151. Recommendations for Vaccination in Adult Patients with Systemic Inflammatory Rheumatic Diseases from the Portuguese Society of Rheumatology

Abstract

Serious infections are a major cause of morbidity and mortality in systemic inflammatory rheumatic disease (SIRD) patients. Although vaccination may prevent numerous infections, vaccination uptake rates are low in this group of patients.To develop evidence-based recommendations for vaccination in SIRD patients.We searched MEDLINE (until 31 October 2014) and EMBASE (until 14 December 2014) databases, as well as the ACR and EULAR congress abstracts (2011-2014). Patients with any systemic inflammatory rheumatic disease were included and all vaccines were considered. Any safety and efficacy outcomes were admitted. Search results were submitted to title and abstract selection, followed by detailed review of suitable studies. Data were subsequently pooled according to the type of vaccine and the SIRD considered. Results were presented and discussed by a multidisciplinary panel and systematic literature review (SLR)-derived recommendations were voted according to the Delphi method. The level of agreement among rheumatologists was assessed using an online survey.Eight general and seven vaccine-specific recommendations were formulated. Briefly, immunization status should routinely be assessed in all SIRD patients. The National Vaccination Program should be followed and some additional vaccines are recommended. To maximize the efficacy of vaccination, vaccines should preferably be administered 4 weeks before starting immunosuppression or, if possible when disease activity is controlled. Non-live vaccines are safe in SIRD, including immunosuppressed patients. The safety of live attenuated vaccines in immunosuppressed patients deserves further ascertainment, but might be considered in particular situations.The present recommendations combine scientific evidence with the multidisciplinary expertise of our taskforce panel and attained desirable agreement among Portuguese rheumatologists. Vaccination recommendations need to be updated on a regular basis, as more scientific data regarding vaccination efficacy and safety, emergent infectious threats, new vaccines as well as new immunomodulatory therapies become available.

Além disso, com o fator ambiental que após a lei nº 12.305/10 de destinação de resíduos sólidos, a qual prevê a redução na geração de resíduos urbanos o gerenciamento de resíduos torna-se condição incontornável na gestão ou implantação de qualquer sistema ou processo. Tendo em vista essa demanda, o Lodo de Fossa Séptica (LFS) é utilizado nesse trabalho com o intuito de realizar uma análise referente ao potencial de geração de energia, e, ao mesmo tempo, sugerindo um modo auto-sustentável de tratamento termoquímico visando a redução do seu volume. Portanto, a influência da umidade do material e do tipo de configuração do leito de combustão (co-corrente ou contra-corrente) foi estudada. Inicialmente, foi realizada uma análise do poder calorífico superior (PCS) do lodo em função da sua umidade, e os resultados obtidos mostraram que para uma redução da umidade de 20% até que o lodo esteja completamente seco, ou seja, a 0%, o PCS elevou-se de 7487 até 8480 kJ.kg-1, o que representa um aumento de, aproximadamente, 14% no potencial energético do LFS. Em seguida, foi realizada a análise da combustão smouldering contra-corrente do lodo em função da umidade. Tal análise mostrou que a redução da umidade acarreta no aumento da temperatura máxima obtida no processo (962 até 1026°C) e, além disso, gera um aumento da velocidade de propagação da frente de combustão, de 2,43 até 2,99 mm.min-1. A análise referente ao processo co-corrente apresentou resultados similares, de forma que, com a redução da umidade elevou-se a temperatura máxima de operação e a velocidade de propagação da frente de combustão. Finalmente, comparou-se os resultados do processo contra-corrente e co-corrente, apontando que para o mesmo teor de umidade, o processo co-corrente apresentou maiores temperaturas e menores velocidades de propagação da frente de combustão. Paralelamente às análises supracitadas, foi verificado o teor de material inerte no lodo comparando a massa da amostra antes e após o experimento. Todos os ensaios apresentaram teor próximo de 52% de matéria inerte.

Abstract

The Strategic Implementation Plan of the European Innovation Partnership on Active and Healthy Ageing (EIP on AHA) proposed six Action Groups. After almost three years of activity, many achievements have been obtained through commitments or collaborative work of the Action Groups. However, they have often worked in silos and, consequently, synergies between Action Groups have been proposed to strengthen the triple win of the EIP on AHA. The paper presents the methodology and current status of the Task Force on EIP on AHA synergies. Synergies are in line with the Action Groups’ new Renovated Action Plan (2016-2018) to ensure that their future objectives are coherent and fully connected. The outcomes and impact of synergies are using the Monitoring and Assessment Framework for the EIP on AHA (MAFEIP). Eight proposals for synergies have been approved by the Task Force: Five cross-cutting synergies which can be used for all current and future synergies as they consider overarching domains (appropriate polypharmacy, citizen empowerment, teaching and coaching on AHA, deployment of synergies to EU regions, Responsible Research and Innovation), and three cross-cutting synergies focussing on current Action Group activities (falls, frailty, integrated care and chronic respiratory diseases).

Abstract

The Strategic Implementation Plan of the European Innovation Partnership on Active and Healthy Ageing (EIP on AHA) proposed six Action Groups. After almost three years of activity, many achievements have been obtained through commitments or collaborative work of the Action Groups. However, they have often worked in silos and, consequently, synergies between Action Groups have been proposed to strengthen the triple win of the EIP on AHA. The paper presents the methodology and current status of the Task Force on EIP on AHA synergies. Synergies are in line with the Action Groups' new Renovated Action Plan (2016-2018) to ensure that their future objectives are coherent and fully connected. The outcomes and impact of synergies are using the Monitoring and Assessment Framework for the EIP on AHA (MAFEIP). Eight proposals for synergies have been approved by the Task Force: Five cross-cutting synergies which can be used for all current and future synergies as they consider overarching domains (appropriate polypharmacy, citizen empowerment, teaching and coaching on AHA, deployment of synergies to EU regions, Responsible Research and Innovation), and three cross-cutting synergies focussing on current Action Group activities (falls, frailty, integrated care and chronic respiratory diseases).

Abstract

In the present work a process of catalytic wet air oxidation of lignin obtained from sugar-cane bagasse is developed with the objective of producing vanillin, syringaldehyde and p-hydroxybenzaldehyde in a continuous regime. Palladium supported on g-alumina was used as the catalyst. The reactions in the lignin degradation and aldehyde production were described by a kinetic model as a system of complex parallel and series reactions, in which pseudo-first-order steps are found. For the purpose of producing aromatic aldehydes in continuous regime, a three-phase fluidized reactor was built, and it was operated using atmospheric air as the oxidizer. The best yield in aromatic aldehydes was of 12%. The experimental results were compatible with those values obtained by the pseudo-heterogeneous axial dispersion model (PHADM) applied to the liquid phase.

Abstract

Objective: The present study aimed to describe breast-feeding, complementary feeding and determining factors for early complementary feeding from birth to 8 months of age in a typical Brazilian low-income urban community.Design: A birth cohort was conducted (n 233), with data collection twice weekly, allowing close observation of breast-feeding, complementary feeding introduction and description of the WHO core indicators on infant and young child feeding. Infant feeding practices were related to socio-economic status (SES), assessed by Water/sanitation, wealth measured by a set of eight Assets, Maternal education and monthly household Income (WAMI index). Two logistic regression models were constructed to evaluate risk factors associated with early complementary feeding.Results: Based on twice weekly follow-up, 65 % of the children received exclusive breast-feeding in the first month of life and 5 % in the sixth month. Complementary feeding was offered in the first month: 29 % of the children received water, 15 % infant formulas, 13 % other milks and 9·4 % grain-derived foods. At 6 months, dietary diversity and minimum acceptable diet were both 47 % and these increased to 69 % at 8 months. No breast-feeding within the first hour of birth was a risk factor for the early introduction of water (adjusted OR=4·68; 95 % CI 1·33, 16·47) and low WAMI index a risk factor for the early introduction of other milks (adjusted OR=0·00; 95 % CI 0·00, 0·02).Conclusions: Data suggest local policies should promote: (i) early breast-feeding initiation; (ii) SES, considering maternal education, income and household conditions; (iii) timely introduction of complementary feeding; and (iv) dietary diversity. Abstract: This work presents a novel texture design for implants surface functionalization, through the creation of line‐shaped textures on Ti6Al4V surfaces and subsequent sintering of hydroxyapatite (HAp) powder into the designated locations. HAp‐rich locations were designed to avoid HAp detachment during insertion, thus guaranteeing an effective osseointegration. This process starts by creating textured lines using a Nd:YAG laser, filling these lines with HAp powder and sintering HAp using a CO2 laser. The adhesion of HAp is known to be influenced by HAp sintering parameters, especially laser power and scanning speed and also by the textured lines manufacturing. Different laser parameters combinations were used to assess the sintering and adhesion of HAp to the textured lines. HAp adhesion was assessed by performing high energy ultrasonic cavitation tests and sliding tests mimicking an implant insertion, with Ti6Al4V/HAp specimens sliding against animal bone. The HAp content retained after these tests was measured and results showed that an excellent HAp sintering and adhesion was achieved when using a scan speed of 1 mm/s and laser power between 9 and 9.6 W. It is important to emphasize that results indicated that the HAp bioactivity was maintained when using these conditions, validating this functionalization process for the production of hip prosthesis with improved bioactivity. © 2017 Wiley Periodicals, Inc. 187. Effect of HAp and β‐TCP incorporation on the tribological response of Ti6Al4V biocomposites for implant parts.

Abstract

Abstract: Titanium and its alloys have been widely used in many engineering areas due to their properties. Despite having a high implant‐tissue osseointegration time, Ti6Al4V has been extensively used in prosthesis and articular implants. To promote a faster bone ingrowth and consequently reduce the implant fixation time, the addition of a bioactive phase to form a biocomposite seems to be an excellent solution. Because of their bioactivity and similarity in composition with the human bone, HAp and β‐TCP are two of the most widely used calcium phosphates in biomedical applications. To guarantee a strong adhesion of the previous bioactive materials in the implants surface, samples of Ti6Al4V, Ti6Al4V+HAp (10 vol %) and Ti6Al4V+β‐TCP (10 vol %) TCP were processed by the hot pressing technique. Tribological tests against Al2O3, lubricated in PBS at 37°C were carried out on a ball‐on‐flat reciprocating sliding geometry. Loads in the range of 3 N to 30 N were applied and their effect on the friction behavior and wear resistance of the tested materials was evaluated. Values of the coefficient of friction as well as the wear rate tend to increase with the addition of a bioactive phase to the Ti alloy. Micrographs of the worn surfaces showed that abrasion and plastic deformation are the prevailing wear mechanisms in the studied tribosystems. For biocomposites, particularly in the case of Ti6Al4V+HAp, pull‐out of bioactive particle clusters has a determinant role on the tribological response, increasing both the friction coefficient and the specific wear rate. © 2017 Wiley Periodicals, Inc. 189. Recommendations for Vaccination in Adult Patients with Systemic Inflammatory Rheumatic Diseases from the Portuguese Society of Rheumatology.

Abstract

Background: Serious infections are a major cause of morbidity and mortality in systemic inflammatory rheumatic disease (SIRD) patients. Although vaccination may prevent numerous infections, vaccination uptake rates are low in this group of patients. Objectives: To develop evidence-based recommendations for vaccination in SIRD patients. Methods: We searched MEDLINE (until 31 October 2014) and EMBASE (until 14 December 2014) databa - ses, as well as the ACR and EULAR congress abstracts (2011-2014). Patients with any systemic inflammatory rheumatic disease were included and all vaccines were considered. Any safety and efficacy outcomes were admitted. Search results were submitted to title and abstract selection, followed by detailed review of suitable studies. Data were subsequently pooled accor - ding to the type of vaccine and the SIRD considered. Results were presented and discussed by a multidisci- plinary panel and systematic literature review (SLR)- -derived recommendations were voted according to the Delphi method. The level of agreement among rheumatologists was assessed using an online survey. Results: Eight general and seven vaccine-specific re - commendations were formulated. Briefly, immunization status should routinely be assessed in all SIRD patients. The National Vaccination Program should be followed and some additional vaccines are recommended. To maximize the efficacy of vaccination, vaccines should preferably be administered 4 weeks before starting immunosuppression or, if possible when disease activity is controlled. Non-live vaccines are safe in SIRD, including immunosuppressed patients. The safety of live attenuated vaccines in immunosuppressed patients deserves further ascertainment, but might be considered in particular situations. Discussion: The present recommendations combine scientific evidence with the multidisciplinary expertise of our taskforce panel and attained desirable agreement among Portuguese rheumatologists. Vaccination re - commendations need to be updated on a regular basis, as more scientific data regarding vaccination efficacy and safety, emergent infectious threats, new vaccines as well as new immunomodulatory therapies become available. 190. Novel synthetic routes of large-pore magnetic mesoporous nanocomposites (SBA-15/Fe3O4) as potential multifunctional theranostic nanodevices.

Abstract

In this paper, novel magnetic silica nanocomposites were prepared by anchoring magnetite nanoparticles onto the outer surface of mesoporous SBA-15 silica; the magnetic nanoparticles were prepared by microemulsion and solvothermal methods, varying the synthesis conditions in order to control the final physicochemical, textural and magnetic properties. The morphology and mesostructure of the materials were characterized by X-ray diffraction (XRD), Fourier-Transform Infrared Spectroscopy (FTIR), N2 adsorption–desorption, and Transmission and Scanning Electron Microscopy (TEM and SEM). Magnetic silica nanocomposites feature a two-dimensional hexagonal arrangement constituted by a homogeneous pore channel system with diameters between 13 and 18 nm and a Brunauer–Emmett–Teller (BET) surface area higher than 260 m2 g−1. The different morphologies of the samples are given by the presence of diverse magnetic nanoparticle arrangements covalently linked onto the outer surface of the mesoporous silica rods. This confers on them a superparamagnetic behaviour with a magnetic response between 50–80 emu g−1, even though the weight percent of magnetite present in the samples does not exceed 21.7%. In addition, the magnetic nanocomposites exhibit magnetic hyperthermia with moderate Specific Absorption Rate (SAR) values. [ABSTRACT FROM AUTHOR]

199. Utilização de argilas esmectíticas do nordeste do Brasil para preparação de um adsorvente organofílico

Abstract

O principal objetivo foi a preparação e a utilização de uma argila organofílica com capacidade de promover a separação do composto fenol, encontrado em larga escala nos efluentes aquosos de indústrias petroquímicas. O trabalho consistiu no preparo de um material adsorvente, partindo de cinco tipos diferentes argilas retiradas de bacias localizadas na região nordeste do Brasil: Verde Lodo, Bofe e Chocolate, retiradas do município de Campina Grande, Boa Vista, PB; e Lagoa de Dentro e S. Jorge, do Pólo Gesseiro do Araripe, PE. As argilas in natura foram tratadas com uma solução de carbonato de sódio visando a troca dos poli-cátions existentes em suas regiões intercamadas pelo cátion sódio, em um reator de bancada, com diferentes concentrações, tempo de contato e temperatura, de acordo com um planejamento experimental 2³. Como resposta ao tratamento com carbonato de sódio, fez-se a leitura das viscosidades plásticas (cP ou mPa.s) das suspensões aquosas desse material com 4,86% em massa. Os resultados mostraram que a argila Chocolate, na melhor condição do planejamento experimental, atingiu uma viscosidade plástica de 4 mPa.s, sendo, portanto, escolhida como material adsorvente. Em seguida, este material foi submetido a um tratamento com sal quaternário de amônio em duas concentrações diferentes visando a troca catiônica de sódio por cátions amônio quaternário. Após o preparo do material adsorvente, foram realizados ensaios adsortivos de equilíbrio, sendo verificado o efeito da concentração do sal quaternário de amônio no processo adsortivo de remoção do fenol. A eficiência adsortiva, para uma relação de massa 1 g de argila organofílica e volume de solução fenólica igual a 100 mL, com concentrações iniciais entre 20 e 110 mg.L-1, foi de aproximadamente 70%. The goal of this research was the preparation and use of organophilic clay capable of promoting the separation of phenol, found in large scale in the aqueous effluent of petrochemical industries. The studies consisted of preparing adsorbent material, based on five different types of clay taken from ponds located in the Northeast of Brazil, which are: Verde Lodo, Bofe and Chocolate, from Campina Grande, Boa Vista, PB, Brazil, and Lagoa de Dentro and S. Jorge, from Araripe's pole of plaster in PE, Brazil. In experimental scale the clays in natura were treated with a solution of sodium carbonate, to the exchange of poly-cations exist in their interlayer regions by the sodium cation, used different concentrations, temperature and time of contact, according to a experimental design 2³. In response to treatment with sodium carbonate, was reading the plastic viscosity (cP or mPa.s) of aqueous suspensions of this material with 4.86% by weight. The result showed that the Chocolate clay, in the best condition of the experimental design, has a plastic viscosity equal to 4 mPa.s and therefore was chosen as adsorbent material to continue the study. The Chocolate clay was subjected to a treatment with quaternary aµmonium salt, in two different concentrations, to cation exchange of sodium by quaternary aµmonium cations. After preparing the adsorbent material, were tested adsorptive equilibrium was verified the effect of the concentration of the quaternary aµmonium salt in the adsorptive process to removal of phenol. The adsorptive efficiency for a ratio (wt/vv) of 1 g of organophilic clay in 100 mL of phenolic solution, with initial concentrations between 20 and 110 mg.L-1, was approximately 70%.
