Purpose The purpose of our study was to evaluate the usefulness of the BiofireFilmArray Meningitis/Encephalitis (FA ME) panel and identify factors associated with positive FA ME panel results. Methods We retrospectively analyzed the medical records of 129 patients (45 children, 84 adults) with central nervous system infections and FA ME panel test results. We compared the results of the FA ME panel and the diagnostic yield between pediatric and adult patients with positive and negative FA ME panel results. Positive FA ME panel results were compared with conventional microbiological testing. Results Forty-three patients (33.3%) had positive results on the FA ME panel. The detected pathogens were enterovirus (n=17, 39.5%), varicella zoster virus (n=10, 23.3%), herpes simplex virus-2 (n=2, 16.3%), herpes simplex virus-1 (n=2, 4.7%), human parechoviruses (n=2), Cryptococcus neoformans/gattii (n=2), Streptococcus agalactiae (n=1, 2.3%), Streptococcus pneumoniae (n=1), and Haemophilus influenzae (n=1). The time to detection of the FA ME panel (4.9±6.6 hours) was significantly shorter than that of conventional culture (77.4±48.2 hours) (P=0.005). Background Laparoscopic cholecystectomy (LC) is a noninvasive surgery, but postoperative pain is a major problem. Studies have indicated that erector spinae plane block (ESPB) has an analgesic effect after LC. We aimed to compare the efficacy of different ESPB anesthetic concentrations in pain control in patients with LC. Methods This retrospective study included patients aged 20 to 75 years scheduled for LC with the American Society of Anesthesiologists physical status classification I or II. ESPB was administered using 0.375% bupivacaine in group 1 and 0.25% in group 2. Both groups received general anesthesia. Postoperative tramadol consumption and pain scores were compared and intraoperative and postoperative fentanyl requirements in the postanesthesia care unit (PACU) were measured. Results Eighty-five patients were included in this analysis. Tramadol consumption in the first 12 hours, second 12 hours, and total 24 hours was similar between groups (p>0.05). The differences between postoperative numeric rating scale (NRS) scores at rest did not differ significantly. The postoperative NRS scores upon bodily movement were not statistically different between the two groups, except at 12 hours. The mean intraoperative and postoperative fentanyl requirements in the PACU were similar. The difference in the requirement for rescue analgesics was not statistically significant (p=0.788). Conclusion Ultrasound-guided ESPB performed with different bupivacaine concentrations was effective in both groups for LC analgesia, with similar opioid consumption. A lower concentration of local anesthetic can be helpful for the safety of regional anesthesia and is recommended for the analgesic effect of ESPB in LC.

Complex perovskite La2(Al1/2MgTa1/2)O6 (LAMT) crystallizes in a monoclinic unit cell with space group P21/n at room temperature. Its B-site cations are ordered in a rock-salt-type arrangement. Previously, the full occupancy of Mg on the 2c-Wyckoff position was deduced from powder X-ray diffraction (PXRD). However, conventional X-rays could not properly resolve the mixed occupation on the B-site, since there is little scattering contrast between the neighbouring elements Mg and Al of the periodic table. Hence, complementary neutron diffraction studies were carried out to verify the exact B-site cation ordering in the unit cell. In this specific configuration of the B-cations, with its occupancy ratio and the presence of a heavy element Ta as well as neighbouring elements Mg and Al, only the strategy of a combined Rietveld analysis using both the X-ray and neutron diffraction data simultaneously succeeded in elucidating an accurate B-site cation ordering in this complex perovskite system. A full occupancy of Mg on the 2c-Wyckoff position and each a half occupancy of Al and Ta on the 2d-Wyckoff position could be resolved for the rock-salt-type ordering of the B-site cations in the monoclinic unit cell of LAMT.

Nowadays, piezoelectric speakers have been actively studied for the applications to smartphone, TV monitor, and car audio system. Accordingly, for developing the piezoelectric speaker composition ceramics, in this paper, (Na,K,Li)(Nb,Sb)O3–(Bi,Na)ZrO3 ceramics with Rhombohedral-Tetragonal (R-T) region were fabricated with iron oxide addition, and their piezoelectric characteristics were investigated. When iron oxide was added to the ceramics, d33and kp were decreased. However, Qm was increased. At the NKN-BNZ ceramics with 0.1 wt% iron oxide. The piezoelectric constant d33, electromechanical coupling factor kp, and dielectric constant εr were optimized as 263[pC/N], 0.450, 1978, respectively, for piezoelectric speaker application. This study aimed to evaluate the effect from usage and autoclaving on torsional fracture resistance of heat-treated nickel-titanium rotary instruments. ProTaper Gold F2 (PG) and TruNatomy Prime (TN) were selected. For group-N, new files were tested to make a baseline torsional resistance. For group-U, files underwent instrumentation in a J-shaped canal block, and files were tested. For group-S, files underwent instrumentation as in the group-U and followed by autoclave sterilization then were tested. Maximum torsional strength and distortion angel until fracture were measured using a customized device and toughness was calculated. Statistical analysis was conducted at the significance level of 95%. The topographic appearances of torsional fracture fragments were examined under scanning electron microscopy. PG showed significantly higher torsional strength after usage and sterilization; however, TN had reduced torsional strength (p < 0.05). PG showed significantly higher torsional strength in group-S than subgroup-N, while TN showed significantly lower torsional strength in group-U than group-N (p < 0.05). Regardless of the test conditions, PG showed significantly higher torsional strength and toughness than TN while TN had bigger distortion angle than PG (p < 0.05). The mechanical loads from usage and heat application during sterilization may result in alteration of torsional resistances according to geometry and alloy characteristics.

Photonic lanterns (PLs) are tapered waveguides that gradually transition from a multimode fiber geometry to a bundle of single-mode fibers (SMFs). They can efficiently couple multimode telescope light into a multimode fiber entrance at the focal plane and convert it into multiple single-mode beams. Thus, each SMF samples its unique mode (lantern principal mode) of the telescope light in the pupil, analogous to subapertures in aperture masking interferometry (AMI). Coherent imaging with PLs can be enabled by the interference of SMF outputs and applying phase modulation, which can be achieved using a photonic chip beam combiner at the backend (e.g., the ABCD beam combiner). In this study, we investigate the potential of coherent imaging by the interference of SMF outputs of a PL with a single telescope. We demonstrate that the visibilities that can be measured from a PL are mutual intensities incident on the pupil weighted by the cross correlation of a pair of lantern modes. From numerically simulated lantern principal modes of a 6-port PL, we find that interferometric observables using a PL behave similarly to separated-aperture visibilities for simple models on small angular scales (< λ / D ) but with greater sensitivity to symmetries and capability to break phase angle degeneracies. Furthermore, we present simulated observations with wave front errors (WFEs) and compare them to AMI. Despite the redundancy caused by extended lantern principal modes, spatial filtering offers stability to WFEs. Our simulated observations suggest that PLs may offer significant benefits in the photon-noise-limited regime and in resolving small angular scales at the low-contrast regime.

Abstract Background Approximately 30% to 40% of older adults have hyperkyphosis, defined as excessive curvature of the thoracic spine. Hyperkyphosis is associated with increased morbidity and mortality. This study aimed to determine whether hyperkyphosis (Cobb’s angle) and upper extremity tasks were independently associated with the 6-min walk test (6MWT) in community-dwelling older adults with hyperkyphosis. Methods In this cross-sectional study, we studied 71 women and 28 men aged 60–87 from the study of hyperkyphosis, exercise, and function trial (SHEAF) who had kyphosis, 3 timed upper extremity tasks and the 6MWT assessed at their baseline visit. We used standing lateral spine radiographs and a standardized protocol for thoracic kyphosis (T4-T12) to measure Cobb angle of kyphosis. In addition, 3 activity of daily living (ADL) extremity tests (putting on and removing a laboratory coat, picking up a penny from the floor, and lifting a 7-lb. book to a shelf) were used. Results The mean ± SD age was 70.1 ± 6.1 years. The mean ± SD Cobb angle of kyphosis was 57.4 ± 12.5 degrees. On average ± SD, the participants walked 504.8 ± 84.2 m in 6 min and took 2.4 ± 2.2 prescription medications. The mean ± SD height was 164.7 ± 8.5 cm, weight was 68.7 ± 13.1 kg, and BMI was 25.2 ± 4.0 kg/m2.
