In Poland, the Aesculapian snake, critically endangered at the edge of its European range, inhabits the lower parts of the Bieszczady Mts. in the SE of the country. This research, carried out between 2009 and 2014, included fieldwork in areas selected on the basis of historical literature data and the results of interviews. 51 breeding mounds were deployed at historical and potential sites of this snake’s occurrence on the forested area of 220,886 ha as a supportive tool for detecting these reptiles. The 371 records of the Aesculapian snake obtained are displayed on maps showing its historical distribution in two periods (1958-1989 and 1995-1998) for comparison. Most records were collected in the centre of the species’ range in the Bieszczady Mts. Most of the breeding mounds (17 out of 29) in this area were colonized by the snake, whereas only two sites were colonized in the outside. The results show that the Aesculapian snake’s range in the Bieszczady Mts has been shrinking. However, most current single records outside the species’ centre of occurrence are from localities close to its historical range; this suggests that relict populations may be surviving there. The proportion of records on the breeding mounds grew in consecutive years. This suggests that breeding mounds deployed near the snake’s existing localities may be a useful tool for monitoring endangered oviparous snakes and their active conservation.

Przedmiotem szczególnej uwagi była analiza ram modalnych, których przekład na język polski wpłynął na ich semantyczno-strukturalną charakterystykę.

Introduction Needle biopsy of enlarged lymph nodes is an accepted method for the diagnostic workup of sarcoidosis, but the optimal endosonography‑guided approach is yet to be determined. Objectives The aim of our study was to assess the relative diagnostic yield of combined ultrasound‑guided needle aspiration (CUS‑b‑NA), which includes endobronchial ultrasound‑guided transbronchial needle aspiration (EBUS‑TBNA) with endoscopic ultrasound fine‑needle aspiration (EUS‑b‑FNA), as well as the role of the cell block (CB) technique and lymph node localization in the diagnostic workup of sarcoidosis. Patients and methods This was a prospective multicenter study including consecutive patients with clinical suspicion of stage I or II sarcoidosis. CUS‑b‑NA with smears and CB technique were performed in the whole study group. If a biopsy result was not conclusive, an invasive diagnostic workup and a 6-month follow‑up were scheduled. Results Out of 77 screened patients, 54 signed written consent and 50 were enrolled for the final analysis. The overall sensitivity of EBUS‑TBNA, EUS‑b‑FNA, and CUS‑b‑NA was 76.6%, 70.2%, and 91.7%, respectively. There were no differences between EBUS‑TBNA and EUS‑b‑FNA (P = 0.52) but CUS‑b‑NA had a higher diagnostic yield (P = 0.005 and P = 0.001, respectively). Adding the CB method to smear technique (P = 0.008) and biopsy of the subcarinal lymph nodes increased the diagnostic yield (P = 0.001). Conclusions: The diagnostic yield of CUS‑b‑NA is higher than that of endosonographic techniques alone in the diagnostic workup of stage I and II sarcoidosis. The preparation of cytological material including CB and the choice of the subcarinal lymph node station for the biopsy increase the diagnostic efficacy.

Background Induced sputum (IS) allows to measure mediators of asthmatic inflammation in bronchial secretions. NSAID‐exacerbated respiratory disease (NERD) is recognized as a distinct asthma phenotype, usually with a severe course, eosinophilic airway inflammation, and increased production of pro‐inflammatory eicosanoids. A more insightful analysis of NERD patients has shown this phenotype to be nonhomogeneous. Objective We aimed to identify possible subphenotypes in a cohort of NERD patients with the means of latent class analysis (LCA). Methods A total of 95 asthma patients with aspirin hypersensitivity underwent sputum induction. High‐performance liquid chromatography or gas chromatography coupled with mass spectrometry was used to profile eicosanoids in induced sputum supernatant (ISS). Sixteen variables covering clinical characteristics, IS inflammatory cells, and eicosanoids were considered in the LCA. Results Three classes (subphenotypes) were distinguished within the NERD cohort. Class 1 subjects had mild‐to‐moderate asthma, an almost equal distribution of inflammatory cell patterns, the lowest concentrations of eicosanoids, and logLTE4/logPGE2 ratio. Class 2 represented severe asthma with impaired lung function despite high doses of steroids. High sputum eosinophilia was in line with higher pro‐inflammatory LTE4 in ISS and the highest logLTE4/logPGE2 ratio. Class 3 subjects had mild‐to‐moderate asthma and were also characterized by eosinophilic airway inflammation, yet increased production of pro‐ (LTE4, PGD2 and 11‐dehydro‐TBX2) was balanced by anti‐inflammatory PGE2. The value of logLTE4/logPGE2 was between values calculated for classes 1 and 3, similarly to disease control and severity. Conclusions LCA revealed three distinct NERD subphenotypes. Our results support a more complex pathobiology of aspirin hypersensitivity. Considering NERD heterogeneity, the relationship between inflammatory pathways and clinical manifestations of asthma may lead to more individualized treatment in difficult to treat patients in the future., Heterogeneity of NERD phenotype reflects differences in inflammatory response measured by airway cells and eicosanoids. Identifying subphenotypes provides a more insightful perception and suggests a need for more individualized approach. Although eutrophication of freshwaters is a natural process, the human impact often leads to inland waters becoming overloaded with nutrients, impoverishing many valuable and vanishing habitats, such as floodplain lakes. These changes need to be reversed if the occurrence of endangered aquatic species is to be restored. In this paper we analyse the impact of a change in the water regime of a naturally eutrophic floodplain lake, which harbours a large diversity of Unionidae (large freshwater mussels), a globally threatened taxonomic group that provides important ecosystem functions and services. We found that a slight increase in the discharge from this waterbody, following the construction of an additional outflow pipe, positively influenced recruitment in three of the five mussel species inhabiting the lake. We also found that, after the construction of this additional outflow, the niches of juveniles of Anodonta cygnea and Unio spp. changed, revealing differences in their hydrological requirements. Our results suggest that, as in lotic habitats, complex hydraulic parameters are highly significant to unionid mussels in lentic conditions.

This paper compares the Anderson-Darling and some Eicker-Jaeschke statistics to the classical unweighted Kolmogorov-Smirnov statistic. The goal is to provide a quantitative comparison of such tests and to study real possibilities of using them to detect departures from the hypothesized distribution that occur in the tails. This contribution covers the case when under the alternative a moderately large portion of probability mass is allocated towards the tails. It is demonstrated that the approach allows for tractable, analytic comparison between the given test and the benchmark, and for reliable quantitative evaluation of weighted statistics. Finite sample results illustrate the proposed approach and confirm the theoretical findings. In the course of the investigation we also prove that a slight and natural modification of the solution proposed by Borovkov and Sycheva (1968) leads to a statistic which is a member of Eicker-Jaeschke class and can be considered an attractive competitor of the very popular supremum-type Anderson-Darling statistic.

BACKGROUND: Aortic valve stenosis (AS) generates a chronic pressure overload that induces left ventricular hypertrophy (LVH). The aim of this study was to assess the prevalence of the electrocardiographic criteria for LVH in patients with clinically significant AS and to evaluate the relationship between the ECG criteria for LVH and echocardiographic parameters. METHODS: The clinical data of 95 patients with moderate to severe AS were retrospectively analyzed. Eight ECG criteria for LVH were used and compared to the results of transthoracic echocardiography (TTE). RESULTS: In 59% of patients, at least one of the ECG criteria for LVH was found. These patients had a greater LVMI (142.1 ± 35.6 vs. 124.1 ± 22.5 g/m(2), p = 0.01) and peak aortic jet velocity (4.2 ± 0.8 vs. 3.8 ± 0.9 m/s, p = 0.01) along with smaller aortic valve area (0.72 ± 0.28 vs. 0.86 ± 0.22 cm(2), p = 0.02) compared to patients with a negative ECG for LVH. The ECG parameters had a low sensitivity (6%–36.9%) with a specificity of up to 100%. In this paper, we develop tests for positive expectation dependence. The proposed tests are based on weighted Kolmogorov−Smirnov type statistics. These originate from the function valued monotonic dependence function, describing local changes of the strength of the dependence. The resulting procedure is supported by a simple and insightful graphical device. This paper presents asymptotic and simulation results for such tests. We show that an inference relying on p -values and wild bootstrap allows to overcome inherent difficulties of this testing problem. Our simulations show that the new tests perform well in finite samples. A Danish fire insurance data set is examined to demonstrate the practical application of the proposed inference methods.

BACKGROUND: Video-assisted (VATS) lung lobectomy can be associated with stronger postoperative pain than is commonly believed. It is generally accepted to introduce multimodal analgaesic strategies based on regional blockade, opioids and non-steroidal anti-inflammatory drugs. However, there is still no consensus regarding the optimal regional technique. The aim of this study was to compare the analgaesic efficacy of continuous thoracic epidural block (TEA) and percutaneous continuous paravertebral block (PVB) in patients undergoing video-assisted lung lobectomy. METHODS: Fifty-one patients undergoing VATS lobectomy were enrolled in the present prospective, randomised clinical trial. The same analgaesic regimen in both groups included continuous infusion of 0.25% bupivacaine with epinephrine, intravenous ketoprofen and paracetamol. The doses of local anaesthetics were determined to achieve the spread of at least 4 segments in both groups. Postoperative static and dynamic visual analogue pain scores, as well as patient-controlled morphine usage, were used to compare the efficacy of analgaesia. Side effects and failure rates of both blocks were analysed. RESULTS: Static and dynamic pain scores at 24 postoperative hours were significantly lower in the paravertebral group, as were the static pain score at 36 and 48 postoperative hours ( P l 0.05). No difference between the treatment groups was identified regarding postoperative morphine usage. The failure rate was higher in the epidural group than in the paravertebral group. No complications were noted in either group, but side effects (urinary retention, hypotension) were more frequent in the epidural group ( P l 0.05). CONCLUSIONS: Postoperative pain following VATS lung resection procedures is significant and requires the application of complex analgaesic techniques. Percutaneous paravertebral block is equally effective as thoracic epidural block in providing analgaesia in patients undergoing VATS lobectomy. Paravertebral block has a better safety profile than thoracic epidural block.

The paper looks at the issues related to the research on and assessment of the contagion effect. Based on several examinations of two selected EU countries, Poland paired with one of the EU member states; it presents the interaction between their economic development. A DCC-GARCH model constructed for the purpose of the study was used to generate a covariance matrix Ht , which enabled the calculation of correlation matrices Rt . The resulting variance vectors were used to present a linear correlation model on which a further analysis of the contagion effect was based. The aim of the study was to test a contagion effect among selected EU countries in the years 2000–2014. The transmission channel under study was the GDP of a selected country. The empirical studies confirmed the existence of the contagion effect between the economic development of the Polish and selected EU economies.

Background:ANCA associated vasculitides (AAV) are a heterogeneous group of rare diseases with unknown etiology and the clinical spectrum ranging from life-threatening systemic disease, through single organ involvement to minor isolated skin changes. Thus there is an unmet need for phenotype identification especially among patients with granulomatosis with polyangiitis GPA, patients with microscopic polyangiitis MPA group seems to be more uniform. Recently, based on previous clustering analysis and clinical, histopathological, serological and prognostic aspects three subcategories of AAV have been proposed and named as: non-severe AAV, severe PR3-AAV and severe MPO-AAV [1].Objectives:In line with these attempts to subcategorize AAV we decided to use latent class analysis (LCA) on a large multicenter cohort of polish AAV patients from POLVAS [2] registry to identify potential new subphenotypes or confirm already proposed ones.Methods:Latent Class Analysis (LCA) approach was used as a model based clustering method of objects described by dichotomous (e.g., gender; ANCA status – cANCA, pANCA; organ involvement - skin, eye, ENT, respiratory, heart, GI, renal, urinary, CNS, peripheral nerves) and polytomous (number of relapses) variables supported by quantitative covariates (e.g., age at diagnosis, CRP at diagnosis, maximal serum creatinine concentration ever).Results:Results of LCA on our AAV group returned four class model of AAV subphenotypes, confirming existence of the previously proposed by Mahr at al. [1] and revealed fourth – previously not described clinically relevant subphenotype. Background Aspirin desensitization (AD) is an effective and safe therapeutic option for patients with nonsteroidal anti-inflammatory drugs (NSAIDs)-exacerbated respiratory disease (N-ERD). The mechanisms driving its beneficial effects remain poorly understood. Objective To investigate the effect of long-term AD on clinical, biochemical and radiological changes in N-ERD patients. Methods The study group consisted of twenty-three individuals with N-ERD who underwent AD, followed by ingestion of 325 mg aspirin twice daily. Twenty patients completed the 52 weeks of AD. The following evaluations were conducted at baseline and in the 52nd week of AD: (i) clinical: asthma exacerbations, Asthma Control Test (ACT), Visual Analogue Scale (VAS) for the assessment of nasal symptoms; (ii) blood and induced sputum supernatant (ISS) periostin, (iii) phenotypes based on induced sputum (IS) cells, (iiii) ISS and nasal lavage (NL) concentration of prostaglandin D2 (PGD2), prostaglandin E2 (PGE2), tetranor-PGD-M, tetranor-PGE-M, 8-iso-PGE2, leukotriene B4 (LTB4), LTC4, LTD4 and LTE4, and urine LTE4. Results A significant improvement was observed in ACT (P = 0.02) and VAS score (P = 0.008) in the 52nd week of AD. ISS periostin and IS eosinophil count decreased significantly in the 52nd week of AD (P = 0.04 and P = 0.01, respectively). ISS and NL eicosanoid concentrations did not change following long-term AD. Conclusion and Clinical Relevance: AD is associated with a decrease in sputum periostin biosynthesis, which may prevent the recruitment of eosinophils into respiratory tissue and be one of explanation of the clinical benefits of AD. Long-term aspirin administration does not lead to an imbalance between pro- and anti-inflammatory ISS eicosanoids.

BACKGROUND A special regulatory role for prostaglandin E2 (PGE2 ) has been postulated in nonsteroidal anti-inflammatory drug (NSAID)-exacerbated respiratory disease (NERD). OBJECTIVE To investigate the effect of systemic aspirin (acetylsalicylic acid) administration on airway PGE2 biosynthesis in induced sputum supernatant (ISS) among subjects with NERD or aspirin-tolerant asthma with chronic rhinosinusitis with nasal polyposis (ATA-CRSwNP), as well as healthy controls (HC). METHODS Induced sputum (IS) was collected from patients with NERD (n = 26), ATA-CRSwNP (n = 17), and HC (n = 21) at baseline and after aspirin challenge. Sputum differential cell count and IS supernatant (ISS) levels of prostanoids, PGE2 , 8-iso-PGE2 , tetranor-PGE-M, 8-iso-PGF2 α, and leukotriene C4 , D4 , and E4 , were determined using mass spectrometry. Urinary excretion of LTE4 was measured by ELISA. RESULTS NERD subjects had elevated sputum eosinophilic count as compared to ATA-CRSwNP and HC (median NERD 9.1%, ATA-CRSwNP 2.1%, and HC 0.4%; P

With a simple generic approach, we develop a classification that encodes and measures the strength of completeness (or compactness) properties in various types of spaces and ordered structures. The approach also allows us to encode notions of functions being contractive in these spaces and structures. As a sample of possible applications we discuss metric spaces, ultrametric spaces, ordered groups and fields, topological spaces, partially ordered sets, and lattices. We describe several notions of completeness in these spaces and structures and determine their respective strengths. In order to illustrate some consequences of the levels of strength, we give examples of generic fixed point theorems which then can be specialized to theorems in various applications which work with contracting functions and some completeness property of the underlying space. Ball spaces are nonempty sets of nonempty subsets of a given set. They are called spherically complete if every chain of balls has a nonempty intersection. This is all that is needed for the encoding of completeness notions. We discuss operations on the sets of balls to determine when they lead to larger sets of balls; if so, then the properties of the so obtained new ball spaces are determined. The operations can lead to increased level of strength, or to ball spaces of newly constructed structures, such as products. Further, the general framework makes it possible to transfer concepts and approaches from one application to the other; as examples we discuss theorems analogous to the Knaster--Tarski Fixed Point Theorem for lattices and theorems analogous to the Tychonoff Theorem for topological spaces.

Populations at the edge of their main range of distribution are often exposed to suboptimal environments. They therefore exhibit a greater susceptibility towards habitat changes and often clearly differ in their structure and dynamics from populations in the main range. Here we define population parameters of the Aesculapian snake Zamenis longissimus, a species endangered in Europe, at its northern range limit based on data obtained in the valley of the River San (Bieszczady Mts., SE Poland) in 2009-2013. We focus on the spatial and temporal sex ratio patterns by comparing data obtained in 2009-2013 with published records from 1990 to 1998 and five other northern populations (1 from the main range, 1 at the northern edge of its contiguous range, and 3 entirely isolated ones). Using the capture-mark-recapture (CMR) method, we estimated the population size to be about 230 snakes. Our data show that among adults the percentage of females (17.74%) in the San valley population was significantly lower than in those other five populations and the same population studied two decades earlier. Because of the male-biased sex ratio, we estimated the effective population size to be about 74 snakes. This disproportion between the sexes in adults may be interpreted as an early stage of the population extinction process at the northern range limit of this species’ distribution. It is probably due to the limited availability of egg-laying sites, compelling females to undertake longer movements, which may heighten the risk of mortality. Purpose: Very small intracranial aneurysms (VSIAs) may cause many neurological complications and even death. Thanks to technological progress and higher quality of non-invasive neuroimaging methods, these pathologies can be investigated sooner and treated earlier. Due to the controversy surrounding invasive treatment of these pathologies, the aim of the study was to analyse methods of treatment, their outcome, and complications in a group of patients with VSIAs. Material and methods: Out of 444 cases of intracranial aneurysms treated in our centre, 65 aneurysms met the radiological criteria of VSIAs. The parameters - width and length of the aneurysm's neck and width, length, and height of the aneurysm's dome - were measured. The analysed parameters were as follows: symptoms upon admission and after treatment, days in hospital, and intraoperative complications. Clinical and radiological intensity of subarachnoid haemorrhage (SAH) was evaluated by using the Hunt-Hess and Fisher scales. The degree of embolisation of the aneurysm after the procedure was assessed using the Montreal Scale. Clinical outcome was assessed by Glasgow Outcome Scale. Results: 50.77% of VSIAs were treated with endovascular procedures and 49.23% with neurosurgical clipping. SAH was presented in 38.46% of patients with VSIAs. Intraoperative complications were presented in 16.92% of patients with VSIAs, and the most common complication was ischaemic stroke. Stents were used in 51.52% of VSIAs. In 69.70% of embolisation procedures at VSIAs complete obliteration was achieved. The average result in the Montreal Scale was 1.31 (SD = 0.66). Conclusion: VSIAs can be treated as effectively and safely as larger aneurysms, by both endovascular and surgical methods.

The problem of testing hypothesis that a density function has no more than μ derivatives versus it has more than μ derivatives is considered. For a solution, the L 2 norms of wavelet orthogonal projections on some orthogonal “differences” of spaces from a multiresolution analysis is used. For the construction of the smoothness test an asymptotic distribution of a smoothness estimator is used. To analyze that asymptotic distribution, a new technique of enrichment procedure is proposed. The finite sample behavior of the smoothness test is demonstrated in a numerical experiment in case of determination if a density function is continuous or discontinuous.

The basic motivation and primary goal of this paper is a qualitative evaluation of the performance of a new weighted statistic for a nonparametric test for stochastic dominance based on two samples, which was introduced in Ledwina and Wy{\l}upek (2012). For this purpose, we elaborate a useful variant of Kallenberg's notion of intermediate efficiency. This variant is general enough to be applicable to other nonparametric problems. We provide a formal defnition of the proposed variant of intermediate efficiency, describe the technical tools used in its calculation, and provide proofs of related asymptotic results. Next, we apply this approach to calculating the intermediate efficiency of the new test with respect to the classical one-sided Kolmogorov-Smirnov test, which is a recognized standard for this problem. It turns out that for a very large class of convergent alternatives the new test is more efficient than the classical one. We also report the results of an extensive simulation study on the powers of the tests considered, which shows that the new variant of intermediate efficiency reflects the exact behavior of the power well., Comment: 41 pages, 4 figures

Cel – Celem artykulu jest zaprezentowanie analizy dokonywanych wydatkow na ochrone zdrowia w Polsce z uwzglednieniem stochastycznych rownan rozniczkowych. Metodologia badania – Analize zaprezentowano z wykorzystaniem modelu procesu stochastycznego przyrostow kosztu otrzymując przedstawione w pracy rownanie roznicowe i modelu przedstawiającego zaleznośc miedzy kosztem, tempem wzrostu i przyspieszeniem wzrostu określonym w rownaniu rozniczkowym. Wynik – Przeprowadzona analiza wydatkow na ochrone zdrowia pozwolila na zaprezentowanie na podstawie wyjściowego szeregu czasowego modelu stochastycznego rozwoju ponoszonych kosztow. Model ten pozwala na wyznaczenie prognoz kosztu na lata nastepne. Oryginalnośc/wartośc – Badania ukierunkowano i podporządkowano wykorzystaniu teorii procesow stochastycznych do zaprezentowania analizy kosztow.

W niniejszym referacie przedstawiono analize spoleczenstwa informacyjnego Polski oraz wybranych panstw (Luksemburg, Bulgaria, Niemcy, Wielka Brytania). Jako okres analizy przyjeto lata od roku 2005 do roku 2015. Zaprezentowano konstrukcje oraz otrzymaną postac pelnoczynnikowego modelu GARCH. Celem zaprezentowanych w niniejszej pracy badan jest analiza wspolzalezności ksztaltowania sie rozwoju spoleczenstwa informacyjnego Polski i wybranych panstw UE. Przedstawione zostaną wielorownaniowe modele GARCH, prezentujące wzajemne relatywne powiązania w zakresie dynamiki rozkladow empirycznych ze szczegolnym zwroceniem uwagi na dynamike wartości oczekiwanych i wariancji.
