Background: The overall goal of this study was to better understand how language can affect the mental states in a manner that can increase the well-being of people who have experienced major physical or psychological trauma. Analysis: We chose to answer this question through a qualitative empirical investigation in which we interviewed 49 individuals (patients, caregivers, professionals). To get at the heart of the "performativity" of language and its ability to alter mental states, we identified phrases and words that "do good" for traumatized individuals. Conclusion and implications: Examining these words and phrases allowed us to identify broad themes that we then linked to basic psychological needs and elementary linguistic morphologies. These helped us to grasp the basic structure on which it is possible to build conversational tools for psychosocial intervention. [ABSTRACT FROM AUTHOR]

De nos jours, le recours à la médecine traditionnelle est de plus en plus fréquent dans le monde. C’est ainsi qu’en Afrique, les médicaments traditionnels à base de plantes dont la qualité, l’efficacité et l’innocuité sont établies par des méthodes scientifiques dont les essais cliniques, sont considérés comme une alternative crédible. Cette revue vise à faire le point sur les cadres éthique et réglementaire des essais cliniques des médicaments traditionnels à base de plantes. Elle est basée sur des recherches dans Scopus, Science Direct et Medline. Les données indiquent que les cadres éthique et réglementaire existant sont le plus souvent édictés sous forme de recommandations dans le monde, qui doivent être transcrites dans les législations sous-régionales ou nationales pour jouir d’une force contraignante. En Afrique de l’Ouest en particulier, il n’existe pas de cadre harmonisé contraignant et la pratique des essais cliniques des médicaments traditionnels à base de plantes reste faible. Cependant, dans un souci d’harmonisation et d’application immédiate du cadre juridique des essais cliniques des médicaments à usage humain, l’Europe a opté pour les Règlements qui de facto s’imposent à tous les États membres. En outre, la législation européennepromet l’expérience d’usage traditionnel comme mesure d’exemption d’essais cliniques pour les médicaments à base de plantes. En général, le processus de régulation (approbation, inspection, pharmacovigilance) est relativement le même aussi bien pour les médicaments conventionnels que ceux traditionnels. L’expérience européenne d’exemption d’essais cliniques pour les médicaments traditionnels à base de plantes pourrait être capitalisée dans les pays et blocs régionaux africains pour promouvoir un cadre réglementaire harmonisé et adapté aux réalités socio-culturelles.

L'histoire des concepts et des principes éthiques en psychiatrie et leur évolution au cours des temps, de l'Antiquité à nos jours, est passée en revue en mettant en valeur les avancées au niveau des droits des patients. Un nouveau Code d'éthique de la WPA a été élaboré en 2020. Il succède à la Déclaration de Hawaï en 1977 et à celle de Madrid en 1996. Ce Code est divisé en quatre sections qui couvrent l'exercice de la psychiatrie dans la pratique clinique, l'éducation, la recherche et la publication ainsi que la santé mentale publique. Nous discuterons aussi les nouveaux enjeux éthiques que ce Code entraîne. This article reviews the concepts and history of ethical principles in psychiatry and their evolution over time from Antiquity to the present highlighting advances made in patient rights. We also discuss the WPA's new Code of Ethics that was developed in 2020 and which succeeds the Declaration of Hawaii of 1977 and the Declaration of Madrid of 1996. The Code is divided into four sections that cover the practice of psychiatry in clinical practice, education, research and publication as well as in public mental health. Ces dernières années, la communauté médicale est traversée par les débats sociétaux autour du genre, du sexe, de la procréation et des droits humains. Aussi les questions autour de la prise en charge médicale des enfants et adolescents transgenres viennent elles très régulièrement sur le devant de la scène médiatique suscitant la mobilisation de collègues à l'origine de prise de positions réclamant entre autres l'interdiction de prescrire des traitements hormonaux jusqu'à 25 ans. Nous proposons dans cet article de reprendre point par point les principaux sujets de controverses autour de la prise en charge médicale contemporaine des transidentités de l'enfant et de l'adolescent, à savoir la transition sociale, le blocage pubertaire, les transitions hormonales, et les transitions chirurgicales, et de les mettre en perspective de la situation contemporaine en France. Si la plupart des études cliniques menées depuis plus de 25 ans montrent les impacts positifs des prises en charge médicales des adolescents transgenres sur leur devenir psychologique soulignant l'innocuité relative des traitements, l'accompagnement psychodynamique n'en demeure pas moins essentiel, prenant en compte la singularité de chaque patient au fil des rencontres. Le temps long est parfois requis, parfois contre-indiqué en fonction de chaque situation clinique. Rappelons qu'il n'y a aucune prescription médicamenteuse avant la puberté, et pour les adolescents la durée moyenne entre la première consultation (pour laquelle ils ont attendu souvent un an) et une éventuelle prescription est d'une année. Dans le contexte où la population des enfants et adolescents transgenres est particulièrement vulnérable, ne pas nuire n'est pas systématiquement s'abstenir de prescrire, chaque situation clinique devant être évaluée dans sa singularité avec précaution et discernement. Ces décisions de transition médicale sont discutées et validées si pertinentes, en France depuis 2015, dans le cadre de Réunions de Concertation Pluridisciplinaire. Par-delà les opinions et les débats sociétaux, de réels enjeux éthiques sont à considérer, en particulier autour de la notion de consentement libre et éclairé chez l'enfant et l'adolescent, et les recherches doivent se poursuivre. In recent years the medical community has been crossed by societal debates around gender, sex, procreation, and human rights. Also, questions around the medical care of transgender children and adolescents very regularly come to the fore in the media leading colleagues to seek prohibiting hormonal treatment until the age of 25 years. In the current report, we propose to explore all controversial proposals in the field of contemporary transgender care, i.e. : social transition, puberty suppression with GnRH, hormonal transition, and surgical transition. In addition, we expose the way such a practice is ongoing in France. If most of the clinical studies carried out internationally for more than 25 years have shown considerable positive impacts of the medical care of adolescents on their psychological future and emphasizing the relative safety of the treatments, the psychodynamic support remains essential considering the uniqueness of each patient over the course of the meetings. A long period of time is sometimes required, while sometimes it is contraindicated depending on each clinical situation. Drug prescriptions before puberty are not permitted, and for adolescents the average time between the first consultation (for which they often waited a year) and a possible prescription is one year. As transgender children and adolescents are a particularly vulnerable population, doing no harm does not always mean refraining from prescribing, and each clinical situation has to be assessed in its uniqueness with care and discernment. Since 2015 in France these decisions are taken after assessment during multidisciplinary concertation meetings. Beyond societal opinions and debates, real ethical issues must be considered, especially around the notion of free and informed consent in children and adolescents, and research must continue. The complexity of university teaching requires the development of studies that allow contrasting the experiences of teachers in different settings, so this work compares the characteristics of the training of students in three universities, one in Spain, one in Mexico and one in Venezuela. A non-experimental design research was carried out with a descriptive level using the Questionnaire “Indicators of Teaching Excellence at the University of Granada”. The results demonstrate the importance attributed in teaching to the following features: knowledge of the subject, planning, communication, didactics, evaluation, and reflection as mechanisms for permanent innovation. The relevance of evaluation and motivation were found as coincident elements in the three institutions, that demonstrates a shared vision of context-independent teaching. This study analyzes the ethical organizational development in early childhood and primary education centers in Catalonia and validates intervention proposals in this regard. The methodology used consisted of 18 exploratory interviews with managers, school inspectors, and experts in organizational ethics and ethical leadership. A total of 442 school principals were also surveyed through questionnaires. This study aims to delimit theoretically and practically the construct of organizational ethics, competences, professional functions of the school management team in this regard and organizational proposals to promote its development. This review addresses this gap by providing comprehensive guidance on how to calculate sample size in nutrition research. Emphasizing the importance of an a priori sample size calculation, the review outlines the key considerations, including the desired levels of significance and power, effect size estimation, and standard deviation assessment. Formulas for determining sample size for various comparisons, including two proportions, two means, three or more groups, and unevenly sized groups, are provided, along with strategies for addressing loss to follow-up. Hypothetical examples illustrate these formulas' application across different research scenarios, highlighting their practical value in ensuring study robustness. 185. [Study of the profile and trajectory of women seeking a maternal medical termination of pregnancy for psychosocial reasons at the University Hospital of Tours from 2017 to 2021].

Objective: The law of 7 July 2011 adopts the concept of therapeutic abortion (Medical Termination of Pregnancy or MTP) for maternal reasons if the pregnancy poses a "serious threat" to the woman's health, allowing for the introduction of the concept of psycho-social distress., Materials and Methods: This was a retrospective monocentric study involving 33 women seeking IMG for psychosocial reasons between January 2017 and December 2021 at the University Hospital of Tours. The study resulted in 29 approvals and 25 MTP procedures performed., Results: Eighty-seven per cent of the requests were accepted. Over half of the women were under 25 years old and were primiparous. The procedures were performed at a mean gestational age of 22 weeks. Sixty percent did not use contraception at the time of conception, and 36% did not use it after the MTP. About a third of the women experienced violence and had psychiatric histories. The majority of the women did not have stable economic resources. Fifty-six per cent of the MTP procedures involved feticide. There were 7 immediate complications with no subsequent consequences. The average time to completion was 16.5 days., Conclusion: This is a delicate and underexplored subject in literature. Although sporadic in France, these procedures have been practiced for many years. The diversity of opinions during multidisciplinary committees ensures the best objectivity. Requests for psychosocial IMG are rare, but responding to them is in accordance with the law. 186. An exploratory study of blind spot bias in psychiatrists: The value of mindfulness and ethical skills'.

Introduction: Psychiatry is an area of medicine that is particularly exposed to the risks of cognitive bias, notably because diagnosis is subjective. However, psychiatrists may develop psycho-cognitive skills that protect against bias, as the reflective approach to their patients favors the activation of system 2 (logical) as opposed to system 1 (intuitive) processes. Furthermore, the collegial practices found in psychiatry, a consequence of the ethical questions psychiatrists must address, can also mitigate the impact of bias. On the other hand, clinical ethics committees have adopted structured deliberation practices that aim to improve collective decision-making and limit the cognitive biases associated with groupthink., Objective: The primary aim was to compare blind spot bias in a group of psychiatrists and a group of 'ethicists', experts in group deliberation. The secondary objective was to study the distribution of skills influencing the balance between systems 1 and 2 (notably, ethical deliberation, feeling of efficacy, mindfulness, and values) in both groups. The general hypothesis is that psychiatrists are less prone to blind spot bias and are more competent in the skills studied., Methods: This descriptive, cross-sectional study was conducted from April 2022 to May 2023. Thirty-two psychiatrists were recruited from several French hospitals, along with 20 'ethicists'. All participants completed the following self-administered questionnaires: the Bias Blind Spot Scale (French version), the Euro Moral Case Deliberation (Euro-MCD, ethical skills), Bandura's self-efficacy scale (self and collective efficacy), the Freiburg Mindfulness Inventory (mindfulness), and the Schwartz Value Survey (values)., Results: The ratio of self/other blind spot bias was lower in the 'ethicists' group than the psychiatrists' group (P<0.001). Euro-MCD scores were higher (P<0.001) and psychiatrists tended to have a better sense of collective efficacy (P=0.08). No significant difference was found between the two groups for total mindfulness or presence and acceptance sub-scores. Scores for hedonic value were higher among psychiatrists (P<0.02)., Conclusion: The results do not confirm our hypotheses. There is less evidence of blind spot bias among our sample of 'ethicists', who are experts in group deliberation, than psychiatrists, who have experience of the patient/physician dyad. “To be with and to go towards” is the minimal definition of the verb to accompany. This perspective makes accompaniment a particular relationship based on the symbolic value of sharing (Paul, 2009). Talking about doctoral accompaniment thus marks a strong conceptual positioning. What are the ethical implications of such an approach and how can they be identified? This contribution addresses this question from an epistemological perspective. The hypothesis suggested is that despite the ethical charters of the doctorate that define the contours of supervision or more broadly of doctoral training and despite academic freedom determined as a general principle of university research, a blind spot linked to epistemological questions remains. Doctoral students are confronted with gaps in meaning and may feel alone, isolated, or even in distress. This contribution proposes to update the ethical implications of doctoral accompaniment by recognising the political dimension of knowledge, by putting the responsibility of researchers back at the centre, by encouraging commitment and by valuing the biographical dimension in the construction of knowledge. These implications are illustrated by examples of insolent acts of research in response to certain positivist injunctions that put doctoral students in difficulty.

Cet article fait état du débat académique sur la posture des universitaires allochtones en contextes autochtones et d’une analyse quant à la légitimité de l’exercice de la recherche, dans le cadre d’un projet doctoral sur l’évaluation de la recherche partenariale en architecture et en aménagement entre communautés autochtones et universités. Une reconnaissance de l’évolution de la recherche sur, avec, pour et par les communautés autochtones permet de comprendre l’importance des balises éthiques aujourd’hui en place. Un exposé des discours en présence, de la culpabilité coloniale paralysante à un rejet de toute responsabilité quant à la colonisation, permet de faire émerger la posture du trait d’union. « Travailler le trait d’union » implique de s’engager dans une démarche transformative et réflexive qui s’attarde aux tensions générées par les différences sans tenter de les effacer, en reconnaissant les impacts de sa présence comme universitaire, de sa posture et de ses privilèges, mais également de ses compétences.

A partir dos apontamentos contemporâneos sobre a ética liberal da comunicação, retomamos as discussões de Arendt e Dewey sobre as noções de público e privado, no sentido de situar em quais territórios estariam localizadas as liberdades de imprensa e de expressão. Posteriormente, partindo da noção de que essas distinções são performadas para reivindicar poder, argumentamos que o descompasso entre essas liberdades ora favorecem o público (regulações sociais), ora violentam os Direitos Humanos através da subalternidade. Diante do problema, sugerimos uma perspectiva de análise que seja alternativa a oposição entre essas liberdades, dadas em espaços rígidos, centrada nos seus atravessamentos, a partir de uma noção de “multiterritorialidade”. A partir de uma visada pós-colonialista, em função da discussão sobre a territorialidade, a subalternidade na assimetria dos espaços de escuta - para além da fala - se apresenta como marcador substancial para uma abordagem ética sobre a comunicação pública.

Résumé : L’iboïsme littéraire est une critique de restauration des fondements et de l’éthique littéraire. En s’appliquant à l’œuvre de Tchicaya U Tam’si, un poète négro-africain considéré par la critique comme le chef de file de « la seconde génération » de poètes négro-africains, cette méthode vise à montrer comment le discours poétique peut être reconfiguré d’un courant à l’autre, d’une époque à l’autre voire d’un territoire à l’autre, tout en maintenant son justificatif identitaire en tant que création de spécificité. Cela, en raison des facteurs esthétiques, éthiques et expressifs de l’œuvre. Le rapport de cette poésie à l’univers civilisationnel ainsi conféré à l’œuvre, n’est pas à négliger d’autant plus qu’il ouvre la voie à la compréhension de la conception littéraire sur ses bases fondamentales. Ainsi, la lecture ibotique de cette rupture initiée à partir de Tchicaya, permettra de mieux apprécier ce poème au regard de ses sources fondamentales, et partant, d’apprécier sereinement son expressivité à partir de ses fondements littéraires.

Face au développement du Big Data, l’intelligence artificielle (IA) a connu un regain d’intérêt depuis quelques années dans les débats scientifiques sur le traitement des données massives dans les organisations. Les universités ne font pas exception. En effet, l’analyse quantitative de données, tributaire des caractéristiques du big data: son Volume, sa Vélocité, sa Variété et sa Valeur, implique de mettre en œuvre des techniques de calcul avancées et de nouvelles modalités de gestion des données, dépassant les modèles classiques. Dans ce contexte, l’intelligence artificielle (IA) représente une opportunité pour les universités qui souhaitent mieux gérer et mieux exploiter leurs données, mais elle soulève aussi de nombreux enjeux au niveaux éthique et légal. Le but de cet article est de présenter les faits saillants d'une revue de littérature portant sur opportunités et promesses de l’IA en matière de traitement et d’analyse des données massives au sein de l’université marocaine et d’en discuter les risques et les enjeux éthiques. Nous concluons que l’IA présente un immense potentiel pour les universités et que les questions éthiques qu’elle soulève ne sont pas nouvelles. C’est le cas d’une part de la protection des données personnelles et de la vie privée des parties prenantes et d’autre part de l’explicabilité et de l’opacité des systèmes de l’IA. Enfin, certaines questions émergent, comme celles qui concernent les conséquences de l’automatisation sur le travail et l’agentivité humaine.

Entendues comme une rupture d'équilibre dans les conditions de pratique des soins selon un fonctionnement conforme à protocoles ou procédures établis, les situations de crise dans les services d'oncologie radiothérapie sont de causes multiples. Leur gravité peut parfois imposer des changements de paradigmes décisionnel, organisationnel ou technique. Une des possibles conséquences peut être la nécessité de procéder à des décisions de priorisation dans l'accès aux soins, lorsque s'installe une inadéquation entre les besoins de soins d'une population et les moyens sanitaires (techniques et humains) disponibles. Les spécificités des parcours de soin et la grande variété des situations cliniques en oncologie radiothérapie rendent ces décisions particulièrement délicates sur le plan éthique. Les réflexions d'anticipation, les procédures décisionnelles collégiales et multi professionnelles ou encore l'intégration des représentants d'usagers dans ces démarches de priorisation sont alors des outils indispensables. Une attention particulière doit être portée à l'information indispensable à fournir aux personnes malades dans un souci de transparence et de respect de la personne humaine. Les situations de priorisation sont d'authentiques mises à l'épreuve de nos services. Elles débordent le seul aspect technique du soin en oncologie radiothérapie. Elles peuvent être source d'une authentique souffrance éthique pour les soignants quand leurs valeurs soignantes se percutent aux limites imposées par les situations de crises. Understood as a disruption of the conditions of care practice according to established protocols or procedures, crisis situations in radiation oncology departments can have multiple causes. Their seriousness can sometimes impose changes in the decision-making, organizational or technical paradigms. A possible consequence may be the need to make prioritization decisions in access to care, when there is a mismatch between the care needs of a population and the available health resources (whether technical or human). The specificities of care pathways and the wide variety of clinical situations in radiation oncology make these ethical decisions particularly difficult. Anticipation, collegial and multi-professional decision-making procedures or the integration of patient representatives in these prioritization processes are essential tools. Particular attention must be paid to the information to be provided to patients in a concern of transparency and respect. Prioritization situations are real tests for our departments. They go beyond the purely technical aspect of radiation oncology. En radiothérapie, de multiples situations peuvent générer une situation de crise (panne de matériel, absentéisme, suractivité, erreur, prise en charge urgente, pandémie, etc.). Elle est ainsi définie comme une situation difficile dans le déroulement de l'activité quotidienne qui peut à tout moment échapper à la vigilance des acteurs concernés lorsqu'ils n'y sont pas préparés. La crise produit une rupture d'équilibre entre le déroulement normal de l'activité de traitement et l'obligation pour les manipulateurs de concilier les conséquences de la crise vécue avec l'objectif de prendre en charge tous les patients prévus au programme. La crise et sa gestion vont inexorablement engendrer stress, fatigue et en corollaire peuvent impacter la sécurité des patients. Le rôle de l'encadrement dans la dynamique de l'équipe a toute son importance lorsqu'une crise survient. L'objectif de cet article est ainsi d'illustrer ces situations pour mettre en évidence les besoins des équipes et plus spécifiquement des manipulateurs exerçant en radiothérapie. Il met en exergue l'organisation et la prise en charge des patients en radiothérapie en cas de gestion de crise (Coronavirus disease 2019 [Covid-19]), lors de radiothérapie palliative avec une opportunité de développement de pratique avancée. La réflexion sur la « souffrance éthique » du manipulateur dans la priorisation des patients nous amène à aborder les savoirs en éthique médicale apportés lors de la formation initiale. Le manipulateur semble confronté au cours de sa pratique à des situations qui font émerger des dilemmes d'ordre éthique. Il est alors question de comprendre comment celui-ci réfléchit autour de sa pratique afin de la faire évoluer. Le manipulateur exerçant en radiothérapie est-il confronté à des problématiques éthiques au cours de sa pratique ? Quelles sont les solutions mises en place afin de dépasser ces problématiques ? Many situations can be considered as a crisis in radiotherapy (failure of equipment, absenteeism, excessive workload, errors, urgent care, pandemic, etc.). Such events are defined as difficult situations in the course of daily activity, they can at any time escape the vigilance of the concerned actors when they are not prepared for it. The crisis can induce a disruption of balance between the normal course of the radiation therapy and the obligation for the manipulators to reconcile the consequences of the ongoing crisis with the objective of taking charge of all patients scheduled for the program. The crisis and its management will inexorably cause stress, fatigue and, as a consequence, impact patient safety. Management has an essential role to play in maintaining team dynamics when a crisis occurs, the work environment may no longer be the same. The aim of this article is to describe these situations in order to highlight the needs of the teams and more specifically the needs of the radiation therapist. It highlights the organization of patient care in the event of crisis management (Covid-19), during palliative radiotherapy with an opportunity to develop advanced practice. The reflection on the ethical concern of the radiation therapist in the prioritization of patients leads us to analyze the knowledge of medical ethics provided during initial training. When they perform their duties, manipulators seem to be confronted with situations that give rise to ethical dilemmas. It is then a question of understanding how they reflect on their practice in order to make it evolve. Is the radiation therapist confronted with ethical issues in the course of his practice? Through comments made by composer and improviser Pierre-Yves Martel on his collaboration with the Impatients, musicologist and teacher Augustin Tiffou on his pedagogical experience with students with disabilities, and philosopher Eric Lewis on the digital instrument aumi (Adaptive Use Musical Instrument), social, ethical, and political responsibility form the basis of our reflections on creativity and disability in particular, and on living together as a community in general. In the interviews collected for this enquête, making music together appears as a very marked form of community living, and creativity opens a space where, politically, we can fight against the segregation of people with disabilities. Introduction: The objectives of this study was to conduct an updated comparative audit involving a larger and more representative group of accredited chiropractic programs in order to determine if (i) if there has been any changes in the delivery of JEB curricula since the first audit was conducted in 2010, and (ii) provide recommendations that could lead toward a standardized or model JEB curriculum worldwide. Methods: This study was approved by the ERB of the University of South Wales. Twenty-one chiropractic programs agreed to provide JEB course outlines for review. Results: A total of 88 different course outlines, which listed 83 different topics pertaining to JEB course content, were submitted for review. Conclusion: The results of this comparative audit revealed there has been an increase in the variability of JEB course content taught to students over time. Recommendations are provided for the next steps that could lead toward a standardized or model JEB curriculum curricula. [ABSTRACT FROM AUTHOR]
