104. Psychometric testing of a person-centred care scale the Eden Warmth Survey in a long-term care home in New Zealand

Abstract

Background: Traditional nursing homes have been viewed as dominated by the medical model. Since the 1990s, the Eden AlternativeTM has become a significant model in systemic transformations in nursing homes. The purpose of this study was to evaluate the psychometric performance of the 20 items of the Eden Warmth Survey – Residents (EWS-R) in an aged-care home. Design: A resident's satisfaction survey was used to collect a sample of 85 long-term care home residents. Methods: Psychometric evaluation included item analyses, reliability including internal consistency and stability, criterion-related validity and construct validity. Results: The reduced 13 items demonstrated adequate reliability (a = 0.82) with two factors, Trust and Connectedness with Others and Care Practices, extracted and contributed to 57.9% of the total variance. Conclusions: The 13-item of EWS-R can be considered as a reliable and predictive scale for assessing quality of life and overall satisfaction on people living in long-term care facilities.

Abstract

Objective: To describe trends in indirect cause-specific pregnancy-related mortality from 1998 to 2015.Design: Secondary analysis of annual, national cross-sectional database of maternal and late maternal deaths, identified through active surveillance of deaths among women aged 10-50 years.Setting: Jamaica, a middle-income Caribbean country.Population: Maternal and late maternal deaths.Methods: Descriptive trend analyses of demographic and cause-specific maternal and pregnancy-related mortality ratios undertaken comparing the periods 1998-2003, 2004-2009 and 2010-2015. Multivariate logistic regression was used to confirm changes in risk of indirect death.Main Outcome Measures: Maternal, pregnancy-related, direct, indirect and cause-specific mortality ratios (deaths/100 000 live births).Results: Maternal deaths from indirect conditions increased between the first two periods (P = 0.004) and stabilised in the third (P = 0.085). Associated with upward movement in cardiovascular deaths (P[trend] = 0.003), women under 25 years were at elevated risk (odds ratio 1.44, 95% CI 1.00-2.08; P = 0.052). Haematological/immunological conditions (69% sickle cell disease) ranked second but did not vary with time. Health service utilisation was similar across age, parity, health region and major cause categories (non-communicable diseases, non-obstetric infections, direct), however women with indirect conditions spent more time in hospital (median 5 days versus 3 days) and more often died after the puerperium.Conclusions: Medical conditions, especially cardiovascular disease, are increasingly associated with maternal and late maternal mortality. Middle-income countries need to simultaneously improve management of indirect conditions, while redoubling efforts to reduce direct deaths. Postpuerperal medical services should be integrated into routine infant health services to improve continuity of care during this high-risk period.Tweetable Abstract: Maternal survival (SDG 3.1) in LMICs requires better care for women with both non-communicable diseases and obstetric conditions. Abstract

Since the 2001 attacks on the twin towers, policies on security have changed drastically, bringing about an increased need for tools that allow for the detection of deception. Many of the solutions offered today, however, lack scientific underpinning. We recommend two important changes to improve the (cost) effectiveness of security policy. To begin with, the emphasis of deception research should shift from technological to behavioural sciences. Secondly, the burden of proof should lie with the manufacturers of the security tools. Governments should not rely on security tools that have not passed scientific scrutiny, and should only employ those methods that have been proven effective. After all, the use of tools that do not work will only get us further from the truth.

Abstract

Autism spectrum disorders (ASDs) are common, heritable neurodevelopmental conditions. The genetic architecture of ASDs is complex, requiring large samples to overcome heterogeneity. Here we broaden coverage and sample size relative to other studies of ASDs by using Affymetrix 10K SNP arrays and 1,168 families with at least two affected individuals, performing the largest linkage scan to date while also analyzing copy number variation in these families. Linkage and copy number variation analyses implicate chromosome 11p12-p13 and neurexins, respectively, among other candidate loci. Neurexins team with previously implicated neuroligins for glutamatergic synaptogenesis, highlighting glutamate-related genes as promising candidates for contributing to ASDs. © 2007 Nature Publishing Group.

A concise exact pointer theory for von Neumann projector measurements of pre- and post-selected quantum systems has previously been used to formalize the notion of weak value persistence and apply it to explain pointer position data collected from a dynamical quantum non-locality detection experiment. This paper applies this exact pointer theory to provide an operational explanation of weak value persistence observed in the data obtained from a recent quantum box experiment. Abstract

This study examined visual search for animal fear stimuli and whether high fear levels influence children's visual search. Experiment 1 was conducted with adults to provide a control for the effects observed in Experiments 2 and 3 with children. Both adults and children were faster to locate snakes and spiders among flowers and mushrooms than vice versa in arrays of nine but not of four pictures. Both groups were also faster to determine target absence from arrays of snakes and spiders than flowers and mushrooms regardless of array size. Experiment 3 showed that compared with low-fearful children, those who feared snakes and spiders did not show a search advantage for determining target absence from arrays containing snakes and spiders compared with flowers and mushrooms. These results support preferential search for animal fear stimuli in children and suggest that high fearfulness affects children's ability to disengage attention from feared stimuli. © The Australian Psychological Society Ltd.

Abstract

In this paper the role of B mode real time diagnostic ultrasonography, both with and without Doppler and colour flow mapping facilities, in the investigation of bovine cardiac disease is discussed. Its uses are illustrated in a number (n = 18) of clinical cases. The examination technique is explained along with the equipment and the success of the simplistic approach of B mode echocardiography, which is compared with the application of Doppler vascular flow analysis and colour flow mapping. Conditions such as pericarditis, endocarditis and some congenital defects can be interrogated satisfactorily with B mode but smaller valvular lesions and complex congenital anomalies require Doppler techniques. In the future, the use of echocontrast agents may permit the investigation of these latter problems with simple equipment in the in vivo application of such a contrast agent is illustrated., The Bovine Practitioner, No. 32.2 (1998 May)

Abstract

We introduce a construction technique for generalized complex linear processing orthogonal designs, which are p x n matrices X satisfying XHX = f I, where f is a complex quadratic form, I is the identity matrix, and X has complex entries. These matrices generalize the familiar notions of orthogonal designs and generalized complex orthogonal designs. We explain the application of these matrices to space-time block coding for multiple-antenna wireless communications. In particular, we discuss the practical strengths of the space-time block codes constructed via our proposed technique
