Resumen La miastenia grave es una enfermedad que afecta los impulsos de los nervios que son transmitidos a los músculos en las coyunturas neuromusculares. Es una enfermedad de origen autoinmune en la que el cuerpo genera un ataque contra su propio sistema muscular. Aunque las expectativas para personas con miastenia grave son generalmente buenas cuando son tratadas apropiadamente, la miastenia grave (MG) puede ser hasta mortal cuando la debilidad muscular interfiere con la respiración. No es heredada exceptuando el síndrome miastenico congénito, ni contagiosa y afecta primordialmente a los músculos voluntarios ó esquelétales. Los músculos faciales y masticatorios pueden estar afectados. De allí su enorme importancia en el campo odontológico ya que la debilidad muscular puede causarse por un medicamento colocado durante el tratamiento. Se reporta el caso de un paciente de sexo femenino de 65 años de edad con Miastenia Grave a quien fue necesario realizarle 3 exodoncias simples en diferentes oportunidades. El objetivo es conocer el protocolo adecuado a seguir para tratar a estos pacientes desde el punto de vista odontológico, de tal forma de poder realizar el tratamiento evitando situaciones de emergencia que pongan en riesgo la vida del paciente. Abstract Myasthenia Gravis is an illness that affect the nerves impulses and this is transmitted to the muscles in the neuromuscular joint. Is a autoimmune disorder in which the body attack his own muscle system. The expectative for this population is good when they are treatment correctly. The Myasthenia Gravis could be mortal when breath is affected. MG is not directly inherited nor is it contagious. MG involves the voluntary muscles of the body. The facials and masticators muscles can be involved. It is important in the dental field because the weakness could be produced for many drugs put in during the treatment. A report one case: female patient, 65 years old with Myasthenia Gravis. Were done 3 simples extractions in different opportunities. The objective is to know the correct protocol we need to follow to treat this patients from the dental point of view in order to avoid dangerous situations for the patient live.

In this paper we address the problem of designing allocation schemas of large database-objects in a Web environment that may be exposed to significant changes in usage and access patterns and scaling of data. In these circumstances, if the design is not adjusted to the new changes, the system can undergo a severe degradation in data access costs and response time. In order to solve this problem, we propose a formal model to generate a new database-object allocation. The model uses current state information of the system and usage statistical data collected during a given period, and adapts the system to the new usage patterns so as to minimize communication costs. Implicitly the mathematical model handles database-object migration and scaling of the number of sites. Model tests have been conducted with satisfactory and promising results. The principles used in the model can be applied to other models for the optimization of Web resources.

El Queratoquiste Odontogénico constituye un tipo histológico de los quistes odontogénicos de desarrollo que se presenta entre la segunda y tercera década de la vida y tiene un segundo pico de aparición hacia la quinta década. La ubicación más frecuente es en la zona del tercer molar inferior, ángulo mandibular desde donde progresan hacia la rama y el cuerpo. Su importancia radica en alta tasa de recurrencia ubicada entre el 30 y 60%. El Queratoquiste Odontogénico presenta características histopatológicas que lo diferencian de otras lesiones quísticas. Se reportan dos casos: el primero se trata un paciente de sexo masculino de 56 años de edad que presentó expansión vestibular en la región de canino y premolar inferior izquierdo. El segundo caso de trata de trata de una joven de 19 años de edad que presentó lesión expansiva de gran tamaño de ambas tablas óseas que se extendía desde la zona del 45 hasta la Escotadura Sigmoidea, del lado derecho. Después de los exámenes clínico, radiográfico e histopatológico se diagnosticó: Queratoquiste Odontogénico en ambos casos. Se realizó tratamiento quirúrgico obteniendo resultados satisfactorios. The Odontogenic Keratocyst is a distinctive histopathologic type of developmental Odontogenic cyst. It occurs in second and third decade of life and it has a second appearance pick in fifth decade. Frequently the lesion is locate in the third molar region, jaw angle, mandible body and ramous. Is important because it has a high recurrence rate which has been estimated at around 30 and 60%. The Odontogenic Keratocyst has histopathologic characteristics which differentiate other cyst lesions. A report of two cases: the first is about a male patient 56 years old. With a vestibule expansion in canine region and lower left premolar. The second case is about a young girl 19 years old. She had a big size expansive lesion of both bone tables, from 45’s region to Sigmoid neckline. After the clinic, radiographic and histopathologic exams the diagnose was: Odontogenic Keratocyst in both cases. The treatment was surgical remove and the results were satisfactory.
