The fornix is a region of greatest neurosurgical interest in regards to its complex anatomy and surgical approaches to this area. The objective of this study was to evaluate the morphology of the triangular recess (TR) and its role in the growth pattern of the colloid cysts (CC) within the third ventricle and in the choice of the surgical approach for their removal. Furthermore, to compare the results of the dissections with measurements performed on a magnetic resonance imaging scan. In the anatomic study, 20 cadaveric specimens were dissected and analyzed. In the radiologic study, a magnetic resonance imaging scan was performed in 20 healthy volunteers. In the clinical study, a retrospective analysis of all the patients affected with CCs microsurgically removed at our institute between 2010 and 2018 was conducted. In the anatomic study, the width, height, and the area of the TR were respectively 0.31 cm, 0.33cm, and 0.051 cm2. In the radiologic study, 3 different typologies of TR were identified: open recess in ventriculomegaly (7 patients); open recess in physiologic ventricular system (3 patients); closed or blind recess (10 patients). Three different growth patterns of CCs were identified: type 1) CCs localized at the foramen of Monro growing behind the fornix and below the third ventricular roof; type 2) CCs growing rostrally between the column of fornix; and type 3) CCs growing above the plane of the third ventricular roof. Colloid cysts are rare benign tumors of the third ventricle and have a wide range of clinical representations from incidentomas to acute death. The growth of these cysts is an uncommon event and may cause obstructive hydrocephalus that lead to sudden deterioration in the patient's condition and may cause lifethreatening complications. The large colloid cyst of third ventricle caused sudden death of children due to clogging, diagnosed in vivo is a very rare case in literature. We are representing here a case which patient admitted to emergency room for sudden loss of consciousness. Deep-seated brain tumors are often best treated by primary surgical excision. Traditional microsurgical techniques can cause retraction injury and require extensive brain dissection. To mitigate this risk, stereotactic-guided tubular retractors were developed; however, the risk of shear injury remains. We created a stereotactic-guided dilatable port system to create a corridor for deep brain tumor surgery along the trajectory of a brain needle to minimize iatrogenic brain injury. Of the 8 included patients (6 colloid cysts, 1 metastasis, 1 intraventricular meningioma), 5 had undergone frameless and 3 frame-based stereotactic targeting. We used a tans-sulcal trajectory and a 2.6-mm stereotactic needle. At the target depth, the cannula was removed and the balloon inflated to 14 mm. The balloon was deflated and removed before placing the port. Pre- and 3-month postoperative magnetic resonance imaging scans were used to measure the T2-weighted signal change and residual cannulation defect. These patients were compared with a case-matched standard endoscopic port surgery cohort. All patients had undergone total lesional resection without new neurologic deficits. Patients undergoing dilatable endoscopic port surgery (DEPS) had significantly smaller residual cannulation defects (P < 0.05) but no significant differences in postoperative T2-weighted signal changes or diffusion restriction volumes at 3 months postoperatively (P > 0.05). DEPS might be a safe alternative to standard endoscopic port surgery or microsurgery for deep-seated brain tumors. The degree of iatrogenic injury using DEPS, as determined by magnetic resonance imaging analysis, might be equivalent to or less than that with standard port surgery techniques, although larger sample sizes are needed for validation. The main criticism of endoscopic excision of colloid cysts of the third ventricle is the decreased ability to completely resect the cyst wall, therefore increasing the risk of recurrence. The extent of resection varies widely across the endoscopic series and is largely influenced by the surgical technique. We report the results of the rotational technique for endoscopic transforaminal excision of colloid cysts in a series of consecutive patients. Our objective is to contribute to the current literature and to shed more light on an effective and safe yet less commonly used technique. Retrospective analysis was performed on 19 patients. Preoperative magnetic resonance imaging (MRI) was evaluated for cyst characteristics. Postoperative MRI was evaluated for residual cyst membranes. Operative records were reviewed for residual cyst components. Excision grade was determined based on the Barrow Neurological Institute grading scale. Symptom resolution was documented clinically. The study included 13 men and 6 women, with a mean age of 35 years (range, 19–56 years) and mean follow-up of 27.74 months (range, 4–55 months). Total excision was achieved in 17 of 19 cysts (89.5%). A small residual was seen intraoperatively but not radiographically in 1 patient. In another patient, residual cyst membrane seen intraoperatively and radiologically led to recurrence 18 months postoperatively. No mortalities or permanent morbidities occurred. Our high total excision rate and low complication profile are in concordance with the recent reports of endoscopic resection of colloid cysts. Colloid cysts (CC) have been associated with neurocognitive function (NCF) decline, both preoperatively and after resection. Factors such as local pressure on the fornix and hydrocephalus are thought to contribute to preoperative NCF decline. The potential cause of postoperative decline is thought to be forniceal injury during surgery. In the current series, we describe NCF outcomes amongst patients with CC, both nonoperated and operated. A total of 36 patients (23 operated, 13 nonoperated) were included in this retrospective study. All patients underwent at least 1 NCF evaluation battery. Of the 13 nonoperated cases, 5 had follow-up tests too. Of the 23 operated, 14 had both pre- and postoperative tests, and 8 had early and late postoperative tests. There was no significant difference in baseline NCF between nonoperated and operated cases (as evaluated preoperatively). Nonoperated patients had a stable NCF test over time. Patients who were operated showed a significant improvement after surgery in several NCF variables. There was no significant change in NCF between early and late postoperative evaluation. None of the operated patients had a postoperative NCF decline. Patients with CC should undergo routine NCF testing with a standardized protocol, whether they are operated or followed. Surgery has a positive impact on NCF; however, it remains to be determined if the improvement is solely secondary to treatment of hydrocephalus, or to a reduction of local pressure on the fornices. Background Colloid cysts (CCs) are rare cystic lesions derived from the endoderm of the central nervous system. Although they appear most commonly in the anterior roof of the third ventricle, there are only a few reports of CCs located in the suprasellar region. Although CCs are considered to be slow-growing benign tumors, their developmental process remains unclear. Case Description A 6-year-old boy was referred to our hospital for a mild head injury. Head computed tomography scan revealed a round, hyperdense suprasellar lesion, while magnetic resonance imaging depicted the mass as an isointense lesion with multiple highly hypointense spots on T2-weighted imaging. Although this lesion had been managed conservatively with annual magnetic resonance imaging follow-up, its size gradually increased, resulting in an increase in diameter by a factor of 1.5 over an 11-year period. The doubling time of this tumor was estimated to be approximately 7 years. Despite its asymptomatic nature, the cystic lesion was totally resected when the patient was 17 years of age using an endoscopic endonasal approach to make a definitive histologic diagnosis and prevent the occurrence of neurologic symptoms. The postoperative course was uneventful, and the histologic diagnosis of the surgical specimen was consistent with CC. Conclusions This case suggests that CC may be one of several possible diagnoses in patients who present with suprasellar cystic lesions. Even if CCs in the suprasellar region remain asymptomatic, radiological follow-up is required due to possible progressive enlargement of the cyst. [ABSTRACT FROM AUTHOR]

Patients who present with a large colloid cyst (CC) and nondilated ventricles represent a therapeutic challenge. 1-3 Although transcallosal approaches provide reliable access to the lateral ventricle and foramen of Monro, direct visualization of the vascular attachment of the CC to the roof of the third ventricle is not always possible. This can be especially true with CCs located more posteriorly and superiorly. 4 Opening of the choroidal fissure can improve access and visualization to the posterior third ventricle; however, this maneuver is associated with some element of risk. 5 There is a paucity of operative video in the literature illustrating the technique of gentle, microblade elevation of the fornix to improve visualization into the third ventricle and, on occasion, avoid the need to open the choroidal fissure. 6 We report the case of a 28-year-old woman who presented with headaches and progressive short-term memory dysfunction (Video 1). Magnetic resonance imaging demonstrated a 17-mm CC associated with distortion and thinning of the bilateral fornices without hydrocephalus. The patient was offered interhemispheric, transcallosal resection. Intraoperatively, gentle elevation of the fornix with a microblade retractor facilitated access to the vascular attachment of the colloid cyst—obviating the need to open the choroidal fissure. The index operative video discusses the technical nuances associated with trans-callosal resection of CC with use of the microblade retractor. Abstract An 11-year-old boy presented with sudden onset of altered consciousness following acute-onset severe headache and vomiting. Computed tomography showed hydrocephalus due to obstruction of the foramen of Monro by an isodense mass in the anterior third ventricle. Ventriculoperitoneal shunt was placed to relieve the hydrocephalus and the patient improved clinically. Magnetic resonance imaging of the brain revealed a well-defined lesion with isointense signal intensity on T1-weighted and hyperintense signal intensity on T2-weighted images. Acute, right occipital lobe and right thalamic infarction was revealed by diffusion-weighted imaging; smaller infarct was seen in the medial left occipital lobe. Imaging findings were compatible with third ventricle colloid cyst causing acute hydrocephalus and occipital lobe infarction due to central transtentorial herniation.

Colloid cysts are mucous-filled masses with an outer fibrous layer. These cysts are rare developmental malformation and not a true neoplasm. They usually found incidentally and are asymptomatic; but in some cases may associate with rapid neurologic deterioration, herniation, and sudden death. Recognition of this rare but important diagnosis may result in decreasing mortality. In this report, we presented a 13-year-old boy with complaint of two times drop attack and final diagnosis of colloid cyst in the third brain ventricle.

Background Although histologically benign, colloid cysts have been an entity of interest to neurosurgeons due to the wide array of possible presentations ranging from asymptomatic to high ICP symptoms to sudden death. It is estimated that colloid cysts represent 0.3 to 2% of all brain tumors. 1 As they are typically located in the third ventricle, multiple approaches have been adapted and developed for the maximum resection with the least complications given the sensitive anatomy in the area. 12 The interhemispheric transcallosal approach can be safely performed to fully resect a third ventricle colloid cyst with close to zero recurrence rate and minimal to none permanent deficits. 3 This video is an educational illustration of the surgical technique and the related anatomy for the interhemispheric transcallosal approach and how to provide best chances of a benign postoperative course. Case Description In this surgical video, we present a case of a 20-year-old male, a known case of hypertension and un-controlled diabetes type-1, presented to our hospital with on and off headache, dizziness, and diplopia that gradually progressed. His neurological exam was unremarkable including memory function. CT and MRI scans demonstrated a rounded sharply demarcated lesion at the at the roof of the third ventricle, measuring 1 × 1 cm in size. The patient underwent an anterior inter-hemispheric transcallosal approach, with gross total resection of the cyst. In the follow-up office visit, the patient headache has resolved, and he had no seizures with preserved memory functions. Conclusion When removal of the colloid cyst is indicated, the interhemispheric transcallosal approach can be performed safely with gross total resection and minimal neurological deficit. Introduction: Colloid cyst of the third ventricle is a rare intracranial benign tumor. Traditionally, treatment is carried out by transcallosal or transcortical transventricular approach. Currently, the endoscopic treatment of these lesions is increasingly used. We reported our preliminary experience about three cases of colloid cysts treated by neuroendoscopy. Methods and Patients: We conducted a retrospective study on the analysis of medical records of three patients with colloid cyst who had been operated endoscopically in our department. We used a rigid neuroendoscope of 2.7 mm in diameter, with an optical 30°. Coagulation of the wall, followed by a puncture aspiration of cystic contents and partial resection of its wall were performed during the surgery. The mean follow-up time was two years. Results: Patients were 35, 47, and 53 years old. They were male. The dominant clinical symptom was headaches with consciousness disorder. Brain MRI allowed the diagnosis in all cases. The mean operation time was 100 minutes. A case of intraoperative hemorrhage was observed, necessitating the installation of an EDV. The outcome was favorable in two patients, and after surgery we observed one case of meningitis treated. One case of transient amnesia and one death were reported. Conclusion: Preliminary results of endoscopic treatment of colloid cysts were encouraging. It is a promising technique in our department. However, more investigation is necessary.

Colloid cyst of the third ventricle (CC) represents approximately 1% of intracranial tumours and 20% of intraventricular tumours. CC usually occurs between 20 and 50 years of age. During the first decade of life, it is diagnosed very rarely (1-2%). It can be most commonly found in the anterior part of the third ventricle at the foramen of Monro (1). It is often visualised during the computed tomography (CT) examination as a hyperdense focal lesion, it has variable change of the signal during magnetic resonance imaging (MRI) (2). CC has a benign character, however, a strategic position which may lead to acute hydrocephalus, intracranial hypertension syndrome, consciousness disorder, and even sudden death. This peracute hydrocephalus is an indication to an acute neurosurgical procedure (3).

Colloid cysts of the third ventricle are considered as benign lesions. The clinical manifestations are not clear in most of cases. Many treatment options are available and endoscopic removal of the cysts proves to be a very successful method especially if the lateral ventricles are moderately or severely dilated. Sometimes, we faced cases with non- or mildly dilated ventricles which may add more risks to the operation, limit the radicality of removal, or abort the procedure entirely. Sixteen cases of colloid cyst with mildly dilated ventricles were operated upon between 2008 and 2016 using the rigid endoscopic system. Twelve were female and four were male. Their ages were between 17 and 40 years old. Headaches and epileptic fits were the presenting symptoms in 13 cases and 2 cases respectively. One case was asymptomatic. The patients were followed up from 1 to 4 years. Total removal in 12 cases and evacuation of the contents and partial removal in 4 cases. Mild transient complications occurred in the form of fever in seven cases, vomiting in four cases or short-term recent memory loss in three cases. No deaths occurred due to the procedures. Working endoscopically in enlarged ventricles is very effective and easy; however, small sized ventricles do not prevent safe and effective complete removal of colloid cysts. Background Primary central nervous system lymphoma is a rare malignant tumor of the central nervous system. It is associated with poor prognosis and accounts for 0.7%–0.9% of all lymphomas and only 0.3%–1.5% of intracranial tumors. Typically, these lesions are in the cerebral white matter near the corpus callosum, the central gray matter, the basal ganglia-thalamus-hypothalamic region, the posterior fossa and the periventricular region. Only 2 cases with pure third ventricular lymphoma have been reported in the literature. Case Description A 72-year-old female patient known to have type II diabetes mellitus treated with insulin, hypertension, chronic kidney disease, dyslipidemia, and obesity presented with a history of acute confusion and urinary incontinence. No headache was reported on admission, but previously she had intermittent mild headaches and generalized body aches. She had no history of visual symptoms. Her family said she was mildly confused and had memory difficulties that started acutely 2 days before presentation to the hospital. The patient had urgent computed tomography of the brain, which showed a hyperdense lesion in the region of the foramen of Monro. Pre-operative images diagnosed the case as colloid cyst, but post-operative histopathology proved the lesion to be primary CNS lymphoma. Conclusions Eleven months after surgery, the patient is fully consciousness and oriented, with no memory issues or neurologic deficit. She is back to her baseline activities. Objective Patients with hydrocephalus secondary to third ventricular colloid cysts can require bilateral external ventricular drain (EVD) placement while awaiting surgery. However, some patients could develop auto-fenestration of the septum pellucidum (AFSP) and only require 1 EVD. We evaluated our experience with bedside iohexol ventriculography and staged EVD placement for patients with obstructive hydrocephalus. Methods We retrospectively identified 34 patients who had been treated for third ventricular colloid cysts (2013–2016). The preoperative and postoperative data, including age, sex, colloid cyst size, preoperative hydrocephalus, preoperative EVD placement, preoperative iohexol ventriculography, operative approach, intraoperative findings, and postoperative ventriculoperitoneal shunt requirements, were reviewed. Results Hydrocephalus was found in 23 patients (68%) on initial presentation. Nine patients (26%) had EVDs placed before surgery. Six patients (18%) underwent iohexol ventriculography after insertion of a right-sided EVD. Five patients (15%) demonstrated no evidence of ventricular communication. Four patients (67%) required left-sided EVD placement. One patient (17%) had robust communication between the lateral ventricles after intraventricular iohexol injection, which was managed with a single EVD. AFSP was observed during surgical resection of this patient's colloid cyst. One other patient who did not undergo preoperative EVD placement was noted to have AFSP intraoperatively, resulting in 2 of 34 patients (6%) with AFSP in our series. Conclusions A subset of patients with obstruction at the foramina of Monro can develop AFSP. Iohexol ventriculography after unilateral EVD placement allows clinicians to assess for the presence of AFSP and identify patients who can be treated with a single EVD before surgery. Background Endoscopic resection of colloid cysts is a widely accepted treatment option instead of microsurgery. However, there is still a debate about a potentially higher rate of incomplete resections and recurrence. Objective The aim of this retrospective study was to evaluate long-term results after endoscopic removal of colloid cysts. Methods Twenty patients underwent endoscopic treatment in our department. Eighteen patients agreed to follow-up examinations. In 17 patients, magnetic resonance images were obtained. Results Total cyst resection was achieved in 16 procedures. In 1 patient, only plexus coagulation and widening of the ipsilateral foramen of Monro were performed. In 3 patients, small remnants of the cyst membrane were left behind. Conversion to microsurgery became necessary in 1 patient. Mild temporary complications occurred in 6 patients. Preoperative symptoms were completely relieved in 16 patients and improved in 2 patients. The average follow-up period was 188 months. In the patient with plexus coagulation, the cyst did not change. Recurrence occurred in 2 of 3 patients with cyst remnants. To date, no cyst remnant or recurrence has caused any symptoms or required surgical treatment. Conclusions Our results indicate that endoscopic treatment of colloid cysts is a safe and effective treatment option that provides excellent long-term results. However, we determined that a significant risk for recurrence exists when even small parts of the cyst capsule were left behind. Therefore, we advocate an attempt at total endoscopic cyst resection. Abstract The MRI appearance of intraventricular neoplasms is often nonspecific, however knowledge of patient age and tumor location facilitates the formulation of a differential diagnosis. Imaging and pertinent clinical features of the following intraventricular neoplasms are reviewed: ependymoma, subependymoma, central neurocytoma, subependymal giant cell tumor, choroid plexus tumors, medulloblastoma, meningioma, chordoid glioma, rosette-forming glioneuronal tumor, central nervous system lymphoma, and metastasis. Background and Study Aim Colloid cysts usually occur in the anterior third ventricle at the level of the foramina of Monro. Colloid cysts may extend from the third toward the lateral ventricle. We present a rare case of multiple intraventricular colloid cysts, two of which were in the third ventricle and one in the lateral ventricle. Clinical Description A 40-year-old female patient presented with three intraventricular cystic lesions: one cyst in the typical localization in the anterior rostral third ventricle, another cyst behind it in the same (third) ventricle, and a larger bulging cyst in the right lateral ventricle. A bilateral ventriculoperitoneal shunt had been inserted 26 years before to treat hydrocephalus. All three cysts had different magnetic resonance imaging (MRI) signal characteristics. We removed the cysts through an endoscopically assisted right transcortical transventricular microsurgical approach, using the right ventricular catheter as a guide to the lateral ventricle. After removal of the lateral ventricular cyst, we observed that the foramen of Monro was greatly enlarged (most likely as a result of the large cyst), which allowed us to remove the cysts in the third ventricle. During surgery, the cysts were found to have different consistencies. MRI 2 years following surgery showed complete removal and no hydrocephalus. The patient had no symptoms, and the clinical examinations were normal. Conclusions Colloid cysts may become large and extend to the lateral ventricle, especially in patients treated with ventriculoperitoneal shunts. We present the case of a patient who died of a fatal meningoencephalitis after removal of a third ventricle colloid cyst. Postoperative clinical and iconographic evolution let us think about an acute disseminated encephalomyelitis probably due to cerebrospinal fluid contamination by inflammatory proteins contained in the colloid cyst. Colloid cyst is a rare benign intracranial lesion. Due to its embryologic origin and development, it is commonly located within the third ventricle, though it also presents at various locations. Its location within cavum septum pellucidum is rarely reported. Operative approaches to such lesion can be challenging and misleading due to intraoperative variations. However, surgical resection in such locations are simplified and perfected with stereotactic guidance. A case of colloid cyst within the cavum septum pellucidum and the importance of stereotactic guidance in precisely reaching the lesion, thus minimizing the dissection of corpus callosum, are discussed. Colloid cysts have been associated with acute neurologic deterioration and sudden death. However, the low incidence of associated sudden deaths has meant that factors influencing outcome in patients who present with acute neurological deterioration have not been extensively published. A PubMed literature search was performed to identify reported patients who presented with acute neurological deterioration with radiographic or histopathologic diagnosis of a colloid cyst. Demographic data, presenting symptoms, physical exam, surgical interventions, and outcomes were recorded. Analysis included 140 patients. Mean cyst size was 2.12 cm in males and 1.59 cm in females (p = 0.155), and 1.64 cm in patients who survived and 2.05 cm in patients who died (p = 0.04). Minimum cyst size was 0.4 cm in females and 0.8 cm in males. All patients without surgical intervention died, versus 48% with surgical intervention (p < 0.0001). Patient age was not significantly associated with outcome. Patients with hydrocephalus who have symptomatic colloid cysts are at extremely high risk for acute neurological deterioration and sudden death. Larger cyst size was associated with higher mortality, regardless of intervention. Prompt surgical intervention in extremis can lead to survival in approximately half the patients. Females, even with smaller cyst sizes, may be more likely to die before any intervention and may therefore benefit from more aggressive treatment approaches. [ABSTRACT FROM AUTHOR]

Objective Clinical significance and management of asymptomatic colloid cysts of the third ventricle is not well defined. The aim of this study was to investigate the risk of cyst progression necessitating surgical intervention during a surveillance period. Methods A systematic pooled analysis of the literature was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. A comprehensive search (conducted in December 2017) in MEDLINE and EMBASE databases, identified eligible studies. Data related to demographic (sex, age, size), clinical (surgical intervention, acute neurological deterioration, cyst related mortality) and radiological outcomes (cyst stability, progression, regression) were extrapolated and analysed. Results Of the 134 manuscripts identified, only 4 retrospective studies (176 patients) met the inclusion criteria. The level of evidence provided by these studies was low. During a median follow up of 61.2 months (IQR 41.6–70.1), 11 patients (8.6%, 95% CI 4.7–14.9) required surgical intervention due to either clinical or radiological progression. One patient experienced an acute neurological decline (0.8%, 95% CI −0.3–4.7), which eventuated in death a few years later. There were no reported cases of sudden death during this period. On radiological follow up, 86.7% (95% CI 78.5–92.2) of cysts remained stable, 11.2% (95% CI 6.2–19.2) progressed, and 2.0% (95% CI 0.1–7.6) regressed in size. Conclusion For incidental colloid cysts deemed appropriate for conservative management, there is a 5–15% risk of future progression necessitating operative intervention in the 5 years following diagnosis. Introduction: Colloid cysts are uncommon lesions in the pediatric age group, which most commonly occur in the fourth through fifth decades. The authors hereby report a series of 36 patients with colloid cysts in the pediatric age group.Material and method: A retrospective chart review was conducted on all patients with colloid cyst who underwent surgery in our institute between November 2003 and December 2016 (13 years). Patients above the age of 18 were excluded from the study. They were analyzed based on clinical presentation, radiological findings, surgical approaches, and outcome.Results: There were 36 pediatric patients selected for the study. Age ranged from 8 to 18 years. The male-to-female ratio was 3.5:1. Headache and vomiting were the most common symptoms, and papilledema is the most common clinical sign. The mean duration of symptoms was 9 months (range 1 day to 5 years), but 27 (75%) of the children had precipitous symptoms just before presentation. Preoperative CT showed a hyperdense non-enhancing lesion in the majority of cases. Endoscopic excision was done in 13; 5 patients underwent transcortical transventricular excision, while transcallosal approach was opted for in 17 patients. In one patient, the colloid cyst could not be removed endoscopically and had to be converted to transcortical transventricular approach. Postoperatively, five patients developed CSF leak. They were successfully managed conservatively. One patient had operative site extra dural hematoma and underwent re-exploration, and two patients had transient hemiparesis which improved spontaneously. The median follow-up period was 9 months in the 30 available patients. Though no formal neuropsychological testing was done at follow-up, all children appeared to be doing well without memory disturbances.Conclusion: Pediatric colloid cysts are less common and may show rapid deterioration. Timely surgery results in a permanent cure with minimum morbidity. The results of either micro neurosurgical or endoscopic operative excision of colloid cysts in children are excellent. All children who are symptomatic with raised intracranial pressure due to a third ventricular colloid cyst should undergo definitive surgery. Objective: To study the efficacy of transcallosal, transforaminal approach in excision of colloid cysts of third ventricle in terms of complete excision and post-operative complications. Study Design: Prospective observational study. Place and Duration of Study: Study was carried out in neurosurgery department of CMH Rawalpindi, from July 2009 to Jun 2014. Material and Methods: All suspected cases were subjected to CT scan and MRI of brain for diagnosis and size and site of colloid cyst. All patients having colloid cyst of anterior third ventricle irrespective of age, sex and size of the cyst were included in the study. Patients with recurrence or previous surgery were excluded from the study. Patients with psychiatric illness and abnormalities of memory were also excluded. A uniform approach in terms of position of patients, craniotomy site and flap elevation was adopted for all patients. Post operatively the complications were recorded at 2nd day and 6th week. Derangement of short and long term memory, beads stringing and touch localization were recorded on a specific proforma. Results: A total of 19 patients with colloid cyst of 3rd ventricle were included, thirteen were male and 6 were female patients. The most common presentation was headache and vomiting. Most frequent cyst size was 1-2 cm and was present in 57.9%, while 4 patients presented with a large cystic swelling measuring 2-3 cm. A preoperative shunt had to be inserted in two cases due to sudden deterioration of the neurological status. Most of the cases (68.4%) presented with hydrocephalus, and it was absent only in 6 cases. Complete excision was obtained in 17 cases while in 2 cases the cyst could not be removed completely. Most frequent complication on 2nd postoperative day was the impaired short term memory which was present in 13 patients. Beads stringing was impaired in 9 patients on 2nd post-operative day. On sixth post-operative week, only 2 cases had short term memory loss as improvement occurred in all complications. Conclusion: Transcallosal, transforaminal approach is an effective and safe approach for the excision of colloid cysts of anterior third ventricle. Background: Colloid cysts are benign lesions of the roof of the third ventricle, often diagnosed incidentally; sometimes they can cause hydrocephalus due to obstruction of the foramina of Monroe. Symptomatic cysts could be resected either microsurgically (transcallosal or transcortical) or endoscopically. Although both strategies are effective and have advantages and disadvantages, there is no consensus on the choice of the optimal approach. Transcallosal resection, although more invasive than endoscopy, allows adequate bimanual manipulation of the cyst and is associated with high rates of complete resection, the use of neuronavigator and intraoperative ultrasound optimizes surgical trajectory and improves safety of the procedure with complication rates comparable to endoscopy. Endoscopy is less invasive but complete resection of solid cysts can be challenging. Case Description: In Video 1, we show resection of a solid partially calcified colloid cyst using a transcallosal bilateral transforaminal approach to anterior third ventricle male, 65 years old; headache and mild memory impairment for 6 months; admitted at our emergency department because of a brief loss of consciousness. Neurologic examination was normal. Computed tomography (CT) and magnetic resonance imaging (MRI) showed a colloid cyst at the level of anterior third of the third ventricle (1.5 cm in diameter) with hypointense appearance in T2 sequences suggesting a solid calcific component. The ventricular system was enlarged. Colloid cyst risk score 3/5 (diameter >0.7 cm, headache, risk zone I) considered an intermediate-risk subgroup according to Alford et al. On this basis, we proposed the surgical treatment. We chose a transcallosal microsurgical resection. The patient gave consent for the procedure. A preoperative planning with a computer-generated 3D model is performed to simulate the approach. Craniotomy, interhemispheric dissection, and callosotomy were planned with the neuronavigator and with the aid of intraoperative ultrasound to optimize the trajectory and perform a limited and tailored callosotomy. The 1.5 cm callosotomy allows to approach both lateral ventricles, the cyst was progressively dissected working bilaterally through both foramina of Monroe without injuries of the fornices. Resection at term is complete. Postoperative MRI and CT scan confirmed complete excision without complications; the patient was discharged after a week in good neurological condition with complete regression of headache. Conclusion: Microscopic transcallosal resection of the colloid cyst of the third ventricle allows for complete resection with low complication rates. ObjectiveOpen microsurgical and endoscopic approaches are the two main surgical options for excision of colloid cysts. Controversy remains as to which is superior. Previous studies consist of small cohort sizes. This topic has not been investigated using national administrative claims data which benefits from larger patient numbers.MethodsCurrent Procedural Terminology (CPT) and International Classification of Disease version 9 (ICD-9) coding at inpatient visit was used to select for index surgical procedures corresponding to microsurgical or endoscopic excision of colloid cysts. Comorbidities, costs, and complications were collected.ResultsWe identified a total of 483 patients. In all, 240 were from the microsurgical cohort and 243 were from the endoscopic cohort. The two groups displayed similar demographic and comorbidity profiles. Thirty-day post-operative complications were also similar between groups with the exception of seizures and thirty-day readmissions, both higher in the open surgical cohort. The seizure rates were 14.7 and 5.4% in the microsurgical and endoscopic cohorts, respectively (p = 0.0011). The thirty-day readmission rates were 17.3 and 9.6% in the microsurgical and endoscopic cohorts, respectively (p = 0.0149). Index admission costs and 90-day post discharge payments were higher in patients receiving microsurgical excision.ConclusionAn analysis of administrative claims data revealed few differences in surgical complications following colloid cyst excision via microsurgical and endoscopic approaches. Post-operative seizures and thirty-day readmissions were seen at higher frequency in patients who underwent microsurgical resection. Despite similar complication profiles, patients undergoing microsurgical excision experienced higher index admission costs and 90-day aggregated costs suggesting that complications may have been more severe in this group.

Background Colloid cyst is a gelatin-containing cyst in the brain almost always found in the third ventricle. The specific shape and location of these cysts, a round well-delineated mass in the rostral part of the third ventricle adjacent to the foramen of Monro, on imaging are the main findings for diagnosis. Several masses of the third ventricle masquerading colloid cysts on images have been reported. Based on different surgical approaches, preoperative misdiagnosis of colloid cyst may have great impact on prognosis. Methods We report 2 cases that presented with severe headache and hydrocephalus, and their preoperative images were highly indicative of colloid cyst. Results Histopathologic investigations after tumor resection showed pilocytic astrocytoma of fornix in both cases. Conclusions Fifteen cases of colloid cyst misdiagnosis with other masses have been reported thus far; among them, 2 cases were pilocytic astrocytoma. In this study we report 2 other cases. Context: Colloid cyst is a rare tumour of the brain that has a good outcome upon surgical removal. Aims: We present the outcome of surgical management of colloid cyst at our setup based on the experience of last ten years. Study Design, Setting and Period: The type of study is a retrospective review of cases of third ventricular colloid cyst operated at the Department of Neurosurgery, Civil Hospital Karachi, Pakistan, from January 2005 to December 2015. Methods and Material: The patient's data including the clinical records, radiographic data, operative and follow up notes were studied. The patient population was divided into groups based upon the size of the cyst. The surgical approaches utilized were the transfrontal transventricular and transcallosal approach. All the patients also underwent a series of tests to determine the function of the somatosensory, motor and memory functions both pre and post operatively and upon follow up. Statistical analysis used: Data was analyzed using SPSS version 23. Mean and standard deviation was used for continuous variables, while frequency and percentage was used for categorical variables. Results: The study population n= 60 patient, n= 36 males and n= 24 females. The size distribution of the cyst was, n= 4 (6.66%) were less than 1.5cm, n=47 (78.33%) were 1.5-3cm and n=9 (15%) were >3cm. Headache was the most common presenting complaint. Papilledema was observed in 71.66% of the patients, while memory changes were observed in 8.33% of the patients. The surgical excision of the cyst was performed in n= 52, patients while ventriculoperitoneal shunt was performed in n= 8 patients. Conclusions: According to the results of our study the transcallosal approach, is a safe technique, in which the neurological damage is limited due to minimal callosotomy and manoeuvring of the fornix. Aim: Colloid cysts are benign tumors originating from primitive neuroepithelial or endodermic origin in the third ventricle. Asymptomatic, as well as paroxysmal headache, gait disturbance, nausea, vomiting, learning difficulty and death may occur. Computed tomography (CT) is also seen as a round or oval, non-contrasting lesion. Magnetic resonance imaging (MRI) also shows hyperintense at T1 weighed and isointens at T2 weighed sequences. Stereotactic aspiration, microscopic or endoscopic approaches, shunt can be applied. Total excision should be targeted. Material and Methods: We aimed to investigate the age, sex, complaint, hydrocephalus presence, neurological examination findings, surgical method and results of 19 cases of colloid cyst in our clinic between 2012-2017. Results: Eleven of 19 cases were female (57.9%) and 8 were male (42.1%).The average age was 27.2 (2-62). The most common complaint was headache. Ptosis due to visual disturbance was seen in 5.3% (1 patient), ataxia in 10.5% (2 patients), hydrocephalus with memory loss in 31.5% (6 patients). Six patients underwent cystectomy with endoscopic third ventriculostomy (ETV) and transcranial surgery in 13 patients. Ventriculoperitoneal shunt was performed to one patient at eight months after surgery. Rhinorrhea and menengitis was seen in one patient. Medical theraphy was given to this patient. One patient died because of acute cerebral anfarct. Conclusion: Asymptomatic old patients must be followed periodically. Symptomatic patients must be treated surgıcally and it must be combined by V-P shunt if patients had hydrocephalus. One of the surgıcal endoscopic or transcranial technics can be selected. Background This study analyzes the results of surgical treatment in 377 patients with colloid cysts (CCs) of the third ventricle who were treated at the Burdenko Neurosurgery Institute from 1981 to 2015. Operations were performed by a single surgeon (the first author of the article). Methods The transcallosal approach was used to remove CCs in 97% of cases. Total cyst removal was performed in 96% of cases (in 4% of cases, a small cyst capsule fragment was left on the veins and fornix); in 3 cases, the cyst recurred, which required its repeated removal. Results Hydrocephalus symptoms regressed in 98% of patients. The most frequent complications in the early postoperative period were memory impairments of varying severity, with a tendency to regress by the time of discharge. Two patients had an intraventricular hematoma, which required operative exploration in 1 patient. There were 4 cases of meningitis (including 1 shunt-associated meningitis) and 5 cases of transient pyramidal insufficiency. Conclusions We made a preliminary assessment of the reasonability of the infratentorial supracerebellar approach for removal of CCs in 10 cases. [ABSTRACT FROM AUTHOR]

Introduction: Colloid cysts are the most common pathologic lesions of the third ventricle. Although they are histologically benign, they may grow and can cause the hydrocephalus. Case summary: A 5-year-old male patient underwent to surgery with the diagnosis of colloid cyst. In operation, after the anterior transcallosal approach, the cyst was punctured by syringe and obviously pus aspirated. Conclusion: Colloid cysts mostly remain silent and detected incidentally. Although it is rarely defined, rhinorrhea, hypopituitarism, diabetes insipidus, spasmodic torticollis, drop attack, aseptic meningitis, and coexistence with neurocysticercosis have been reported, but this is the first reported case of an infected colloid cyst.
