Abstract

BACKGROUND: The offer of prenatal Down's syndrome screening is part of routine antenatal care in most of Europe; however screening uptake varies significantly across countries. Although a decision to accept or reject screening is a personal choice, it is unlikely that the widely differing uptake rates across countries can be explained by variation in individual values alone.The aim of this study was to compare Down's syndrome screening policies and programmes in the Netherlands, where uptake is relatively low (<30%) with England and Denmark where uptake is higher (74 and > 90% respectively), in an attempt to explain the observed variation in national uptake rates.METHODS: We used a mixed methods approach with an embedded design: a) documentary analysis and b) expert stakeholder analysis. National central statistical offices and legal documents were studied first to gain insight in demographic characteristics, cultural background, organization and structure of healthcare followed by documentary analysis of primary and secondary sources on relevant documents on DSS policies and programme. To enhance interpretation of these findings we performed in-depth interviews with relevant expert stakeholders.RESULTS: There were many similarities in the demographics, healthcare systems, government abortion legislation and Down's syndrome screening policy across the studied countries. However, the additional cost for Down's syndrome screening over and above standard antenatal care in the Netherlands and an emphasis on the 'right not to know' about screening in this country were identified as potential explanations for the 'low' uptake rates of Down's syndrome screening in the Netherlands. The social context and positive framing of the offer at the service delivery level may play a role in the relatively high uptake rates in Denmark.CONCLUSIONS: This paper makes an important contribution to understanding how macro-level demographic, social and healthca

Abstract

Since the 1970s, intrapartum monitoring of the distressed foetus has been managed by continuous registration of the foetal heart rate, together with uterine activity (cardiotocogram; CTG). Use of CTG without additional foetal information leads to unnecessary interventions because of the high number of false-positive signals. Foetal blood sampling (FBS) is a solution to this problem, but is not always consistently carried out. Automated ST analysis of the foetal electrocardiogram (STAN method), combined with the CTG, may lead to reduction of metabolic acidosis, fewer interventions and fewer foetal blood samples. A disadvantage of application of the STAN method is that it is based on visual interpretation of the CTG, with large inter- and intraobserver variability. The purpose of tocolytic drug administration is to postpone threatening preterm delivery for 48 hours to allow maximal effect of antenatal corticosteroids and maternal transportation to a center with specialized neonatal care facilities. There is uncertainty about the value of atosiban (oxytocin receptor antagonist) and nifedipine (calcium channel blocker) as first-line tocolytic drugs in the management of preterm labor. For nifedipine, concerns have been raised about unproven safety, lack of placebo-controlled trials, and its off-label use. The tocolytic efficacy of atosiban has also been questioned because of a lack of reduction in neonatal morbidity. Abstract

OBJECTIVE: The objective of the study was a comparison of insulin aspart (IAsp) with human insulin (HI) in basal-bolus therapy with neutral protamine Hagedorn for fetal and perinatal outcomes of type 1 diabetes in pregnancy. STUDY DESIGN: This was a randomized, parallel, open-label, controlled, multicenter, multinational study. Subjects were pregnant (gestational age; <10 weeks) or planning pregnancy at enrollment. Three hundred twenty-two women with type 1 diabetes received IAsp (n = 157) or HI (n = 165). RESULTS: For IAsp and HI, respectively, there were 137 and 131 live births and 14 and 21 fetal losses. Perinatal mortality was 14 and 22 per 1000 births; number of congenital malformations were 6 and 9; mean (SEM) birthweight corrected for gestational age was 3438 g (71.5) and 3555 g (72.9; P = .091). Mean gestational age was 37.6 vs 37.4 weeks. Preterm delivery occurred in 20.3% (IAsp) and 30.6% (HI) of pregnancies (P = .053). Abstract

Poco se sabía hasta hace poco sobre el proceso por el que los padres toman la decisión de terminar el embarazo tras el diagnóstico de que su hijo tiene síndrome de Down. En 2005, Skotko publicó los resultados de un estudio realizado en Estados Unidos mediante encuesta por correo a 141 madres que decidieron mantener el embarazo tras el diagnóstico de que su hijo tenía síndrome de Down (ver en Canal Down21: http://www.down21.org/revista/2005/Abril/Resumen.htm). El presente estudio se ha realizado en Holanda, en 71 mujeres que decidieron terminarlo antes del nacimiento, a pesar de que el feto no mostraba ninguna anomalía incompatible con la vida. El cuestionario fue entregado a las madres a los cuatro meses de haber abortado. En el cuestionario se tuvieron en cuenta diversas variables: motivos de la decisión, razones para dudar, si percibieron o no influencias o presiones para tomar la decisión, y si se sintieron satisfechas con los cuidados sanitarios recibidos.

Abstract

Las relaciones entre hermanos constituyen el lazo familiar de mayor duración: comienzan con el nacimiento del hermano menor y terminan con la muerte de un miembro del par de hermanos. Los hermanos comparten una herencia familiar común, tanto genética como experiencial, y quizá por esta razón, la relación entre hermanos se caracteriza normativamente como igualitaria, recíproca y mutua. ¿Cómo cambia la relación entre hermanos cuando uno de los miembros tiene una discapacidad del desarrollo? Las características de los lazos fraternos podrían ser atípicas en este contexto, en varios aspectos. Por ejemplo, la duración de la relación podría ser más corta, pues algunos individuos con discapacidades del desarrollo tienen una vida más corta. Además, puede haber menos similitud genética y experiencial entre miembros de la pareja de hermanos y menos igualitarismo e intercambio recíproco debido a las desiguales capacidades y resultados de patrones de vida. Pero puede haber también una mayor continuidad en el contacto a lo largo de la vida cuando el hermano tiene una discapacidad del desarrollo. La mayor parte de los investigadores en el pasado que han analizado las relaciones de hermanos de personas con discapacidad del desarrollo se han centrado en la pareja de hermanos en la infancia (Stoneman, 1998; Stoneman y Berman, 1993). De cualquier modo, los individuos con discapacidades del desarrollo tienden a vivir cada vez más en la edad adulta y ahora, por lo común, llegan a edad avanzada. Por tanto, es necesario extender nuestra investigación sobre las relaciones entre hermanos cuando uno de ellos tiene discapacidad del desarrollo a lo largo de todo el ciclo vital, para conocer mejor si esta relación cambia y cómo lo hace conforme los hermanos crecen. Abstract

Background: Fetal growth restriction (intra-uterine growth restriction [IUGR]) has a considerable impact on perinatal morbidity. Preterm IUGR infants are prone to impaired intestine function. Near-infrared spectroscopy (NIRS) has been used to monitor oxygenation status of the brain and of the intestine.Patients and Methods: We conducted a prospective case-control study at our NICU in 20 preterm infants of whom 10 infants complicated by compared with 10 non-IUGR preterm infants. Splanchnic and cerebral regional oximetry values were measured with NIRS. Three hours of consecutive recordings were performed in the first 24 h of life, T0, and during the transitional period, T1. The cerebral/splanchnic oxygenation ratio, CSOR, (cerebral regional saturations [rScO2]/splanchnic regional saturations [rSsO2]) was also calculated.Results: Both in the IUGR and the non-IUGR infants, at T0 and T1 monitoring time-points, the rSO2 values were higher in the cerebral district when compared to those of the splanchnic area. Comparison of the NIRS parameters between the IUGR and non-IUGR infants at T0 showed no difference in rScO2, while rSsO2 was significantly lower in the IUGR group. At T1, rScO2 was significantly lower and rSsO2 higher in the IUGR group.Conclusions: Cerebral/splanchnic vascular adaptation of IUGR infants to the extra-uterine environment is characterized by a postnatal persistence of the brain sparing effect with reperfusion in the transitional period.
