OBJECTIVE: To describe an outbreak of Cryptosporidium gastroenteritis in a swimming pool in Melbourne in early 1998 that was not detected through routine surveillance, and discuss difficulties in identifying such outbreaks. METHODS: The Water Quality Study (WQS) was a large community-based study of gastroenteritis. Following suspicion of an outbreak of cryptosporidiosis within the study group, due to pool 'X', a nested case control study was performed. Each case of Cryptosporidium gastroenteritis was matched with six controls and data from weekly Health Diaries from the WQS were reviewed. The Department of Human Services also instigated active surveillance among patrons at pool 'X' using a systematic sample of 50 people from the pool's swim-school enrolment list. RESULTS: There were seven cases of Cryptosporidium gastroenteritis in the case control study. Five cases and eight controls swam at pool 'X' during the outbreak period. The adjusted odds of developing cryptosporidial diarrhoea if an individual swam at pool 'X' was 34.5 (CI 2.3 - 2548). DHS identified another 11 laboratory confirmed cases associated with pool 'X' as well as cases not linked to pool 'X'. 125 cases were identified throughout Melbourne with the suspected involvement of seven swimming pools. CONCLUSIONS: Despite a high odds ratio of developing cryptosporidiosis this outbreak was not detected by routine surveillance methods. Current outbreak detection methods lack sensitivity, specificity or timeliness. IMPLICATIONS: Improved surveillance systems are required if outbreaks of gastroenteritis are to be detected early so an intervention can be instigated to reduce the amount of subsequent illness.

The association between the use of swimming pools and illness has long been recognized. Discovery of hardier varieties of pathogens and increased use of spa pools and hot tubs has resulted in higher recommended levels of disinfectants. Chlorine is the most widely used disinfectant in pools. When maintained at proper levels, chlorine can inactivate or control most pathogens in swimming pool water. The exception is Cryptosporidium oocysts which are extremely resistant to most disinfectants and require high levels of chlorine for long periods of time to achieve deactivation. With the prevalence of Cryptosporidium infection estimated as high as 4.5% in the general population and the recent occurrence of five outbreaks of Cryptosporidiosis at public pools, more extreme measures must be taken to protect the health of pool users. Depending on the circumstances, when a fecal accident occurs at a public pool, it is prudent to consider closing the pool for up to a day so that the pool water can be properly disinfected and filtered., The association between the use of swimming pools and disease has been well documented in literature (1). An outbreak of cryptosporidiosis among swimmers who visited a water park illustrates that this infection may be caused by ingestion of fecally-contaminated pool water. Cryptosporidiosis is a parasitic infection that causes diarrhea, abdominal cramps, or vomiting. Between July and September of 1992, laboratory workers in Lane County, Oregon reported an increasing number of Cryptosporidium-infected stool samples. Researchers studied 52 people with positive stool specimens for cryptosporidiosis between June and October of 1992. Common symptoms of cryptosporidiosis included diarrhea, abdominal pain, vomiting, and low-grade fever. Eighteen infected people were compared to 18 uninfected people to determine the source of the illness. Half of the infected people and none of the uninfected people reported swimming at a nearby wave pool. The wave pool filter backwash contained a Cryptosporidium oocyst. In early September of 1992, the wave pool water was drained and replaced, after which the outbreak subsided., Objective. - To determine the cause of a community-wide outbreak of cryptosporidiosis. Design. - A matched case-control study. Setting. - General community of Lane County, Oregon. Patients and Other Participants. - Persons with Cryptosporidium detected in their stool from June to October 1992 were identified by contacting laboratories serving the area. Exposures of the first 18 case patients identified were compared with those of 18 age- and neighborhood - matched controls selected from a reverse telephone directory. Main Outcome Measures. - Reported exposures to risk factors for cryptosporidiosis and abatement of cryptosporidiosis outbreak. Results. - Fifty-five patients with cryptosporidiosis were detected, including 37 who were the first individuals ill in their households. The case-control study involving the first 18 case patients showed no association between illness and attendance at day care or drinking municipal water or drinking untreated surface waters (river or lake water) in the 2 weeks before onset of illness. However, nine of 18 case patients reported swimming at a local wave pool, compared with none of 18 controls. We ultimately identified 17 case patients who reported swimming at the same wave pool during their incubation periods, whose exposure dates spanned a 2-month period. Inspection of the pool's filtration system did not detect any abnormalities. The outbreak subsided after the pool water was drained and replaced. Conclusions. - This prolonged outbreak of cryptosporidiosis was likely caused by exposure to fecally contaminated wave pool water. Since Cryptosporidium is highly chlorine resistant and inadequately removed by sand filters, such outbreaks may represent an unrecognized hazard of wave pools, where the likelihood of inadvertent water ingestion is high. Such outbreaks may go undetected in areas where cryptosporidiosis is not reportable or laboratory screening is infrequent. (JAMA 1994;272:1597-1600)
