Abstract

Background: Uncoupling protein 2 ( UCP2) plays an important role in regulating body weight, energy expenditure and insulin secretion. UCP2 is upregulated in white fat in response to fat feeding, and negatively controls insulin secretion. UCP2 also has a function that protects cells from apoptosis and oxidative stress, which shows UCP2 might be a senescence-related gene. Previously, UCP2-866G/A polymorphism in the promoter region has been reported to alter adipose tissue mRNA expression and is associated with obesity in Caucasians. Methods: In this study, we investigated the association between this polymorphism and obesity, insulin secretion and hypertension in the general Japanese population. Results: The allele frequency of UCP2-866G/A polymorphism was significantly higher in Japanese subjects compared to Caucasians. It revealed that subjects only in the obese group with the AA type of UCP2-866G/A polymorphism had significantly higher levels of body mass index (BMI) and waist circumference. Multiple logistic regression analysis showed that this polymorphism was independently associated with waist circumference. This positive association remained in the analysis of the subgroup younger than 65 years, but not in the older group. This polymorphism did not affect levels of insulin and homeostasis model assessment ratio (HOMA-R). Conclusions: These results suggest that the AA type of UCP2-866G/A polymorphism is related to abdominal obesity, which indicates the possible role of this polymorphism in causing metabolic syndromes. 128. Hypoparathyroidism Potentiates Cardiovascular Complications through Disturbed Calcium Metabolism: Possible Risk of Vitamin D[sub 3] Analog Administration in Dialysis Patients with End-Stage Renal Disease.

Abstract

Background/Aim: Progressive cardiovascular calcification in dialysis patients with end-stage renal disease (ESRD) is a serious complication; however, the precise mechanism remains uncertain. We tested whether metabolic calcium abnormalities and hypoparathyroidism might have a correlation with cardiovascular complications in ESRD patients. Methods: A series of 48 ESRD patients with cardiovascular diseases and/or congestive heart failure, aged 36–82 (61 ± 12) years, 23 male and 25 female, were enrolled in this study. Serum total calcium (Ca, mmol/l), inorganic phosphate (mmol/l), and intact parathyroid hormone (iPTH, pg/ml) levels were determined in all cases. Results: Organic heart disease was confirmed in 28 patients (58.3%), including 15 with coronary artery disease: 8 with aortic aneurysm, 8 with stenotic valvular heart disease, 9 with excessive mitral annular calcification, 3 with dialysis cardiomyopathy, and 7 with obstructive arterial disease. Serum iPTH measurement revealed hypoparathyroidism (iPTH <60) in 20 of 48 (41.7%) and hyperthyroidism (iPTH ≥200) in 13 of 48 (27.1%) subjects. The 20 patients with low iPTH had a higher prevalence of valvular heart disease, a higher total Ca level corrected for serum albumin (2.70 ± 0.30 in low iPTH vs. 2.47 ± 0.30 in normal iPTH, 2.35 ± 0.20 in high iPTH, p = 0.003) and a higher tendency of vitamin D[sub 3] analog use (65% in low iPTH vs. 33% in normal iPTH and 46% in high iPTH, p = 0.078). Moreover, corrected serum Ca exhibited a negative logarithmic correlation with serum iPTH: corrected Ca = –0.284× log (iPTH) + 3.021 (r = 0.637, p = 0.0001). Multiple logistic regression analysis revealed diabetes and hypoparathyroidism (iPTH <60) as risk factors for cardiovascular complications in ESRD. Conclusion: These results suggest that hypercalcemia and hypoparathyroidism in conjunction with vitamin D[sub 3] use might play an important role in cardiovascular complications of chronic dialysis patients.Copyright © 2000 S. Abstract

Background:The concentration of heart-type fatty acid-binding protein (hFABP), a promising novel marker for detection of acute or persistent myocardial damage, is significantly influenced by renal clearance and thus has limitations to its usefulness in patients with renal dysfunction. We evaluated whether the serum ratio of hFABP to myoglobin (F/M) might be a useful marker for assessing cardiac damage in hemodialysis patients. Methods:Serum hFABP and myoglobin were measured, and the value of F/M was calculated in 21 hemodialysis patients. Cardiac markers (cardiac troponin T [cTnT], atrial natriuretic peptide [ANP], and brain natriuretic peptide [BNP]) and echocardiographic indices (left ventricular end-diastolic dimension [LVDd], left ventricular mass index [LVMI], and inferior vena cava [IVC] dimension) were examined and compared with hFABP, myoglobin, and F/M ratios. Results:Serum hFABP and myoglobin levels were significantly elevated in hemodialysis patients and reduced by 30–40% during hemodialysis. The value of F/M after hemodialysis, but not the concentration of hFABP or myoglobin, had significant linear correlations with ANP, BNP, cTnT, LVDd, LVMI, and IVC. Conclusion:The value of F/M after hemodialysis, but not the concentration of hFABP itself, might be a newly useful marker for estimation of cardiac damage and volume overload in hemodialysis patients.

The article discusses a study to evaluate the factors that are responsible for creating burden among family caregivers of regular hemodialysis patients. Participants included 108 pairs of family caregivers and regular hemodialysis patients in Hokkaido Prefecture, Japan. The study concluded that caregivers tend to feel burdened by looking after patients for a long time as they spent a lot of time with their charges as well as paying a lot of money for their care.

141. Increased Serum Soluble (Pro)renin Receptor Levels in Patients with Sarcopenia.

Abstract

Purpose. Sarcopenia is an advancing and widespread skeletal muscle condition characterized by the depletion of skeletal muscle mass. The (pro)renin receptor ((P)RR) is a multifaceted protein with both pathophysiological and physiological functions. (P)RR is cleaved to generate soluble (P)RR, whose blood concentration may reflect the expression level of tissue (P)RR. We have discovered that the expression of (P)RR was elevated in the atrophied skeletal muscles of aged mice and humans. Methods. In this study, we examined the correlation between muscle atrophy and serum soluble (P)RR levels in bedridden patients with sarcopenia. Results. In total, 45 subjects were enrolled. Their ages were 81 (59–89) y.o., and serum soluble (P)RR levels were 30.5 ± 7.3 ng/ml. Lean body mass ratio (lean body mass/body weight), an index of muscle mass, was independently significantly and negatively correlated with serum soluble (P)RR concentrations in all patients. In addition, the lean body mass ratio was independently significantly and negatively correlated with serum soluble (P)RR concentrations in older patients (≥82 y.o.) but not in younger patients (<82 y.o.). Conclusion. These data suggested that patients with sarcopenia, especially in elderly patients, may be associated with increased (P)RR expression. The serum soluble (P)RR level could be a biomarker of sarcopenia although this issue should be investigated by future studies. [ABSTRACT FROM AUTHOR]
