Abstract

Background: Metabolic and bariatric surgery (MBS) is an effective treatment for adolescents with severe obesity. Objectives: This study examined the safety of MBS in adolescents during the coronavirus disease 2019 (COVID-19) pandemic. Methods: This was a global, multicentre and observational cohort study of MBS performed between May 01, 2020, and October 10,2020, in 68 centres from 24 countries. Data collection included in-hospital and 30-day COVID-19 and surgery-specific morbidity/mortality. Results: One hundred and seventy adolescent patients (mean age: 17.75 ± 1.30 years), mostly females (n=122, 71.8%), underwent MBS during the study period. The mean pre-operative weight and body mass index were 122.16 ± 15.92 kg and 43.7± 7.11 kg/m2, respectively. Although majority of patients had pre-operative testing for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) (n=146; 85.9%), only 42.4% (n=72) of the patients were asked to self-isolate pre-operatively. Two patients developed symptomatic SARS-CoV-2 infection post-operatively (1.2%). The overall complication rate was 5.3% (n=9). There was no mortality in this cohort. Conclusions: MBS in adolescents with obesity is safe during the COVID-19 pandemic when performed within the context of local precautionary procedures (such as pre-operative testing). The 30-day morbidity rates were similar to those reported pre-pandemic. These data will help facilitate the safe re-introduction of MBS services for this group of patients.

Abstract

Background: There is a paucity of data comparing 30-day morbidity and mortality of sleeve gastrectomy (SG), Roux-en-Y gastric bypass (RYGB), and one anastomosis gastric bypass (OAGB). This study aimed to compare the 30-day safety of SG, RYGB, and OAGB in propensity score-matched cohorts. Materials and methods: This analysis utilised data collected from the GENEVA study which was a multicentre observational cohort study of bariatric and metabolic surgery (BMS) in 185 centres across 42 countries between 01/05/2022 and 31/10/2020 during the Coronavirus Disease-2019 (COVID-19) pandemic. 30-day complications were categorised according to the Clavien–Dindo classification. Patients receiving SG, RYGB, or OAGB were propensity-matched according to baseline characteristics and 30-day complications were compared between groups. Results: In total, 6770 patients (SG 3983; OAGB 702; RYGB 2085) were included in this analysis. Prior to matching, RYGB was associated with highest 30-day complication rate (SG 5.8%; OAGB 7.5%; RYGB 8.0% (p = 0.006)). On multivariate regression modelling, Insulin-dependent type 2 diabetes mellitus and hypercholesterolaemia were associated with increased 30-day complications. Being a non-smoker was associated with reduced complication rates. When compared to SG as a reference category, RYGB, but not OAGB, was associated with an increased rate of 30-day complications. A total of 702 pairs of SG and OAGB were propensity score-matched. The complication rate in the SG group was 7.3% (n = 51) as compared to 7.5% (n = 53) in the OAGB group (p = 0.68). Similarly, 2085 pairs of SG and RYGB were propensity score-matched. The complication rate in the SG group was 6.1% (n = 127) as compared to 7.9% (n = 166) in the RYGB group (p = 0.09). And, 702 pairs of OAGB and RYGB were matched. The complication rate in both groups was the same at 7.5 % (n = 53; p = 0.07). Conclusions: This global study found no significant difference in the 30-day morbidity and mortality of SG, RYGB, and OAGB in propensity score-matched cohorts. © 2021, The Author(s).

Abstract

Objective: MicroRNAs represent an emerging class of small non-coding RNAs that play important roles in the post-transcriptional regulation of gene expression. This study was aimed to evaluate the relevance of microRNA in classical mixed cellularity Hodgkin lymphoma (MCHL) pathogenesis. Methods: The expressions of 157 microRNAs in lymph nodes from 20 paediatric and 20 adult patients with MCHL and 20 normal lymph nodes (controls) were analysed. Results: The patients' mean age was 7.4 years for the children and 47.4 years for the adults. Most of the patients were male (n = 14, 70%) in both groups. Stage III disease (n = 9, 45%) was common in the paediatric group, while stage II disease (n = 18, 90%) in others. Thirty-six cases (90%) were alive, while four (10%) were deceased. With microRNA sequencing, it was determined that miR-1273e, miR-4322, miR-5008-5p, and miR-6511b-5p were upregulated, while miR-508-5p was down-regulated in only paediatric tumours. Conclusion: These results suggest that microRNAs may play an important role in the biology of paediatric MCHL and may be useful in developing therapies targeting microRNAs. [ABSTRACT FROM AUTHOR]

Abstract

Neuroblastic tumours exhibit heterogeneity, which results in different therapeutic outcomes. Neuroblastoma is categorized into three major risk groups (low, intermediate, high risk). Recent identification of new genes raised the possibility of new biomarkers to identify sub-risk groups. In this retrospective cross-sectional study, we aimed to assess new biomarkers defining the ultra-high-risk subgroup within the high-risk group that differ in clinical situation with very bad prognosis. Twenty-five low- and 29 high-risk groups of patients were analysed for their expression of ALK, ATRX, HIF1a, HIF2a (EPAS), H2AFX, and ETV5 genes at the RNA level. Immunohistochemistry was performed to confirm the protein expression level of ALK. The risk group of patients was determined according to the International Neuroblastoma Risk Group Stratification System. Spearman correlation analysis and Mann-Whitney-U nonparametric test were used to assess the importance of expression levels among the groups. P < 0.05 was considered as significant. Sensitivity of the results was checked by ROC curve analysis. All analysed genes were found to be highly expressed in the high-risk group compared to the low-risk group, except for ETV5. When the ultra-high-risk and highrisk groups were compared, ALK was found to be highly expressed in the ultra-high-risk group. Our results show that ALK may be a candidate gene whose mRNA expression levels can distinguish the ultrahigh- risk subgroup of patients in the high-risk group of patients with non-familial neuroblastoma. Abstract

A laboratory-scale study is presented of ruthenium recovery from spent ruthenium sulphate bath solutions (from a decorative ruthenium-plating workshop) by a cementation reaction of Ru3+ with zinc powder in a temperature-controlled water bath at 20-70 degrees C and atmospheric pressure. The effects of quantity of zinc, reaction temperature, reaction time, shaking speed and addition of sodium chloride on the reaction are described. An activation energy of 12.48 kJ/mol for the ruthenium-recovery process is calculated, indicative of a diffusion-controlled reaction mechanism. The results show that addition of sodium chloride accelerates the cementation reaction and increases the ruthenium-precipitation efficiency. At 25 degrees C with no sodium chloride addition, the ruthenium recovery is less than 75%; at 65 degrees C with 1 000-mg sodium chloride addition, the use of 20 mg zinc, 10 ml solution, 200 rpm shaking and 2 h reaction time achieves a ruthenium recovery of over 95%., A laboratory-scale study is presented of ruthenium recovery from spent ruthenium sulphate bath solutions (from a decorative ruthenium-plating workshop) by a cementation reaction of Ru3+ with zinc powder in a temperature-controlled water bath at 20-70 degrees C and atmospheric pressure. The effects of quantity of zinc, reaction temperature, reaction time, shaking speed and addition of sodium chloride on the reaction are described. An activation energy of 12.48 kJ/mol for the ruthenium-recovery process is calculated, indicative of a diffusion-controlled reaction mechanism. The results show that addition of sodium chloride accelerates the cementation reaction and increases the ruthenium-precipitation efficiency. Abstract

Introduction: In this multicenter study, we analysed the magnitude of healthcare worker (HCW) [infection control practitioner (ICP), nurses and others] workforce in hospitals participated in the study. Materials and Methods: This study was performed in 41 hospitals (with intensive care units-ICU) located in 22 cities from seven regions of Turkey. We analysed the ICP workforce, nursing and auxiliary HCW (AHCW) workforce in ICUs, number of ICU beds and occupied beds in four different days [two of which were in summer during the vacation time (August 27 and 31, 2016) and two others in autumn (October 12 and 15, 2016)]. The Turkish Ministry of Health (TMOH) requires two patients per nurse in level 3 ICUs, three patients per nurse in level 2 ICUs and five patients per nurse in level 1 ICUs. There is no standardization for the number of AHCW in ICUs. Finally, one ICP per 150 hospital beds is required by TMOH. Results: The total number of ICUs, ICU beds and ICPs were 214, 2377 and 111, respectively in he 41 participated centers. The number ICPs was adequate only in 12 hospitals. Abstract

Introduction Ozone may promote moderate oxidative stress, which increases antioxidant endogenous systems. There are a number of antioxidants that have been investigated therapeutically for improving peripheral nerve regeneration. However, no previous studies have reported the effect of ozone therapy on facial nerve regeneration. Objective We aimed to evaluate the effect of ozone therapy on facial nerve regeneration. Methods Fourteen Wistar albino rats were randomly divided into two groups with experimental nerve crush injuries: a control group, which received saline treatment post-crush, and an experimental group, which received ozone treatment. All animals underwent surgery in which the left facial nerve was exposed and crushed. Treatment with saline or ozone began on the day of the nerve crush. Left facial nerve stimulation thresholds were measured before crush, immediately after crush, and after 30 days. After measuring nerve stimulation thresholds at 30 days post-injury, the crushed facial nerve was excised. All specimens were studied using light and electron microscopy. Results Post-crushing, the ozone-treated group had lower stimulation thresholds than the saline group. Although this did not achieve statistical significance, it is indicative of greater functional improvement in the ozone group. Significant differences were found in vascular congestion, macrovacuolization, and myelin thickness between the ozone and control groups. Significant differences were also found in axonal degeneration and myelin ultrastructure between the two groups. Conclusion We found that ozone therapy exerted beneficial effect on the regeneration of crushed facial nerves in rats. © 2016 Associação Brasileira de Otorrinolaringologia e Cirurgia Cérvico-Facial
