlation between the calculated energies of formation of copper complexes in the gas phase and the observed strength of comparably substituted reagents in solvent extraction experiments is remarkable. Analysis of the energies of formation suggests that big differences in strength of extractants arise principally from a combination of the effects of the substituents on the ease of deprotonation of the proligands and, for the ortho-substituted ligands, their propensity to buttress inter-ligand hydrogen bonding.

Abstract

Diabet. Med. 29, e398–e401 (2012) Abstract Background We describe an unplanned pregnancy in a 19-year-old with lipodystrophic diabetes caused by a mutation in the peroxisome proliferator-activated receptor gamma (PPARG) gene. The pregnancy was complicated by poor compliance with treatment, severe hypertriglyceridaemia and pancreatitis. Case report The patient presented at 6 weeks’ gestation with an HbA1c of 140 mmol/mol (15%), cholesterol 8.1 mmol/l and triglycerides 20.1 mmol/l. She wished to continue the pregnancy so lipid-lowering therapy was discontinued. She was severely insulin resistant and poorly compliant with diet and medication. A continuous subcutaneous insulin infusion was required for efficient delivery of large doses of basal insulin, alongside injected mealtime boluses, (up to 300 units insulin per day). At 17 weeks’ gestation she developed acute pancreatitis secondary to hypertriglyceridaemia (triglycerides > 100 mmol/l) and required plasmapheresis. Lipid-lowering therapy was reinstated in the third trimester and plasmapheresis was required repeatedly to maintain triglycerides

Abstract

Mapping, recognizing, and defining sedimentary formations in poorly exposed terrains, especially in units with significant stratigraphic variations and disparate definitive diagenetic characteristics, focuses attention on both the problems of field mapping and the contrast between classical field mapping practices and modern stratigraphic and sedimentological analyses. Field mappers must contend with issues of scale and mappability in defining formations, diagenetic versus primary depositional characteristics in delineating contacts, and facies changes, while making standard 1:24,000–1:25,000 scale maps. Formations were defined as mappable units, even before codification of stratigraphic practice, and pre–twentieth-century "legacy formations" remain as widely recognized units. Many "modern formations" recognized and defined in the late twentieth century also meet stratigraphic code definitions. However, postlegacy analyses of exceptionally well-exposed sections using new techniques and insights, while yielding knowledge of stratigraphic variability, may yield newly named, but not necessarily mappable "formations." Resulting problems of formation recognition, formation definition, and field mapping of units and contacts on 1:24,000 or 1:25,000 scale maps are faced repeatedly by STATEMAP and other mappers. In some cases, mapping of "hybrid" formations, composed of two or more previously defined units is required. In other cases, stratigraphic position and general character must override details of type section stratigraphy in mapping formations. We recommend latitude in correlation with type sections and suggest that, while well-exposed sections provide important insights into paleoenvironments and stratigraphic variations, unless units are demonstrably mappable at the 1:24,000 or 1:25,000 scale, these variations should not be used to subdivide previously defined and mappable formations into new unmappable "formations."

Abstract

Interstitial pulmonary fibrosis is caused by the excess production of extracellular matrix (ECM) by Fb in response to TGF-β1. Here, we show that the peptidyl-prolyl isomerase Pin1 modulates the production of many pro- and antifibrogenic cytokines and ECM. After acute, bleomycin injury, Pin1−/− mice showed reduced, pulmonary expression of collagens, tissue inhibitors of metalloproteinases, and fibrogenic cytokines but increased matrix metalloproteinases, compared with WT mice, despite similar levels of inflammation. In primary fibroblasts, Pin1 was required for TGF-β-induced phosphorylation, nuclear translocation, and transcriptional activity of Smad3. In Pin1−/− cells, inhibitory Smad6 was found in the cytoplasm rather than nucleus. Smad6 knockdown in Pin1−/− fibroblasts restored TGF-β-induced Smad3 activation, translocation, and target gene expression. Therefore, Pin1 is essential for normal Smad6 function and ECM production in response to injury or TGF-β and thus may be an attractive therapeutic target to prevent excess scarring in diverse lung diseases. Background: Interstitial pulmonary fibrosis is caused by the excess production of extracellular matrix (ECM) by Fb in response to TGF-β1. Results: The peptidyl-prolyl isomerase Pin1 modulates the production of many pro- and antifibrogenic cytokines and ECM after bleomycin injury. Conclusion: Pin1 controls Smad6 function. Abstract

Background The prevalence of vitamin D deficiency in lung disease is greater than in the general population. Vitamin D deficiency may negatively affect immune and lung function. Accordingly, we hypothesized that lung transplant recipients with vitamin D deficiency are more susceptible to rejection and infections after transplantation. Methods Transplant outcomes were reviewed in a retrospective cohort of 102 lung transplant recipients who had 25-hydroxyvitamin D [25(OH)D] levels drawn during the near-transplant period (100 days pre- or post-transplant). Results In the near-transplant period, 80% of recipients were 25(OH)D-deficient and 20% were not 25(OH)D-deficient. Episodes of acute cellular rejection in the deficient group were more frequent than in the non-deficient group [mean 1.27 (0.99 to 1.55) vs 0.52 (0.12 to 0.93), p = 0.006]. The rejection rate in the deficient group was more than double that of the the non-deficient group [IRR 2.43 (1.30 to 4.52), p = 0.005]. Infectious episodes were also more frequent in the deficient group than in the non-deficient group [mean 4.01 (3.24 to 4.79) vs 2.71 (1.47 to 3.96), p = 0.04]. The mortality rate of recipients who remained 25(OH)D-deficient 1 year after transplant was almost 5-fold higher than in recipients who were not 25(OH)D-deficient [IRR 4.79 (1.06 to 21.63), p = 0.04]. Conclusions Low serum 25(OH)D levels in lung transplant recipients were associated with increased incidence of acute rejection and infection. The mortality of recipients who remained deficient 1 year post-transplant was higher than that of recipients who maintained normal vitamin D levels at 1 year post-transplant.

Abstract

The prevalence of gastroesophageal reflux disease (GERD) in lung transplant patients is twice that of the general population, yet it is unclear which pathophysiological factors play a role in the development or exacerbation of GERD in these patients.1–4 It has been speculated that the lung transplantation itself might be causative in the pathogenesis of GERD after lung transplantation, probably by increasing the risk of esophageal or vagal nerve injury or by increasing the transdiaphragmatic gastroesophageal pressure gradient.1,2 For example, Young et al2 demonstrated that GERD worsens after lung transplantation, reporting that the mean esophageal acid contact time in the supine position before and after transplant doubled from 5.1% to 11.4%. However, they failed to demonstrate that the type of transplant may be responsible for the deterioration of the reflux profile in these patients. Conversely, in a small study that we conducted to determine the prevalence and extent of GERD, as well as the frequency of risk factors and complications of GERD in lung transplant patients, we incidentally found that GERD was more common after bilateral transplantation and retransplantation than after a unilateral lung transplant.1 Therefore, to understand if the type of transplant may lead more frequently to GERD and proximal reflux, we have investigated the prevalence of distal and proximal reflux, as well as the radiologic, endoscopic, and manometric characteristics, in a much larger series of lung transplant patients, which have been stratified according to the type of transplant received (unilateral, bilateral, or retransplant).

Abstract

A series of polydentate dual-compartment, Schiff-base pyrrole macrocycles has been prepared through the straightforward Lewis acid catalysed [1 + 1] condensation reactions between ONO or O(5)-linked aryldiamines and dipyrromethane dialdehydes. These macrocycles display hydrogen-bond acceptor and donor properties and provide distinct N(4) and O(5)/ONO donor sets for metallation reactions, so forming alkali, alkaline earth, and transition metal complexes that were characterised spectroscopically and crystallographically. While the conformationally flexible O(5) donor set allows the formation of helical potassium salt structures, the transition metal complexes of all variants of these macrocycles invariably adopt wedged, Pacman-shaped structures in which the metal is bound in the pyrrole-imine N(4) donor set, so leaving the ONO/O(5) donor set pendant and apical. In some cases (V, Cr, and Co), this proximate combination of Lewis acid binding site and hydrogen bond acceptor facilitates the coordination of water within the molecular cleft; alternatively, direct interaction between the pendant arm and the metal is seen (e.g. Ti). Higher order [2 + 2] macrocycles were also prepared as minor, inseparable by-products of cyclisation, and Fe(2), Mn(2), and Co(2) complexes of these larger macrocycles were found to adopt binuclear helical structures by X-ray crystallography.

Lymphangioleiomyomatosis (LAM) leads to hyperproliferation of abnormal smooth muscle cells in the lungs, associated with diffuse pulmonary parenchymal cyst formation and progressive dyspnea on exertion. The disease targets women of child-bearing age. Complications include pneumothoraces and chylous pleural effusions. Ten-year survival is estimated at 70%, and lung transplantation remains the only validated treatment. It has been observed that LAM cells express markers associated with melanocytic differentiation, including gp100 and MART-1. Other melanocytic markers have also been observed. The same proteins are targeted by T cells infiltrating melanoma tumors as well as by T cells infiltrating autoimmune vitiligo skin, and these antigens are regarded as relatively immunogenic. Consequently, vaccines have been developed for melanoma targeting these and other immunogenic melanocyte differentiation proteins. Mono- and dinuclear calcium complexes of the Schiff-base macrocycles H4L have been prepared and characterized spectroscopically and crystallographically. In the formation of Ca(THF)2(H2L(1)), Ca2(THF)2(μ-THF)(L(1)), and Ca2(THF)4(L(2)), the ligand framework adopts a bowl-shaped conformation instead of the conventional wedge, Pacman-shaped structure as seen with the anthracenyl-hinged complex Ca2(py)5(L(3)). The mononuclear calcium complex Ca(THF)2(H2L(1)) reacts with various equivalents of LiN(SiMe3)2 to form calcium/alkali metal clusters and dinuclear transition metal complexes when reacted subsequently with transition metal salts. The dinuclear calcium complex Ca2(THF)2(μ-THF)(L(1)), when reacted with various equivalents of NaOH, is shown to act as a platform for the formation of calcium/alkali metal hydroxide clusters, displaying alternate wedged and bowl-shaped conformations.

Abstract

Lung transplantation is now a well established treatment option for several end-stage respiratory diseases. Survival after lung transplantation has significantly improved over the last decade. The primary limitation to increased utilization of lung transplantation remains donor scarcity. Suitable allografts have been procured from donors after determination of neurologic death and from donors after determination of cardiac death (DDCD or DCD). Historically, the first human lung transplantation performed, utilized an allograft procured after cardiovascular death, also referred to as nonheart-beating donor.The experience at University of Wisconsin in 1993 reintroduced DCD lung transplantation with the first successful clinical case.A potential additional lung allograft source, DCD lung transplantation has been established with very acceptable outcomes observed by several centers. We provide the relevant background for the rationale of donor allograft expansion to include DCD lungs from controlled (Maastricht category III donors).This review considers the available evidence for DCD lung transplantation and compares reported primary graft dysfunction rates and current survival data available.

Abstract

Long-term morbidity and mortality after lung transplantation are largely attributable to bronchiolitis obliterans syndrome (BOS), a form of chronic rejection.1,2 In turn, BOS and rejection have been associated with gastroesophageal reflux disease (GERD), although direct causality has not been established.3,4 The mechanism by which GERD may cause or exacerbate BOS is not known; however, it is thought that aspiration of gastroesophageal contents may trigger a nonallogenic injury to the transplanted lungs. The investigation of the causal relationship between GERD, aspiration, and BOS is important because GERD can be treated before lung function deteriorates.4–6 In fact, studies have shown that operative control of GERD may stabilize or improve lung function in some patients with BOS, especially when a laparoscopic fundoplication is performed early after lung transplantation.7–10 Control of reflux is key, because GERD may be a modifiable risk factor for the progression of BOS; GERD and aspiration of gastroduodenal substances, like pepsin, might be stopped by laparoscopic antireflux surgery (LARS). Nevertheless, the role and the indications for LARS in the management of patients with BOS or rejection is less clear because it is not known if aspiration can be prevented by LARS, or if aspiration of gastroesophageal refluxate indeed represents a nonallogenic injury to the transplanted lungs. The aim of this study was to determine if LARS after lung transplantation could represent an effective means to prevent aspiration as defined by the presence of pepsin in the bronchoalveolar lavage fluid (BALF), and if aspiration of gastroesophageal refluxate is associated with a worse clinical outcome than patients without evidence of aspiration. We hypothesized that recovery of pepsin in the BALF confirms aspiration and that LARS could be protective against aspiration of pepsin.

Abstract

Sirolimus (rapamycin) is a potent anti-proliferative agent with immunosuppressive properties that is increasingly being used in solid-organ and hematopoietic stem cell transplantation. In addition, this drug is being investigated for treatment of a broad range of disorders, including cardiovascular disease, malignancies, tuberous sclerosis, and lymphangeioleiomyomatosis. In this study, we found an increased risk of venous thromboembolism (VTE) in lung transplant recipients treated with a sirolimus (SIR)-based immunosuppressive regimen.One hundred eighty-one lung transplant recipients were enrolled in a prospective, multicenter, randomized, open-label trial comparing a tacrolimus (TAC)/SIR/prednisone immunosuppression regimen with a TAC/azathioprine (AZA)/prednisone immunosuppressive regimen. The differences in rates of VTE were examined.There was a significantly higher occurrence of VTE in the SIR cohort [15 of 87 (17.2%)] compared with the AZA cohort [3 of 94 (3.2%)] (stratified log-rank statistic = 7.44, p0.01). When adjusted for pre-transplant diagnosis and stratified by transplant center, this difference remained essentially unchanged (hazard ratio for SIR vs AZA = 5.2, 95% confidence interval 1.4 to 19.5, p = 0.01).Clinicians prescribing SIR should maintain a high level of vigilance for VTE, particularly among patients with other risk factors for this complication.

Abstract

Lung transplantation is currently performed on selected patients with end-stage lung disease. However, the median survival of patients after lung transplantation is inferior to that of any other solid organ transplantation, only 5 years.1 This low survival rate is primarily due to the development of obliterative bronchiolitis, which clinically manifests as bronchiolitis obliterans syndrome (BOS).2 BOS develops in almost half of lung transplant recipients within 5 years and is a source of considerable morbidity because of its detrimental effect on lung function and quality of life.1 The pathophysiology of obliterative bronchiolitis is not fully understood. However, evidence suggests that obliterative bronchiolitis might represent a nonimmunologic aberrant response to a chronic stimulus injury.3–6 Recently, gastroesophageal reflux disease (GERD) has been proposed as a potential factor responsible for chronic injury. Several studies have indirectly shown that the presence of GERD in lung transplant recipients is a risk factor for the development and progression of BOS, because of the high prevalence of GERD demonstrated after lung transplantation and because operative control of GERD may control the decline in lung function characteristic of BOS.4–7 Despite the evidence that supports the detrimental effect of GERD before and after lung transplantation, the prevalence of risk factors of GERD, including esophageal dysmotility, hiatal hernia, and delayed gastric emptying, are still unknown. In addition, the prevalence of Barrett’s esophagus, a known complication of GERD, has not been determined in lung transplant patients. The purpose of this study was to determine the prevalence and extent of GERD, as well as the frequency of these risk factors and complications of GERD in lung transplant patients.

Abstract

Objective We sought to examine long-term outcomes at the University of Wisconsin for all lung transplant recipients who received lungs from donation after cardiac death donors since the initiation of this program in 1993. Methods Eighteen (4.2%) of the 424 lung transplantations performed in 406 patients between January 1993 and April 2009 used lungs from donation after cardiac death donors. Outcomes for this recipient cohort were compared with those for recipients who received organs from brain-dead donors. Results Warm ischemic time (from withdrawal of support to reperfusion of organs) was 30 ± 17 minutes (11–93 minutes). The patient survival rates in the donation after cardiac death group (DCD group) at 1, 3, and 5 years were 88.1% ± 7.9%, 81.9% ± 9.5%, and 81.9% ± 9.5%, respectively. These survival rates were not different from those of the brain-dead donor group (BDD group, P = .66). The incidence of primary graft dysfunction in the DCD group was similar to that of the BDD group (P = .59). However, the incidence of airway complications was somewhat higher in the DCD group. Freedom from bronchiolitis obliterans syndrome at 1, 3, and 5 years in the DCD group was 80.4% ± 10.2%, 80.4% ± 10.2%, and 72.3% ± 11.9%, respectively, and did not differ from the incidence of bronchiolitis obliterans syndrome in the BDD group (P = .59). Conclusions Our data show that the long-term patient and graft survival rates after donation after cardiac death lung transplantation were equivalent to those after brain-dead donor lung transplantation. Our findings suggest that the use of donation after cardiac death donors can safely and substantially expand the donor pool for lung transplantation.

This recent evidence has underscored gastroesophageal reflux (GER) and its associated aspiration risk as a non-alloimmune mechanism of chronic lung transplant failure. Given the suggested safety and efficacy of laparoscopic anti-reflux procedures in the lung transplant population, identifying those at risk for aspiration is of prime importance, especially concerning the potential for long-term improvements in morbidity and mortality. Conventional diagnostic methods for GER and aspiration, such as pH monitoring and detecting pepsin and bile salts in BAL fluid, have gaps in their effectiveness. Therefore, we review the applications and controversies of a non-invasive method of defining reflux injury in the lung transplant population: the detection of biomarkers of aspiration in the exhaled breath condensate. Only by means of assay standardization and directed collaboration may such a non-invasive method be a realization in lung transplantation.

Abstract

Rat lung allograft rejection is mediated by collagen type V (col(V)) specific T-helper-cell 17 (Th17) cells. Adoptive transfer of these cells is sufficient to induce rejection pathology in isografts, whereas tolerance to col(V) suppresses allograft rejection. Therefore, we tested whether regulatory T cells from tolerant rats could suppress the Th17-mediated rejection in the syngeneic model of lung transplantation.Rats were subjected to syngeneic left lung transplantation, and acute rejection was induced by adoptive transfer of lymph node cells from col(V)-immunized rats. Tolerance was induced by intravenous injection of col(V), and spleen lymphocytes were used for adoptive transfer. CD4+ T cells were depleted using magnetic beads. Lung isografts were analyzed using micro-positron emission tomography imaging and histochemistry. The transvivo delayed type hypersensitivity assay was used to analyze the Th17 response.Adoptive cotransfer of col(V)-specific effector cells with cells from col(V)-tolerized rats suppressed severe vasculitis and bronchiolitis with parenchymal inflammation, and the expression of interleukin (IL)-17 transcripts in mediastinal lymph nodes induced by effector cells alone. Analysis by transvivo delayed type hypersensitivity showed that the reactivity to col(V) was dependent on the presence of tumor necrosis factor-alpha and IL-17 but not interferon-gamma. Depletion of CD4+ T cells from the suppressor cell population abrogated the col(V)-specific protection.Th17-mediated acute rejection after lung transplantation is ameliorated by CD4+ col(V)-specific regulatory T cells. The mechanism for this Th17 suppression is consistent with tolerance induction to col(V). The goal of transplantation treatment, therefore, should target Th17 development and not suppression of T-cell activation by suppressing IL-2.

Abstract

The uranyl dication, [UO2]2+, is the most prevalent and most thermodynamically stable form of uranium and is a soluble and problematic environmental contaminant. It is also extraordinarily chemically robust due to the strongly covalent trans-UO2 bonding. In contrast, the pentavalent uranyl cation [UO2]+ is unstable in an aqueous environment with respect to disproportionation into tetravalent uranium species and [UO2]2+. Aside from fundamental interest, an understanding of the pentavalent [UO2]+ cation is desirable since it is important environmentally as a key intermediate in the precipitation of uranium from groundwater. In the last 2 years, the use of anaerobic coordination chemistry techniques and organometallic reagents has allowed the isolation of a few kinetically inert complexes containing the f1 [UO2]+ cation. The synthesis and characterisation of these, and the insight they give into subsequent reactivity of the trans-UO2 unit, is discussed in this review.

Abstract

PHENIX has measured the electron-positron pair mass spectrum from 0 to 8 GeV/c(2) in p + p collisions at root s = 200 GeV. The contributions from light meson decays to e(+)e(-) pairs have been determined based on measurements of hadron production cross sections by PHENIX. Within the systematic uncertainty of similar to 20% they account for all e(+)e(-) pairs in the mass region below similar to 1 GeV/c(2). The e(+)e(-) pair yield remaining after subtracting these contributions is dominated by semileptonic decays of charmed hadrons correlated through flavor conservation. Using the spectral shape predicted by PYTHIA, we estimate the charm production cross section to be 544 +/- 39(stat) +/- 142(syst) +/- 200(model) pb. which is consistent with QCD calculations and measurements of single leptons by PHENIX. Abstract

Rationale: The pathogenesis of primary graft dysfunction (PGD), a serious complication of lung transplantation, is poorly understood. Human studies and rodent models have shown that collagen type V (col[V]), stimulates IL-17–dependent cellular immunity after lung transplantation. Objectives: To determine whether patients with end-stage lung disease develop pretransplant col(V)-specific cellular immunity, and if so, the impact of this response on PGD. Methods: Trans-vivo delayed-type hypersensitivity (TV-DTH) assays were used to evaluate memory T-cell responses to col(V) in 55 patients awaiting lung transplantation. PaO2/FIO2 index data were used to assess PGD. Univariate risk factor analysis was performed to identify variables associated with PGD. Rats immunized with col(V) or irrelevant antigen underwent lung isografting to determine if prior anti-col(V) immunity triggers PGD in the absence of alloreactivity. Measurements and Main Results: We found that 58.8% (10/17) of patients with idiopathic pulmonary fibrosis, and 15.8% (6/38) of patients without idiopathic pulmonary fibrosis tested while on the wait list for a lung transplant were col(V) DTH positive. Col(V) reactivity was CD4+ T-cell and monocyte mediated, and dependent on IL-17, IL-1b, and tumor necrosis factor (TNF)-a .P aO2 /FIO2 indices were impaired significantly 6–72 hours after transplantation in col(V)-reactive versus nonreactive patients. Univariate risk factor analysis identified only preoperative TV-DTH to col(V) and ischemic time as predictors of PGD. Finally, in a rat lung isograft model, col(V) sensitization resulted in significantly lower PaO2 /FIO2 , increased local TNF-a and IL-1b production, and a moderate-to-severe bronchiolitis/ vasculitis when compared with control isografts. Abstract

Bronchiolitis obliterans syndrome (BOS), a process of fibro-obliterative occlusion of the small airways in the transplanted lung, is the most common cause of lung transplant failure. We tested the role of cell-mediated immunity to collagen type V [col(V)] in this process. PBMC responses to col(II) and col(V) were monitored prospectively over a 7-year period. PBMCs from lung transplant recipients, but not from healthy controls or col(IV)-reactive Goodpasture's syndrome patients after renal transplant, were frequently col(V) reactive. Col(V)-specific responses were dependent on both CD4+ T cells and monocytes and required both IL-17 and the monokines TNF-alpha and IL-1beta. Strong col(V)-specific responses were associated with substantially increased incidence and severity of BOS. Incidences of acute rejection, HLA-DR mismatched transplants, and induction of HLA-specific antibodies in the transplant recipient were not as strongly associated with a risk of BOS. These data suggest that while alloimmunity initiates lung transplant rejection, de novo autoimmunity mediated by col(V)-specific Th17 cells and monocyte/macrophage accessory cells ultimately causes progressive airway obliteration.

Abstract

The synthesis and structures of dinuclear manganese, iron, and zinc complexes of chiral di-iminodipyrromethane ligands (L) are reported. Schiff base condensation reactions between 5,5′-diformyl-2,2′-dipyrromethane and the chiral amines (–)-(R)-CH(Me)tBu and (+)-(R)-CH(Me)Ph result in the straightforward synthesis of the new, chiral ligands H2L2 and H2L3, respectively. Salt elimination reactions between K2L and divalent Mn and Fe halides, and protonolysis reactions between ZnMe2 and H2L result in the formation of the new dinuclear complexes [M2(L)2]. Investigation of the structures of these compounds in solution and in the solid state reveal that chiral mesocates are formed for L = L2, whereas for L = L3, a racemic mixture of helicates is present.(© Wiley-VCH Verlag GmbH & Co. Abstract

Study Objective. To determine whether lung transplant recipients would have a less vigorous T-cell response to hepatitis B surface antigen (HBsAg) than that of patients awaiting lung transplantation and healthy subjects, we sought to measure and compare T-cell responses among these three groups. Design. Prospective study. Setting. Lung transplant clinic at a university hospital. Subjects. Twelve lung transplant recipients, 12 patients awaiting lung transplantation, and 15 healthy subjects. All participants had received the hepatitis B vaccine series and had a documented antibody response to it. Intervention. Blood samples were obtained from each participant. Measurements and Main Results. Participants' sex, age, time since lung transplantation (if applicable), and time since hepatitis B immunization were recorded. Peripheral blood mononuclear cells were isolated from the participants' blood samples for the trans vivo delayed-type hypersensitivity (DTH) assay. These cells were mixed with saline, tetanus toxoid, or HBsAg and injected into the footpads of immunodeficient mice. Resultant swelling of the footpad was used as an index of human T-cell response. The healthy subjects were younger than the patients in both transplant groups. However, we found no significant difference in DTH response elicited by HBsAg among the healthy subjects, patients awaiting lung transplantation, and lung transplant recipients (mean ± standard error [SE] 34.7 ± 4.3, 32.1 ± 3.1, and 33.5 ± 4.0 × 10−4 in., respectively, p>0.8) or when tetanus toxoid was used as a positive control (15.7 ± 2.8, 22.8 ± 6.5, and 21.7 ± 3.9 × 10−4 in., respectively, p>0.3). No correlation between age or time since immunization and DTH response was noted. Conclusion. Lung transplant recipients maintained a T-cell response to HBsAg that was similar in vigor to that of both patients awaiting transplantation and healthy subjects even though their antibody concentrations waned rapidly after transplantation. The role of these T cells as a correlate of protection from infection remains to be investigated.

Abstract

Background Although new approaches to the treatment of patients with cystic fibrosis (CF) are significantly prolonging their lives, most patients will eventually develop respiratory failure due to progressive bronchiectasis caused by chronic lung infection and inflammation and die from to respiratory failure. We examined our center's (University of Wisconsin Hospital and Clinics) experience with lung transplantation for patients with CF and reviewed the literature to examine current and evolving approaches to transplantation for this indication. Methods We reviewed all published literature pertaining to lung transplantation for CF through 2006, and we reviewed all aspects of transplantation for patients with CF at our institution from 1994 to 2005. Results Major complications following lung transplantation include acute rejection, bacterial infection, and bronchiolitis obliterans. Five-year survival at UWHC (Kaplan–Meier) is 67%, and survival was not adversely affected by transplanting patients receiving mechanical ventilation. The major cause of death for transplant recipients was bronchiolitis obliterans syndrome (BOS). Conclusions Lung transplantion for CF is associated with acceptable survival rates and can improve quality of life. Lung transplant should be offered to all patients with advanced CF lung disease if they meet currently accepted inclusion and exclusion criteria.

Abstract

Everolimus is an immunosuppressive agent that reduces cardiac allograft vasculopathy. This report presents the 24-month results of a multicenter trial of everolimus vs azathioprine in heart transplantation.A total of 634 patients were randomized to receive 1.5 mg everolimus, 3 mg everolimus or azathioprine, with cyclosporine and steroids. A 12-month, double-blind, double-dummy period was followed by a 12-month open-label period.At 24 months, the percentage of patients reaching the composite efficacy end-points was significantly lower with everolimus (1.5 mg: 45.9%, p = 0.016; 3 mg: 36.0%, p0.001) than with azathioprine (57.5%). The change in maximal intimal thickness from baseline to 24 months was significantly smaller with everolimus 1.5 mg (0.07 mm, p = 0.014) and 3 mg (0.06 mm, p = 0.004) compared with azathioprine (0.15 mm). The 24-month incidence of vasculopathy was 33.3% with everolimus 1.5 mg, 45.5% with everolimus 3 mg and 58.3% with azathioprine (p = 0.017 vs everolimus 1.5 mg). Incidence of cytomegalovirus infection was 3-fold lower in patients receiving everolimus compared with azathioprine (7.2% and 7.1% in the 1.5-mg and 3-mg everolimus cohorts, respectively, and 21% in the azathioprine group; p0.0001). Median serum creatinine levels at 24 months were higher with everolimus than with azathioprine, but decreased when cyclosporine exposure was reduced (everolimus 1.5 mg: baseline 167 micromol, after 6 months 157.5 micromol; everolimus 3 mg: baseline 185.6 micromol, after 6 months 160 micromol; azathioprine: baseline 123.3 micromol, after 6 months 127 micromol).Everolimus significantly reduced acute rejection and limited the progression of allograft vasculopathy at 24 months compared with azathioprine. Although graft and patient survival was comparable at 24 months, everolimus therapy may improve longer-term outcomes after heart transplantation.

Abstract

Correlations between p and (p) over bar at transverse momenta typical of enhanced baryon production in Au + Au collisions are reported. The PHENIX experiment has measured same and opposite sign baryon pairs in Au + Au collisions at root s(NN) = 200 GeV. Correlated production of p and p with the trigger particle from the range 2.5 < p(T) < 4.0 GeV/c and the associated particle with 1.8 < p(T) < 2.5 GeV/c is observed to be nearly independent of the centrality of the collisions. Same sign pairs show no correlation at any centrality. The conditional yield of mesons triggered by baryons (and anti-baryons) and mesons in the same p(T) range rises with increasing centrality, except for the most central collisions, where baryons show a significantly smaller number of associated mesons. These data are consistent with a picture in which hard scattered partons produce correlated p and (p) over bar in the p(T) region of the baryon excess. (c) 2007 Elsevier B.V. All rights reserved.

Abstract

The use of a Schiff-base calixpyrrole (L) as a dinucleating ligand for early transition metals is described. Salt elimination reactions between the crystallographically-characterised [K4(THF)3(PhMe)(L)] and titanium(III) and vanadium(III) halides form the new dinuclear complexes [(MCl)2(L)] (M = Ti, V). Adventitious, and partial, oxidation of [(VCl)2(L)] resulted in the formation of the unusual mixed-valence vanadyl complexes [(V O)(S)(VCl)(L)] (S = THF or pyridine), which both adopt desired Pacman geometries in the solid state in which the oxo ligand is accommodated within the dinuclear molecular cleft.

Abstract

A 30-year old African American woman presents with multiple scarred plaques on her scalp, ears, and neck. The lesions are occasionally pruritic and even painful, especially in the early stages when they first appear. She notes that outbreaks are worse during the summer, and she admits that she is very conscious of her appearance year-wide, often wearing her hair past her shoulders and clothing that covers her neck to help hide her skin. She denies joint pain, shortness of breath, chest pain, or ever noticing any ulcers in her mouth.
