Abstract

In Fall of 2017, a series of measurement laboratory exercises, built around a microphone preampifier and filter, were used in teaching an engineering measurement techniques course. The microphone circuit included a preamplifier that introduced JFETs, a virtual ground and buffer to illustrate loading effects, a high pass filter and a low pass filter, and a zener-diode-based limiter. The data was sampled using a beaglebone microcontroller’s built-in analog-to-digital converter. The data was transmitted to matlab for post-processing and analysis. The microphone was then used in the Spring 2018 acoustics class. A variety of projects, including a Kundt’s tube, a driven closed-open pipe, a 3D printed horn for a blue-tooth speaker, and an absorptivity measurement for a restaurant’s wall treatment tiles, were completed in the course. It was observed that students showed improved interest in the material (increased in-class and out-of-class questions about specifics of the course material related to the project, increased exploration of material outside the provided course materials, full participation by all members of the class in the project) and learning (improved test scores after the project began relative to scores before the project began) relative to earlier classes where no experimental component was included. Abstract

Aims:\ud The primary aim of this study is to provide data to inform the design of a randomized controlled clinical trial (RCT) of a palliative care (PC) intervention in heart failure (HF). We will identify an appropriate study population with a high prevalence of PC needs defined using quantifiable measures. We will also identify which components a specific and targeted PC intervention in HF should include and attempt to define the most relevant trial outcomes.\ud \ud Methods:\ud An unselected, prospective, near-consecutive, cohort of patients admitted to hospital with acute decompensated HF will be enrolled over a 2-year period. All potential participants will be screened using B-type natriuretic peptide and echocardiography, and all those enrolled will be extensively characterized in terms of their HF status, comorbidity, and PC needs. Quantitative assessment of PC needs will include evaluation of general and disease-specific quality of life, mood, symptom burden, caregiver burden, and end of life care. Inpatient assessments will be performed and after discharge outpatient assessments will be carried out every 4 months for up to 2.5 years. Participants will be followed up for a minimum of 1 year for hospital admissions, and place and cause of death. Methods for identifying patients with HF with PC needs will be evaluated, and estimates of healthcare utilisation performed.\ud \ud Conclusion:\ud By assessing the prevalence of these needs, describing how these needs change over time, and evaluating how best PC needs can be identified, we will provide the foundation for designing an RCT of a PC intervention in HF.

Abstract

Heel ulceration, most frequently the result of prolonged pressure because of patient immobility, can range from the trivial to the life threatening. Whilst the vast majority of heel pressure ulcers (PUs) are superficial and involve the skin (stages I and II) or underlying fat (stage III), between 10% and 20% will involve deeper tissues, either muscle, tendon or bone (stage IV). These stage IV heel PUs represent a major health and economic burden and can be difficult to treat. The worst outcomes are seen in those with large ulcers, compromised peripheral arterial supply, osteomyelitis and associated comorbidities. Whilst the mainstay of management of stage I-III heel pressure ulceration centres on offloading and appropriate wound care, successful healing in stage IV PUs is often only possible with surgical intervention. Such intervention includes simple debridement, partial or total calcanectomy, arterial revascularisation in the context of coexisting peripheral vascular disease or using free tissue flaps. Amputation may be required for failed surgical intervention, or as a definitive first-line procedure in certain high-risk or poor prognosis patient groups. This review provides an overview of heel PUs, alongside a comprehensive literature review detailing the surgical interventions available when managing such patients.

Abstract

This pilot study examined the use of early HbA1c in screening for gestational diabetes mellitus and adverse pregnancy outcomes in Singapore. One hundred and fifty-one pregnant women with a gestational age of under 14 weeks had an HbA1c test measured with their antenatal bloods prior to a second trimester oral glucose tolerance test. Patient characteristics and pregnancy outcome data were collected. Gestational diabetes mellitus prevalence was 11%. A receiver operating characteristic curve showed an HbA1c level of 5.2% (33 mmol/mol), had an 82% sensitivity, 72% specificity, 97% negative predictive value and 27% positive predictive value to predict gestational diabetes mellitus. Women with HbA1c of 5.2% (33 mmol/mol) or over 5.2% (33 mmol/mol) were older, had higher BMI and were less likely to be Chinese than those with HbA1c less than 5.2% (33 mmol/mol). There was no difference in pregnancy outcomes. Early HbA1c less than 5.2% (33 mmol/mol) may be useful to exclude low-risk Singaporean women from further testing, while those with HbA1c of 5.2% (33 mmol/mol) or greater would still need a oral glucose tolerance test between 24 and 28 weeks' gestation. Abstract

The demographic shift to an ageing population in contemporary Western society offers a new cultural frontier, with rapidly shifting contours that demand creative and critical examination. Rather than take the predominant bio-medical perspective that views ageing in terms of decline and deficit, in this paper I emphasise the poetics and potential of ageing, within a framework of cultural gerontology. From an existential, phenomenological perspective, I weave together current socio-political and psycho-biological research into gendered ageing, with feminist literature on the topic and my own auto-ethnographic narrative as an ageing woman. Drawing on Simone de Beauvoir and Paul Ricoeur, I examine how narrative identity can shift in older age, particularly if a new sense of creative agency is developed that supports the re-storying of lives. I argue that the diverse group of "baby boomer" women currently in their 60s, who I focus on in this essay, will reinvent the face of ageing, as they once transformed the makeup of the workplace.
