An experiment was conducted under normal (with six irrigation during crop period) and stressed condition during Rabi 2019 in All India Coordinated Research Project (AICRP) on Wheat, MARS, Dharwad, Karnataka with the view of assessing genetic variability, heritability and expected genetic advance in 104 F4 families of the cross HD2864 × DBW14. The progenies exhibited wide range of variation for all the traits indicating the presence of enough genetic variability in the material under study. Under timely sown condition, the traits like leaf waxyness, NDVI at different stages, SPAD at different stages, number of productive tillers per plant, seeds per spike, grain filling duration, spike length, thousand grains weight and seed set percentage showed high variability. The traits like plant height, canopy temperature at different stages and membrane stability index showed low variability. Under late sown condition, the traits like leaf waxyness, plant height, NDVI at different stages, SPAD at different stages, number of productive tillers per plant, seeds per spike, yield per plant, row bulk yield and seed set percentage showed high variability. The traits like days to maturity, spike length and membrane stability index showed low variability. The findings shows that all traits related to crop yield are positively linked, except for canopy temperature at different growth stages. Promising lines identified through Heat susceptibility index (HSI) and Heat tolerance index (HTI) can be utilized in future breeding programs to enhance crop productivity.

Purpose: The percentage of cataract in the working population in the age group of less than 50 years is on the rise. This study was done to assess the work profile and identify the modifiable risk factors for presenile cataract. Methods: Patients in the age group of 20–50 years presenting to the Out-Patient Department of Ophthalmology with presenile cataracts were recruited as cases. Controls were age-matched patients without cataract. All details were collected using a questionnaire. A complete ocular examination and blood investigation were done. For normally distributed continuous data, for differences between the two groups, the Student’s t test was used, as was the Wilcoxon rank sum test for non-normal data and the Chi-square test for categorical variables. To look for an association between cataract and the risk factors, logistic regression was done, and the results were reported as the odds ratio, 95% confidence interval, and P value. Results: Seventy-five cases and 75 controls were recruited for the study after informed consent. The odds of developing presenile cataract were eight times higher for blue-collar workers, with an odds ratio of 8.154 (P < 0.001). 52% were of lower socio-economic status in cases compared to 34% of controls (P 0.04). Underweight was a risk factor for presenile cataract, with an odds ratio of 28.064. (P 0.02). Conclusions: Blue-collar workers, people belonging to lower socio-economic status, and underweight were risk factors for presenile cataract in the current study. We recommend a periodic screening of cataracts in occupational health centres for all blue-collar workers. Measures to improve the general health of the presenile population need to be taken, which will also address the risk factors for presenile cataracts.

Evidence-based practices (EBPs) have been taught to students by identifying the best evidence/evidence from research results. However, the experiences of Indonesian nurse preceptors in helping students implement research findings have not been explored thoroughly. This study aimed to explore Indonesian nurse preceptors in guiding nursing students to use research findings.

Aims: The importance of screening for hemoglobinopathies is well-documented in India. However, information on the distribution of hemoglobinopathies in Karnataka is lacking. The present study focuses on determining the spectrum of hemoglobinopathies for various districts of Karnataka. Materials and Methods and Results: A retrospective analysis of samples registered for hemoglobinopathies for a period of 5 years (2017–2021) was carried out. A total of 17066 records registered only from the Karnataka region, were anonymized and retrieved. The data included gender, age, district, and results of the tests. The results were based on complete blood count, peripheral smear, and capillary electrophoresis (CE) pattern. The data were revalidated by pathologists, and the unambiguous data were analyzed for the study. One-fourth of the records (25%) showed abnormal hematological parameters. The number of female records (66%) was twice that of males and both genders showed higher distribution of thalassemia, followed by variants and double heterozygotes (DH). Several cases of thalassemia major were identified below the age of 17 years. The majority of thalassemia cases were β thal and 93% of them were β thal trait. Among the variants, HbS was more prevalent than HbE. Among the districts, Hassan had a 35.2% thal, Mysuru had a 7.2% variant, and Chitradurga had a 5.5% DH. Thalassemia, variants, and DH were distributed across several districts of Karnataka to various levels. Conclusion: The comprehensive retrospective analysis of the spectrum of hemoglobinopathies in various districts of Karnataka serves as evidence to carry out a prospective study on population screening where the incidence of thalassemia and structural variants is high.

Abstract Background Physician burnout is rising, especially among academic physicians facing pressures to increase their clinical workload, lead administrative tasks and committees, and be active in research. There is a concern this could have downstream effects on learners’ experiences and academic physician’s ability to teach learners on the team. Methods A 29-question RedCap survey was electronically distributed to 54 attending physicians within an academic learning health system who oversaw the General Medicine inpatient teaching services during the 2022–2023 academic year. The aims were to assess this cohort of attending physicians’ experiences, attitudes, and perceptions on their ability to effectively teach learners on the team, feeling valued, contributors to work-life balance and symptoms of burnout, Fisher’s Exact Tests were used for data analysis. Results Response rate was 56%. Attendings splitting time 50% inpatient / 50% outpatient felt that team size and type of admissions model affected their ability to effectively teach learners (p = 0.022 and p = 0.049). Attendings with protected administrative time felt that non-patient care obligations affected their ability to effectively teach the learners (p = 0.019). Male attendings and attendings with ≤ 5 years of General Medicine inpatient teaching experience felt less valued by residency leadership (p = 0.019 and p = 0.026). 80% of attendings experienced emotional exhaustion, and those with > 10 weeks on a General Medicine inpatient teaching service were more likely to experience emotional exhaustion (p = 0.041). Attendings with > 10 weeks on a General Medicine inpatient teaching service and those who were a primary caregiver were more likely to experience depersonalization (p = 0.012 and p = 0.031). 57% of attendings had reduced personal achievement. Conclusions Institutions should seek an individual and organizational approach to professional fulfillment. Special attention to these certain groups is warranted to understand how they can be better supported. Further research, such as with focus groups, is needed to address these challenges.

Background Alzheimer disease (AD) leads to cognitive decline and alters functional connectivity (FC) in key brain regions. Resting-state functional magnetic resonance imaging (rs-fMRI) assesses these changes using static-FC for overall correlation and dynamic-FC for temporal variability.

Background The purpose of the study was to assess the obstacles faced by diabetes patients in their self-care and determine the factors associated with these obstacles. The management of diabetes mellitus (DM) extends beyond the clinician’s efforts, with the responsibility of the care also being shared by the patient to achieve better treatment outcomes and prevent complications. Self-care management is the most important part of DM treatment, which includes diet, regular exercise, blood glucose monitoring, medication and foot care. Methods A facility-based cross-sectional study was conducted among 107 type 2 DM patients aged >18 years using the Diabetes Obstacles Questionnaire-30. Patients scoring a mean response score >3 were considered to have an obstacle. We included age, gender, socioeconomic status (SES), duration of DM and blood glucose levels as factors for regression analysis and a P value Results A large majority (64.5%, n = 69) of our participants were aged above 55 years and belonged to lower socio-economic status (65.4%, n = 70). Family history of DM was present in 41% (n=44) of the participants. The median duration of DM among the participant was 10 (4 – 7) years. In our study, the participants faced obstacles for two items in the domains: Support from Friends & Family (mean score: 3.73) and Knowledge of the Disease (mean score: 3.58). A multinominal regression analysis revealed SES was predictive of participants who could not understand information from literature with a P. value of 0.002 (OR: 3.65, CI: 1.60-8.338). Conclusion The two major obstacles to self-management practices that were identified were in the domains of Support from Friends and Family, and Knowledge of the Disease. Socioeconomic status was identified to be a predictive factor associated with the participants who are not able to understand information from the literature.

BackgroundComprehensive Behavioral Intervention for Tics (CBIT) is a first-line treatment for tic disorders that aims to improve controllability over tics that an individual finds distressing or impairing. However, it is only effective for approximately half of patients. Supplementary motor area (SMA)-directed neurocircuitry plays a strong role in motor inhibition, and activity in this region is thought to contribute to tic expression. Targeted modulation of SMA using transcranial magnetic stimulation (TMS) may increase CBIT efficacy by improving patients' ability to implement tic controllability behaviors.MethodsThe CBIT + TMS trial is a two-phase, milestone-driven early-stage randomized controlled trial. The trial will test whether augmenting CBIT with inhibitory, non-invasive stimulation of SMA with TMS modifies activity in SMA-mediated circuits and enhances tic controllability in youth ages 12-21 years with chronic tics. Phase 1 will directly compare two rTMS augmentation strategies (1 Hz rTMS vs. cTBS) vs. sham in N = 60 participants. Quantifiable, a priori "Go/No Go Criteria" guide the decision to proceed to phase 2 and the selection of the optimal TMS regimen. Phase 2 will compare the optimal regimen vs. sham and test the link between neural target engagement and clinical outcomes in a new sample of N = 60 participants.DiscussionThis clinical trial is one of few to date testing TMS augmentation of therapy in a pediatric sample. The results will provide insight into whether TMS is a potentially viable strategy for enhancing CBIT efficacy and reveal potential neural and behavioral mechanisms of change.Trial registrationClinicalTrials.gov NCT04578912 . Cardiovascular diseases (CVDs) account for about 32% of global deaths. While digital stethoscopes can record heart sounds, expert analysis is often lacking. To address this, we propose LightCardiacNet, an interpretable, lightweight ensemble neural network using Bi-Directional Gated Recurrent Units (Bi-GRU). It is trained on the PASCAL Heart Challenge and CirCor DigiScope datasets. Static network pruning enhances model sparsity for real-time deployment. We employ various data augmentation techniques to improve resilience to background noise. An ensemble of the two networks is constructed by employing a weighted average approach that combines the two light-weight attention Bi-GRU networks trained on different datasets, which outperforms several state-of-the-art networks achieving an accuracy of 99.8%, specificity of 99.6%, sensitivity of 95.2%, ROC-AUC of 0.974 and inference time of 17 ms on the PASCAL dataset, accuracy of 98.5%, specificity of 95.1%, sensitivity of 90.9%, ROC-AUC of 0.961 and inference time of 18 ms on the CirCor dataset, and an accuracy of 96.21%, sensitivity of 92.78%, specificity of 93.16%, ROC-AUC of 0.913 and inference time of 17.5 ms on real-world data. We adopt the SHAP algorithm to incorporate model interpretability and provide insights to make it clinically explainable and useful to healthcare professionals.

Visual crypto-system is a class of cryptography intended to secure images. Random-grid crypto-system is a type of visual cryptosystem that generates an encrypted grid of the secret image utilizing a pre-encoded grid and the secret image. The random grid research is still engaging in three dimensions: security, quality, and efficiency. Though there are many works in improving security, there is scope for investigation in the other two directions from the perspective of technological advancements. There has been a significant increase in the number of Graphical Processing Unit (GPU) cores for which the random grid models are intuitively amenable. The random grid secret sharing models demand more improvement in the quality of the reconstructed image as they achieved only 50% contrast. In this paper, we proposed a GPU based random-grid model to improve its efficiency by exploiting the data-parallelism inherent in the model. In addition to this speedup of [Formula: see text] we restored the secret image with a quality almost equal to the original secret image using autoencoder super-resolution. Objective quality measures such as MSE, NCC, NAE and SSIM for the proposed model empirically confirm the improvement in image quality compared to other state-of-the-art models.

Among the various types of recently released composite materials, particle-reinforced metal matrix composites (MMCs), in particular aluminum as the matrix material, have been shown to provide substantial industrial benefits in the automotive and aerospace sectors. The current research focused on the corrosion behavior of an Al7075 hybrid composite in 0.1 M hydrochloric acid (HCl) and 3.5% NaCl media at different temperatures. Electrochemical techniques, such as Tafel polarization (TP) and electrochemical impedance spectroscopy (EIS), were employed to study the corrosion behavior in the respective solutions. The results showed that the corrosion rate of the studied specimen increased from 31.77 to 47.61 mmy−1 in 0.1 M HCl and from 0.26 to 1.28 mmy−1 in 3.5% NaCl with increasing temperature. The corrosion current density of the Al7075 hybrid composite in 0.1 M HCl is on the order of 10−3 Acm2, and that in 3.5% NaCl is on the order of 10−5 Acm2. This confirms the increased rate of composite corrosion in the 0.1 M HCl medium compared to that in the 3.5% NaCl medium. A suitable mechanism was proposed for the corrosion of the Al7075 hybrid composite in both media.

This article examines the precision of medical terminology commonly used to diagnose and understand the pathogenesis of electrical shock injuries. As everyday technology increasingly depends on advanced electrical mechanisms that utilize more efficient modes of electrical energy transmission, waveforms, and frequencies, emergency and trauma physicians will continue to encounter a broader array of electrical injury manifestations. This phenomenon prompts a closer examination of the diagnostic terminology associated with electrical shocks. The pathogenesis of electrical injury depends on the tissue electric field strength, frequency, current duration, and tissues involved. Some traditional diagnostic terms, for example, “entry” and “exit” wounds, arc-flash burns, and “high-voltage” and “low-voltage” electrical injuries, obscure the complexity of this pathogenesis, likely impeding medical management and advances in electrical safety science. This article presents the scientific rationale for suggested changes to medical terminology and aims to encourage future refinement.

ABSTRACT Background There is a paucity of research on the effects of commonly used substances, such as cannabis and other drugs, on quality of life as a contributor to head and neck cancer (HNC) prognosis. We examined associations between non‐alcohol or tobacco substance use (cannabis and other illicit drug) and self‐reported quality of life in patients with HNC prior to starting treatment. Methods This was a cross‐sectional study of patients who presented for routine psych‐oncologevaluation prior to treatment between 11/2015 and 9/2022. Primary exposures were cannabis use (never, past, or current users) and current illicit drug use (yes/no). The primary outcome measure was the Functional Assessment of Cancer Therapy—Head and Neck (FACT‐HN) subscales (physical, social/family, functional and emotional). Linear regression models examined associations between pretreatment substance use and FACT‐HN subscales adjusting for demographic, socioeconomic, and clinical factors. Results Of 570 patients, 13.9% endorsed current cannabis and 13.9% current illicit drug use. The mean (SD) scores for FACT‐HN subscales were physical well‐being = 22.8 (5.0), social well‐being = 22.7 (5.5), emotional well‐being = 17.5 (4.5), and functional well‐being = 18.7 (6.9). In the adjusted models, cannabis use was not independently associated with any FACT‐HN subscales. However, patients who currently used illicit drugs reported worse emotional well‐being (β = −1.32; 95% CI −2.45 to −0.20). No independent association was found between current illicit drug use and other subscales (physical, social, and functional). Conclusions Illicit drug use, but not cannabis use, is negatively associated with pretreatment emotional well‐being in patients with HNC. Further research exploring the relationships between longitudinal cannabis and illicit drug use and methods of consumption on QoL and cancer outcomes in patients with head and neck cancer is warranted.

Background Anti-N-methyl-D-aspartate (NMDA) receptor encephalitis has been recognised to present with the syndrome of catatonia. In severe cases dysautonomia is representative of malignant catatonia. The treatment with benzodiazepines (BZDs) and electroconvulsive therapy (ECT) may decrease morbidity and mortality in patients presenting with anti-NMDA receptor encephalitis and catatonia.Methods This is a retrospective case series of eight patients with anti-NMDA receptor encephalitis treated with ECT. We use clinical prediction scores (Clinical Assessment Scale for Autoimmune Encephalitis [CASE] and anti-NMDAR Encephalitis One-Year Functional Status scores) to compare expected outcomes and observed outcomes.Results CASE scores in our group ranged between 5 and 19, with a mean score of 13.8 (median 15.5). NEOS scores ranged from 2 to 4, with a mean and median of 3. Of the eight patients, six had a favourable modified Rankin Score (0–2) at a follow-up of 8 to 12 months. Patients received an average of 29.9 ECT treatments in total.Conclusions Based on clinical prediction scores, this cohort had better than expected functional outcomes. We discuss the use of BZDs and ECT in these cases and propose a treatment algorithm for patients who present with catatonic syndrome in anti-NMDA receptor encephalitis.

In subjects with peculiar susceptibility to severe infections by common pyogenic bacteria, mutations of interleukin-1 receptor-associated kinase proteins (IRAK)1 and IRAK4 had been identified. The IRAK kinases function as downstream signal transductors following the activation of pathogen recognition receptors. In two patients with sequential or repeated invasive infections: herpes simplex virus-triggered hemophagocytic lymphohistiocytosis with tuberculosis, and Streptococcus pneumoniae bacteremia with candidemia respectively, novel mutations of IRAK2 were identified. These mutations compromised the capacity to ubiquinate (or functionally modify) the signal adaptor tumor necrosis factor receptor-associated factor 6. The result is impairment of the cytokine tumor necrosis factor-alpha production. This susceptibility to a varied range of pathogens underlines a potential central role played by IRAK2 in mediating host defense in infectious diseases.

Corrosion is one of the crucial topics of concern, as it has caused a lot of economic losses in many countries across the world. Various measures have been taken over the years to tackle corrosion, amongst which corrosion inhibitors have taken the limelight. As far as we know, there hasn't been any published work that brings together all the research on how different types of surfactants can prevent corrosion on different materials. This review covers 100+ surfactants acting as corrosion inhibitors for different metals, providing a comprehensive and accessible resource. Surfactants possess unique properties such as their adequate hydrophilic to hydrophobic ratio, presence of halogen atoms, and diversity due to charge variations, making them a potential corrosion inhibitor. Surfactants containing long hydrophobic tails, electronegative atoms, and smaller spacer sizes showed increased inhibition efficacy. This paperarticle summarizes the type of metal and medium upon which the surfactant acted and hence protected metal corrosion. In addition, studies have explained about the type of surfactants, the electrochemical, spectroscopic, computational simulations, and different adsorption isotherms. The significance of the literature is to provide the readers with a library of surfactants possessing corrosion inhibition properties against various materials and to maintain sustainability.

Shrimp aquaculture has been growing rapidly over the last three decades. However, high-density aquaculture together with environmental degradation has led to increased incidence of shrimp infections. Thus, devising and implementing effective strategies to predict, diagnose and control the spread of infections of shrimps are crucial, also to ensure biosecurity and sustainability of the food industry. With the recent advancements in biotechnology, more attention has been given to develop novel promising therapeutic tools with potential to prevent disease occurrence and better manage shrimp health. Furthermore, owing to the advent of the next-generation sequencing (NGS) platforms, it has become possible to analyze the genetic basis of susceptibility or resistance of different stocks of shrimps to infections and how sustainable aquaculture could be made free of shrimp diseases.

The global aquaculture industry is increasingly seeking sustainable alternatives to fishmeal (FM) due to its high price and shortfall in supply. In this context, fermented aquatic weed meal (FAWM) could emerge as a viable plant protein source for aquafeed. Four isoproteic diets [30 % crude protein (CP)] were formulated, incorporating 50 % total protein from FAWM comprising fermented Azolla diet (D1), Pistia diet (D2), and Eichhornia diet (D3). The control diet (D0) did not contain FAWM. At the end of the 90 days feeding trial, their growth performance, whole-body proximate composition, gut microbial load, haemato-biochemical indices, liver and gut health, and economic efficiency were determined. Fish fed with D0 had significantly (p < 0.05) improved growth performance and feed utilization compared to other treatment groups. Meanwhile, the fish supplemented with D1 diet exhibited significantly (p < 0.05) higher final weight (g), specific growth rate (%/day), weight gain (%), total biomass (g), and protein efficiency ratio among the FAWM dietary groups. The D1 group also demonstrated the significantly (p < 0.05) highest whole-body CP (64.27 ± 0.40 %) and lower crude lipid (8.24 ± 0.28 %) compared to other test diets. The total bacteria (TB) and lactic acid bacteria (LAB) in the fish gut were found to be significantly (p < 0.05) higher in D1 group. Furthermore, most of the hemato-biochemical indices of fish were significantly (p < 0.05) affected by FAWM inclusions, with few exceptions. The histological findings indicated that amongst the FAWM groups, D1 fish exhibited improved intestinal health. Nonetheless, the gut of the control fish demonstrated substantially (p < 0.05) lower villi width and crypt depth than other treatments. The D1 and D2 diet groups had significantly improved liver health. Moreover, FAWM inclusion enhanced economic efficiency by considerably reducing farm feed cost (US$/kg) and increasing return on investment (%). In summary, dietary inclusion of fermented Azolla pinnata (D1) protein at 50 % in aquafeed promoted feed utilization, growth, health, and farm economics of Asian catfish fingerlings compared to other FAWM diets.

Background:. Hidradenitis suppurativa (HS) is a chronic skin disease characterized by recurrent nodules that affect areas with a high density of apocrine sweat glands, such as the axillae and groin. Androgens are implicated in the pathophysiology of HS. Therefore, spironolactone, an antiandrogen therapy, is recommended. However, data on its use in women of childbearing age are limited, especially since its antiandrogenic effects may affect menstruation, fertility, and pubertal development. Objective:. To evaluate the efficacy and safety of spironolactone in the treatment of hidradenitis suppurativa in women of childbearing age and to identify factors associated with treatment response. Methods:. A retrospective analysis was conducted on female patients aged 12 to 50 with HS treated with spironolactone at Michigan Medicine dermatology clinics from 2000 to 2021. The patients’ demographic data, HS characteristics, and spironolactone responses were examined. Statistical assessments were performed to determine the efficacy indicators. Results:. Of the 157 patients reviewed, 31 showed an improvement in treatment. Variables such as axillary involvement, previous treatment failures, and use of intralesional steroids were linked to a lack of improvement in spironolactone. Through adjusted multiple logistic regression analysis, a significant association was observed between improvement status and Hurley stage 3 (odds ratio = 0.15 [95% CI: 0.02–0.79], P = .036), suggesting that patients with Hurley stage 3 were 85% less likely to exhibit improvement in spironolactone therapy. Limitations:. The study’s retrospective nature and reliance on single-center data can limit generalizability. The sample size is limited and therefore affects the study’s statistical power. Conclusion:. Thus, spironolactone may offer therapeutic benefits for HS in women of childbearing age. However, patients with severe disease (Hurley stage 3) had reduced response rates. Further prospective studies are recommended to validate these findings and determine the most suitable patient profile for spironolactone therapy for HS.
