Cerebrospinal fluid rhinorrhea is an uncommon but dangerous disease. Many lethal complications, such as bacterial meningitis and pneumoencephalus, may be the result of cerebrospinal fluid rhinorrhea. Otolaryngologist, neurosurgeons and radiologists must know how to diagnose, how to localize the site of leakage and how to choose the best method of treatment. A case of cerebrospinal fluid rhinorrhea and meningitis due to improper sinus surgery is presented. Satisfactory result, such as avoidance of unnecessary brain tissue damage, can be obtained by extracranial endonasal repairing of the fistula with a composite septal flap.

We reported the purification and characterization of a pancreatic cancer-associated antigen from normal colonic mucosa (PCAAc) and pancreas-specific antigen (PaA) according to Kitada's and Loor's methods as well as the established method of double antibody enzyme-linked immunoassay for the detection of serum PaA and PCAAc; respectively. In our study, 155 serum samples were detected (including normal controls 40; pancreatic cancer 31; other malignant disease 56 and benign diseases 28). The results indicated that the overall sensitivity and specificity of a single PCAAc test for pancreatic cancer were calculated to be 70.98% and 96.43%; a single PaA to be 80.65% and 95.23%, respectively. Furthermore, the sensitivity and specificity of combined tests (PCAAc and/or PaA) for pancreatic cancer were calculated to be 96.77% and 90.91%. The diagnostic efficiency was 90.91%. None of 84 cases of nonpancreatic cancer and benign diseases has been found elevated PCAAc and PaA simultaneously. The results suggest that simultaneous detection of PCAAc and PaA would have increased the sensitivity of diagnosis for pancreatic cancer.

Effect of a magnetic field on martensitic transformation in the NiCoMnGa alloys was investigated. A field-induced reversible martensitic transformation from the martensitic phase of low magnetization to the parent phase of high magnetization has been realized. The substitution of Co for Ni atoms has turned the magnetic ordering of the parent phase from partially antiferromagnetic to ferromagnetic, resulting in a large magnetization change across the transformation, which dramatically enhances the magnetic field driving force. The transformation temperature can be downshifted by magnetic field at a rate up to 14K∕T in Ni37Co13Mn32Ga18. Other mechanism details were also discussed.

In this letter the authors present the observation of giant magnetothermal conductivity in NiMnIn single crystals. Upon cooling, a martensitic transformation is accompanied by a ferromagnetic metal→ferrimagnetic poor-metal transition. Most strikingly, this transition can be shifted to lower temperature and even totally suppressed by a magnetic field. The magnetic field-induced phase transition leads to a large magnetoresistance and a large magnetothermal conductivity up to 70% and 120%, respectively. The specific heat measurements indicate that the large magnetotransport properties are due to the increasing the density of free electrons, suggesting existence of superzone gap in the low-temperature, ferrimagnetic martensite.

Magnetic field-induced martensitic transformation was realized in Ni50−xCoxMn39Sb11 alloys. The partial substitution of Co for Ni has turned the antiferromagnetically aligned Mn moments in the starting material Ni50Mn39Sb11 into a ferromagnetic ordering, raising the magnetization at room temperature from 8emu∕g for NiMnSb to ∼110emu∕g for Ni41Co9Mn39Sb11. In the same quaternary sample, a magnetization difference up to 80emu∕g was measured across the martensitic transformation, and the transformation temperature (T0=259K) could be lowered by 35K under a field of 10T. Also a magnetoresistance over 70% was observed through this field-induced transformation.

Variation of electrical resistance in single-crystalline Ni50Mn50−xInx alloys (x=14–16) upon martensitic transformation was investigated. In Ni50Mn35In15 with Tm∼295K, a negative magnetoresistance (MR) over 60% is attainable at moderate field strengths; in Ni50Mn34In16 with Tm∼190K, the MR can exceed 70% over a temperature of approximately 100K. The significant change in electric resistance upon martensitic transformation originates primarily from the altered electronic structure, while the large effect of a magnetic field follows its ability to manipulate the transformation in materials of low Tm and large ΔM∕ΔS. The extremely large MR promises more innovative applications for these important alloys.

Context Glucose-insulin-potassium (GIK) infusion is a widely applicable, low-cost therapy that has been postulated to improve mortality in patients with acute ST-segment elevation myocardial infarction (STEMI). Given the potential global importance of GIK infusion, a large, adequately powered randomized trial is required to determine the effect of GIK on mortality in patients with STEMI. Objective To determine the effect of high-dose GIK infusion on mortality in patients with STEMI. Design, setting, and participants Randomized controlled trial conducted in 470 centers worldwide among 20,201 patients with STEMI who presented within 12 hours of symptom onset. The mean age of patients was 58.6 years, and evidence-based therapies were commonly used. Intervention Patients were randomly assigned to receive GIK intravenous infusion for 24 hours plus usual care (n = 10,091) or to receive usual care alone (controls; n = 10,110). Main outcome measures Mortality, cardiac arrest, cardiogenic shock, and reinfarction at 30 days after randomization. Results At 30 days, 976 control patients (9.7%) and 1004 GIK infusion patients (10.0%) died (hazard ratio [HR], 1.03; 95% confidence interval [CI], 0.95-1.13; P = .45). There were no significant differences in the rates of cardiac arrest (1.5% [151/10 107] in control and 1.4% [139/10,088] in GIK infusion; HR, 0.93; 95% CI, 0.74-1.17; P = .51), cardiogenic shock (6.3% [640/10 107] vs 6.6% [667/10 088]; HR, 1.05; 95% CI, 0.94-1.17; P = .38), or reinfarction (2.4% [246/10,107] vs 2.3% [236/10,088]; HR, 0.98; 95% CI, 0.82-1.17; P = .81). The rates of heart failure at 7 days after randomization were also similar between the groups (16.9% [1711/10,107] vs 17.1% [1721/10,088]; HR, 1.01; 95% CI, 0.95-1.08; P = .72). The lack of benefit of GIK infusion on mortality was consistent in prespecified subgroups, including in those with and without diabetes, in those presenting with and without heart failure, in those presenting early and later after symptom onset, and in those receiving and not receiving reperfusion therapy (thrombolysis or primary percutaneous coronary intervention). Conclusion In this large, international randomized trial, high-dose GIK infusion had a neutral effect on mortality, cardiac arrest, and cardiogenic shock in patients with acute STEMI.

Two distinct chloride (Cl - ) species were detected on and/or in the passive oxides of polycrystalline Al samples, which were anodically polarized below the stable pitting potential in Cl - -containing solutions. Chloride was found to be present as an adsorbed specie at the surface of the Al oxide, as well as an incorporated specie within the passive oxide. The two species of Cl - were recorded by X-ray absorption near edge structure (XANES), using both an electron yield detector and an X-ray fluorescence detector, and by X-ray photoelectron spectroscopy (XPS). Electron yield XANES and XPS results indicate that adsorbed Cl - migrates from the solution/Al oxide interface into the passive Al oxide film, prior to stable pit initiation. Cl - migration occurs once a critical anodic potential or critical adsorbed Cl - concentration is reached. The migration of Cl - is followed by a loss of oxidized Al from the passivating film, as determined by XPS, and can be attributed to (i) metastable pitting events or (ii) oxide dissolution. The ingress of Cl - into the oxide appears to be a key factor for the onset of metastable pitting or passive film dissolution.

The formation of daily growth increments of otoliths was studied in the reared larvae of milkfish. The first growth increment was formed during the yolk–sac reabsorption period c.2 days after hatching, and increment formation continued on a daily schedule regardless of growth rate. The initial incremental zone was of amorphous structure, and the subsequent incremental zone was of needle–shaped crystalline structure; the former structure was formed in the yolk–sac reabsorption period of the larva and the latter in the exogenous feeding period. Accordingly, ageing of milkfish larvae is possible by counting growth increments, and timings within the developmental stage of the larvae can be understood by examining the otolith microstructure.

Under the same process conditions in the conventional diffusion and anisotropic etching technology, short channel MOST with V-shape and U-shape arc fabricated under the same substrate conditions by the standard photolithographical technology. The short channel effect in which the threshold voltage and transconductance vary with the channel length is measured for both VMOS and UMOS structures. Detailed comparisons between both structures are made, and a simple physical interpretation is given

Carriers are injected into the delay path of the BARITT structure with zero drift velocity, hence, diffusion can not be neglected in the small signal analysis. The inclusion of a diffusion region adjacent to the injection plane produces a more physically reasonable approach to the small signal analysis. This analysis is accomplished through a regional approximation. The three regions are the diffusion region (where diffusion dominates the conduction process) a low field region (where drift dominates but the carrier velocity has not reached the saturation velocity) and a high field region (where the carriers drift with a saturated velocity). The inclusion of the diffusion region removes the zero-velocity singularity which results in the conventional analysis and accounts for an appreciable delay which has been previously neglected. The device characteristics are optimized for various device parameters. The optimized device length and most probable value of the negative resistance are given, and a qualitative discussion of the physical origin of the behavior of the device characteristics is given.

This paper presents a theoretical analysis of the second‐breakdown phenomenon. The analysis makes the use of the coupled current‐continuity, the Fourier heat‐flow, and the Poisson equations. The geometry used in the analysis allows the directions of current and heat flow to be separated self‐consistently. The trigger temperature is derived and a plot of its variation with respect to applied voltage is given. The approximate expressions for the steady‐state voltage states is also given.

Lung collagen was studied in hamsters which had been intratracheally instilled with elastase. Total and insoluble collagens in lungs among the elastase-instilled were higher than those of the age-matched controls and continued to be elevated even 4 months after the elastase-instillation. A transient increase in salt-soluble collagen during the period 6 to 20 days after the elastase-instillation was also found among the experimentals. Studies of incorporation of l -[ 14 C]proline into lung collagen showed higher specific radioactivities in both hydroxyproline and collagen fractions as well as a many-fold increase in total radioactivities in collagen fractions of lungs among the elastase-instilled hamsters. The results indicated that collagen metabolism in hamster lungs was activated after the elastase-injury which led to an increase of collagen in lung.

Pulmonary arterial occlusion (PAO) produces multiple alterations in the physiological/biochemical environment of lung cells as well as dysfunction of the lung's surfactant system, which is considered to play a significant role in mediating lung injury. The present studies were performed using 66 excised dog lungs to evaluate the impact of alterations in ventilation, substrate availability, alveolar CO2 tension, hydrogen ion and bicarbonate concentrations, and temperature and neural denervation on the lamellar body (LB) volume density of type II pneumocytes. Ventilating excised nonperfused dog lungs with room air (0% CO2) for 4 h at 38 degrees C resulted in severe reductions (6813;77%) in LB volume density. Supplementing inspired gas with 5% CO2 prevented LB depletion, while ventilation with 2.5% CO2 moderated the severity of depletion to 17–27% of control. Ventilation with 10% CO2 tended to increase LB volume density by increasing the number of LBs per cell, whereas reductions in LB volume density predominantly resulted from a decrease in LB size. The level of ventilation had no significant effect on LB volume density independent of inspired CO2 concentration. Reducing temperature to 5 degrees C prevented LB depletion. Lung perfusion with autologous whole blood failed to moderate the severity of LB depletion during room air ventilation despite the increased availability of metabolic substrates for cellular metabolism. Adding hydrochloric acid to maintain physiologically normal hydrogen ion concentrations in the perfusing blood had a small effect in ameliorating the severity of LB depletion. These results indicate that alveolar CO2 tension and bicarbonate concentration are major factors regulating the LB content of type II pneumocytes and suggest an important link between the gas exchange and phospholipid metabolic functions of the lung.

The current‐voltage characteristics of the p+np+ diode are derived for bias voltages below reach‐through condition. The electron current continuity equation is used to find the relationship between the voltage across the forward‐biased junction and the total applied voltage. For a narrow central neutral region a joint probability is defined so as to give the probability that a carrier has a velocity above some minimum value and travels across the neutral region without being scattered. The thermionic emission current is then obtained, the J‐V characteristics is plotted, and the singularity from the conventional theory at the reach‐through is then removed.

The cross-links histidinoalanine (HA); pyridinoline (Pyr); desmosine (Des); and isodesmosine (Ides) in human atherosclerotic aortas were studied. Only HA showed a significant increase in calcified aortas, with a high concentration in the insoluble "mineralized" fraction, which was separated out after treatment of tissues with pronase E. The cross-links composition was similar among "mineralized" fractions prepared from tissues of varying degrees of calcification: values were 2.40; 0.10; 0.17; and 0.16 moles per 1000 moles of amino acid residues for HA; Pyr; Des; and Ides, respectively. The findings suggest that the HA-containing peptide may play an important role in the calcification process of aortic tissues.

Chronic hypoxic pulmonary hypertension in the rat is associated with increased collagen and elastin in the pulmonary artery. We investigated whether excess vascular collagen contributes to chronic hypoxic pulmonary hypertension by administering the proline analogue cis-4-hydroxy-L-proline (cHyp), an agent relatively specific for inhibiting collagen production, to rats exposed to chronic hypoxia. Sprague-Dawley rats (weighting 200 g) were exposed to air or hypoxia (10% O2-90% N2) for 3 wk. Groups studied were: air-exposed injected with saline, air-exposed injected with cHyp, hypoxic injected with saline, and hypoxic injected with cHyp. At the end of 3 wk, we measured mean right ventricular pressure (RVP) of animals breathing room air and hydroxyproline and desmosine contents of the main pulmonary artery trunk. Hypoxia increased RVP from 13 +/- 1 to 27 +/- 1 mm Hg (p less than 0.05); cHyp partially prevented this increase since RVP was 17 +/- 1 mm Hg (p less than 0.05). There was no effect of cHyp on cardiac output. Hypoxia increased collagen from 0.9 +/- 0.1 to 2.0 +/- 0.3 mg/artery (p less than 0.05); cHyp completely prevented this increase since collagen was 1.0 +/- 0.2 mg/artery (p less than 0.05). Hypoxia increased elastin from 1.1 +/- 0.1 to 2.4 +/- 0.2 mg/artery (p less than 0.05); cHyp had no apparent effect since elastin was 2.1 +/- 0.1 mg/artery. Also, cHyp did not affect RVP or vascular collagen or elastin in air-breathing animals. The cHyp treatment prevented luminal narrowing and thickening of arteriolar walls by hypoxia.(ABSTRACT TRUNCATED AT 250 WORDS)

Both clinical and experimental evidence implicate proteolytic enzymes active against elastin in the pathogenesis of emphysema. Paradoxically, however, the elastin content of emphysematous human lungs at autopsy has been normal. When emphysema was produced in hamsters by a single intratracheal injection of 25 units of porcine pancreatic elastase, the elastin content of the lungs was reduced from 1.40 +/- 0.22 mg. in controls to 0.43 +/- 0.10 mg. 24 hours after injection, and histologic sections showed that many elastic fibers had disappeared. The elastin content of the lungs gradually increased, approaching normal values by 2 months after injection. The incorporation of 14C-proline into elastin was markedly elevated during the first 2 weeks after injection, decreasing nearly to normal by 2 months. The synthesis of collagen was also increased, indicated by an increase in the collagen content of the lung, an increase in the prolyl hydroxylase activity, and an increase in incorporation of labeled proline into collagen. During the period of active resynthesis of elastin, small clumps of microfibrils and elastic fibrils were visible by electron microscopy within grooves on the surface of septal connective tissue cells in the lungs. Many elastic fibers seen in histologic sections up to 4 months after injection were of abnormal configuration and disorganized.

NBT-PABA test was performed in 72 subjects with simultaneous determination of the plasma PABA time-concentration curve. The curve in 25 controls after oral administration of N-benzoyl-L-tyrosyl-P-aminobenzoic acid (NBT-PABA) peaked (3.88 +/- 0.78 micrograms/ml, M +/- SD) at the third hour. In patients with chronic pancreatitis (24) and pancreatic carcinoma (6), the curves were flattened, and peaked at the fifth hour (2.7 +/- 1.4 micrograms/ml, M +/- SD), and fourth hour (3.14 +/- 2.26 micrograms/ml, M +/- SD), respectively. The discrimination between the controls and patients with chronic pancreatitis was most significant in the third-hour plasma PABA concentration (P less than 0.0001). This study shows that determination of the third-hour plasma PABA concentration is better in sensitivity and specificity than the 6-hour urinary excretion of PABA, suggesting that the third-hour plasma PABA concentration after oral administration of NBT-PABA might be a sensitive index of exocrine pancreatic function test.

The aerobic glycolysis rate and mitochondria-bound hexokinase activity of ascitic liver cancer cells are 8 and 50 times as high as those of the normal liver cells, and they can be inhibited by Na2SeO3 in concentration below 10 micrograms/ml. The inhibitory effect increases with selenium concentration. In contrast, Na2SeO3 at the same concentration does not significantly affect the glycolysis rate and mitochondria-bound hexokinase activity of the normal liver cells. The selective action of selenium on energy metabolism of the liver cancer cells may account for its inhibitory effect on cancer cells.

We studied damage and repair of lung connective tissue in rats exposed to toxic amounts of oxygen by measuring lung content of collagen and elastin and the number of collagen fragments in lung lavage fluid after exposure to 98% O2 for 60 h. Lung collagen was decreased 17%, and collagen fragments in lavage fluid were increased 4- to 5-fold in O2-exposed rats compared with those in control rats. No biochemical evidence of elastin degradation was found. Mild emphysematous changes and a leftward shift of fluid-filled, pressure-volume curves were induced within 2 wk after recovery from exposure to O2. Administration of the lathyrogen beta-aminopropionitrile worsened the emphysematous lesion produced by hyperoxia, suggesting that replacement of connective tissue during repair limits the extent of emphysema. We conclude that lung collagen is degraded and an emphysematous lesion is produced by relatively short exposure to toxic amounts of oxygen.

Serum ribonuclease (RNase) activity and its isoenzymes were determined by biochemical and PAGE electrophoretic separation technique in 20 patients with pancreatic cancer, in 27 with other gastroenterologic malignant tumors, 8 with acute pancreatitis, 7 with chronic pancreatitis, 5 with leukemia, 3 with chronic uremia of glomerulonephritis, and in 30 adult normal controls. Serum A1AT rocket immunoelectrophoresis and carcinoembryonic antigen (CEA) radioimmunoassay were also carried out simultaneously.

Degradation of elastin was studied in rats after intraperitoneal implantation of a suspension of 14C-elastin that had been prepared by labeling bovine elastin with 14C-formaldehyde. Most of the degradation of 14C-elastin, measured by releasing the radioactivity in urine and feces, occurred between 6 and 15 days after the implantation. During this period an increase of leukocytes and macrophages in the implanted site was observed. This followed a brief increase in neutrophils. Furthermore, an elevation of the elastolytic activity, measured by the 14C-elastin, was found in cell lysates of both macrophages and neutrophils, which were recovered by lavage of the peritoneal cavity. But the enzymatic activity was not found in the cell-free lavage. On the contrary, there was elastase inhibitor in the lavage, increased by the implantation. The mechanism of the elastin degradation in the peritoneal cavity was discussed.

Desmosine is a cross-link amino acid unique to elastin. Previous work has shown that during turnover in the body, desmosine is not reused, and that desmosine is not absorbed from the intestine. Instead, all desmosine released in the course of elastin metabolism is excreted in the urine attached to low molecular weight peptides. Therefore, measurement of desmosine in acid-hydrolysates of urine might be used to monitor elastin breakdown in several pathologic states, including pulmonary emphysema. In the present report, we have described a sensitive, highly specific radioimmunoassay capable of detecting as little as 200 pg of desmosine in acid-hydrolysates of urine. The assay was specific for desmosine; cross-reactivity with merodesmosine, isodesmosine, lysine, and mixed amino acids was 0.25%, 0.1%, less than 0.0003%, and 0%, respectively. Twenty-three normal, nonsmoking subjects had a mean 24-hr desmosine excretion of 47 +/- 15 microgram. In a group of smokers with evidence of chronic obstructive disease and/or lung infection, the values for desmosine excretion ranged from 40 to 400 microgram/24 h. Desmosine radioimmunoassay may find application in the study of diseases involving increased destruction of elastin in the body.

Sodium selenite in normal saline was administered intraperitoneally (1 mg/kg) into mice bearing ascitic hepatocarcinoma for 4 days. The cyclic AMP-dependent protein kinase isozymes (type I and type II) in normal liver and hepatocarcinoma cells were separated and assayed. The results show that the level of type I/II is markedly higher in hepatocarcinoma than in the normal liver cells. Sodium selenite is able to reduce it towards the normal level. Further analysis shows that the chief function of sodium selenite is to reduce the raised level of type I/II in hepatocarcinoma cells which, in fact, is due to the increase of total amount of type I cyclic AMP-dependent protein kinase. This paper presents the speculation that one of the mechanisms of the inhibitory effect of sodium selenite on carcinogenesis may be due to the selective action of this compound on the cyclic AMP-dependent protein kinase isozymes in tumor cells, thus inhibiting cancer cell division and facilitating differentiation and reversion.

After treating human lung adenocarcinoma cells by Na2SeO3 (1 microgram/ml) for 24 hours, its mitotic index decreased by 50%. The reduction rate was directly proportional to selenite concentration (1-5 micrograms/ml). In the meantime, the labelling index, growth rate and progression of the cell cycle were also inhibited. In contrast to the above observation, there was no marked change in the cell count, mitotic index, labelling index and the average silver granule number in the human embryonic lung diploid cells treated by Na2SeO3 (1-5 micrograms/ml) for 1-3 days. When human embryonic lung diploid cells were mix-cultured with human lung adenocarcinoma cells in the presence of 5 micrograms/ml Na2SeO3 for 24 hours, the former maintained a normal morphology, while the lung cancer cells showed heavily vacuolated cytoplasms and distorted nuclei.
