102. Association Between HIV Infection and Cancer Stage at Presentation at the Uganda Cancer Institute

Abstract

Purpose: The HIV epidemic has contributed to the increasing incidence of cancer in sub-Saharan Africa, where most patients with cancer present at an advanced stage. However, improved access to HIV care and treatment centers in sub-Saharan Africa may facilitate earlier diagnosis of cancer among patients who are HIV positive. To test this hypothesis, we characterized the stage of cancer and evaluated the factors associated with advanced stage at presentation among patients in Uganda. Methods: We conducted a retrospective analysis of adult patients with any of four specific cancers who presented for care in Kampala, Uganda, between 2003 and 2010. Demographic, clinical, and laboratory data were abstracted from the medical record, together with the outcome measure of advanced stage of disease (clinical stage III or IV). We identified measures for inclusion in a multivariate logistic regression model. Results: We analyzed 731 patients with both AIDS-defining cancers (cervical [43.1%], and non-Hodgkin lymphoma [18.3%]), and non–AIDS-defining cancers (breast [30.0%] and Hodgkin lymphoma [8.6%]). Nearly 80% of all patients presented at an advanced stage and 37% had HIV infection. More than 90% of patients were symptomatic and the median duration of symptoms before presentation was 5 months. In the multivariate model, HIV-positive patients were less likely to present at an advanced stage as were patients with higher hemoglobin and fewer symptoms. Conclusion: Patients with limited access to primary care may present with advanced cancer because of a delay in diagnosis. However, patients with HIV now have better access to clinical care. Use of this growing infrastructure to increase cancer screening and referral is promising and deserves continued support, because the prognosis of HIV-positive patients with advanced cancer is characterized by poor survival globally.

Abstract

Arsenic is a widespread serious environmental pollutant as a food chain contaminant and non-threshold carcinogen. Arsenic transfer through the crops-soil-water system and animals is one of the most important pathways of human exposure and a measure of phytoremediation. Exposure occurs primarily from the consumption of contaminated water and foods. Various chemical technologies are utilized for As removal from contaminated water and soil, but they are very costly and difficult for large-scale cleaning of water and soil. In contrast, phytoremediation utilizes green plants to remove As from a contaminated environment. A large number of terrestrial and aquatic weed flora have been identified so far for their hyper metal removal capacity. In the panorama presented herein, the latest state of the art on methods of bioaccumulation, transfer mechanism of As through plants and animals, and remediation that encompass the use of physicochemical and biological processes, i.e., microbes, mosses, lichens, ferns, algae, and macrophytes have been assessed. Since these bioremediation approaches for the clean-up of this contaminant are still at the initial experimental stages, some have not been recognized at full scale. Abstract

Arsenic is one of the regulated hazard materials in the environment and a persistent pollutant creating environmental, agricultural and health issues and posing a serious risk to humans. In the present review, sources and mobility of As in various compartments of the environment (air, water, soil and sediment) around the World are comprehensively investigated, along with measures of health hazards. Multiple atomic spectrometric approaches have been applied for total and speciation analysis of As chemical species. The LoD values are basically under 1 mu g L-1, which is sufficient for the analysis of As or its chemical species in environmental samples. Both natural and anthropogenic sources contributed to As in air, while fine particulate matter tends to have higher concentrations of arsenic and results in high concentrations of As up to a maximum of 1660 ng m(-3) in urban areas. Sources for As in natural waters (as dissolved or in particulate form) can be attributed to natural deposits, agricultural and industrial effluents, for which the maximum concentration of 2000 mu g L-1 was found in groundwater. Sources for As in soil can be the initial contents, fossil fuel burning products, industrial effluents, pesticides, and so on, with a maximum reported concentration up to 4600 mg kg(-1). Sources for As in sediments can be attributed to their reservoirs, with a maximum reported concentration up to 2500 mg kg(-1). It is notable that some reported concentrations of As in the environment are several times higher than permissible limits. Item does not contain fulltext, Traditional vaccines are difficult to deploy against the diverse antimicrobial-resistant, nosocomial pathogens that cause health care-associated infections. We developed a protein-free vaccine composed of aluminum hydroxide, monophosphoryl lipid A, and fungal mannan that improved survival and reduced bacterial burden of mice with invasive blood or lung infections caused by methicillin-resistant Staphylococcus aureus, vancomycin-resistant Enterococcus faecalis, extended-spectrum beta-lactamase-expressing Escherichia coli, and carbapenem-resistant strains of Acinetobacter baumannii, Klebsiella pneumoniae, and Pseudomonas aeruginosa. The vaccine also conferred protection against the fungi Rhizopus delemar and Candida albicans. Efficacy was apparent by 24 hours and lasted for up to 28 days after a single vaccine dose, with a second dose restoring efficacy. The vaccine acted through stimulation of the innate, rather than the adaptive, immune system, as demonstrated by efficacy in the absence of lymphocytes that were abrogated by macrophage depletion. A role for macrophages was further supported by the finding that vaccination induced macrophage epigenetic alterations that modulated phagocytosis and the inflammatory response to infection. Together, these data show that this protein-free vaccine is a promising strategy to prevent deadly antimicrobial-resistant health care-associated infections.

Abstract

Informed analysis of policies related to food security, global climate change, wetland ecology, environmental nutrient flux, element cycling, groundwater weathering, continental denudation, human health, and others depends to a large extent on quantitative estimates of solute mass fluxes into and out of all global element pools including the enigmatic global aquifer systems. Herein for the first time, we proffer the mean global solute concentration of all major and selected minor and trace solutes in the active groundwater that represents 99% of liquid fresh water on Earth. Concentrations in this significant element pool have yielded to a geospatial machine learning kNN-nearest neighbors' algorithm with numerous geospatial predictors utilizing a large new lithology/climate/aquifer age/elevation based solute database. The predicted concentrations are consistent with traditional solute ratios, concentrations, and thermodynamic saturation indices.

Abstract

The Committee on Polymer Education (PolyEd) had its initial planning meeting in 1974. The Intersociety Polymer Education Council (IPEC) has been active since 1990 as an outgrowth of PolyEd. Because of the activities of PolyEd and IPEC, basic polymer concepts have become a required part of undergraduate education and a more common topic in the curriculum for K-12 students. A brief history of polymer education is given. The current and future activities of PolyEd and IPEC are described.

122. Global Nitrogen Mass Flux From the Active Freshwater Aquifer Element Pool.

Abstract

The estimated current global mean nitrogen concentration (geogenic + anthropogenic) in the active continental freshwater aquifer element pool is 1.1 mg/L as N, or between four and five times greater than the assumed geogenic mean. This concentration, combined with groundwater flux, generates a continental mass flux of 17 Tg N/y (teragrams of nitrogen, as N, per year) as a result of direct ocean discharge (0.67 Tg N/y), endorheic basins (1.2 Tg N/y), and cold‐wet (0.82 Tg N/y); cold‐dry (1.4 Tg N/y); warm‐dry (1.6 Tg N/y); and warm‐wet (11 Tg N/y) exorheic basins. These values are derived from a geospatial machine learning algorithm and combined groundwater‐modeled recharge in an ArcGIS environment. This active continental freshwater aquifer mass flux is between 35% and 40% of the continental integrated riverine system discharge, thus a significant component of the Earth's active continental freshwater nitrogen budget. We estimate the active continental freshwater aquifer volume to be between 1.4 and 2.8 million km3 suggesting a legacy of between 1.5 and 3.1 Pg as N (petagrams nitrogen as N) with mean residences of 90–180 years. Abstract

An analytical method to determine tin concentrations in natural samples, by coupling atomic fluorescence spectrometry, hydride generation and flow injection system was developed. The sample digestion procedure and the conditions of hydride generation were fully optimized. The best conditions for the generation of SnH4 were obtained with NaBH4 4% (m v-1) and HCl 0.6 mol L-1. The optimized flow rates were 1.2 mL min-1 for sample, 1.2 mL min-1 for HCl and 1.6 mL min-1 for NaBH4 solution. The sample volume was 500 µL. The best acid volume for digestion of solid samples was stablished to be 3 mL HNO3 + 1 mL HCl. The accuracy of the method was established by the analysis of certified reference materials and by comparative determinations, using inductively coupled optical emission spectrometry. The approach developed was shown to offer a robust method with excellent accuracy and precision for the determination of Sn in environmental samples. Finally, the method was applied to analyse the soil, sediment and water samples, collected in area under anthropogenic activities: processing area of cassiterite in Corumbataí/SP. The obtained data indicated that there was alteration in the natural quality of the environment as a result of the activities developed in that site

Abstract

Establishing the abundance and physical properties of regolith and boulders on asteroids is crucial for understanding the formation and degradation mechanisms at work on their surfaces. Using images and thermal data from NASA's Origins, Spectral Interpretation, Resource Identification, and Security-Regolith Explorer (OSIRIS-REx) spacecraft, we show that asteroid (101955) Bennu's surface is globally rough, dense with boulders, and low in albedo. The number of boulders is surprising given Bennu's moderate thermal inertia, suggesting that simple models linking thermal inertia to particle size do not adequately capture the complexity relating these properties. At the same time, we find evidence for a wide range of particle sizes with distinct albedo characteristics. Our findings imply that ages of Bennu's surface particles span from the disruption of the asteroid's parent body (boulders) to recent in situ production (micrometre-scale particles).
