Linear porokeratosis of Mibelli (LPM), a variant of porokeratosis, usually appears in childhood, but may occur in middle life. Unilateral lesions frequently occur and are often misdiagnosed. We herewith report a case of linear porokeratosis of Mibelli in a 4 year old girl child clinically mistaken as linear verrucous epidermal nevus.

We report a 26 year old female patient of Kindler syndrome associated with squamous cell carcinoma of the leg. This syndrome is a rare inherited disorder characterized by trauma induced acral blistering, photosensitivity, poikiloderma and cutaneous atrophy. Occurrence of overt cutaneous malignancy on glabrous skin in this syndrome has not been reported before.

Pellagra is a nutritional disorder that occurs due to niacin deficiency. In the present day context, in western world, pellagra is confined to individuals who have improper food intake, such as psychiatry patients, alcoholics and recluses. Here with we report a case of pellagra occurring in a 60 year old male who was a chronic alcoholic.

Papillon-Lefevre syndrome is an inherited disorder of Keratinization characterized by palmoplantar keratoderma, associated with periodontosis and a tendency to frequent pyogenic skin infection. The clinical expression of this condition is highly variable. We report here, two cases of this rare syndrome occurring in 18 year old unmarried girl and a 54 year old married woman. Differentiating points from two recently described syndromes have been highlighted in the discussion.

We herewith report a case of extramedullary myeloid cell tumour presenting as leukaemia cutis for its rarity. It occurred in a 50 year old male patient who presented to us with a 40 days history of painless raised solid skin swellings over the trunk. Histopathological examination of the skin biopsy and bone marrow biopsy showed features suggestive of non-Hodgkinâ€s lymphoma. Immunophenotyping on skin biopsy specimens and bone marrow biopsy found tumour cells expressing CD43 and Tdt but were negative for CD3 and CD20. These features were consistent with extramedullary myeloid cell tumour involving skin and subcutis (cutaneous manifestation of acute myeloid leukaemia.

A 17 year old South Indian female born of consanguineous marriage presented with a widespread non-itchy eruption over scalp, face, below breasts and flexural aspects of limbs including axillae, inguinal regions and popliteal fossae since 3 months of age. The eruption consisted of individual small, flat, brownish black hyperkeratotic papules, which at some places, coalesced to from a reticulate pattern. Histopathological examination of biopsies from the axilla and forearm showed typical hyperkeratosis and papillomatosis without acanthosis. The dermis demonstrated a mild perivascular infiltrate. On the basis of the clinical and histopathological findings, a diagnosis of confluent and reticulate papillomatosis of Gourgerot and Carteaud was made. Partial response to doxycline in our case point towards this being a keratinization disorder where microflora may be playing an exacerbating rather than a primary role.

A 14 year old female with acute lymphoblastic leukaemia had a tumoral lesion over the face of 3 months duration. Excision biopsy of the lesion confirmed it to be molluscum contagiosum. Giant molluscum contagiosum attaining polypoidal character as seen in our patient is an uncommon presentation and hence being reported for its rarity.

Toxic epidermal necrolysis (TEN) is a severe mucocutaneous reaction pattern characterized by fever, systemic toxicity, tenderness, erythema and wide-spread exfoliation. A retrospective analysis from January 1990 to December 1996 was undertaken to see the changing trends and outcome of this disorder in a third world country. A total of eleven cases were found. Their mean age was 33.09 years and male to female ratio was 2.6:1. Various drugs implicated were paracetamol (2 cases), co-trimoxazole (2 cases), aspirin (1 case), thiacetazone (1 case), streptomycin (1 case), etc. On an average, the patients developed TEN at the interval of 4.68 days following the drug intake. Fever was presenting symptom in all cases and burning sensation preceded it in 6 of them. Percentage area. Involved ranged from 40% to 90%. Other target areas involved in order of decreasing frequency were oral mucosa (9 cases) genitalia (8 cases), eye (5 cases), palms and soles (3 cases). Oral prednisolonoe therapy was started in all cases in the range of 20 to 80 mg. According to the severity, extent of lesion and body weight. Epithelisation of denuded areas took an average of 10.6 days. Complications in the form of secondary infection (3 cases), septicaemia and death (1 case) and symblepheron formation (1 case) were noted.

Reticulate acral and flexural pigmentary disorders are a rare group of pigmentary genodermatoses. We report a family showing a combination of features described in the two disorders; reticulate acropigmentation of kitamura (RAPK) and Dowling Degos disease, along with palmoplantar keratoderma. We speculate that RAPK and DD are part of a spectrum of reticulate pigmentary disorders with varied phenotypic expression.

Background: Spinal dysraphism occurs due to incomplete fusion of the midline mesenchymal, bony, or neural elements of the spine. The defects in the spinal cord can be associated with skin lesion since both have same embryonic origin. Aims and Objectives: This study was conducted to determine the association of midline and paramedian cutaneous lesions with spinal dysraphism by using spinal ultrasonography. Materials and Methods: Two thousand apparently healthy neonates were screened in the postnatal ward of a tertiary care center in South India. Those neonates with cutaneous lesions in the midline and paramedian region were screened for evidence of spinal dysraphism by using spinal ultrasonography. Results: Among 2000 neonates, 120 (6%) had at least one cutaneous lesion, of which 114 (5.7%) were in the midline and 6 (0.3%) were on the paramedian region of dorsal and ventral aspect of the body. Among these neonates, two cases had more than one skin lesions. The most common cutaneous lesion observed was typical dimple (82, 68%) followed by hypertrichosis (12, 10%). Ultrasonography revealed spinal anomaly in six (5%) of them. The cutaneous lesions associated with spinal dysraphism were obvious midline swelling, dermal sinus, and multiple skin lesions. Conclusion: Congenital midline and paramedian skin lesions may be the marker of spinal dysraphism. In the presence of such cutaneous lesions, only 5% of them had associated spinal anomaly in our study. [ABSTRACT FROM AUTHOR]

Onychomatricoma is a rare tumor of the nail matrix. A 28 year male presented with asymptomatic gradual appearance of yellowish discoloration and thickening of lateral part of nail of the right index finger of 4 years duration. On examination, a yellow longitudinal band of discoloration involving medial one third of the nail was seen over the index finger of the right hand. The affected part of the natil was thickened and showed over curvature and longitudinal riding. Histopathology showed fibroepithelial elements, confirming the diagnosis of onychomatricoma.

Linear lichen planus is uncommon and only occasionally has a relationship with Blaschkoâ€s lines been observed. We describe herewith a patient who had this uncommon variant of linear lichen planus along with erosive lesion of lichen planus over the glans.
