Abstract

Fistula in ano is a common anorectal disease in adults. Currently, surgery remains the definitive therapeutic approach, but in some cases, it can lead to serious complications as faecal or gas incontinence. Therefore, sphincter sparing treatments should be considered for complex fistulas. One of the sphincteric preserving treatment is the filling with a dermal extract commonly called “collagen glue” as Salvecoll-E® gel. This is a multicentric, prospective, observational study on the use of Salvecoll-E® gel in treatment of complex anal fistulas. We treated 70 patients from May 2016 to May 2017. In the first phase, we debrided the fistula tract using a loose seton kept for 4–6 weeks. In the second phase, the seton was removed and the fistula tract was filled with Salvecoll-E® gel. In this article, we report results at 36 months of follow-up. Fifty patients (71.4%) had completely healed fistula within 36 months of follow-up. Twenty-eight patients (28.2%) had recurrences. Among these failures, 65% were within 6 months. All low transphincteric fistulas healed. Recurrences occurred only in median and high transphincteric fistulas. No patient had a worsening of continence status measured with Cleveland Clinic Florida Incontinence Severity score. Salvecoll-E® gel is a recent finding among sphincter-sparing treatments. In this study, we demonstrate that it is a safe option in the treatment of complex fistulas. Final results are satisfactory and in line with the best results published in literature among mini-invasive treatments.

Abstract

The project for the second competition for the Parliament of the Kingdom of Italy, drawn up between 1888 and 1889 sees Ernesto Basile (Palermo 1857-1932) as winner ex aequo. Basile's project envisages the construction of a large monumental building (182.70 m by 171.40 m), in which the three halls, the Chamber, the Senate and the Royal Sessions Hall, develop around a large central courtyard; it will not be realized due to the fall of the Crispi government in 1891, but presents, in embryo, solutions adopted in the expansion of Palazzo Montecitorio (1903 - 1918), entrusted to Basile, in which the Chamber of Deputies, still existing today, is may consider “a sort of budding, from the organism of his second Parliament Building” (Mauro, Sessa 2000b).The original drawings are kept and cataloged in the Basile Endowment of the Scientific Collections of the Department of Architecture of the University of Palermo, edited by Professor Ettore Sessa (Mauro, Sessa, 2000a; Sessa, 2014, Mauro, Sessa, 2015).In a first phase, the conjectural reconstruction and digital modeling was carried out (Avella, 2020), and in a second phase, a first 3D printing model was created, limited to the main front, which involved the design of the decomposition and assembly according to criteria respectful of architectural parties (Avella, 2021).This work presents the progress of the work, extended to the bodies containing the Chamber of Deputies and that of the Senate and the integration with Augmented Reality systems, in view of the future permanent exhibition at the premises of the Basile Endowment. The integration with Augmented Reality visualization systems stems from the desire to allow the observer to visualize the external envelope of the physical model and the internal spaces of the parliamentary halls.

Abstract

Em um contexto de desenvolvimento profissional docente, este artigo tem por objetivo caracterizar o perfil do professor de língua inglesa inserido no âmbito da educação profissional. Para tanto, recorre à análise do Plano de Curso e do Edital de Contratação docente de língua inglesa atuante no Ensino Médio Técnico em Administração da ETEC, vinculada ao CEETEPS. A análise documental também inclui a Base Nacional Comum Curricular (BNCC) a fim de analisar como a língua inglesa é contemplada em paralelo com o Plano de Curso. No contexto histórico sobre o ensino de línguas no ensino profissionalizante e em autores que versam sobre formação identitária, buscou-se suporte teórico para a realização deste trabalho. Os resultados indicam que o professor de língua inglesa tem seu desenvolvimento profissional docente eminentemente atrelado à sua prática de ensino, tendo que revisitar sua identidade docente de forma a ter uma atuação consoante com os conceitos técnicos de cada curso em que venha a atuar.

Abstract

According to the modular hypothesis for the control of movement, muscles are recruited in synergies, which capture muscle coordination in space, time, or both. In the last two decades, muscle synergy analysis has become a well-established framework in the motor control field and for the characterization of motor impairments in neurological patients. Altered modular control during a locomotion task has been often proposed as a potential quantitative metric for characterizing pathological conditions. Therefore, the purpose of this systematic review is to analyze the recent literature that used a muscle synergy analysis of neurological patients’ locomotion as an indicator of motor rehabilitation therapy effectiveness, encompassing the key methodological elements to date. Searches for the relevant literature were made in Web of Science, PubMed, and Scopus. Most of the 15 full-text articles which were retrieved and included in this review identified an effect of the rehabilitation intervention on muscle synergies. However, the used experimental and methodological approaches varied across studies. Despite the scarcity of studies that investigated the effect of rehabilitation on muscle synergies, this review supports the utility of muscle synergies as a marker of the effectiveness of rehabilitative therapy and highlights the challenges and open issues that future works need to address to introduce the muscle synergies in the clinical practice and decisional process.

Abstract

Sensor-based assessments in medical practice and rehabilitation include the measurement of physiological signals such as EEG, EMG, ECG, heart rate, and NIRS, and the recording of movement kinematics and interaction forces. Such measurements are commonly employed in clinics with the aim of assessing patients’ pathologies, but so far some of them have found full exploitation mainly for research purposes. In fact, even though the data they allow to gather may shed light on physiopathology and mechanisms underlying motor recovery in rehabilitation, their practical use in the clinical environment is mainly devoted to research studies, with a very reduced impact on clinical practice. This is especially the case for muscle synergies, a well-known method for the evaluation of motor control in neuroscience based on multichannel EMG recordings. In this paper, considering neuromotor rehabilitation as one of the most important scenarios for exploiting novel methods to assess motor control, the main challenges and future perspectives for the standard clinical adoption of muscle synergy analysis are reported and critically discussed.

We derive concentration inequalities for differentially private median and mean estimators building on the "Propose, Test, Release" (PTR) mechanism introduced by Dwork and Lei (2009). We introduce a new general version of the PTR mechanism that allows us to derive high probability error bounds for differentially private estimators. Our algorithms provide the first statistical guarantees for differentially private estimation of the median and mean without any boundedness assumptions on the data, and without assuming that the target population parameter lies in some known bounded interval. Abstract

Pseudo-density matrices are a generalisation of quantum states and do not obey monogamy of quantum correlations. Could this be the solution to the paradox of information loss during the evaporation of a black hole? In this paper we discuss this possibility, providing a theoretical proposal to extend quantum theory with these pseudo-states to describe the statistics arising in black-hole evaporation. We also provide an experimental demonstration of this theoretical proposal, using a simulation in optical regime, that tomographically reproduces the correlations of the pseudo-density matrix describing this physical phenomenon.

Abstract

Abstract Background Very long‐chain fatty acids (VLCFAs) composed of more than 20 carbon atoms are essential in the biosynthesis of cell membranes in the brain, skin, and retina. VLCFAs are elongated beyond 28 carbon atoms by ELOVL4 enzyme. Variants in ELOVL4 are associated with three Mendelian disorders: autosomal dominant (AD) Stargardt‐like macular dystrophy type 3, AD spinocerebellar ataxia, and autosomal recessive disorder congenital ichthyosis, spastic quadriplegia and impaired intellectual development (ISQMR). Only seven subjects from five unrelated families with ISQMR have been described, all of which have biallelic single‐nucleotide variants. Methods We performed clinical exome sequencing on probands from four unrelated families with neuro‐ichthyosis. Results We identified three novel homozygous ELOVL4 variants. Two of the families originated from the same Saudi tribe and had the exact homozygous exonic deletion in ELOVL4, while the third and fourth probands had two different novel homozygous missense variants. Seven out of the eight affected subjects had profound developmental delay, epilepsy, axial hypotonia, peripheral hypertonia, and ichthyosis. Delayed myelination and corpus callosum hypoplasia were seen in two of five subjects with brain magnetic rosonance imaging and cerebral atrophy in three. Conclusion Our study expands the allelic spectrum of ELOVL4‐related ISQMR. The detection of the same exonic deletion in two unrelated Saudi family from same tribe suggests a tribal founder mutation.

Abstract

A comprehensive study of the behavior of the Mott insulator Ca_2RuO_4 under electrical current drive is performed by combining two experimental probes: the macroscopic electrical transport and the microscopic X-Ray diffraction. The resistivity, rho, vs electric current density, J, and temperature, T , rho(J,T), resistivity map is drawn. In particular, the meta-stable state, induced between the insulating and the metallic thermodynamic states by current biasing Ca_2RuO_4 single crystals, is investigated. Such an analysis, combined with the study of the resulting RuO_6 octahedra energy levels, reveals that a metallic crystal phase emerges in the meta-stable regime. The peculiar properties of such a phase, coexisting with the well-established orthorhombic insulating and tetragonal metallic phases, allow to explain some of the unconventional and puzzling behaviors observed in the experiments, as a negative differential resistivity.

184. Privacy-preserving parametric inference: a case for robust statistics

Abstract

Differential privacy is a cryptographically-motivated approach to privacy that has become a very active field of research over the last decade in theoretical computer science and machine learning. In this paradigm one assumes there is a trusted curator who holds the data of individuals in a database and the goal of privacy is to simultaneously protect individual data while allowing the release of global characteristics of the database. In this setting we introduce a general framework for parametric inference with differential privacy guarantees. We first obtain differentially private estimators based on bounded influence M-estimators by leveraging their gross-error sensitivity in the calibration of a noise term added to them in order to ensure privacy. We then show how a similar construction can also be applied to construct differentially private test statistics analogous to the Wald, score and likelihood ratio tests. We provide statistical guarantees for all our proposals via an asymptotic analysis. An interesting consequence of our results is to further clarify the connection between differential privacy and robust statistics. In particular, we demonstrate that differential privacy is a weaker stability requirement than infinitesimal robustness, and show that robust M-estimators can be easily randomized in order to guarantee both differential privacy and robustness towards the presence of contaminated data. We illustrate our results both on simulated and real data.

Abstract

We investigate the collective behavior of a globalized society under the influence of endogenous mass media trends. The mass media trend is a global field corresponding to the statistical mode of the states of the agents in the system. The interaction dynamics is based on Axelrod's rules for the dissemination of culture. We find situations where the largest minority group, possessing a cultural state different from that of the predominant trend transmitted by the mass media, can grow to almost half of the size of the population. We show that this phenomenon occurs when a critical number of long-range connections are present in the underlying network of interactions. We have numerically characterized four phases on the space of parameters of the system: an ordered phase; a semi-ordered phase where almost half of the population consists of the largest minority in a state different from that of the mass media; a disordered phase; and a chimera-like phase where one large domain coexists with many very small domains., Comment: 7 pages and 7 figures

Abstract

Closed timelike curves are striking predictions of general relativity allowing for time-travel. They are afflicted by notorious causality issues (e.g. grandfather's paradox). Quantum models where a qubit travels back in time solve these problems, at the cost of violating quantum theory's linearity - leading e.g. to universal quantum cloning. Interestingly, linearity is violated even by open timelike curves (OTCs), where the qubit does not interact with its past copy, but is initially entangled with another qubit. Non-linear dynamics is needed to avoid violating entanglement monogamy. Here we propose an alternative approach to OTCs, allowing for monogamy violations. Specifically, we describe the qubit in the OTC via a pseudo-density operator - a unified descriptor of both temporal and spatial correlations. We also simulate the monogamy violation with polarization-entangled photons, providing a pseudo-density operator quantum tomography. Remarkably, our proposal applies to any space-time correlations violating entanglement monogamy, such as those arising in black holes.

Abstract

We present the effects of spin-orbit coupling on the low-energy bands and Fermi surface of the recently discovered pressure-induced superconductor CrAs. We apply the L\"owdin down-folding procedure to a tight-binding hamiltonian that includes the intrinsic spin-orbit interaction, originating from the Cr 3d electrons as well as from As 4p ones. Our results indicate that As contributions have negligible effects, whereas the modifications to the band structure and the Fermi surface can be mainly ascribed to the Cr contribution. We show that the inclusion of the spin-orbit interaction allows for a selective removal of the band degeneracy due to the crystal symmetries, along specific high symmetry lines. Such release of the band degeneracy naturally determines a reconstruction of the Fermi surface, including the possibility of changing the number of pockets., Comment: 7 pages, 6 figures, 1 table

Abstract

We tackle the problem of estimating a location parameter with differential privacy guarantees and sub-Gaussian deviations. Recent work in statistics has focused on the study of estimators that achieve sub-Gaussian type deviations even for heavy tailed data. We revisit some of these estimators through the lens of differential privacy and show that a naive application of the Laplace mechanism can lead to sub-optimal results. We design two private algorithms for estimating the median that lead to estimators with sub-Gaussian type errors. Unlike most existing differentially private median estimators, both algorithms are well defined for unbounded random variables that are not even required to have finite moments. We then turn to the problem of sub-Gaussian mean estimation and show that under heavy tails natural differentially private alternatives lead to strictly worse deviations than their non-private sub-Gaussian counterparts. This is in sharp contrast with recent results that show that from an asymptotic perspective the cost of differential privacy is negligible.

Abstract

We study numerically a one-dimensional mixture of spin-$\tfrac{1}{2}$ fermions and scalar bosons in the hard-core limit. Considering repulsive fermion-fermion and boson-fermion interactions, we find superfluid and insulator states whose phase diagram is calculated. We determine that given a fermionic density $\rho_F$, the insulator states are located at the bosonic densities $\rho_B=1-\rho_F$ and $\rho_B=1-\tfrac{1}{2}\rho_F$, and emerge even in the absence of fermion-fermion coupling. In addition, the boson-fermion repulsion drives quantum phase transitions inside the insulator lobes with $\rho_B=1/2$. Abstract

Quantum correlations become formidable tools for beating classical capacities of measurement. Preserving these advantages in practical systems, where experimental imperfections are unavoidable, is a challenge of the utmost importance. Here we propose and realize a quantum ghost imaging protocol able to compensate for the detrimental effect of detection noise and losses. This represents an important improvement as quantum correlations allow low brightness imaging, desirable for reducing the absorption dose. In particular, we develop a comprehensive model starting from a ghost imaging scheme elaborated for bright thermal light, known as differential ghost imaging and particularly suitable in the relevant case of faint or sparse objects. We perform the experiment using SPDC light in microscopic configuration. The image is reconstructed exploiting non-classical intensity correlation rather than photon pairs detection coincidences. On one side we validate the theoretical model and on the other we show the applicability of this technique by reconstructing a biological object with 5 micrometers resolution.
