151. PERDAS QUANTITATIVAS NA COLHEITA MECANIZADA DE SOJA EM FUNÇÃO DAS REGULAGENS INTERNAS DA MÁQUINA

Abstract

O aumento da produção agrícola passou a exigir maior qualidade e rapidez na colheita, já que a colheita da soja é um processo que influencia diretamente no rendimento final, devendo ser executada com o mínimo de perdas. O objetivo deste trabalho foi avaliar as perdas na colheita de soja em regulagens de abertura de côncavo e rotação do rotor. O ensaio foi conduzido em março de 2022, na fazenda Segredo, em Nova Crixás. O delineamento utilizado foi o inteiramente casualizado em esquema fatorial 2x2, sendo 2 regulagens de côncavo (12 mm e 14 mm) e duas rotações do rotor de trilha (526 rpm e 620 rpm), com dez repetições por tratamento. Foram avaliadas as perdas naturais (PN), perdas de plataforma (PP), perdas internas (PI), perdas totais da máquina (PTM), perdas totais da colheita (PTC) e umidade dos grãos. Os dados foram submetidos ao controle estatístico de processo com a confecção de cartas de controle por variáveis. As perdas naturais foram de 9,26 kg ha-1, totalizando 58,03% das perdas totais na colheita. As perdas da plataforma foram em média 4,22 kg ha-1, representando 62,99% das perdas totais da máquina, que ainda foram compostas pelas perdas internas, com 2,48 kg ha-1, não havendo diferenças entre os tratamentos, ficando abaixo dos limites aceitáveis.

153. Effect of Centralization on Surgical Outcome of Children Operated for Liver Tumors in Switzerland: A Retrospective Comparative Study

Abstract

Background: Pediatric liver surgery is complex, and complications are not uncommon. Centralization of highly specialized surgery has been shown to improve quality of care. In 2012, pediatric liver surgery was centralized in Switzerland in one national center. This study analyses results before and after centralization. Methods: Retrospective monocentric comparative study. Analysis of medical records of children (0–16 years) operated for any liver tumor between 1 January 2001 and 31 December 2020. Forty-one patients were included: 14 before centralization (before 1 January 2012) and 27 after centralization (after 1 January 2012). Epidemiological, pre-, intra-, and post-operative data were collected. Fischer’s exact and t-test were used to compare groups. Results: The two cohorts were homogeneous. Operating time was reduced, although not significantly, from 366 to 277 min. Length of postoperative stay and mortality were not statistically different between groups. Yet, after centralization, overall postoperative complication rate decreased significantly from 57% to 15% (p = 0.01), Clavien > III complications decreased from 50% to 7% (p < 0.01), and hepatic recurrences were also significantly reduced (40% to 5%, p = 0.03). Conclusion: Centralization of the surgical management of liver tumors in Switzerland has improved quality of care in our center by significantly reducing postoperative complications and hepatic recurrence.

Abstract

Dieu au Collège de France, le sixième volume de la collection « Passage des disciplines », est issu d’un colloque qui s’est tenu au Collège de France en juin 2021, après un retard d’une année provoqué par la pandémie. Il prenait la suite d’une série de quatre colloques organisés dans le cadre du programme de recherche « Passage des disciplines », portant sur l’histoire de l’émergence (ou de la disparition) des disciplines enseignées au Collège de France, ainsi que dans le haut enseignement parisien, français et international, à partir du poste d’observation privilégié qu’offre le Collège de France. « Dieu » n’est certes ni une nouvelle discipline au xixe siècle ni un « fondateur de discursivité », à l’instar des autres savants que Foucault qualifiait ainsi pour désigner des inventeurs non seulement de nouveaux discours, mais d’un nouveau continent discursif, d’une nouvelle épistémè. Dieu est manifestement un « inventeur de discursivité » très ancien et le continent discursif qu’il a inauguré est considérablement plus diffus que ceux qui portent les noms de Freud, Darwin, Einstein ou Durkheim, qui ont fait l’objet des colloques précédents, et pour les trois premiers, de trois volumes de la collection. Il nous a pourtant semblé possible de nous interroger, en collaboration avec Thomas Römer, titulaire de la chaire des milieux bibliques, sur la science de Dieu et sur la manière dont le Collège, à différents moments de son histoire, et, à travers cet établissement, l’université, le haut enseignement en général, la vie des idées ont fait leur place aux sciences des religions. Dieu au Collège de France, volume 6 of the « Passage des disciplines » series, includes texts that were presented during the symposium “Dieu at the Collège de France”, which was held in June 2021, after the delays caused by the pandemic, as part of “Passage des disciplines. Global history of the Collège de France, 19th-20th century”. This research program and the series associated with it examines the genesis of scientific and literary fields of knowledge, their mapping and their evolution during the 19th and 20th centuries, on both national and international levels, from the standpoint afforded by the history of the renewal of academic chairs at the Collège de France. “Dieu” is, to be sure, not a new 19th discipline, or a “founder of discursivity”, as Foucault called the scholars who had invented not only new discourses, but, more broadly, discursive continents, new “épistemes”. God is certainly an ancient “inventor of discursivity” and the discursive continent that he has launched is considerably more diffuse than those which come from Freud, Darwin, Einstein or Durkheim, which were the object of preceding symposia, and, for the first three ones, of the first three volumes of the series. Yet, it seemed to us possible to examine, in collaboration with Thomas Römer, holder of the chair The Hebrew Bible and its Contexts, the science of God and how the Collège de France, at different times of its history, and through this institution, the university, higher education in general, the life of ideas, made way for the sciences of religion.

Abstract

Background: Remifentanil, a synthetic opioid used for analgesia during cesarean sections, has been shown in ex vivo experiments to exert anti-apoptotic activity on immature mice brains. The present study aimed to characterize the impact of remifentanil on brain lesions using an in vivo model of excitotoxic neonatal brain injury.Methods: Postnatal day 2 (P2) mice received three intraperitoneal injections of remifentanil (500 ng/g over a 10-min period) or saline just before an intracortical injection of ibotenate (10 μg). Cerebral reactive oxygen species (ROS) production, cell death, in situ labeling of cortical caspase activity, astrogliosis, inflammation mediators, and lesion size were determined at various time points after ibotenate injection. Finally, behavioral tests were performed until P18.Results: In the injured neonatal brain, remifentanil significantly decreased ROS production, cortical caspase activity, DNA fragmentation, interleukin-1β levels, and reactive astrogliosis. At P7, the sizes of the ibotenate-induced lesions were significantly reduced by remifentanil treatment. Performance on negative geotaxis (P6-8) and grasping reflex (P10-12) tests was improved in the remifentanil group. At P18, a sex specificity was noticed; remifentanil-treated females spent more time in the open field center than did the controls, suggesting less anxiety in young female mice.Conclusions:In vivo exposure to remifentanil exerts a beneficial effect against excitotoxicity on the developing mouse brain, which is associated with a reduction in the size of ibotenate-induced brain lesion as well as prevention of some behavioral deficits in young mice. The long-term effect of neonatal exposure to remifentanil should be investigated.

Abstract

Purpose: Neurocritical care patients receive prolonged invasive mechanical ventilation (IMV), but there is poor specific information in this high-risk population about the liberation strategies of invasive mechanical ventilation. Methods: ENIO (NCT03400904) is an international, prospective observational study, in 73 intensive care units (ICUs) in 18 countries from 2018 to 2020. Neurocritical care patients with a Glasgow Coma Score (GCS) ≤ 12, receiving IMV ≥ 24 h, undergoing extubation attempt or tracheostomy were included. The primary endpoint was extubation failure by day 5. An extubation success prediction score was created, with 2/3 of patients randomly allocated to the training cohort and 1/3 to the validation cohort. Secondary endpoints were the duration of IMV and in-ICU mortality. Results: 1512 patients were included. Among the 1193 (78.9%) patients who underwent an extubation attempt, 231 (19.4%) failures were recorded. The score for successful extubation prediction retained 20 variables as independent predictors. The area under the curve (AUC) in the training cohort was 0.79 95% confidence interval (CI95) [0.71–0.87] and 0.71 CI95 [0.61–0.81] in the validation cohort. Patients with extubation failure displayed a longer IMV duration (14 [7–21] vs 6 [3–11] days) and a higher in-ICU mortality rate (8.7% vs 2.4%). Three hundred and nineteen (21.1%) patients underwent tracheostomy without extubation attempt. Patients with direct tracheostomy displayed a longer duration of IMV and higher in-ICU mortality than patients with an extubation attempt (success and failure). Conclusions: In neurocritical care patients, extubation failure is high and is associated with unfavourable outcomes. A score could predict extubation success in multiple settings. However, it will be mandatory to validate our findings in another prospective independent cohort.

Abstract

In this study, we identified new lipid species associated with the loss of pancreatic β-cells triggering diabetes. We performed lipidomics measurements on serum from prediabetic mice lacking β-cell prohibitin-2 (a model of monogenic diabetes) patients without previous history of diabetes but scheduled for pancreaticoduodenectomy resulting in the acute reduction of their β-cell mass (∼50%), and patients with type 2 diabetes (T2D). We found lysophosphatidylinositols (lysoPIs) were the main circulating lipid species altered in prediabetic mice. The changes were confirmed in the patients with acute reduction of their β-cell mass and in those with T2D. Increased lysoPIs significantly correlated with HbA1c (reflecting glycemic control), fasting glycemia, and disposition index, and did not correlate with insulin resistance or obesity in human patients with T2D. INS-1E β-cells as well as pancreatic islets isolated from nondiabetic mice and human donors exposed to exogenous lysoPIs showed potentiated glucose-stimulated and basal insulin secretion. Finally, addition of exogenous lysoPIs partially rescued impaired glucose-stimulated insulin secretion in islets from mice and humans in the diabetic state. Overall, lysoPIs appear to be lipid species upregulated in the prediabetic stage associated with the loss of β-cells and that support the secretory function of the remaining β-cells.
