Abstract

Aim: The purpose of this study was to evaluate the clinical response to augmenting an ultrasound-guided percutaneous plantar fasciotomy (UGPF) with an amniotic membrane (AM) allograft injection. Design: Retrospective, single-center, matched, case-controlled study. Methods: Patients underwent either an UGPF (n = 15) or a combined UGPF and AM injection (n = 16). Results: The UGPF plus AM group demonstrated a significant reduction in pain (p = 0.02) from baseline at the short-term follow-up, but there was no significant difference in pain or patient satisfaction between groups at the 52-week follow-up. Conclusion: Both groups demonstrated a significant reduction in pain and high level of patient satisfaction, but the combination of UGPF with an AM injection may provide a greater reduction in pain earlier in the post-operative period.

Abstract

Introduction As of April 19, 2021, over 141 million cases of coronavirus disease 2019 (COVID-19) associated with severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) had been reported worldwide (1). Available data [...], BACKGROUND. Although convalescent plasma has been widely used to treat severe coronavirus disease 2019 (COVID-19), data from randomized controlled trials that support its efficacy are limited. METHODS. We conducted a randomized, double-blind, controlled trial among adults hospitalized with severe and critical COVID-19 at 5 sites in New York City (USA) and Rio de Janeiro (Brazil). Patients were randomized 2:1 to receive a single transfusion of either convalescent plasma or normal control plasma. The primary outcome was clinical status at 28 days following randomization, measured using an ordinal scale and analyzed using a proportional odds model in the intention-to-treat population. RESULTS. Of 223 participants enrolled, 150 were randomized to receive convalescent plasma and 73 to receive normal control plasma. At 28 days, no significant improvement in the clinical scale was observed in participants randomized to convalescent plasma (OR 1.50, 95% confidence interval [CI] 0.83-2.68, P = 0.180). However, 28-day mortality was significantly lower in participants randomized to convalescent plasma versus control plasma (19/150 [12.6%] versus 18/73 [24.6%], OR 0.44, 95% CI 0.22-0.91, P = 0.034). The median titer of anti-SARS- CoV-2 neutralizing antibody in infused convalescent plasma units was 1:160 (IQR 1:80-1:320). In a subset of nasopharyngeal swab samples from Brazil that underwent genomic sequencing, no evidence of neutralization-escape mutants was detected. CONCLUSION. In adults hospitalized with severe COVID-19, use of convalescent plasma was not associated with significant improvement in day 28 clinical status. However, convalescent plasma was associated with significantly improved survival. A possible explanation is that survivors remained hospitalized at their baseline clinical status. TRIAL REGISTRATION. Abstract

Context. A large sample of long-period giant planets has been discovered thanks to long-term radial velocity surveys, but only a few dozen of these planets have a precise radius measurement. Transiting gas giants are crucial targets for the study of atmospheric composition across a wide range of equilibrium temperatures and, more importantly, for shedding light on the formation and evolution of planetary systems. Indeed, compared to hot Jupiters, the atmospheric properties and orbital parameters of cooler gas giants are unaltered by intense stellar irradiation and tidal effects. Aims. We aim to identify long-period planets in the Transiting Exoplanet Survey Satellite (TESS) data as single or duo-transit events. Our goal is to solve the orbital periods of TESS duo-transit candidates with the use of additional space-based photometric observations and to collect follow-up spectroscopic observations in order to confirm the planetary nature and measure the mass of the candidates. Methods. We use the CHaracterising ExOPlanet Satellite (CHEOPS) to observe the highest-probability period aliases in order to discard or confirm a transit event at a given period. Once a period is confirmed, we jointly model the TESS and CHEOPS light curves along with the radial velocity datasets to measure the orbital parameters of the system and obtain precise mass and radius measurements. Results. We report the discovery of a long-period transiting Neptune-mass planet orbiting the G7-type star TOI-5678. Our spectroscopic analysis shows that TOI-5678 is a star with a solar metallicity. The TESS light curve of TOI-5678 presents two transit events separated by almost two years. In addition, CHEOPS observed the target as part of its Guaranteed Time Observation program. After four non-detections corresponding to possible periods, CHEOPS detected a transit event matching a unique period alias. 114. Telemedical Monitoring of Plantar Temperature in Diabetic Patients at Risk of Foot Ulcers: The Randomized Smart Prevent Diabetic Feet Trial.

Abstract

Background: The worldwide prevalence of diabetic foot ulcers (DFUs) among persons with diabetes is estimated at 6.3%, with an annual incidence of 9.1 to 26.1 million persons. The early detection of asymmetrical plantar temperature elevation, followed by reduction of weight -bearing on the affected foot, may be an effective mode of prevention. Methods: Patients with diabetes and peripheral neuropathy (DFU risk groups 2/3) were monitored for plantar abnormalities with a telemedical system consisting of sole inserts with temperature sensors and photographic documentation. An open, prospective, randomized controlled trial was performed to determine whether this system prevented DFUs. The intervention and control groups were also trained in ulcer prevention and observed in follow-up at 6-month intervals for 24 months. Results: 283 patients were recruited. In 85 137 observation days, DFUs arose in five patients in the control group (n = 143) and in no patient in the intervention group (n = 140). The primary outcome measure was the hazard ratio, which was calculated to be 0.015 (95% confidence interval [0; 19,717]; p = 0.25) after adjustment for age, sex, severity of neuropathy, and risk class. There were 239 alarms and 75 instructions to reduce weight-bearing on the foot. The subjects carried out the telemedical application on about 70% of the days of observation. Quality of life improved in both groups. Conclusion: The tele-health system used in this trial is practical and enables the early detection of morbidity. Likely explanations for the unexpectedly low ulceration rate in this trial (and, in turn, for the lack of statistical significance) include the availability of a training program and regular follow-up examinations to patients in both arms of the trial, along with lower mobility levels due to the COVID pandemic. Abstract

A variety of intra-articular injections are used for the management of osteoarthritis. A rare complication of intra-articular injections is acute pseudoseptic arthritis, which mimics true septic arthritis. To the authors knowledge, pseudosepsis has not been reported as a complication after platelet-rich plasma (PRP) injections. We present a case report of pseudoseptic arthritis resulting in acute postinjection pain and a joint effusion after an intra-articular PRP injection into the knee. Clinically, pseudosepsis can occur after PRP treatment with significantly elevated white blood cell counts in the synovial fluid, and should be a consideration in cases of post-PRP injection flares.

Abstract

The astrophysical journal / Supplement series 266(2), 37 (2023). doi:10.3847/1538-4365/acc181, We study the broadband emission of Mrk 501 using multiwavelength observations from 2017 to 2020 performed with a multitude of instruments, involving, among others, MAGIC, Fermi's Large Area Telescope (LAT), NuSTAR, Swift, GASP-WEBT, and the Owens Valley Radio Observatory. Mrk 501 showed an extremely low broadband activity, which may help to unravel its baseline emission. Nonetheless, significant flux variations are detected at all wave bands, with the highest occurring at X-rays and very-high-energy (VHE) $γ$-rays. A significant correlation (>3σ) between X-rays and VHE $γ$-rays is measured, supporting leptonic scenarios to explain the variable parts of the emission, also during low activity. This is further supported when we extend our data from 2008 to 2020, and identify, for the first time, significant correlations between the Swift X-Ray Telescope and Fermi-LAT. We additionally find correlations between high-energy γ-rays and radio, with the radio lagging by more than 100 days, placing the γ-ray emission zone upstream of the radio-bright regions in the jet. Furthermore, Mrk 501 showed a historically low activity in X-rays and VHE $γ$-rays from mid-2017 to mid-2019 with a stable VHE flux (>0.2 TeV) of 5% the emission of the Crab Nebula. The broadband spectral energy distribution (SED) of this 2 $γ$r long low state, the potential baseline emission of Mrk 501, can be characterized with one-zone leptonic models, and with (lepto)-hadronic models fulfilling neutrino flux constraints from IceCube. Abstract

HD$\,$172555 is a young ($\sim$20$\,$Myr) A7V star surrounded by a 10$\,$au wide debris disk suspected to be replenished partly by collisions between large planetesimals. Small evaporating transiting bodies, exocomets, have also been detected in this system by spectroscopy. After $\beta\,$Pictoris, this is another example of a system possibly witnessing a phase of heavy bombardment of planetesimals. In such system, small bodies trace dynamical evolution processes. We aim at constraining their dust content by using transit photometry. We performed a 2-day-long photometric monitoring of HD$\,$172555 with the CHEOPS space telescope in order to detect shallow transits of exocomets with a typical expected duration of a few hours. The large oscillations in the lightcurve indicate that HD$\,$172555 is a $\delta\,$Scuti pulsating star. Once removing those dominating oscillations, we find a hint for a transient absorption. If fitted with an exocomet transit model, it corresponds to an evaporating body passing near the star at a distance of $6.8\pm1.4\,$R$_\star$ (or $0.05\pm 0.01\,$au) with a radius of 2.5 km. These properties are comparable to those of the exocomets already found in this system using spectroscopy, as well as those found in the $\beta\,$Pic system. The nuclei of solar system's Jupiter family comets, with radii of 2-6$\,$km, are also comparable in size. Abstract

Residual limb pain is common after amputation. Painful osteophytes are one of many causes of residual limb pain, and cases that fail conservative management may require open surgical resection. Here we present a novel percutaneous ultrasound-guided approach to remove a painful osteophyte in a transfemoral amputee. This procedure successfully treated this patient's residual limb pain while minimizing the risks associated with open surgical resection.

Abstract

A sports medicine physician manages musculoskeletal (MSK) injuries and sport-related medical and MSK conditions of patients of all ages and abilities. Physical medicine and rehabilitation physicians (physiatrists) must be adequately trained to provide this care for all patients including, but not limited to, athletes participating in organized sports, the weekend warrior as well as athletes with disabilities. Accreditation Council of Graduate Medical Education core requirements and basic guidelines help physiatry residency training programs develop and implement residency curriculums. The goal of this article is to provide suggested curricular guidelines to optimize physiatrist training in MSK and sports medicine.
