105. Clinical application guidelines for blood glucose monitoring in China (2022 edition).

Abstract

Glucose monitoring is an important component of diabetes management. The Chinese Diabetes Society (CDS) has been producing evidence‐based guidelines on the optimal use of glucose monitoring since 2011. In recent years, new technologies in glucose monitoring and more clinical evidence, especially those derived from Chinese populations, have emerged. In this context, the CDS organised experts to revise the Clinical application guidelines for blood glucose monitoring in China in 2021. In this guideline, we focus on four methods of glucose monitoring that are commonly used in clinical practice, including capillary glucose monitoring, glycated haemoglobin A1c, glycated albumin, and continuous glucose monitoring. We describe the definitions and technical characteristics of these methods, the factor that may interfere with the measurement, the advantages and caveats in clinical practice, the interpretation of glucose metrics, and the relevant supporting evidence. The recommendations for the use of these methods are also provided. The various methods of glucose monitoring have their strengths and limitations and cannot be replaced by one another. We hope that these guidelines could aid in the optimal application of common methods of glucose monitoring in clinical practice for better diabetes care. [ABSTRACT FROM AUTHOR]

123. Diagnostic value of quantitative sensory testing in diabetic peripheral neuropathy in patients with type 2 diabetes mellitus.

Abstract

Objective To explore the diagnostic value of quantitative sensory testing (QST) in diabetic peripheral neuropathy in patients with type 2 diabetes mellitus (T2DM). Methods Patients with T2DM aged 18-70 years in Nanjing Drum Tower Hospital from May 2020 to February 2021 were retrospectively analyzed. QST parameters [warm detection threshold (WDT), cold detection threshold (CDT)], heat pain threshold (HPT) and cold pain threshold (CPT) were collected, and nerve conduction velocity (NCV) parameters of both lower limbs were measured, including common peroneal nerve conduction velocity, sural nerve conduction velocity, tibial nerve conduction velocity, etc. Patients received QST and NCV for allocation into different neuropathy groups according to the diagnostic criteria: nondiabetic peripheral neuropathy (NDPN) group, small fiber neuropathy (SFN) group and mixed fiber neuropathy (MFN) group. The t-test or one-way analysis of variance (ANOVA), Mann Whitney U or Kruskal Wallis H test, or χ2 test were used to compare the general clinical data between groups. Multivariate logistic regression analysis was used to analyze the risk factors of DPN. Spearman correlation method was used to analyze the correlation between QST parameters and NCV. Receiver operating characteristic curve (ROC) analysis was used to evaluate the sensitivity, specificity and diagnostic value of QST for MFN. Results A total of 279 patients were included, including 111 in NDPN group, 116 in SFN group and 52 in MFN group. Compared with SFN group, MFN group presented lower CDT and CPT, higher WDT and HPT (all P< 0.05). The results of correlation analysis showed that the common peroneal nerve conduction velocity, sural nerve conduction velocity and tibial nerve conduction velocity of both lower limbs were positively correlated with CDT (r=0.264, 0.366 and 0.288 respectively) and CPT (r=0.226, 0.253 and 0.236 respectively) (all P<0.05), They were negatively correlated with WDT (r=-0.274, -0.338 and -0.268 respectively) and HPT (r=-0.246, -0.281 and -0.192 respectively) (all P<0.05). The results of ROC model showed that when WDT+CDT was used to diagnose MFN, the product under the curve was 0.840, the sensitivity was 86.3%, and the specificity was 71.8% (P<0.001). 138. Comparison of Beinaglutide Versus Metformin for Weight Loss in Overweight and Obese Non-diabetic Patients.

Abstract

Purpose We compared the efficacy and safety of beinaglutide, a glucagon-like peptide-1 (GLP-1) analogue with metformin in lowering the bodyweight of patients who were overweight/obese and non-diabetic. Patients and Methods Seventy-eight non-diabetic patients were randomly selected and beinaglutide or metformin was administered for 12 weeks. The primary endpoints were changes in body weight and the proportions of patients who lost≥5 and≥10% of their baseline body weights. Results A total of 64 patients completed the study; patients in the beinaglutide group exhibited more bodyweight loss than those in the metformin group [(9.5±0.8%; 9.1±0.9 kg) and (5.1±0.9%; 4.5±0.8 kg), respectively, corresponding to a difference of approximately 4.5 kg (95% confidence interval, 2.2–6.9 kg; P <0.01)]. In the beinaglutide group, 90.6 and 40.6% of the patients lost≥5 and≥10% of their body weight, respectively, whereas, in the metformin group, these rates were 46.9 and 12.5%, respectively (P <0.01 and P< 0.05). Weight loss following beinaglutide treatment mainly resulted from the loss of fat mass. Compared to metformin, beinaglutide induced a greater decrease in the body mass index, weight circumference, percent body fat, and body fat mass (total, trunk, limb, android, and gynoid). Additionally, beinaglutide decreased serum insulin levels and ameliorated insulin resistance. Conclusions Beinaglutide is more efficient than metformin at reducing weight and fat mass in patients who are overweight/obese and non-diabetic. Abstract

Antenna-in-Package(AiP) technology has an advantage that a system can maintain antenna performance while being small in size and highly integrated. In this paper, based on LTCC technology, a rectangular corner cut microstrip antenna for 5G communication band is designed, and then integrated into a package of the chip with AiP technology. The simulation results of HFSS software show that after miniaturized and highly integrated, the AiP system achieves the goal with operation frequency of 4.8 ~ 4.9GHz, bandwidth of 100MHz and the peak gain of 6.52dB.

Abstract

Aims To assess the effects on glycaemic control of lixisenatide vs placebo as add‐on treatment to basal insulin (BI) ± metformin and effects on glycated haemoglobin (HbA1c) reduction in patients with insufficiently controlled type 2 diabetes (T2D). Methods Patients (n = 448) with inadequately controlled T2D were randomized (1:1) to lixisenatide or placebo as add‐on to BI ± metformin for 24 weeks after an 8‐week run‐in phase, during which BI was titrated to a target self‐monitored plasma glucose (SMPG; 4.4‐5.6 mmol/L). The primary endpoint was absolute change in HbA1c from baseline to week 24. Secondary efficacy endpoints included: percentage of responders; changes in 2‐hour postprandial plasma glucose (PPG); 7‐point SMPG (daily average); body weight (BW); total daily BI dose; fasting plasma glucose; and safety assessments. Results Baseline demographics were similar in the two treatment groups. After insulin optimization during run‐in, lixisenatide was superior to placebo in mean change from baseline (7.9% [standard deviation {s.d.}, 0.66] and 7.9% [0.70], respectively) to week 24 in HbA1c (least squares mean [standard error {s.e.}] change −0.62% [0.09] vs −0.11% [0.09]; P
