Abstract

Little is known about the effects of discontinuing mesalamine therapy for patients with Crohn's disease (CD) who initiate therapy with an anti-tumor necrosis factor (anti-TNF) agent. We analyzed data from 2 national population cohorts to compare outcomes of patients with CD already on mesalamine therapy who started treatment with an anti-TNF agent and then either stopped or continued mesalamine.The primary outcome was any adverse clinical event, defined as a composite of new corticosteroid use or CD-related hospitalization or surgery. We collected data from the Truven MarketScan (IBM, Armonk, NY) health claims database in the United States and the Danish health registers. Our analysis included patients with CD who started anti-TNF therapy after at least 90 days of oral mesalamine therapy. Patients were classified as stopping mesalamine if therapy was discontinued within 90 days of starting anti-TNF. We performed multivariable Cox regression models controlling for demographics, clinical factors, and health care utilization. Adjusted hazard ratios with 95% CIs were calculated, comparing stopping mesalamine with continuing mesalamine.A total of 3178 patients with CD were included in our final analysis (2960 in the United States and 218 in Denmark). Stopping mesalamine after initiating anti-TNF therapy was not associated with an increased risk of adverse clinical events in the US cohort (adjusted hazard ratio, 0.89; 95% CI, 0.77-1.03; P = .13) or in the Danish cohort (adjusted hazard ratio, 1.13; 95% CI, 0.68-1.87; P = .63). Similar results were obtained from sensitivity analyses of concomitant immunomodulator use and duration of mesalamine treatment before initiation of anti-TNF therapy.In a retrospective analysis of 2 national databases, we found that stopping mesalamine therapy for patients with CD who were starting anti-TNF therapy did not increase their risk of adverse clinical events. These results should be validated in a prospective study.

Abstract

Background We aimed to describe the long-term impact of achieving endoscopic and deep remission among participants in the effect of tight control management on CD (CALM) trial. Methods We analysed medical records from patients with follow-up data since end of CALM. Patients were stratified by outcomes in CALM at 1 year: clinical remission (Crohn’s disease activity index, CDAI Results One hundred twenty-two patients with median age 29 years (IQR 22.5–37) and median disease duration 0.2 years (IQR 0.1–0.8) were included. Median follow-up time from end of CALM was 3.02 years (range 0.05–6.26 years). Fifty per cent were randomised to the tight control arm. There were no significant differences in baseline characteristics in patients with follow-up data and those lost to follow-up with the exception of a slightly higher CDEIS score in patients lost to follow-up (14.6 vs. 12.9, p = 0.04). Thirty-four patients (27.9%) had a major adverse outcome during follow-up. Patients in clinical remission at 1 year did not have significantly lower rates of the composite endpoint (log-rank p = 0.15). Patients in endoscopic and deep remission at the end of CALM were significantly less likely to have a major adverse event over time (Figures 1 and 2). After adjusting for age, disease duration, prior surgery, prior stricture, and randomisation arm, endoscopic remission (aHR 0.44, 95% CI 0.20–0.96, p = 0.038) and deep remission (aHR 0.25, 95% CI 0.09–0.72, p = 0.01) were significantly associated with lower risk of major adverse events. Conclusions Early CD patients who achieve endoscopic or deep remission after 1 year of intensive treatment are less likely to have disease progression over a median of 3 years.

Résumé: L'endoscopie est l'examen de référence pour le diagnostic et la surveillance des maladies inflammatoires chroniques intestinales (MICI). À l'heure des stratégies de « tight-control » et de « treat-to-target », le développement de biomarqueurs ou d'examen non invasif, bien toléré, répétable et peu coûteux est indispensable. Comparativement, à l'endoscopie ou à l'imagerie en coupe, l'échographie intestinale s'est avérée performante dans l'évaluation et la surveillance de l'activité de la maladie de Crohn et de la rectocolite hémorragique. Elle peut être réalisée « au chevet du malade » et permettre ainsi une prise de décision clinique en temps réel. L'objectif de cette revue est d'effectuer une mise au point sur la place actuelle et future de l'échographie dans les MICI. The gold standard for the diagnosis and monitoring of inflammatory bowel disease (IBD) is the colonoscopy. In the era of "tight control" and "treat to target" strategies, the development of non-invasive, well-tolerated, repeatable and inexpensive biomarkers are needed. Compared to colonoscopy and Magnetic resonance imaging, bowel ultrasound (US) has shown to have the same level of accuracy in assessing and monitoring disease activity and severity of both Crohn's disease and ulcerative colitis. It can be performed at the point-of-care and therefore allow for real-time clinical decision-making. The aim of this review is to discuss the interest of intestinal ultrasound in the management of patients with IBD. Abstract

The identification of Crohn's disease (CD)‐associated adherent and invasive Escherichia coli(AIEC) is time‐consuming and requires ileal biopsies. We aimed to identify a faster and less invasive methods to detect ileal colonization by AIEC in CD patients. CD patients requiring ileo‐colonoscopy were consecutively enrolled in this prospective multicenter study. Samples from saliva, serum, stools, and ileal biopsies of CD patients were collected. Among 102 CD patients, the prevalence of AIEC on ileal biopsies was 24.5%. The abundance and global invasive ability of ileal‐associated total E. coliwere respectively ten‐fold (p= 0.0065) and two‐fold (p= 0.0007) higher in AIEC‐positive (vs. AIEC‐negative), while abundance of total E. coliin the feces was not correlated with AIEC status in the ileum. The best threshold of ileal total E. coliwas 60 cfu/biopsy to detect AIEC‐positive patients, with high negative predictive value (NPV) (94.1%[80.3–99.3]), while the global invasive ability (>9000 internalized bacteria) was able to detect the presence of AIEC with high positive predictive value (80.0% [55.2–100.0]). Overall, 78.1% of the AIEC + patients were colonized by two or less different AIEC strains. The level of serum anti‐total E. coliantibodies (AEcAb) was higher in AIEC‐positive patients (p= 0.038) with a very high negative predictive value (96.6% [89.9–100.0]) (p= 0.038) for a cut‐off value > 1.9 × 10−3. More than two thirds of AIEC‐positive CD patients were colonized by two or less AIEC strains. While stools samples are not accurate to screen AIEC status, the AEcAb level appears to be an attractive, rapid and easier biomarker to identify patients with Crohn's disease harboring AIEC.

L'infliximab (IFX) a rapidement été associé à un risque de réaction à l'injection potentiellement grave. Les biothérapies sous-cutanées (adalimumab, golimumab et ustékinumab) peuvent être responsables de réactions locales, qui justifient exceptionnellement l'arrêt du traitement. Les incertitudes sur les mécanismes physiologlogiques engagés et le peu d'études d'intervention ont abouti à des protocoles d'administration et de prise en charge des réactions multiples et empiriques. Biologics used for the treatment of Inflammatory Bowel Diseases are associated with a risk of immediate reaction (< 24h). Infliximab (IFX) was associated with a potentially serious infusion reaction risk. Subcutaneous biologics (adalimumab, golimumab and ustekinumab) may be responsible for local reaction, which exceptionally justifies stopping treatment. 142. Is there concordance between the Cellvizio system and pathology in determining microscopic inflammation at the ileal resection margins in Crohn's ileal disease? The CELLVICROHN study

Abstract

Crohn's disease is an inflammatory bowel disease for which surgery is required in approximately 80% of cases. However, surgery does not cure Crohn's disease. The type of surgery is important, as there are intraoperative risk factors for recurrence, including microscopic inflammation at the resection margins. This point is crucial, as a too large resection may result in short bowel syndrome and a too short resection (microscopic inflammation at the resection site) a higher risk of postoperative recurrence (75% vs 46% at 18 months). Based on the results of a randomised control trial, surgeons are encouraged to perform a limited resection (2 cm from macroscopic Crohn's disease). However, such a macroscopically non-inflamed resection margin may show microscopic inflammation (up to 80%). Thus, it would be useful to evaluate whether there is microscopic inflammation at the resection margin. Moreover, there is an increasing interest in the role of the mesentery in recurrence, which is still unclear. Thus, it would be informative to also clarify the border between inflamed and non-inflamed mesentery. Cellvizio is a type of confocal laser endomicroscopy system that provides the possibility to obtain high-magnification in-vivo images of the gut epithelium. This allows real-time examination of the gastrointestinal mucosa at the cellular and subcellular level. Cellvizio has never been used during surgery for Crohn's disease.

149. No Benefit of Concomitant Immunomodulator Therapy on Efficacy of Biologics That Are Not Tumor Necrosis Factor Antagonists in Patients With Inflammatory Bowel Diseases: A Meta-analysis.

Abstract

There is debate over whether patients with inflammatory bowel diseases (IBD) treated with biologics that are not tumor necrosis factor antagonists (such as vedolizumab or ustekinumab) should receive concomitant treatment with immunomodulators. We conducted a meta-analysis to compare the efficacy and safety of concomitant immunomodulator therapy vs vedolizumab or ustekinumab monotherapy. In a systematic search of publications, through July 31, 2019, we identified 33 studies (6 randomized controlled trials and 27 cohort studies) of patients with IBD treated with vedolizumab or ustekinumab. The primary outcome was clinical benefit, including clinical remission, clinical response, or physician global assessment in patients who did vs did not receive combination therapy with an immunomodulator. Secondary outcomes were endoscopic improvement and safety. We performed random-effects meta-analysis and estimated odds ratio (OR) and 95% CIs. Overall, combination therapy was not associated with better clinical outcomes in patients receiving vedolizumab (16 studies: OR, 0.84; 95% CI, 0.68–1.05; I2=13.9%; Q test P =.17) or ustekinumab (15 studies: OR, 1.1; 95% CI, 0.87–1.38; I2 = 11%; Q test P =.28). Results were consistent in subgroup analyses, with no difference in clinical remission or response in induction vs maintenance studies or in patients with Crohn's disease vs ulcerative colitis in studies of vedolizumab. Combination therapy was not associated with better endoscopic outcomes in patients receiving vedolizumab (3 studies: OR, 1.13; 95% CI, 0.48–2.68; I2 = 0; Q test P=.96) or ustekinumab (2 studies: OR, 0.58; 95% CI, 0.21–1.16; I2 = 47%; Q test P =.17). Combination therapy was not associated with an increase in adverse events during vedolizumab therapy (4 studies: OR, 1.17; 95% CI, 0.75–1.84; I2 = 0; Q test P =.110). In a meta-analysis of data from studies of patients with IBD, we found that combining vedolizumab or ustekinumab with an immunomodulator is no more effective than monotherapy in induction or maintenance of remission.
