Relevance. Assessment of the functional state of the body is one of the leading tasks of physiology. The article deals with the analysis of the initial vegetative status of students with different levels of motor activity. Materials and Methods. Registration and analysis of the heart rate variability was carried out with the help of a modern complex electrophysiological laboratory CONAN - 4.5. The heart activity of students engaged in physical culture within the educational process was evaluated on the basis of heart rate variability analysis. Results and Discussion. It was revealed that among the entire studied array of students (with the differentiation of the initial vegetative status calculated according to muscle tension index), normotonics are characterized by an optimal ratio between the parasympathetic and sympathetic divisions of the autonomic nervous system. At the same time, the value of the coefficient of physical activity in the studied group was determined at the level of 1.730.1. Conclusion. For vagotonics, the value of the triangular index was 2.50.2 conventional units (CU), which confirms the idea of an increase in the influence on the autonomic nervous system. The value for normotonics is 2.20.1 CU. This group was characterized by the balance between the sympathetic and parasympathetic parts of the autonomic nervous system. In sympathicotonics - 1.90.5 CU, which confirms the idea of increasing the influence of the sympathetic division of the autonomic nervous system. In hypersympathicotonics-1.10.4 CU. To ensure adequate functioning of the cardiovascular system and for normal adaptation to physical exertion in students, it is necessary to form a level of motor activity that quantitatively corresponds to a coefficient of physical activity of at least 1.75.

Abstract Preterm infants are deficient in long-chain polyunsaturated fatty acids, especially docosahexaenoic acid (DHA), a fatty acid (FA) associated with an increase in bronchopulmonary dysplasia (BPD). In two previous randomized control trials, DHA supplementation did not reduce the risk of BPD. We examined the breast milk FA profile, collected 14 days after birth, of mothers who delivered before 29 weeks of gestation and who were supplemented with DHA-rich algae oil or a placebo within 72 h after birth as part of the MOBYDIck trial. Milk FA were analyzed by gas chromatography. The total amount of FA (mg/mL) was similar in both groups but the supplementation increased DHA (expressed as % of total FA, mean ± SD, treatment vs placebo, 0.95 ± 0.44% vs 0.34 ± 0.20%; P

Background: The CardioSTAT is a single-lead ambulatory electrocardiography monitor that has been validated for use in adult patients. Recording is made through 2 electrodes positioned in a lead-I configuration, and the device allows monitoring for 2, 7, or 14 days. We sought to investigate the efficacy of this device in children with paroxysmal palpitations. Methods: In phase I, the quality of tracings from simultaneous CardioSTAT recordings and D1-lead recordings of a standard 12-lead electrocardiography machine in 23 children were compared. Phase II was a prospective observational cohort study comparing arrhythmia detection using the CardioSTAT vs currently used devices (24-hour Holter monitor and the Cardiomemo loop recorder) in 52 children complaining of palpitations. Results: In Phase I, all but 3 rhythm strips were correctly identified. The pacing spikes on 3 strips were not adequately identified by the observers for the CardioSTAT recording. In Phase II, symptomatic episodes were reported in 42%, 73%, and 100% of subjects during monitoring with the Holter, Cardiomemo, and CardioSTATdevices, respectively. An abnormal rhythm was detected in 13%, 23%, and 35% of subjects by the Holter, Cardiomemo, and CardioSTAT monitors, respectively. The underlying rhythm during symptomatic events was determined in 90% of cases with the CardioSTAT monitor, whereas it was determined in only 19% and 29% of cases using the Holter and Cardiomemo monitors, respectively. Conclusions: The CardioSTAT monitor provided good-quality tracings and was superior to the 24-hour Holter monitor and the Cardiomemo loop recorder in determining the presence or absence of pathologic arrhythmia in the study cohort. Résumé: Contexte: Le CardioSTAT est un moniteur d’électrocardiographie ambulatoire à dérivation unique dont l’utilisation a été validée chez les patients adultes. L’enregistrement se fait au moyen de 2 électrodes positionnées dans une configuration de type « dérivation I », et l’appareil permet un suivi pendant 2, 7 ou 14 jours. Nous avons cherché à étudier l’efficacité de cet appareil chez les enfants atteints de palpitations paroxystiques. Méthodologie: Dans la phase I, la qualité des tracés provenant d’enregistrements simultanés de CardioSTAT et d’enregistrements de la dérivation I d’un appareil d’électrocardiographie standard à 12 dérivations a été comparée chez 23 enfants. La phase II était une étude de cohorte observationnelle prospective comparant la détection de l’arythmie à l’aide du moniteur CardioSTAT par rapport aux appareils utilisés actuellement (moniteur Holter à surveillance sur 24 heures et enregistreur d’événements portatif Cardiomémo) chez 52 enfants se plaignant de palpitations. Résultats: Dans la phase I, toutes les bandes d’enregistrement sauf trois ont été correctement identifiées. Les spicules de stimulation n’ont pas été correctement détectés par les observateurs sur trois bandes d’enregistrement du moniteur CardioSTAT. Dans la phase II, des épisodes symptomatiques ont été signalés chez 42 %, 73 % et 100 % des sujets pendant la surveillance avec les appareils Holter, Cardiomémo et CardioSTAT, respectivement. Un rythme anormal a été détecté chez 13 %, 23 % et 35 % des sujets par les moniteurs Holter, Cardiomémo et CardioSTAT, respectivement. Le rythme sous-jacent pendant les événements symptomatiques a été déterminé dans 90 % des cas avec le moniteur CardioSTAT, alors qu’il n’a été déterminé que dans 19 % et 29 % des cas, respectivement, avec les moniteurs Holter et Cardiomémo. Conclusions: Le moniteur CardioSTAT a fourni des tracés de bonne qualité et s’est révélé supérieur à l’appareil Holter à surveillance sur 24 heures et à l’enregistreur d’événements portatif Cardiomémo pour déterminer la présence ou l’absence d’arythmie pathologique dans la cohorte étudiée.

Abstract Background Precision medicine, as a personalized medicine approach based on biomarkers, is a booming field. In general, physicians and patients have a positive attitude toward precision medicine, but their knowledge and experience are limited. In this study, we aimed at assessing the expectations and educational needs for precision medicine among rheumatologists, rheumatology fellows and patients with rheumatic diseases in Canada. Methods We conducted two anonymous online surveys between June and August 2018, one with rheumatologists and fellows and one with patients assessing precision medicine expectations and educational needs. Descriptive statistics were performed. Results 45 rheumatologists, 6 fellows and 277 patients answered the survey. 78% of rheumatologists and fellows and 97.1% of patients would like to receive training on precision medicine. Most rheumatologists and fellows agreed that precision medicine tests are relevant to medical practice (73.5%) with benefits such as helping to determine prognosis (58.9%), diagnosis (79.4%) and avoid treatment toxicity (61.8%). They are less convinced of their usefulness in helping to choose the most effective treatment and to improve patient adherence (23.5%). Most patients were eager to take precision medicine tests that could predict disease prognosis (92.4%), treatment response (98.1%) or drug toxicity (93.4%), but they feared potential negative impacts like loss of insurability (62.2%) and high cost of the test (57.5%). Conclusions Our study showed that rheumatologists and patients in Canada are overall interested in getting additional precision medicine education. Indeed, while convinced of the potential benefits of precision medicine tests, most physicians don’t feel confident in their abilities and consider their training insufficient to incorporate them into clinical practice.

Background and Objectives: Index equal to the spectral power of the low-frequency oscillations from the time series of the time intervals between the hearts contractions are often used when investigating the cardiovascular system. Experimental studies have shown that this spectral index was a preclinical marker of cardiovascular diseases, including arterial hypertension and diabetes. However, physiological understanding of this index, in particular, its relation to the tone of autonomic control is still largely not understood. Materials and Methods: This problem was studied using mathematical models of the cardiovascular system, which simulated a healthy subject and an arterial hypertension patient. Conclusion: The decrease in the power of low-frequency oscillations in the time-series of the time intervals between the heart contractions in arterial hypertension patients was due to decreased dynamic range of the arterial baroreceptors.

Abstract Background Early recurrences of atrial arrhythmias (ERAA) after atrial fibrillation (AF) catheter ablation do not predict procedural failure. A well-demarcated homogeneous lesion delivered by cryoballoon is less arrhythmogenic, and the recommended three-months blanking period may not refer to cryoballoon ablation (CBA). Objective We aimed to evaluate the predictive role of ERAA after second-generation CBA using an implantable loop recorder. Methods This prospective observational study enrolled 100 patients (58 males, median age 58) with paroxysmal/persistent AF undergoing pulmonary vein (PV) CBA using second-generation cryoballoon with simultaneous ECG loop recorder implantation. The duration of follow-up was 12 months, with scheduled visits at 3, 6 and 12 months. Results 99 patients from 100 completed the 12-month follow-up period. ERAA occurred in 31.3 % of patients. 83.9 % of patients with ERAA also developed late recurrences. Abstract CRPS is a type of severe pain syndrome and can be triggered by previous surgery or trauma. CRPS involves vasomotor changes such as changes in color and temperature of the skin, edema, increased sensitivity to touch, and a limited range of movement. Depending on the presence of nerve damage, CRPS is divided into two types. CRPS type II is associated with a confirmed peripheral nerve injury, while CRPS type I is not associated with an apparent peripheral nerve injury. Despite the ongoing therapy, sometimes, patients still have persistent, burning pain. Intractable CRPS that fail more conservative treatments may undergo neuromodulation. We want to present to your attention a case report of the successful treatment of a patient with CRPS type II using epidural unilateral stimulation. The 44‐year‐old woman came to us with complaints of burning pain and numbness of 1–3 fingers of the right hand, the lateral surface of the right wrist, and lower quarter of the forearm, and shooting pain in the projection of the right median nerve from the shoulder to the wrist. A clinical diagnosis was made—CRPS type II. During the stimulation trial, the most effective pain relief was obtained when the electrode was located in the right side of epidural space at the C4‐Th1 level. The implantation of a pulse generator was performed, and the final selection of the stimulation parameters was carried out (Pulse width: 60 ms, Rate: 210 Hz, and Amplitude: 0.9–1.6 V). The severity of pain syndrome was measured using validated scales in the preoperative period (VAS: 8–9, Pain Detect: 22, NTSS‐9: 4.62, and DN4: 8), in the early postoperative period (VAS: 0–1, Pain Detect: 6, NTSS −9: 0.66, and DN4: 1), and after 12 months (VAS: 0–2, Pain Detect: 6, NTSS‐9: 0.99, and DN4: 1). Observation during 12 months showed that a stable analgesic effect of neurostimulation was achieved using standard neuromodulation regimens and the adaptive stim option. Unilateral stimulation is an effective type of SCS in the treatment of pain syndromes. adaptive stim is usually not applicable for lead implantation at the cervical level. Nevertheless, the rational use of stimulation at threshold values allowed our patient to use adaptive stim in a non‐standard situation.

Abstract Background Health literacy refers to the ability of individuals to gain access to, use, and understand health information and services in order to maintain a good health. It is especially important in nephrology due to the complexity of chronic kidney disease (CKD). The present study sought to define health literacy levels in patients followed in predialysis clinic, in-center dialysis (ICHD), peritoneal dialysis (PD) and home hemodialysis (HHD). Methods This transversal monocentric observational study analysed 363 patients between October 2016 and April 2017. The Brief Health Literacy Screen (BHLS) and the Health Literacy Questionnaire (HLQ) were used to measure health literacy. Multivariate linear regressions were used to compare the mean scores on the BHLS and HLQ, across the four groups. Results Patients on PD had a significantly higher BHLS’score than patients on ICHD (p = 0.04). HLQ’s scores differed across the groups: patients on HHD (p = 0.01) and PD (p = 0.002) were more likely to feel understood by their healthcare providers. Compared to ICHD, patients on HHD were more likely to have sufficient information to manage their health (p = 0.02), and patients in the predialysis clinic were more likely to report high abilities for health information appraisal (p

More precisely, we examined the proportion of blue light-filtering IOL in a wet AMD patients’ sample and compared it with a general North American pseudophakic population sample.Design: Retrospective case–control study.Methods: Case patients were diagnosed and treated for wet AMD and had prior IOL implantation at least 3 years before the diagnosis of wet AMD. Control patients were randomly selected among patients who had cataract surgery at our institution. They were exempt of AMD and paired for the year of surgery, sex and age at cataract surgery. A total of 196 patients were included in each study group.Results: Among patients with wet AMD, 62.8% had a blue light-filtering IOL compared with 63.3% among control patients (p = 0.92). Mean time between implantation and injection of anti-VEGF in AMD patients was 6.62 years (95% confidence interval (CI): 6.04– 7.19) in non-blue light-filtering IOL group and 5.76 years (95% CI: 5.41– 6.11) in blue light-filtering IOL group (p = 0.0120).Conclusion: No correlations could be established between the presence of a blue light filter in the IOL and the occurrence of wet AMD. AMD patients without blue light-filtering IOL were injected significantly later than patients with an IOL filtering blue light, which contradict the potential clinical benefit of the blue light filter.Keywords: blue-light filter, intraocular lens, cataract surgery, age-related macular degeneration, anti-VEGF injection
