Abstract

Although agriculture is now the globally predominant mode of food production, studies of the skeletal remains of early agriculturalists have indicated high levels of physiological stress and poor health relative to hunter-gatherers in similar environments. Previous studies identifying this trend in different regions prompt further research of the causes and effects of subsistence transitions in human societies. Here, 242 dentitions from five ancient Egyptian and Nubian populations are examined: 38 individuals from Jebel Sahaba (Upper Paleolithic), 56 from Badari (Predynastic), 54 from Naqada (Predynastic), 47 from Tarkhan (Dynastic), and 47 from Kerma (Dynastic). These populations span the early period of agricultural intensification along the Nile valley. Skeletal remains were scored for the presence of linear enamel hypoplasia (LEH) of the dentition, an established indicator of physiological stress and growth interruption. The prevalence of LEH was highest in the 'proto-agricultural' (pastoralist) Badari population, with a gradual decline throughout the late Predynastic and early Dynastic periods of state formation. This suggests that the period surrounding the emergence of early agriculture in the Nile valley was associated with high stress and poor health, but that the health of agriculturalists improved substantially with the increasing urbanization and trade that accompanied the formation of the Egyptian state. This evidence for poor health among protoand early agriculturalists in the Nile valley supports theories that agricultural intensification occurred as a response to ecological or demographic pressure rather than simply as an innovation over an existing stable subsistence strategy. Abstract

IMPORTANCE: Phthalate exposure is widespread among pregnant women and may be a risk factor for preterm birth. OBJECTIVE: To investigate the prospective association between urinary biomarkers of phthalates in pregnancy and preterm birth among individuals living in the US. DESIGN, SETTING, AND PARTICIPANTS: Individual-level data were pooled from 16 preconception and pregnancy studies conducted in the US. Pregnant individuals who delivered between 1983 and 2018 and provided 1 or more urine samples during pregnancy were included. EXPOSURES: Urinary phthalate metabolites were quantified as biomarkers of phthalate exposure. Concentrations of 11 phthalate metabolites were standardized for urine dilution and mean repeated measurements across pregnancy were calculated. MAIN OUTCOMES AND MEASURES: Logistic regression models were used to examine the association between each phthalate metabolite with the odds of preterm birth, defined as less than 37 weeks of gestation at delivery (n = 539). Models pooled data using fixed effects and adjusted for maternal age, race and ethnicity, education, and prepregnancy body mass index. The association between the overall mixture of phthalate metabolites and preterm birth was also examined with logistic regression. G-computation, which requires certain assumptions to be considered causal, was used to estimate the association with hypothetical interventions to reduce the mixture concentrations on preterm birth. RESULTS: The final analytic sample included 6045 participants (mean [SD] age, 29.1 [6.1] years). Overall, 802 individuals (13.3%) were Black, 2323 (38.4%) were Hispanic/Latina, 2576 (42.6%) were White, and 328 (5.4%) had other race and ethnicity (including American Indian/Alaskan Native, Native Hawaiian, >1 racial identity, or reported as other). Most phthalate metabolites were detected in more than 96% of participants. Higher odds of preterm birth, ranging from 12% to 16%, were observed in association with an interquartile range increase in urinary concentrations of mono-n-butyl phthalate (odds ratio [OR], 1.12 [95% CI, 0.98-1.27]), mono-isobutyl phthalate (OR, 1.16 [95% CI, 1.00-1.34]), mono(2-ethyl-5-carboxypentyl) phthalate (OR, 1.16 [95% CI, 1.00-1.34]), and mono(3-carboxypropyl) phthalate (OR, 1.14 [95% CI, 1.01-1.29]). Among approximately 90 preterm births per 1000 live births in this study population, hypothetical interventions to reduce the mixture of phthalate metabolite levels by 10%, 30%, and 50% were estimated to prevent 1.8 (95% CI, 0.5-3.1), 5.9 (95% CI, 1.7-9.9), and 11.1 (95% CI, 3.6-18.3) preterm births, respectively. CONCLUSIONS AND RELEVANCE: Results from this large US study population suggest that phthalate exposure during pregnancy may be a preventable risk factor for preterm delivery.

Abstract

Background: DNA methylation has been shown to be associated with adiposity in adulthood. However, whether similar DNA methylation patterns are associated with childhood and adolescent body mass index (BMI) is largely unknown. More insight into this relationship at younger ages may have implications for future prevention of obesity and its related traits. Methods: We examined whether DNA methylation in cord blood and whole blood in childhood and adolescence was associated with BMI in the age range from 2 to 18 years using both cross-sectional and longitudinal models. We performed meta-analyses of epigenome-wide association studies including up to 4133 children from 23 studies. We examined the overlap of findings reported in previous studies in children and adults with those in our analyses and calculated enrichment. Results: DNA methylation at three CpGs (cg05937453, cg25212453, and cg10040131), each in a different age range, was associated with BMI at Bonferroni significance, P < 1.06 × 10−7, with a 0.96 standard deviation score (SDS) (standard error (SE) 0.17), 0.32 SDS (SE 0.06), and 0.32 BMI SDS (SE 0.06) higher BMI per 10% increase in methylation, respectively. DNA methylation at nine additional CpGs in the cross-sectional childhood model was associated with BMI at false discovery rate significance. The strength of the associations of DNA methylation at the 187 CpGs previously identified to be associated with adult BMI, increased with advancing age across childhood and adolescence in our analyses. In addition, correlation coefficients between effect estimates for those CpGs in adults and in children and adolescents also increased. Abstract

Infant adiposity may be influenced by several environmental risk factors, but few studies have explored these interactions.To examine the interaction between exposure to secondhand smoke and breastfeeding exclusivity on adiposity at age 5 months.We studied 813 mother-offspring pairs from the longitudinal Healthy Start study. Fat mass and fat-free mass were measured by air displacement plethysmography. Linear regression analyses were used to estimate the association between household smokers (none, any) with fat mass, fat-free mass, percent fat mass, weight-for-age z-score, weight-for-length z-score and BMI-for-age z-score as separate outcomes. Interaction terms between household smokers and breastfeeding exclusivity (5 months, ≥5 months) were added to separate models.The combination of exposure to secondhand smoke and a lack of exclusive breastfeeding was associated with increased adiposity at age 5 months. For example, within the not exclusively breastfed strata, exposure to secondhand smoke was associated with increased fat mass (0.1 kg; 95% CI: 0.0-0.2; P = 0.05). Conversely, within the exclusively breastfed strata, there was virtually no difference in fat mass between exposed and non-exposed infants (coefficient: -0.1; 95% CI: -0.3-0.1; P = 0.25).Our findings may inform new public health strategies with potential relevance for both smoking cessation and obesity prevention.

Abstract

BACKGROUND: Certain perfluoroalkyl and polyfluoroalkyl substances (PFAS) are widespread, persistent environmental contaminants. Prenatal PFAS exposure has been associated with lower birth weight; however, impacts on body composition and factors responsible for this association are unknown. OBJECTIVES: We aimed to estimate associations between maternal PFAS concentrations and offspring weight and adiposity at birth, and secondarily to estimate associations between PFAS concentrations and maternal glucose and lipids, and to evaluate the potential for these nutrients to mediate associations between PFAS and neonatal outcomes. METHODS: Within the Healthy Start prospective cohort, concentrations of 11 PFAS, fasting glucose, and lipids were measured in maternal midpregnancy serum (n = 628). Infant body composition was measured using air displacement plethysmography. Associations between PFAS and birth weight and adiposity, and between PFAS and maternal glucose and lipids, were estimated via linear regression. Associations were decomposed into direct and indirect effects. RESULTS: Five PFAS were detectable in >50% of participants. Maternal perfluorooctanoate (PFOA) and perfluorononanoate (PFNA) concentrations were inversely associated with birth weight. Adiposity at birth was approximately 10% lower in the highest categories of PFOA, PFNA, and perfluorohexane sulfonate (PFHxS) compared to the lowest categories. PFOA, PFNA, perfluorodecanoate (PFDeA), and PFHxS were inversely associated with maternal glucose. Up to 11.6% of the effect of PFAS on neonatal adiposity was mediated by maternal glucose concentrations. Perfluorooctane sulfonate (PFOS) was not significantly associated with any outcomes studied. 139. Maternal Gestational Diabetes Mellitus and Newborn DNA Methylation: Findings From the Pregnancy and Childhood Epigenetics Consortium.

Abstract

OBJECTIVE Maternal gestational diabetes mellitus (GDM) has been associated with adverse outcomes in the offspring. Growing evidence suggests that the epigenome may play a role, but most previous studies have been small and adjusted for few covariates. The current study meta-analyzed the association between maternal GDM and cord blood DNA methylation in the Pregnancy and Childhood Epigenetics (PACE) consortium. RESEARCH DESIGN AND METHODS Seven pregnancy cohorts (3,677 mother-newborn pairs [317 with GDM]) contributed results from epigenome-wide association studies, using DNA methylation data acquired by the Infinium HumanMethylation450 BeadChip array. Associations between GDM and DNA methylation were examined using robust linear regression, with adjustment for potential confounders. Fixed-effects meta-analyses were performed using METAL. Differentially methylated regions (DMRs) were identified by taking the intersection of results obtained using two regional approaches: comb-p and DMRcate. RESULTS Two DMRs were identified by both comb-p and DMRcate. Both regions were hypomethylated in newborns exposed to GDM in utero compared with control subjects. One DMR (chr 1: 248100345–248100614) was located in the OR2L13 promoter, and the other (chr 10: 135341870–135342620) was located in the gene body of CYP2E1. Individual CpG analyses did not reveal any differentially methylated loci based on a false discovery rate–adjusted P value threshold of 0.05. CONCLUSIONS Maternal GDM was associated with lower cord blood methylation levels within two regions, including the promoter of OR2L13, a gene associated with autism spectrum disorder, and the gene body of CYP2E1, which is upregulated in type 1 and type 2 diabetes. 143. Perfluoroalkyl Substances in Pregnancy and the Risk of Preeclampsia

Abstract

Perfluoroalkyl substances (PFASs) are persistent, ubiquitous environmental contaminants and may be related to preeclampsia, a common pregnancy complication. Previous studies have found serum concentrations of PFASs to be positively associated with serum cholesterol in non-pregnant individuals, and also associated with pregnancy-induced hypertension, including preeclampsia. Using data from the large, population-based Norwegian Mother and Child Cohort (MoBa) Study, we estimated associations between PFAS concentrations measured during pregnancy and an independently validated diagnosis of preeclampsia. Additionally, we estimated associations between mid-pregnancy PFAS concentrations and plasma lipid concentrations in order to evaluate one possible mechanism of association between PFASs and preeclampsia. A case-cohort study was conducted to estimate associations between mid-pregnancy plasma PFAS concentrations and preeclampsia. In proportional hazards models adjusted for maternal age, pre-pregnancy body mass index, education and smoking, we observed no positive associations between PFASs and preeclampsia. However, we found an inverse association between perfluoroundecanoic acid (PFUnDA) and preeclampsia, with a hazard ratio of 0.8 (95% confidence interval [CI]=0.7, 0.9) per natural-log unit increase in PFUnDA. In a separate cross-sectional analysis, we found positive associations between mid-pregnancy plasma concentrations of seven PFASs and high-density lipoprotein cholesterol (HDL). The greatest change in HDL per natural-log unit of exposure was associated with PFUnDA. HDL increased 3.7 mg/dL per interquartile shift in PFUnDA (95% CI=2.5, 4.9). A multi-pollutant model for HDL including seven PFAS exposures also showed the strongest association with PFUnDA compared with the other six PFASs. Additionally, perfluorooctane sulfonate (PFOS) was positively associated with plasma total cholesterol. Total cholesterol increased 4.2 mg/dL per interquartile shift in PFOS (95% CI=0.8, 7.7) in adjusted models. We found positive associations between certain PFASs and plasma lipid parameters, and we observed that different PFASs may have different strengths of association with lipid parameters during pregnancy. We did not observe any positive associations between mid-pregnancy plasma PFAS concentrations and a validated diagnosis of preeclampsia. The plasma concentrations of PFOA in this study, while within the range of previous studies of non-occupationally exposed populations, are substantially lower than exposure levels in the previous study that found associations between PFOA and preeclampsia.

Abstract

This research paper, Contemplating the Terrain Vague, is an examination of the post- industrial landscapes of Rhodes and Port Kembla in New South Wales and Fremantle in Western Australia. Through direct observation of these areas, I have developed a series of images, which explore degradation, pollution and waste of the surrounding foreshores and waterways. My art practice is informed by scientific documentation of this landscape and environs by organisations such as Greenpeace, The Environmental Protection Agency, Rhodes Remediation and The National Toxic Network. These documents examine the effect of pollutants on the foreshores, waterways and inhabitants. Specific artworks by the Dutch photographer Hans Scholten along with South African artists John Muafangejo and Jo Ratcliffe are referenced for their social and political subject matter. The notion of the print as an important social document is discussed, as alluded to by Dr Elizabeth Rankin, writer and curator of South African history. My work examines the legacy of manufacturing by chemical companies for those who reside on contaminated land. In this context, the principle of the Terrain Vague is presented, highlighting the contradictions of those abandoned spaces that can be crucial or harmful to cities. Similarly, the Urbanist theoretical writing of Dr Helen Armstrong in her paper Time, Dereliction and Beauty: an argument for ‘Landscapes of Contempt’ and Professor Ignasi de Solà-Morales, author of The Terrain Vague have informed my practice. The exploration and recording of these abandoned spaces is addressed in the photographic imagery of Urbexer art - a movement that comments on the legacy of waste. This resonates with my own investigation of industrial degradation and decay. Similarly, the artistic manifestations of Romantic, English born, American artist Thomas Cole and American, Frederic Edwin Church are referenced in terms of their aesthetic appreciation of the ruin and the picturesque. I have created a series of images, sited in chapters: Hidden Agenda, The Modern Ruin and Living on the Edge, that feature recurring symbolic motifs, including images of swallows, mangroves, deformed mullet and the prefabricated and affordable post - World War 2 Australian domestic dwelling. Each are used as symbols of the changing face of a post-industrial landscape. Within this body of work some images deal with satirical references and present an ironic view of the comfortable suburban home, albeit, surrounded by smelters and pollution. Contemplating the Terrain Vague refers to actual places and events. It highlights past mistakes, caused through inadequate knowledge and monitoring and sounds a timely warning for future industrial development.

Abstract

Background: Despite widespread use of weight- and length-based anthropometric indexes as proxies for adiposity, little is known regarding the extent to which they correspond with fat mass (FM) or fat-free mass (FFM) during infancy. Objective: This study aimed to examine associations of 3 derived indicators-weight-for-age z score (WFAZ), weight-for-length score (WFLZ), and body mass index z score (BMIZ)-with FM, percentage of FM, and FFM measured by air-displacement plethysmography during the first 5 mo of life. Design: Applying prospectively collected data from 1027 infants in a Colorado prebirth cohort, we used multivariate regression to evaluate associations between the derived indicators and body composition at birth and at 5 mo, and with change (Δ) during follow-up. Results: At birth, all 3 derived indicators were more strongly associated with FFM than with FM. Each unit of WFAZ corresponded with 0.342 kg FFM (95% CI: 0.331, 0.351 kg FFM), compared with 0.121 kg FM (95% CI: 0.114, 0.128 kg FM) (P < 0.0001); similar trends were observed for WFLZ and BMIZ. By 5 mo, WFLZ and BMIZ were more strongly associated with FM than with FFM, whereas WFAZ correlated similarly with the 2 components of body composition. ΔWFLZ and ΔBMIZ were both more strongly related to ΔFM than to ΔFFM; however, a direct comparison of the 2 indexes with respect to change in the percentage of FM indicated that ΔBMIZ was the optimal proxy of adiposity gain (P < 0.0001, pairwise difference). Conclusions: Weight- and length-based indexes are poor surrogates for newborn adiposity. However, at 5 mo, WFLZ and BMIZ are suitable proxies of FM.
