In an attempt to find out the association of metabolic dysregulation with poor endometrial receptivity and pregnancy loss, serum metabonomic profiling of women with idiopathic recurrent spontaneous miscarriage (IRSM) is carried out and compared with fertile controls. (1)H nuclear magnetic resonance (NMR)-based metabonomics was used to obtain serum metabolic profiles of 36 women with IRSM and 28 proven fertile women during the window of implantation. The acquired data were analyzed using multivariate principal component analysis, partial least-squares-discriminant analysis, and orthogonal projection to latent structure with discriminant analysis. A clear metabolic differentiation was evident between IRSM and control samples. The distinguishing metabolites, l-lysine, l-arginine, l-glutamine, l-histidine, l-threonine, l-phenylalanine, and l-tyrosine are significantly up-regulated in IRSM as compared to controls. These altered metabolites may be involved in the molecular mechanism of exaggerated inflammatory response and vascular dysfunction associated with poor endometrial receptivity in women with IRSM. The present work proposes a vital association of metabolic dysfunction with the disease pathogenesis.

Background: Balance between endometrial cell proliferation and apoptosis is crucial for successful embryo implantation. PTEN (phosphatase and tensin homolog deleted on chromosome 10), a pro-apoptotic factor, is proposed to be one of the signaling proteins through which estrogen and progesterone act to affect cellular homeostasis. Although reports in literature have suggested role of PTEN in regulating endometrial cell proliferation and apoptosis during window of implantation, its involvement in women with unexplained infertility is not clear. In the present study, we examined expression, cellular distribution and activation status of PTEN, cell proliferation, and apoptosis in midsecretory endometrium from women with unexplained infertility as compared to fertile controls.Methods: Endometrial biopsies from infertile (n=11) and fertile women (n=22) were used for immunohistochemical evaluation of PTEN, phospho-PTEN and Ki67. Terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate nick end labeling assay was performed for detection of apoptotic cells.Results: Biopsies from infertile women as compared to fertile controls demonstrated statistically significant: i) decrease in nuclear PTEN (P < 0.001), increase in nuclear phospho-PTEN (P < 0.05), increase in nuclear and cytoplasmic phospho-PTEN/PTEN ratio (P < 0.001 and P < 0.05 respectively) in endometrial stroma, ii) increase in cytoplasmic phospho-PTEN (P < 0.001) and phospho-PTEN/PTEN ratio (P < 0.05) in glandular epithelium (GE), iii) increase in Ki67 labeling in GE (P < 0.01) and stroma (P < 0.05) and, iv) decrease in (P < 0.001) apoptosis.Conclusions: Altered PTEN expression and associated modulation in cellular homeostasis during the implantation window might contribute to mechanism underlying unexplained infertility.

The endometrium is receptive to an intrusive blastocyst during a limited time span, defined as the implantation window. No definite clinically applicable receptivity marker has been discovered yet. Current approaches of the endometrium are focusing on factors emerging in the implantation window or retrieved from animal or in-vitro studies. These data fail to correlate endometrial parameters with ongoing pregnancy. Morphological tools are limited by difficulties in interpretation and inter-observer variability. In recent studies, extremely deviant endometrial morphology on the day of oocyte retrieval in IVF cycles with embryo transfer has been associated with a deleterious effect on clinical pregnancy. Gene expression data have identified clusters of genes involved in the regulation of the implantation window. Ideally, future studies should link endometrial morphology and function in precisely timed biopsies, analyzing functional biological pathways in a selected endometrial target area. • A significant proportion of couples undergoing IVF experience recurrent implantation failure (RIF). • Among all different elements that affect implantation failure, the immune system has been the most reasonable and debated. • By stimulation of immune system and initiation of humoral immunity, immunotolerance panel is completely changed in RIF. • Inflammatory responses as pNK cells, Th17 signaling pathway and TLR signaling pathway are activated in RIF patient. A significant part of couples in IVF-ICSI cycles experience Repeated Implantation Failure (RIF). Screening of the embryos with new methods like Next Generation Sequencing and arrays showed that even euploid embryos fail to implant. Immunology is a potent window maybe resolve the RIF problem. In this investigation we employed innate and adaptive immune system PCR array to compare the transcriptome profiles of endometrium in unexplained RIF and healthy fertile women. A total of 21 women were enrolled in the present study, 11women with unexplained RIF and 10 healthy fertile women. After RNA extraction and cDNA synthesis PCR array was performed using RT2 profiler PCR array human innate and adaptive immune responses kit (Qiagen, Cat.No: PAHS-052A). PCR Array data analysis identified significantly greater expression of IL6, IFNG, IL17A, IL23A, IFNA1, IFNB1, CD40 L, CCR4, CCR5, CCR6, CXR3, CCL2, IL2, TLR4, IRF3, STAT3, RAG1, IFNAR1 in unexplained RIF women than in controls (P < 0.05). However, expression of IL1B, IL8, NFKB, HLA-A, HLA-E, CD80, CD40 was significantly lower in unexplained RIF group than in controls (P < 0.05). Our results showed that modulation of immune system in RIF patient is shifted to inflammatory responses as pNK cells, Th17 signaling pathway and TLR signaling pathway are activated. So, by stimulation of immune system and initiation of humoral immune responses the panel of immunity and immunotolerance is completely changed in RIF patients comparing normal. BACKGROUND Frozen-thawed embryo transfer (FET) enables surplus embryos derived from IVF or IVF-ICSI treatment to be stored and transferred at a later date. In recent years the number of FET cycles performed has increased due to transferring fewer embryos per transfer and improved laboratory techniques. Currently, there is little consensus on the most effective method of endometrium preparation prior to FET. METHODS Using both MEDLINE and EMBASE database a systematic review and meta-analysis of literature was performed. Case-series, case-control studies and articles in languages other than English, Dutch or Spanish were excluded. Those studies comparing clinical and ongoing pregnancy rates as well as live birth rates in (i) true natural cycle FET (NC-FET) versus modified NC-FET, (ii) NC-FET versus artificial cycle FET (AC-FET), (iii) AC-FET versus artificial with GnRH agonist cycle FET and (iv) NC-FET versus artificial with GnRH agonist cycle FET were included. Forest plots were constructed and relative risks or odds ratios were calculated. RESULTS A total of 43 publications were selected for critical appraisal and 20 articles were included in the final review. For all comparisons, no differences in the clinical pregnancy rate, ongoing pregnancy rate or live birth rate could be found. Based on information provided in the articles no conclusions could be drawn with regard to cancellation rates. CONCLUSIONS Based on the current literature it is not possible to identify one method of endometrium preparation in FET as being more effective than another. Therefore, all of the current methods of endometrial preparation appear to be equally successful in terms of ongoing pregnancy rate. However, in some comparisons predominantly retrospective studies were included leaving these comparisons subject to selection and publication bias. Also patients' preferences as well as cost-efficiency were not addressed in any of the included studies. Therefore, prospective randomized studies addressing these issues are needed.

A total of 60 patients with hydrosalpinx and 30 patients with tubal obstruction were recruited, and immunohistochemistry was performed to detect the expression of LIF and L-selectin ligand in the endometrium of hydrosalpinx patients before and after surgery and in the endometrium of patients with tubal obstruction. To investigate the clinical importance of increased integrin αvβ3 expression in the endometrium following the surgical treatment for hydrosalpinx, a total of 60 patients with hydrosalpinx and 30 patients with fallopian tube obstruction were recruited. In the implantation window, immunohistochemistry was performed to detect integrin αvβ3 expression in the endometrium of the hydrosalpinx patients before and after surgery and of patients with fallopian tube obstruction. - Abstract
Objective: To observe the effect of warming acupuncture on uterine blood perfusion in the patients with failed high-quality freeze-thawed embryo transfer (FET) and explore its effect mechanism on the improvement of clinical pregnancy rate after re-tranfer., Methods: A total of 72 patients of failed high-quality FET were randomized into an observation group and a control group, 36 cases in each one. In the observation group, after the menstrual period ended, warming acupuncture started at the acupoints located on the abdomen, e.g. Qihai (CV 6), Guanyuan (CV 4), Zhongji (CV 3) and Qugu (CV 2) and those on the lumbar sacral region, e.g. Shenshu (BL 23), Mingmen (GV 4) and Yaoyangguan (GV 3), 50 min in each treatment, once daily, at the interval of 1 day after 4-day treatment. The treatment was discontinued till the patients were at the ovulatory stage. In the control group, nuangong yunzi capsules were taken orally and continuously after the end of menstrual period, 3 capsules each time, three times a day and stopped at the ovulatory stage. The treatment of one menstrual cycle was taken as one course and the treatment for 3 menstrual cycles was required. Before and after treatment, the uterine artery pulsation index (PI), endometrial thickness, endometrial type, uterine blood perfusion, the recovery time of sufficient uterine blood flow, the endomentrial receptivity (ER) during the implantation window period and the clinical pregnancy rate were observed in the two groups., Results: After treatment, the endometrial thickness was increased and PI decreased obviously in the two groups ( P <0.05) and PI in the observation group was lower than that in the control group ( P <0.05). After treatment, the proportion of type a and type A of endometrium was increased markedly in the two groups ( P <0.05) and the proportion in the observation group was higher than the control group ( P <0.05). After treatment, the case proportion of sufficient uterine blood flow was increased obviously in the two groups ( P <0.05) and the value in the observation group was higher than the control group [83.3% (30/36) vs 69.4% (25/36), P <0.05]. After treatment, the proportion of ER during the implantation window period was increased remarkably in the two groups ( P <0.05) and the value in the observation group was higher than the control group [72.2% (26/36) vs 50.0% (18/36), P <0.05]. The recovery time of sufficient uterine blood flow in the observation group was shorter than the control group ( P <0.05) and the clinical pregnancy rate was higher than the control group [47.2% (17/36) vs 33.3% (12/36), P <0.05]., Conclusion: Warming acupuncture enhances uterine blood perfusion and improves uterine endometrial receptivity so that the clinical pregnancy rate is increased after re-transfer in the patients with failed high-quality freeze-thawed embryo transfer.

The objective of this study was to investigate the effect of ovarian stimulation for IVF on endometrial secretion and tissue markers of receptivity in the mid-luteal phase. In 10 oocyte donors, endometrial secretions and biopsies were sampled 5 days after spontaneous ovulation and oocyte retrieval in consecutive cycles. Four subjects received progesterone in the luteal phase of the stimulated cycles. Mid-luteal endometrial maturation in the stimulated cycle was compared with the spontaneous cycle, by histological dating, Ki-67, oestrogen receptor (ER) and progesterone receptor (PR) expression, secretion levels of leukaemia inhibitory factor (LIF), glycodelin A (GdA) and progesterone, and protein profile. No significant differences in histological markers, expression of Ki-67, PR, ER, secretion protein profiles or con central ions of LIF GdA, or progesterone were observed when comparing natural with stimulated cycles. Progesterone supplementation of stimulated cycles was associated with significantly lower Ki-67 (P = 0.03) and ER (P = 0.04) expression compared with the non-supplemented stimulated cycle. In this pilot Study, ovarian stimulation was not demonstrated to alter the studied markers of endometrial maturation in the mid-luteal phase.

Despite the significant advances in the last decades, low implantation rate per transferred embryo still remains a major concern in assisted reproductive techniques, highlighting a need to better characterize endometrial receptivity also by mean of specific biomarkers. Based on physiology and on the intimate contact with endometrium as the tissue of interest, in this study we developed and validated an optimized protocol that uses extracellular vesicles (EVs) recovered from uterine flushings and from a cervical brush, the latter never used until now as an EVs source, as surrogates for endometrial biopsies. This method combines the safety of sampling with the ability to study the expression profile across the uterine cycle. We have compared the yield and composition of EVs recovered from different biofluids samples and fractions thereof, opting for chemical precipitation as the EV isolation procedure, assuring the highest yield without introducing any bias in specific EV recovery. Moreover, collected EVs, in particular exosome-like vesicles, express putative endometrial markers, such as glycodelin A and receptors for estrogen and progesterone, thus confirming their endometrial origin. We also identified uterine flushing EVs, in particular those recovered from its mucous fraction, as the richest source of endometrial transcripts, likely correlated to cellular (epithelial) origin of these vesicles. Finally, our pilot quantitative assessment of three endometrial gene profiles, in samples collected at different time points along the luteal phase, revealed the fluctuations apparently recapitulating gene expression variability prior reported during the menstrual cycle. Unlike tissue biopsy that is subjected to inter- and intra-sample differences, our data suggest that EVs from liquid biopsies (from uterine flushings and a cervical brush) obtained through less-invasive procedures, can be substrate to detect and track the tissue representative expression profiles, better depicting the total endometrium complexity. International audience; Purpose: Few studies have investigated the endometrial receptivity status of oocyte-donation (OD) recipient patients at the specific hormone replacement therapy (HRT)-cycle timing (5 to 6 days after progesterone administration) where embryos at blastocyst stage were mostly replaced. The aim of our study was to analyse, during the implantation window, (i) the global endometrial gene expression profile, and (ii) the endometrial receptivity exploration by the Win-Test® in OD recipient patients under HRT compared to spontaneous cycle in patients awaiting for IVF.Material and methods: This study included OD recipient patients without (n=7) or with (OD RIF, n=20) repeated implantation failures and 12 normal responder patients in spontaneous cycles used as control. Endometrial biopsies were performed during the peri-implantation phase. Samples were analysed using DNA microarrays and the endometrial gene expression profiles of HRT-treated OD recipient patients and of patients in spontaneous cycles were compared. Then, specific biomarkers of endometrial receptivity were assessed in the two groups of HRT-treated OD patients in comparison to control patientsResults: The global gene expression profile of peri-implantation endometrial samples from HRT-treated OD recipients and from patients in spontaneous cycles was different with significant alterations in the oestrogen receptor signalling [GTF2H2B, POLR2B, POLR2E], VEGF family ligand-receptor interactions [VEGFR1, VEGFB] and integrin signalling [ITGAL, PAK7, ILK]. Using specific biomarkers of human endometrial receptivity, we found that endometrium was non-receptive (29 % and 43 % in OD and RIF OD patients, respectively) or partially receptive (71 and 43 % in OD and RIF OD patients, respectively), at Pg+5/+6, in majority of HRT-treated patients. In OD RIF patients, a delay of the implantation window was observed. However, by targeting personalized embryos transfer by identifying for each patient the HRT-cycle day where endometrium is receptive, with respect of the synchronization of embryo-endometrium dialogue, high pregnancy rate per frozen-thawed embryo replacement (50%) was obtained in OD RIF patients.Conclusion: This study underlines the need to take into account the individual patient’s response to HRT cycles and to move to a patient-tailored care management.

We investigated the effects of a modified Shoutaiwai recipe on integrin β3 and leukemia-inhibitory factor (LIF) in the endometrium of controlled ovarian hyperstimulation (COH) mice during the implantation window. Seventy non-pregnant mice were randomly divided into 3 groups: a traditional medicine (TCM) treatment group (N = 30), an aspirin treatment (N = 30) group, and a control group (N = 10). After the model was successfully established, mice in the drug treatment groups and the control group were respectively treated with the modified Shoutaiwai recipe, aspirin, or 0.9% physiological saline. During the implantation window of mice, the middle segment of the mouse uterus was recovered, and integrin β3 and LIF expressions in the endometrium were respectively detected using an immunohistological two-step method and reverse transcription-PCR. Expressions of integrin β3 and LIF in the endometrium of mice in the TCM treatment group were significantly increased compared to aspirin-treated and control mice, and those of aspirin-treated mice were increased compared to the control group. Our modified Shoutaiwai recipe may improve the endometrial receptivity of COH mice by increasing the expression of integrin β3 and LIF in the endometrium during the implantation window.

Infertility remains a challenge to modern medicine. Despite extensive diagnostic and therapeutic procedures, the achievement of pregnancy remains an elusive goal in some patients. The endometrium is one of the key factors in embryo implantation. Older methods of describing endometrial receptivity like histology or ultrasound, did not bring noticeable improvement in pregnancy rates. New technologies, including genomics, proteomics, lipidomics, and secretomics promise to improve the detection of the implantation window in the endometrium and result in better counseling of patients with infertility.

To evaluate the expression of endometrial matrix metalloproteinases (MMPs) 2 and 9 and E-cadherin in peri-implantation phase of infertile women who have undergone in vitro fertilization (IVF) cycles.This prospective study included 51 patients who underwent endometrial biopsy during the receptive phase in a menstrual cycle prior to IVF treatment. The samples were evaluated by tissue microarray for immunohistochemical study.The expression of MMP-2, MMP-9, and E-cadherin in the endometrium prior to IVF treatment was not associated with pregnancy. There was a decrease in E-cadherin immunodetection, the higher the age of the patients, a negative relationship between E-cadherin and MMP-2, and a positive association between MMP-9 and E-cadherin.The MMP-2, MMP-9, and E-cadherin are expressed in the endometrium of infertile patients during the receptive phase of the natural menstrual cycle. However, there is no correlation between the expression of these molecules and the clinical IVF outcomes.

Introduction: A successful embryo implantation is crucial for a positive outcome of in vitro fertilization. But there is only a short period during which the endometrium is receptive for embryo, this so called implantation window can be detected by a molecular diagnostic method endometrial receptivity analysis (ERA)., Objective: To find out the percentage of patients with a non-receptive endometrium in the time of ERA and to learn what part of them got pregnant after the identification of their personalized implantation window., Design: A retrospective study., Setting: REPROMEDA Biology Park, Centre of Reproductive Medicine and Preimplantation Genetic Diagnosis, Brno., Methods: A cohort of 85 patients undergoing ERA from August 2015 to October 2018 was studied. 74 patients experienced a previous implantation failure, the average number of preceding unsuccessful frozen embryo transfers was 2,5 in this group, 11 women went through ERA due to the preventive reason before the first FET. In all women one euploid embryo was transferred. 48 patients were prepared either for ERA or FET in a natural menstrual cycle, 37 women in HRT cycle. We were interested in a percentage of non-receptive patients in the time of ERA and wanted to discover what part of non-receptive women got pregnant after the identification of their personal implantation window. The average number of frozen embryo transfers needed to achieve the pregnancy was also calculated., Results: 31 of 85 patients (36.5%) were found to have a non-receptive endometrium. In the natural cycle 13 of 48 (27.1%) were non-receptive: five were pre-receptive, three early receptive, two late receptive and three post-receptive. In the HRT cycle 18 of 37 patients (48.6%) were non-receptive: 12 were pre-receptive, four early receptive, one late receptive, one post-receptive. Personalized FET was done in 26 of total 31 initially non-receptive patients, 18 of them got pregnant (69.2%). In the natural cycle 6 of 11 (54.5%) achieved the pregnancy, in the HRT cycle 12 of 15 women (80.0%) got pregnant. To achieve the clinical pregnancy 1.5 frozen embryo transfer in average was needed., Conclusion: A displaced implantation window was found in more than 1/3 of patients undergoing an assisted reproductive treatment. After the personalized FET the clinical pregnancy was noticed in 69.2% of them. This result supports an individual approach to patients in IVF programme besides other at the timing of embryo transfer after the identification of pWOI.

Sophoricoside (SOPH) is an isoflavone glycoside isolated from the fruits of Sophora japonica. Since its first isolation in 1961, there are rare findings about the effects of SOPH on reproductive system. In the present study, the pregnant mice administrated by different doses of SOPH were used to explore the effect of SOPH on embryo implantation, especially on the endometrial receptivity. The statistical results showed that the number of implanted embryos was gradually declining along the increasing dose of SOPH. When the administrated dose of SOPH was 600 mg/kg per day, great changes were observed in the exposed uterine morphology and up-regulated progesterone receptor (PR) and down-regulated estrogen receptor α (ERα), E-cadherin, matrix metalloproteinase-2 (MMP-2) and integrin β3 were also found in SOPH-exposed uterine. These findings demonstrated that SOPH exposure reduced the number of implanted embryos in a dose-dependent manner and failed the embryo implantation through altering the morphology of uterine and compromising the endometrial receptivity.

Therefore, a prospective study was conducted during 03/2013-09/2013 in Iakentro, Infertility Treatment Center and 3rd Department of Obstetrics and Gynaecology of Aristotle University of Thessaloniki. Infertile women consisted the patients��� group (group 1) while fertile women the study���s control group (group 2). An endometrial biopsy was performed by Pipelle on 7th-8th menstrual day after ovulation in each woman and expression of total progesterone receptors (��+��) as well as PR-B was studied by immunohistochemistry. Positive nuclei percentage, intensity of staining and h-score in epithelial and stromal cells were the primary outcomes of our study. The former parameters were compared between fertile and infertile women, while expression was also compared between epithelial and stromal cells in the two groups of our study. There were overall 20 women meeting the inclusion criteria of our study, of which 10 fertile (group 1) and 10 infertile (group 2). Endometrial sample was finally obtained by 8 women of each group. Mean age was 32.0 �� 0.9 years for group 1 vs. 37.3 �� 3.5 for group 2(P=.002). Expression of total progesterone receptors (��+��) was significantly increased in the epithelial cells of fertile women compared with epithelial cells of infertile women. Specifically, h-score was significantly increased (P=.05) while there was marginally non-significant difference regarding positive nuclei percentage and intensity of staining (P=.06). Expression of type B receptors was also increased, yet not significantly. No significant difference was detected in any of the parameters between stromal cells of two groups of women. Finally, while epithelial cells presented increased positive nuclei percentage of type B receptors compared with stromal cells in fertile women (62.8 �� 13.8% vs 30.0 �� 9.4% respectively, P=.07), h-score was significantly increased in stromal compared with epithelial cells in infertile women (2.2 �� 0.2 vs. 0.7 �� 0.5 respectively, P=.01). In conclusion, expression of progesterone receptors in epithelial cells is significantly decreased in infertile compared with fertile women. Furthermore, expression pattern of type B receptors between epithelial and stromal cells is also reversed in infertile compared with fertile women.

LH surges are the start of a period of optimal endometrial receptivity. Missing these surges in an unstimulated-cycle frozen-thawed embryo transfer (FET) based on ultrasound alone might lead to incorrect timing of embryo transfer. This prospective, non-randomized trial established the incidence and effect of spontaneous LH surges on ongoing pregnancy rates and assessed the use of ultrasound without LH monitoring in planning FET. All patients undergoing unstimulated-cycle FET in the study centre over a 2-year period were included in this analysis (n = 233). All patients had regular menstrual cycles. Serum LH analysis took place before human chorionic gonadotrophin administration. The main outcome measure was ongoing pregnancy. LH surges occurred in over half of patients. Overall pregnancy rate was 34.3%. Relative risks for ongoing pregnancy for cycles with or without a spontaneous LH surge were not significantly different (ongoing pregnancy rate 33.4% versus 34.8%; RR 1.02, 95% CI 0.7-1.5). Based on these results, it was concluded that LH surges >= 10 IU/l occurred in over 50% of patients, but LH surges demonstrated no significant effect on pregnancy rates. Embryo implantation is the process that results in attachment of the conceptus to the uterine wall. In this histochemical study, we investigated the early stage of embryo implantation in rats by morphological analysis and by the detection of total proteins and glycosaminoglycans using hematoxylin-eosin, toluidine blue at pH 4.0 (TB), and Xylidine Ponceau at pH 2.5 (XP). In non-pregnant females, the uterine layers could be clearly distinguished and showed the normal histology of the organ. In pregnant females, an increase in the number of cells and a reduction in the interstitial space were observed in the endometrium close to the implantation sites. The blastocyst was partially inserted in the endometrium, with the observation of the inner cells mass around the blastocyst cavity surrounded by trophoblastic cells. TB staining revealed mild metachromatic basophilia, which was more evident in the endometrial stroma around the implantation site. Histochemical staining with XP was also more intense in the stroma close to the site of implantation. On the other hand, histochemical staining with either TB or XP was more discrete at sites distant from the conceptus. This study demonstrated changes in the endometrial stroma in areas adjacent to the site of embryo implantation, with variations in glycosaminoglycans and proteins as demonstrated by the detection of anionic and cationic radicals, respectively.

The endometrium is receptive to an intrusive blastocyst during a limited time span, defined as the implantation window. No definite clinically applicable receptivity marker has been discovered yet. Current approaches of the endometrium are focusing on factors emerging in the implantation window or retrieved from animal or in-vitro studies. These data fail to correlate endometrial parameters with ongoing pregnancy. Morphological tools are limited by difficulties in interpretation and inter-observer variability. In recent studies, extremely deviant endometrial morphology on the day of oocyte retrieval in IVF cycles with embryo transfer has been associated with a deleterious effect on clinical pregnancy. Gene expression data have identified clusters of genes involved in the regulation of the implantation window. Ideally, future studies should link endometrial morphology and function in precisely timed biopsies, analyzing functional biological pathways in a selected endometrial target area.

Successful embryo implantation depends on the quality of the embryo, as well as on the receptivity of the endometrium. The aim of this study was to investigate the endometrial gene expression profile in women with unexplained infertility in comparison with fertile controls at the time of embryo implantation in order to find potential predictive markers of uterine receptivity and to identify the molecular mechanisms of infertility. High-density oligonucleotide gene arrays, comprising 44 000 gene targets, were used to define the endometrial gene expression profile in infertile (n = 4) and fertile (n = 5) women during the mid-secretory phase (day LH +7). Microarray results were validated using real-time PCR. Analyses of expression data were carried out using non-parametric methods. Hierarchical clustering and principal component analysis showed a clear distinction in endometrial gene expression between infertile and fertile women. In total we identified 145 significantly (>3-fold change) up-regulated and 115 down-regulated genes in infertile women versus controls. Via Database for Annotation, Visualization and Integrated Discovery functional analysis we detected a substantial number of dysregulated genes in the endometria of infertile women, involved in cellular localization (21.1%) and transport (18.8%) and transporter activity (13.1%) and with major localization in extracellular regions (19.2%). Ingenuity Pathways Analysis of the gene list showed dysregulation of gene pathways involved in leukocyte extravasation signalling, lipid metabolism and detoxification in the endometria of infertile women. In conclusion, endometrial gene expression in women with unexplained infertility at the time of embryo implantation is markedly different from that in fertile women. These results provide new information on genes and pathways that may have functional significance as regards to endometrial receptivity and subsequent embryo implantation.

It is well known that an adequate endometrial receptivity is required for successful implantation in both natural and assisted reproductive cycles. In particular, a brief "implantation window", during which endometrium undergoes anatomical and molecular changes necessary for embryo implantation, has been observed. The hormonal treatment applied to induce ovulation seems to be able to modify the normal development of the prenidatory endometrium, with possible negative effect on the implantation rate. For this reason, several attempts have been made to identify specific markers of endometrial receptivity, useful for predicting implantation outcome in clinical practice. Even if different histological, immunohistochemical, and ultrasonographic parameters are studied, none unfortunately has been univocally shown to be predictive of pregnancy outcome. Therefore, the evaluation of endometrial receptivity remains a challenge in clinical practice.

This review introduces the field of cytokines and implantation and then recalls the specialized role of the uterus and the notion of the ‘implantation window’. The role of inflammatory and angiogenic cytokines is presented, as well as the involvement of cytokines such as -interferons in corpus luteum maintenance in non-chorionic gonadotrophin-producing species. -Interferons are reviewed, before dealing with the more in depth analysis of cytokine networks in the pre- and peri-implantation uterus. The emerging involvement of cytokines in controlling uterine vascularization angiogenesis is reviewed, with emphasis on NK activating factors.

A major problem in IVF procedures is a high rate of induced iatrogenic complications including multiple gestations. Until now, transfer of at least three embryos followed by the subsequent elective reduction of triplet or higher order gestations to twins, single embryo transfer (SET) with cryopreservation of the remaining embryos, as well as the application of SET in unstimulated cycles, serves to illustrate the diversity characterizing the worldwide effort of achievement of pregnancy that aims to avoid possible complications. The BESST (birth emphasizing a successful singleton at term) endpoint constitutes an interesting parameter imposing the safety of SET. However, the observed success rate (11.1%) requires elucidation of the specific pattern of endometrial behaviour around the implantation window as well as its involvement in the further support of gestation. Consequently, research has to focus primarily on the improvement of technical parameters to achieve an acceptable success rate during the IVF procedures as compared with spontaneous gestations.

The ultimate function of the endometrium is to allow the implantation of a blastocyst and to support pregnancy. Cycles of tissue remodeling ensure that the endometrium is in a receptive state during the putative 'implantation window', the few days of each menstrual cycle when an appropriately developed blastocyst may be available to implant in the uterus. A successful pregnancy requires strict temporal regulation of maternal immune function to accommodate a semi-allogeneic embryo. To preparing immunological tolerance at the onset of implantation, tight temporal regulations are required between the immune and endocrine networks. This review will discuss about the action of steroid hormones on the human endometrium and particularly their role in regulating the inflammatory processes associated with endometrial receptivity.
