Confidentiality is a core tenet of medical professionalism, which enables the maintenance of trust in a doctor-patient relationship. However, both the amount of personal data stored and the number of third parties who might access this data have increased dramatically in the digital age, necessitating the introduction of various national data protection acts. Paradoxically, the Freedom of Information Act 2000 allows United Kingdom citizens to access information held by public bodies, including hospitals. Furthermore, the Public Interest Disclosure Act 1998 actively protects physicians who breach their duty of confidentiality in the name of public interest ('whistleblowing'). This article explores the evolution of the law and ethics in this area, and draws attention to the difficulties in balancing confidentiality against freedom of information. In addition, the role and responsibilities of the non-physician anaesthetist are examined.

This study was undertaken to document the phenotype of Kabuki (Niikawa–Kuroki) syndrome in patients from Australia and New Zealand, with particular emphasis on growth patterns, behavior, and relationship between head circumference and intellectual level. Data on 27 children and adults with Kabuki (Niikawa–Kuroki) syndrome from Australia and New Zealand were collected by questionnaire and clinical assessment. The patients ranged in age from 7 months to 36 years with a mean age of 7 years and 2 months. The mean age at diagnosis was 3⅚ years, but in most cases, the facial phenotype was evident from infancy. The minimum birth prevalence was calculated at 1 in 86,000. Three of our patients died. Parents reported a behavior phenotype characterized by an excellent long-term memory and avoidance of eye contact. No correlation was found between head circumference and severity of intellectual disability. Eight of 14 patients over the age of 5 years were overweight or obese. Six of these eight patients had failure to thrive in infancy. One patient developed insulin-dependent diabetes mellitus in adolescence. Some individuals with Kabuki (Niikawa–Kuroki) syndrome show a characteristic growth profile with failure to thrive in infancy progressing to obesity or overweight in middle childhood or adolescence. A behavior phenotype was noted which requires further investigation. Head size is not a predictor of degree of intellectual disability. © 2004 Wiley-Liss, Inc.

This paper reviews the contrasting properties of radio and EUV/X-ray observations for the study of the solar atmosphere. The emphasis is placed on explaining the nature of radio observations to an EUV/X-ray audience. Radio emission is produced by mechanisms which are well-understood within classical physics. Bremsstrahlung tends to be dominant at low frequencies, while gyro-resonance emission from strong magnetic fields produces bright sources at higher frequencies. At most radio frequencies the images of the Sun are dominated almost everywhere by bremsstrahlung opacity, which may be optically thick or thin depending on circumstances. Where gyro-resonance sources are present they may be used as sensitive probes of the regions above active regions where magnetic field strengths exceed several hundred gauss, and this unique capability is one of the strengths of radio observations. Typically a gyro-resonance radio source shows the temperature on an optically thick surface of constant magnetic field within the corona. Since each radio frequency corresponds to a different magnetic field strength, the coronal structure can be `peeled away' by using different frequencies. The peculiarities of radio observing techniques are discussed and contrasted with EUV/X-ray techniques. Radio observations are strong at determining temperatures and coronal magnetic field strengths while EUV/X-ray observations better sense densities and reveal coronal magnetic field lines: in this way the two wavelength domains are nicely complementary.

Observations of extremely high brightness temperatures in astrophysical objects imply that coherent emission processes must be occurring. An emission process may be coherent due to an inverted energy distribution leading to maser action, or due to bunching of particles in a region less than or of order of the wavelength of the radiation. Calculations of emission by bunches have generally used either a fluid model, e.g. Buschauer and Benford (1976), or a single-particle approach which requires all particles to have the same velocity, e.g. Saggion (1975) uses elements of both. In this paper we investigate gyromagnetic emission by bunches using a single-particle approach which includes the effects of differing particle velocities on the radiation.
