In this study, we investigated an energy harvesting effect of tensile stress using piezoelectric polymers and flexible electrodes. A chemical-vapor-deposition grown graphene film was transferred onto both sides of the PVDF and P(VDF-TrFE) films simultaneously by means of a conventional wet chemical method. Output voltage induced by sound waves was measured and analyzed when a mechanical tension was applied to the device. Another energy harvester was made with a metallic electrode, where Al and Ag were deposited by using an electron-beam evaporator. When acoustic vibrations (105 dB) were applied to the graphene/PVDF/graphene device, an induced voltage of 7.6 Vpp was measured with a tensile stress of 1.75 MPa, and this was increased up to 9.1 Vpp with a stress of 2.18 MPa for the metal/P(VDF-TrFE)/metal device. The 9 metal/PVDF/metal layers were stacked as an energy harvester, and tension was applied by using springs. Also, we fabricated a full-wave rectifying circuit to store the electrical energy in a 100 μF capacitor, and external vibration generated the electrical charges. As a result, the stored voltage at the capacitor, obtained from the harvester via a bridge diode rectifier, was saturated to ~7.04 V after 180 s charging time.

Background We investigated the real-life effectiveness and safety of vedolizumab maintenance treatment among Korean patients with Crohn’s disease (CD) or ulcerative colitis (UC) who previously failed anti-tumour necrosis factor (anti-TNF) therapy. Methods Adult patients with CD or UC who have previously failed anti-TNF therapy and received vedolizumab were prospectively enrolled from 16 hospitals in Korea. The primary outcome was clinical remission at week 54. Clinical remission was defined as a Crohn’s disease activity index (CDAI) Results Between August 2017 to July 2020, a total of 165 patients (81 with CD and 84 with UC) received vedolizumab therapy, of whom 154 patients (93.3%) (75 with CD and 79 with UC) received vedolizumab maintenance therapy (Table 1). Clinical remission and response rates at week 54 were 22.2% and 24.1% among patients with CD and 41.4% and 45.7% among patients with UC, respectively (Figure 1A and 1B). Among 70 patients with UC with baseline Mayo endoscopic subscore ≥2, endoscopic remission (Mayo endoscopic subscore ≤1) at week 54 was observed in 19 patients (27.1%). Out of 50 patients with CD with ulcers in baseline endoscopy, 2 patients (4%) showed a disappearance of ulcers at week 54 (Figure 1C). In the multivariable analysis, age at baseline (adjusted odds ration [aOR] 1.065, 95% confidence interval [CI] 1.003–1.131, P=0.041) and Mayo endoscopic subscore at baseline (aOR 0.141, 95% CI 0.026–0.746, P=0.021) were significantly associated with clinical remission at week 54 among patients with UC (Table 2). No factors were found to be associated with clinical remission at week 54 among patients with CD. Among patients who experienced one or more adverse events (n=134, 81.2%), serious adverse events occurred in 82 patients (49.7%) (Table 3). Disease exacerbation was the most common adverse events (n=89, 53.9%). Conclusion The real-life effectiveness of vedolizumab maintenance treatment for Korean patients with UC who failed anti-TNF therapy was generally similar with the outcomes reported from the previous Western studies. A substantial proportion of patients with CD experienced a loss of response during the first year of treatment. Less severe disease at baseline was associated with clinical remission at 1 year of vedolizumab therapy among patients with UC.

Background We aimed to compare trough infliximab levels and the development of anti-drug antibody (ADA) for 1 year between Crohn’s disease (CD) and ulcerative colitis (UC) patients who were biologic-naïve and to evaluate their impact on clinical outcomes. Methods This was a prospective, multi-center, observational study. Biologic-naïve patients with moderate to severe CD and UC who started CT-P13 therapy were eligible for the study. The trough drug and ADA levels were measured serially for 1-year after CT-P13 initiation. Clinical outcomes were assessed with intention-to-treat purpose. Results 267 patients who received CT-P13 treatment were enrolled in the study (CD 168, UC 99). The rates of clinical remission (72% vs. 32.3%, p Conclusion CD shows favorable pharmacokinetics of infliximab including high trough drug and low ADA level compared with UC which might be related with better clinical outcomes for 1-year of infliximab.

Mitochondrial dysfunction is associated with neuronal damage in Huntington's disease (HD), but the precise mechanism of mitochondria-dependent pathogenesis is not understood yet. Herein, we found that colocalization of XIAP and p53 was prominent in the cytosolic compartments of normal subjects but reduced in HD patients and HD transgenic animal models. Overexpression of mutant Huntingtin (mHTT) reduced XIAP levels and elevated mitochondrial localization of p53 in striatal cells in vitro and in vivo. Interestingly, XIAP interacted directly with the C-terminal domain of p53 and decreased its stability via autophagy. Overexpression of XIAP prevented mitochondrially targeted-p53 (Mito-p53)-induced mitochondrial oxidative stress and striatal cell death, whereas, knockdown of XIAP exacerbated Mito-p53-induced neuronal damage in vitro. In vivo transduction of AAV-shRNA XIAP in the dorsal striatum induced rapid onset of disease and reduced the lifespan of HD transgenic (N171-82Q) mice compared to WT littermate mice. XIAP dysfunction led to ultrastructural changes of the mitochondrial cristae and nucleus morphology in striatal cells. Knockdown of XIAP exacerbated neuropathology and motor dysfunctions in N171-82Q mice. In contrast, XIAP overexpression improved neuropathology and motor behaviors in both AAV-mHTT-transduced mice and N171-82Q mice. Our data provides a molecular and pathological mechanism that deregulation of XIAP triggers mitochondria dysfunction and other neuropathological processes via the neurotoxic effect of p53 in HD. Together, the XIAP-p53 pathway is a novel pathological marker and can be a therapeutic target for improving the symptoms in HD.

Omental torsion and infarction are rare causes of acute abdominal pain, and it is often difficult to diagnose these conditions preoperatively. There are several reports showing that computed tomography is useful to diagnose these conditions. Here, we report a case of a 59-year-old man with omental infarction due to secondary omental torsion associated with an untreated inguinal hernia. Preoperative computed tomography showed a fatty mass in the right lower quadrant and a right inguinal hernia containing torsion of the greater omentum. He underwent laparoscopic partial omentectomy and herniorrhaphy, and the postoperative course was uneventful.

Development of tungsten brazing technology for the upgraded Korea superconducting tokamak advanced research divertor was launched in early 2013. ITER grade tungsten block was brazed on the CuCrZr alloy in vacuum at 980 °C for 30 min using silver free brazing alloy. An OFHC-copper was used as an interlayer between tungsten and the CuCuZr. The brazing alloy is a 0.05 mm thickness of plate of which component is a Ni–Cu–Mn. It is found that the optimal loading on mock-up is about 20 kPa. Ultrasonic test and shear strength test were carried out to check the strength between tungsten and substrate material. And the joint conditions were analyzed by scanning electron microscopy. Tungsten-brazed mock-ups with a cooling tube were tested at the electron beam facility, Korea heat load test facility-electron beam in Korea Atomic Energy Research Institute. The high heat flux test was performed for tungsten-brazed mock-ups under heat flux of about 5 MW/ $\text{m}^{2}$ with more than 2000 cycles. All the tungsten-brazed mock-ups met the requirements and there was no delamination or failure at the bonding joints.

Background As the treatment paradigm of inflammatory bowel disease changes towards patient-centred treatment, it is becoming increasingly important to measure patient-reported outcomes (PROs). We aimed to identify the changes of emotional health and work- or activity-related outcomes one year after the diagnosis of ulcerative colitis (UC) and its predictors in patients enrolled in the moderate-to-severe UC in Korea (MOSAIK) cohort (ClinicalTrials.gov: NCT02229344). Methods The MOSAIK cohort is the first nationwide, prospective, inception cohort on moderate-to-severe UC in Korea. Between August 2014 and March 2017, consecutive patients from 30 tertiary hospitals were enrolled. PRO data including hospital anxiety and depression scale (HADS) for emotional health, and work productivity and activity impairment (WPAI) questionnaire for work- or activity-related outcomes, were collected within the first 4 weeks of diagnosis via patient surveys. Wilcoxon-signed rank tests and linear mixed-effects regression models were used for paired comparisons between baseline and 1 year and assessing the predictors of HADS and WPAI. Results Of the 368 enrolled patients, 333 eligible patients were analyzed. The mean age at diagnosis was 39 years and 57.7% (192/333) were male. A considerable number of patients had moderate to high (≥11 by HADS) levels of anxiety and depression (16.0% and 20.5%, respectively), and about half of patients had work and activity impairment (45.5% and 45.8%%, respectively) at baseline. After 1 year follow-up, significant reduction of anxiety and depression (mean difference [MD] in HADS score -1.3 for both anxiety and depression, P Conclusion Newly diagnosed moderate-to-severe UC patients had considerable anxiety, depression, and work and activity impairment at baseline, but significant improvement was noted after 1 year. Controlling symptoms and disease activity was the most important factor to improve PROs after 1 year.

Background Vedolizumab (VDZ) inhibits gut lymphocyte trafficking by binding to α4β7 integrin, which can be effective for patients with Crohn’s disease (CD) or ulcerative colitis (UC). We aimed to investigate the clinical outcomes and response predictors of VDZ treatment for Korean patients with CD or UC, who were previously failed to anti-tumour necrosis factor (TNF) therapy. Methods Between August 2017 and November 2019, a total of 159 patients with CD (n = 81) or UC (n = 78) received a VDZ induction therapy from 16 centres and were prospectively enrolled. Of those, patients who were evaluated at week 14 after three induction doses of VDZ (week 0, 2, and 6) were analysed. The co-primary endpoints were corticosteroid-free clinical remission and endoscopic remission/response (for UC) at week 14. We also analysed predictors of corticosteroid-free clinical remission, persistence of vedolizumab and safety. Results A total of 153 patients were analysed (CD, 77 [50.3%]; male, 94 [61.4%]; median age, 40 years [range, 17–80]; median disease duration, 8.0 years [range, 0.1–38.0]). All patients had previously experienced failures to at least one anti-TNF agent (one, 105 [68.6%]; two, 44 [28.8%]; three, 4 [2.6%]). Corticosteroid-free clinical remission/response rates in CD and UC patients were 44.6%/51.8% and 39.4%/62.0%, respectively. In patients with UC, endoscopic remission and response rates defined by Mayo endoscopic subscore/ulcerative colitis endoscopic index of severity were 33.8%/14.1% and 55.4%/39.1%, respectively. Multivariate analysis revealed that a clinical response at week 6 were associated with a corticosteroid-free clinical remission at week 14 in both CD (Odds ratio [OR] 33.84, 95% confidence interval [CI] 6.25–183.31, p < 0.001) and UC (OR 12.22, 95% CI 1.30–115.28, p = 0.029). In addition, UC patients with higher baseline levels of C-reactive protein (CRP) and faecal calprotectin were less likely to be in corticosteroid-free clinical remission (CRP > 0.31 mg/dl: OR 0.05, 95% CI 0.00–0.60, p = 0.019; faecal calprotectin > 2,000 μg/g: OR 0.04, 95% CI 0.00–0.93, p = 0.045). The cumulative probabilities of continuing VDZ after one year were 48.7% for CD and 65.7% for UC, respectively. During median 10 months of follow-up periods (range, 3–26 months), disease exacerbation was the most common adverse event (n = 73, 47.7%), followed by nasopharyngitis (n = 23, 15.0%) and arthralgia (n = 19, 12.4%). Conclusion In anti-TNF-failed Korean patients with CD and UC, VDZ induction therapy was effective with an acceptable safety profile. Early clinical response and higher inflammatory burden at baseline were associated with corticosteroid-free clinical remission after VDZ induction therapy.

Background Despite proven efficacy of anti-tumour necrosis factor-α agents (ant-TNF) for inflammatory bowel disease (IBD), some patients have to be discontinuation of anti-TNF for various reasons in a real-world clinical setting. The aim of this study was to evaluate the long-term outcomes and risk factors of relapse after discontinuation of anti-TNF in IBD patients with clinical remission. Methods A retrospective multicenter cohort study was conducted at 10 referral hospitals, affiliated in IBD Study Group of the Korean Association for the Study of Intestinal Diseases. The study population comprised patients diagnosed with Crohn’s disease (CD) or Ulcerative colitis (UC) who had been treated with anti-TNF (infliximab (IFX) or adalimumab (ADA)) to induce remission and in whom ant-TNF had been discontinued after clinical remission was achieved. The patients were excluded for follow-up of Results A total of 125 IBD patients were eligible. Among them, 109 IBD patients including 71 CD and 38 UC were analyzed and median follow-up period was 56 months (interquartile range, 35–90 months). The reasons of discontinuation of anti-TNF was physician’s decision (n = 32, 29.4%), patient’s own preference (n = 30, 27.5%), anti-TNF-related adverse events/opportunistic infection (n = 18, 16.5%), and other reasons (n = 29, 26.6%). After discontinuation of anti-TNF, relapse occurred in 49 CD patients (69%) and 19 UC patients (50%). Relapse-free survival rate at 1, 2, 3, and 5 years in patients with CD were 11.3%, 31.4%, 46.7%, and 62.5%, respectively, and that in patients with UC was 28.9%, 34.8%, 45.3%, and 60.9%, respectively. Multivariate Cox regression analysis identified the risk of relapse was associated with adalimumab use (vs. infliximab: hazard ratio [HR], 4.41; 95% confidence interval [CI], 1.18–16.41; p = .027) and discontinuation due to physician’s decision (vs. patient’s preference: HR, 0.13, 95% CI, 0.04–0.49, p = .002) in patients with CD, whereas that was decreased in UC patients with mucosal healing (vs. non-mucosal healing: HR=0.07, 95% CI, 0.01–0.58, p = .014). Retreatment with anti-TNF was done in 54 patients (49.5%) and effective in 45 patients(83.3%). Conclusion The discontinuation of anti-TNF was associated with increased risk of relapse. Although retreatment of anti-TNF seems to be effective and safe, the discontinuation of anti-TNF should be carefully considered based on the type of anti-TNF, the reason for discontinuation, and the mucosal healing status.

A 5-channel two color interferometer (TCI) system has been designed on KSTAR. TCI system is designed for tangential beam paths, which will combine with two existing interferometer systems of vertical and radial beam paths, so that it will provide 3-dimensional measurement of electron density evolution. TCI system uses wavelengths of 10.6 μm by a CO2 laser and 0.633 μm by a HeNe laser. The system compensates the vibrational noise by using two colors and avoids refraction by short wavelengths. The main purpose of the TCI is to generate routine measurement of the line integrated plasma density for the real time density control on KSTAR. The 5-channels will provide profile data for the density. Time resolution of the system is expected to be 500 kHz or higher in order to measure 3-dimensional density fluctuations for ELMs and other MHD activities including TAE modes. The system is planned to be working on KSTAR 2016 campaign with 1–2 channels.

OBJECTIVE: To examine the relationship between high-sensitivity C-reactive protein (hs-CRP) levels and lung function in a community-based cohort of South Korea.DESIGN: The Ansung-Ansan cohort database (an ongoing prospective study of a community-based population) was used in the analysis. We defined airway obstruction as the ratio between forced expiratory volume in 1 sec:forced vital capacity ratio (FEV1:FVC) of RESULTS: A total of 5528 individuals were eligible for the study. The average age was 55.1 years, and 47.8% were males. The prevalence of airway obstruction was 9.0%, and the mean hs-CRP level was 1.51 mg/dl. Serum hs-CRP levels increased with the severity of airway obstruction, and the latter worsened with an increase in the hs-CRP level. In multivariate analysis, as the hs-CRP level increased, FEV1 and FVC decreased. A higher FEV1:FVC ratio was associated with lower hs-CRP levels in males.CONCLUSION: Higher hs-CRP levels were associated with decreased FEV1 and FVC in a general population of Korea. The FEV1:FVC ratio decreased with an increase in the hs-CRP level in males.

The random dynamics of two different tumor cell types on a substrate were observed under a microscope and analyzed based on three parameters—mean square displacement (MSD), turning angle distribution (TAD), and velocity autocorrelation function (VACF)—under well-controlled experimental conditions wherein only thermal energy existed. A two-dimensional analysis on the substrate showed linear MSD but a lowered diffusion coefficient compared to the prediction by Stokes–Einstein equation due to particle substrate interaction. Moreover, anomalous characteristics based on the MSD and TAD of the two different cells, A549 (adenocarcinomic human alveolar basal epithelial cells) and MCF-7 (breast cancer cell line; estrogen, progesterone receptors +, HER2 −), were observed, and the nonlinear MSD at a short time scale was detected. This result is attributed to the localization error from the bright-field image of a different cell taken, while the cell moved randomly on the substrate. Parameters related to random process and localization errors were employed to distinguish the two cell types. This study could be useful for the characterization of cellular properties and cell types.

It is necessary for nuclear power plant operation and spent fuel canisters to provide a sound technical basis for the safety and security of long-term operation and storage respectively. A new code case for mitigation of Primary Water Stress Corrosion Cracking (PWSCC) and Chloride Induced Stress Corrosion Cracking (CISCC) in Section III components by using an advanced surface stress improvement technology (ASSIT) is being developed by Task Group ASSIT which is one of the task groups under the ASME (The American Society of Mechanical Engineers). The necessary technical reports supporting this code case are being developed as part of joint research projects conducted by Doosan Heavy Industries and Construction (DOOSAN), Electric Power Research Institute (EPRI) and Sun Moon University (SMU). A well-known approach to prevent PWSCC and CISCC are to be performed using materials resistant to PWSCC and CISCC. The objective is to eliminate residual tensile stresses, or to induce compressive residual stress using ASSIT methods such as laser peening, water jet/cavitation peening, ultrasonic peening and ultrasonic nanocrystal surface modification (UNSM). Performance and measurement criteria for mitigation of PWSCC by ASSIT will be established based on the magnitude of surface stress and depth of compressive residual stress, sustainability, inspectability and lack of adverse effects. Additionally, for mitigation of CISCC by ASSIT, the evaluation of chloride induced corrosion pitting, the depth and density of corrosion pits and stress corrosion crack initiation and growth under chloride salt chemistry conditions are also being examined. This paper explains the approach, and progress of testing and analysis. The results and details from testing and analysis will be presented in a future PVP paper upon completion.

The PHENIX collaboration presents first measurements of low-momentum ($0.4$1\,GeV/$c$) direct-photon yield $dN_{\gamma}^{\rm dir}/d\eta$ is a smooth function of $dN_{\rm ch}/d\eta$ and can be well described as proportional to $(dN_{\rm ch}/d\eta)^\alpha$ with $\alpha{\approx}1.25$. This scaling behavior holds for a wide range of beam energies at the Relativistic Heavy Ion Collider and the Large Hadron Collider, for centrality selected samples, as well as for different, $A$$+$$A$ collision systems. At a given beam energy the scaling also holds for high $p_T$ ($>5$\,GeV/$c$) but when results from different collision energies are compared, an additional $\sqrt{s_{_{NN}}}$-dependent multiplicative factor is needed to describe the integrated-direct-photon yield., Comment: 673 authors from 82 institutions, 10 pages, 4 figures. v2 is version accepted for publication in Physical Review Letters. Atomistic-isotropic film removal has been demonstrated using noble techniques. Its surface modification such as oxidation or halogenation has been used to form a volatile by-product of mono layer by decoupled plasma. Surface modification has been developed using various conditions to adsorb with specific chemistries such as ligands or halogens at low water temperature, and in-situ thermal pulse with flash lamp as a desorption step has been performed to remove films atomistically to avoid unwanted thermal budget and extremely high selectivity. Finally performing continuous sequential cycle of adsorption and desorption step enables to remove films consistently with infinite selectivity.

Stress-related mucosal disease (SRMD), or stress ulceration, is a group of conditions ranging from stress-related superficial gastric mucosal damage to deep gastric ulcers that are primarily correlated with mucosal ischemia, and pharmacologic interventions that optimize tissue perfusion or preserve defensive mucus aim to decrease the occurrence of conditions, such as gastric acidity, or enhance gastric defenses. However, the identification of multifactorial pathogenesis may be effective in preventing SMRD, and the use of stress prophylaxis is generally preferred. Since threonine is a component in the polymerization and synthesis of gastric mucin and possibly enhanced defense actions and lignin may provide structural support for defense and antioxidative function, we hypothesized that dietary intake of threonine and/or lignin can enhance defense against SRMD. The water immersion-restraint stress (WIRS) was used in rats and additional groups were pretreated with threonine alone or the combination of threonine and lignin. Based on gross and microscopic evaluations, threonine alone or the combination of threonine and lignin, a natural antioxidant, significantly reduced the development of SRMD (P0.05). According to molecular explorations, the levels of inflammatory mediators, such as interleukin (IL)-8, IL-6, IL-1β, inducible nitric oxide synthase (iNOS), tumor necrosis factor alpha (TNF-α), and interferon gamma (IFN-γ), all of which are mediators that play a significant role in controlling WIRS, significantly decreased in the groups pretreated with either threonine alone or the combination of threonine and lignin (P0.01). WIRS significantly increased apoptosis in the stomach. However, the apoptotic index significantly decreased with threonine pretreatment. According to periodic acid Schiff staining results, the expression of gastric mucin was significantly preserved in groups pretreated with threonine but remarkedly decreased in the WIRS group. The gastric heme oxygenase-1 levels significantly increased in the group treated with threonine. In conclusion, the dietary intake of threonine or the combination of threonine and lignin is effective in preventing SRMD.

The e4 allele of the apolipoprotein E gene (APOE4) is the strongest genetic risk factor for late-onset Alzheimer’s disease (AD) compared to the common e3 allele. Because APOE4 is also associated with an increased risk for several vascular conditions including small vessel disease and vascular cognitive impairment (VCI), apoE4 effects on cerebrovasculature might be a converging pathway impacting brain homeostasis. As apoE is abundantly expressed in multiple brain cell types including astrocytes, microglia, and vascular mural cells (VMCs), defining the in vivo effect of apoE4 expressed by individual cell types is critical for addressing its pathogenic mechanism. Here, using conditional mouse models, we show that VMC-specific apoE4 expression impairs cognition and glio-vascular functions. Expression of either apoE3 or apoE4 in VMCs was sufficient to rescue the hypercholesterolemia and atherosclerosis phenotypes seen in the Apoe-knockout mice. Intriguingly, vascular specific expression of apoE4, but not apoE3, led to decreased arteriole blood flow, impaired spatial learning, and increased anxiety-like phenotypes. Single cell RNA sequencing of vascular and glial cells isolated from these mice revealed that conditional expression of apoE4 in VMCs was associated with astrocyte activation while expression of apoE3 in VMCs was associated with angiogenic signatures in pericytes. Together, our data support cell-autonomous effects of vascular apoE on multiple brain homeostatic processes in an isoform-dependent manner, suggesting a critical contribution of vascular apoE in the pathogenesis of AD and VCI.

We describe a compact and stable setup for detecting the optical second harmonics, in which the incident plane rotates with respect to the sample. The setup is composed of rotating Fresnel-rhomb optics and a femtosecond ytterbium-doped fiber-laser source operating at the repetition frequency of 10 MHz. The setup including the laser source occupies an area of 1 m2 and is stable so that the intensity fluctuation of the laser harmonics can be less than 0.2 % for 4 h. We present the isotropic harmonic signal of a gold mirror of 0.5 pW and demonstrate the integrity and sensitivity of the setup. We also show the polarization-dependent six-fold pattern of the harmonics of a few-layer WSe2, from which we infer the degree of local-field effects. Finally, we describe the extendibility of the setup to investigate the samples in various conditions such as cryogenic, strained, ultrafast non-equilibrium, and high magnetic fields.

Development of inflammation modulating polymer scaffolds for soft tissue repair with minimal postsurgical complications is a compelling clinical need. However, the current standard of care soft tissue repair meshes for hernia repair is highly inflammatory and initiates a dysregulated inflammatory process causing visceral adhesions and postsurgical complications. Herein, the development of an inflammation modulating biomaterial scaffold (bioscaffold) for soft tissue repair is presented. The bioscaffold design is based on the idea that, if the excess proinflammatory cytokines are sequestered from the site of injury by the surgical implantation of a bioscaffold, the inflammatory response can be modulated, and the visceral adhesion formations and postsurgical complications can be minimized. The bioscaffold is fabricated by 3D-bioprinting of an in situ phosphate crosslinked poly(vinyl alcohol) polymer. In vivo efficacy of the bioscaffold is evaluated in a rat ventral hernia model. In vivo proinflammatory cytokine expression analysis and histopathological analysis of the tissues have confirmed that the bioscaffold acts as an inflammation trap and captures the proinflammatory cytokines secreted at the implant site and effectively modulates the local inflammation without the need for exogenous anti-inflammatory agents. The bioscaffold is very effective in inhibiting visceral adhesions formation and minimizing postsurgical complications.

Amyotrophic lateral sclerosis (ALS) is a devastating neurodegenerative disorder that leads to a progressive muscle wasting and paralysis. The pathological phenotypes are featured by severe motor neuron death and glial activation in the lumbar spinal cord. Proposed ALS pathogenic mechanisms include glutamate cytotoxicity, inflammatory pathway, oxidative stress, and protein aggregation. However, the exact mechanisms of ALS pathogenesis are not fully understood yet. Recently, a growing body of evidence provides a novel insight on the importance of glial cells in relation to the motor neuronal damage via the non-cell autonomous pathway. Accordingly, the aim of the current paper is to overview the role of astrocytes and microglia in the pathogenesis of ALS and to better understand the disease mechanism of ALS.

Tungsten (W) monoblock that consists of W according to ITER specifications, oxygen-free high conductive copper (Cu-OFHC), and CuCrZr has been designed in preparation for KSTAR upgrade for steady-state long-pulse operation with 27.5-MW heating power. Finite-element analysis has been performed by ANSYS Workbench. The hydraulic thermomechanical analysis is performed to investigate the optimal design parameters and the expected fatigue lifetime under the operating conditions with 5.4-MW/ $\text{m}^{2}$ incident heat flux at the divertor. For a single W monoblock, the optimization gives a W thickness of 5 mm, a side thickness of 2.5 mm, a thickness in the back part of 6 mm, and a Cu-OFHC interlayer thickness of 0.5 mm. For a mock-up consisting of five optimized W monoblocks, the thermal fatigue analysis is performed. As a result, the maximum value of equivalent total strain in the CuCrZr tube is found to be 0.266%, which corresponds to for a fatigue lifetime of 23 000 cycles.

Primary mediastinal leiomyoma is an extremely rare benign tumor of smooth muscle. Most common radiographic appearance is a well circumscribed heterogeneous solid mass. We reported a case of giant cyst-like lesion at the middle mediastinum, which was pathologically confirmed as a primary mediastinal leiomyoma.

The present study involves a detailed experimental investigation of the concurrent compositional clustering and long-range ordering tendencies in a Ni-33 at.%Cr alloy, carried out by coupling synchrotron-based X-ray diffraction (XRD), transmission electron microscopy (TEM), and atom probe tomography (APT). Synchrotron-based XRD results clearly exhibited progressively increasing lattice contraction in the matrix with increasing isothermal aging time, at 475 °C, eventually leading to the development of long-range ordering (LRO) of the Pt 2 Mo-type. Detailed TEM and APT investigations revealed that this LRO in the matrix is manifested in the form of nanometer-scale ordered domains, and the spatial distribution, size, morphology and compositional evolution of these domains have been carefully investigated. APT results also revealed the early stages of compositional clustering prior to the onset of long-range ordering in this alloy and such compositional clustering can potentially be correlated to the lattice contraction and previously proposed short-range ordering tendencies.

Recently, the global demand for reducing pedestrian injuries that result from vehicle accidents has increased. The lower legs of pedestrians, in particular, are more frequently injured than other parts of the body in accidents because they make direct contact with the front bumper systems of vehicles. Thus, it is necessary to reduce injuries by improving the structure of the frontal bumper. EPP (expanded polypropylene) foam is commonly used to absorb energy in a bumper system, but the foam is not sufficient to absorb the impact in the limited inner bumper space. Thus, a crumple bumper structure made from engineering plastics could be more effective in reducing the level of injury. The dynamic material properties for engineering plastics are required in pedestrian-to-vehicle collision analysis to determine the design shape for this structure. In general, it is very difficult to extract these properties. In this study, an indirect method to extract the dynamic material properties for the engineering plastics is applied: a correlation method between the falling-dart impact test and finite element analysis. Finally, an optimized plastic energy absorber is proposed to improve pedestrian safety performance by conducting a parameter study for its design values.

Gamma-rays are an important mutagenic agent that can induce new, useful genetic variations in plants. However, γ-irradiation can also cause damage that negatively affects the use of such mutagens in plant breeding. The aim of the present study was to investigate the effects and damage caused by γ-radiation in a Cymbidium hybrid, RB001. The relative growth rate of protocorm-like bodies (PLBs) was reduced by 50% at a γ-ray dose of approximately 40 Gy. Malondialdehyde concentrations increased significantly with increasing radiation dose. However, almost no difference was observed between untreated control PLBs and PLBs treated with 200 Gy 8 weeks after γ-irradiation. The activities of several antioxidant defence enzymes increased gradually with increasing γ-ray dose, 24 h after irradiation. A flexible polymer-dispersed liquid crystal (PDLC) film with graphene layers as transparent electrodes was fabricated by combining a UV curable polymer and a nematic liquid crystal. The PDLC film with a dimension of 5 × 10 cm 2 was flexible and operated normally with no damage when it was bent with a radius of 2 mm. Instead of using conventional transparent conducting film, the single-layer-graphene transferred on PET film was used as transparent electrodes. The thickness of the graphene-PDLC was about 210–220 μm. The single-layer-graphene grown by thermal chemical vapor deposition was transferred onto the PET film, and its sheet resistance was about 1.2 kΩ/□. The controllable transmission range between the on and off states was about 60% in the visible range. The response time for the turn-on and off processes were estimated to be 0.3 and 32 ms, respectively. This successful fabrication of graphene based PDLC is a crucial step toward paving the way for the commercialization of the emerging material, graphene.

Background The aim of this study to assess the efficacy and safety of adalimumab (ADA), a monoclonal antibody against tumour necrosis factor α (TNF-α), and to explore predictors of response in Korean patients with ulcerative colitis (UC). Methods We conducted a prospective observational multicenter study over 56 weeks in adult patients with moderately to severely active UC. Clinical response and remission were assessed by Mayo score. Mucosal healing was defined as Mayo subscore 0 or 1. Faecal calprotectin (FC) were assessed at baseline, week 8 and 56. Adalimumab drug levels were checked at week 8 and at loss of response. Missing or incomplete data were handled using the nonresponder imputation method. Results A total of 146 patients were enrolled and included in the analysis. Clinical response rates were 52.1% (76/146) and 37.7% (55/146) at week 8 and 56, respectively. Clinical remission was achieved in 24.0% (35/146) and 21.9% (32/146) of patients at week 8 and 56. Steroid-free remission rates were 21.2% (31/146) at week 56. Mucosal healing rates were 39.0% (57/146) and 30.1% (44/146) at week 8 and 56. Prior use of anti-TNF-α did not affect the clinical and endoscopic responses. Treatment persistence was achieved in 57.5% (84/146) of patients at week 56. Adalimumab drug level was significantly higher in patients with clinical response (10.8 vs. 8.0, p = 0.004), clinical remission (11.7 vs. 8.8, p = 0.007) and mucosal healing (11.0 vs. 8.5, p = 0.010) at week 8. Adalimumab dose was escalated to 40 mg weekly in 25 (17.1%) patients, and clinical response and remission were achieved in 40% and 20% of patients at week 56, respectively. Mean faecal calprotectin levels were significantly more decreased in clinical responders compared with non-responders at week 8 (336.3 mg/kg vs. 628.8 mg/kg, p < 0.001). The Fecal calprotectin levels are well correlated with endoscopic severity, and the best cut-off value to predict mucosal healing was 274 mg/kg. The lower endoscopic severity, higher body mass index and higher serum albumin level at baseline were associated with a clinical response at week 8. The lower Mayo score, lower C-reactive protein level, clinical response (74.5% vs. 38.5%, p < 0.001) and mucosal healing (52.7% vs. 30.8%, p = 0.008) at week 8 were associated with clinical response at week 56. Serious adverse drug reactions were identified in 2.7% (4/146) of patients including 1 case of pulmonary tuberculosis. Conclusion Adalimumab is safe and effective for induction and maintenance in Korean patients with UC, regardless of prior anti-TNF therapy. Adalimumab drug level is associated with the efficacy of induction therapy. A better response to induction therapy can predict a better long-term response.

Objective To evaluate the acceptance of, adherence to, and outcomes of latent tuberculous infection (LTBI) treatment among health care workers (HCWs). Design This was a retrospective study in a tertiary hospital in Korea. From May to August 2017, 2190 HCWs simultaneously underwent a tuberculin skin test (TST) and interferon-gamma release assay (IGRA). LTBI was diagnosed if the TST induration was 10 mm or IGRA results were positive. Results Of 2190 HCWs tested, 1006 (45.9%) were diagnosed with LTBI. Of these, 655 (65.1%) HCWs visited out-patient clinics, 234 (35.7%) of whom were advised treatment by physicians. Among these, 120 (51.3%) accepted the physicians' recommendations. In general, HCWs who were older, male and smoked were less likely to visit out-patient clinics. Sixty (50%) HCWs received 3 months of isoniazid plus rifampicin (3HR) and 57 (47.5%) HCWs received 4 months of rifampicin (4R). The proportion of HCWs with 2 side effects (3HR 20% vs. 4R 7.0%, P = 0.041) and drug stoppage rate (3HR 20% vs. 4R 5.3%, P = 0.017) were higher in the 3HR group than in the 4R group. Of the 120 HCWs, 78 (65%) completed LTBI treatment. Conclusion Overall, the acceptance and completion rate for LTBI treatment was not adequate. For effective LTBI management in HCWs, further programmatic strategies are needed.

Administration of dextran sulfate sodium (DSS) in drinking water led to significant bout of colitis simulating ulcerative colitis of human. However, colitis usually developed 5 - 7 days after DSS administration. Therefore, we hypothesized host defense system might protect colitis up to 5 days of DSS administration. 2.5% DSS-induced colitis were administered to C57BL/6 mice and sequential measurements of pathology, cyclooxygenase-2 (COX-2), nuclear factor-κB (NF-κB), heme oxygenase-1 (HO-1), NADPH quinone oxidoreductase-1 (NQO1), γ-glutamylcysteine synthetase (γ-GCS), nuclear factor erythroid 2-related factor-2 (Nrf2), and keap1 were done at 2, 6, 12, 24, 48, 96, 120, and 168 hour of DSS administration, respectively. Exosomes are attractive as nucleic-acid carriers because of their favourable pharmacokinetic and immunological properties and their ability to penetrate physiological barriers that are impermeable to synthetic drug-delivery vehicles. However, inserting exogenous nucleic acids, especially large messenger RNAs, into cell-secreted exosomes leads to low yields. Here we report a cellular-nanoporation method for the production of large quantities of exosomes containing therapeutic mRNAs and targeting peptides. We transfected various source cells with plasmid DNAs and stimulated the cells with a focal and transient electrical stimulus that promotes the release of exosomes carrying transcribed mRNAs and targeting peptides. In particular, optical and chemical properties of nanomaterials are being exploited to improve the effectiveness of neuro-oncological and neurosurgical interventions. Modern nanotechnology-driven clinical applications may have significant impact on management of brain tumors.

Ion decoupled plasma technique with low ion energy have been used to demonstrate conformal shallow junctions of phosphorous with higher than l E20 of dopants for germanium (Ge). Adding antimony (Sb) in plasma-assisted doping was found to enhance the phosphorous (P) dopant level dramatically. Various annealing techniques were compared to understand the impact to dopant activation and levels to form shallow junctions with enhanced P level.

__Plasma-assisted atomic layer deposition (PALD) was carried out for growing thin phosphorus oxide films onto flat and high-aspect ratio substrates. The applicability of these films as dopant sources for shallow phosphorus doping using different rapid thermal annealing (RTP) methods, such as conventional RTP and flash lamp anneal (FLA), was investigated. The phosphorus oxide films were, as expected, highly unstable in ambient air and needed to be stabilized for further ex-situ investigation. Conformal growth of such films could be obtained on silicon trench structures up to ~ 15: 1 aspect ratio with pitch below 50 nm. Shallow phosphorus doping of silicon could be obtained using the respective oxide films as dopant sources.

Solitary fibrous tumor (SFT) is an uncommon neoplasm of mesenchymal origin, and it most often occurs in the pleura. Mammary gland involvement is extremely rare in this disease. Herein, we report a case of SFT that presented as a slowly growing large mass in the breast of a 64-year-old man. On ultrasonographic examination, the mass was oval and circumscribed with heterogeneous echogenicity and increased intralesional blood flow. Moreover, the mass showed hard elasticity as revealed by shear-wave elastography (SWE), which has not been reported in English literature so far. We also briefly review the literature on the radiologic findings. Because of the nonspecific imaging findings and difficulty in its differentiation from other soft tissue tumors, complete excision with detailed immunohistochemistry is essential.

Repeated bouts of ulcerative colitis featured troublesome course of inflammatory bowel disease leading to fatal colitis-associated cancer, which is strongly associated with oxidative stress and sustained inflammation. Since oligonol, low molecular weighted polyphenol extracted from fruit lychee, showed antioxidative and anti-inflammatory actions, we hypothesized that oligonolcan prevent relapse of colitis. We compared oligonol with current gold standard therapeutics, sulfasalazine in preventive efficacy of relapse. First, dextran sulfate sodium (DSS)-induced colitis were made following pretreatment with oligonol, 10, 50, and 100 mg/kg for 7 days to measure therapeutic effect of oligonol and relapse model via repeated DSS administration was made following with either 50 mg/kg oligonol or 30 mg/kg sulfasalazine to explore relapse preventing action of oligonol in C57BL/6 mice. Detailed changes in colon were measured to explain molecular mechanisms. Pretreatment of 10, 50, 100 mg/kg oligonol (p.o.), significantly reduced DSS-induced colitis; total pathologic scores, colon length, and clinical symptom scores (P0.05). Oligonol pretreatment significantly decreased the levels of interleukin (IL)-1, IL-6, and tumor necrosis factor-α (TNF-α) as well as nuclear factor-κB (NF-κB), c-Fos, and c-Jun in affected colon tissues, but the expression of heme oxygenase-1 (HO-1) and NADH: quinone oxidoreductase-1(NQO-1) as well as total antioxidant concentration (P0.005) was significantly increased with oligonol. A relapse model established with repeated DSS administration led to high mortality. However, oligonol significantly ameliorated exacerbations of colitis, while sulfasalazine did not (P0.01). Significantly decreased expressions of cyclooxygenase-2 (COX-2), TNF-α, and macrophages inhibition were relapse preventing actions of oligonal, but significant action of oligonol relevant to relapse prevention was either significantly increased expressions of NQO-1 or significantly preserved mucin (P0.05). Concerted anti-inflammatory, antioxidative, and host defense enhancing actions of oligonol can be applied during maintenance therapy of IBD to prevent relapse of IBD.

Adnexal torsion occurs when the ovary and fallopian tube twist on the axis created between the infundibulopelvic ligament and the utero-ovarian ligament. The symptoms are mostly unspecific and diagnosis is therefore not simple. Early diagnosis is essential to preserve organ function and fertility. The increased use of assisted reproductive technologies has led to an increase in the risk of adnexal torsion, particularly in pregnant women or women with ovarian hyperstimulation syndrome (OHSS). A gestational age eight-week pregnant woman who received in vitro fertilization (IVF) came to the clinic and was suspected of adnexal torsion. The patient underwent an operation and the biopsy histologically confirmed ischemia. Here the authors report a case with comparison to other studies, the early diagnosis, and early operation that could save adnexa.
