Abstract

Mathematics teachers frequently provide concrete manipulatives to students during instruction; however, the rationale for using certain manipulatives in conjunction with concepts may not be explored. This article focuses on area models that are currently used in classrooms to provide concrete examples of integer and binomial multiplication. The innovation of combining the representations for negative numbers with both algebra tiles and "Algeblocks" is provided, with a mathematical justification for its development. Teachers' effective integration of tools such as these in mathematics instruction can help students develop conceptual understanding and procedural fluency.

Abstract

Tissue-resident and infiltrating immune cells are continuously exposed to molecules derived from the local cells that often come in form of secreted factors, such as cytokines. These factors are known to impact the immune cells' biology. However, very little is known about whether the tissue resident immune cells in return also affect the local environment. In this study, with the help of RNA-sequencing, we show for the first time that long-term absence of epidermal resident Langerhans cells led to significant gene expression changes in the local keratinocytes and resident dendritic epidermal T cells. Thus, immune cells might play an active role in maintaining tissue homeostasis, which should be taken in consideration at data interpretation.

115. 'The Blacker the Berry, the Sweeter the Juice': African American Student Writers and the National Assessment of Educational Progress.

Abstract

A study analyzed the degree to which an African American verbal tradition (Black English Vernacular) survives in the writing of Black students across a generational time span. A total of 867 essays from the 1984 and the 1988/89 National Assessment of Educational Progress (NAEP) were subjected to primary trait and holistic scoring analysis, and were ranked in terms of the degree of African American discourse. These scores were compared to the scores given by NAEP raters, and to scores from the 1969 and 1979 NAEP. Results indicated that: (1) no correlation existed between a discernibly African American discourse style and the production of BEV syntax, supporting results of earlier studies of 1969 and 1979 NAEP essays; (2) the more discernibly African American the discourse, the higher the primary trait and holistic scores, and the less discernibly African American the discourse, the lower the primary trait and holistic scores, contrary to earlier studies; and (3) "imaginative/narrative" essays continued to be Black students' strong suit. Findings suggest that students who employed a Black expressive discourse style received higher NAEP scores than those who did not. Recommendations for writing instructors include: capitalize on the strengths of African American cultural discourse; encourage students toward the field dependency style, which enables them to produce more powerful, meaningful, and more highly rated essays; and deemphasize concerns about BEV grammar. 116. A Rare STXBP2 Mutation in Severe COVID-19 and Secondary Cytokine Storm Syndrome

Abstract

Background: Primary (familial) hemophagocytic lymphohistiocytosis (pHLH) is a potentially lethal syndrome of infancy, caused by genetic defects in natural killer (NK) cell and CD8 T cell cytotoxicity, leading to hyperinflammation, elevated cytokine levels, and a disorganized immune response resulting in multi-organ system failure and frequently death. Secondary HLH (sHLH) can be triggered in the setting of malignances, diseases of chronic immune system activation, or by infectious etiologies. While pHLH is usually a result of homozygous gene mutations, monoallelic hypomorphic and dominant-negative mutations in pHLH genes have been implicated in sHLH. Coronavirus disease 2019 (COVID-19) has been an omnipresent viral infection since its arrival, and severe cases can present with cytokine storm and have clinical features and laboratory findings consistent with sHLH. Herein, we report an adolescent with severe COVID-19, decreased NK cell function, and features of sHLH. Her genetic evaluation identified a monoallelic missense mutation in the pHLH gene STXBP2, and NK cell assays of her blood showed decreased cytolysis and degranulation ex vivo. Methods: Patient data was extracted through an electronic medical record review. Using a lentiviral approach, the patient’s STXBP2 mutation and wild-type (WT) STXBP2 were separately transduced into the NK-92 human NK cell line. The WT and mutant STXBP2 transduced NK-92 cells were stimulated with NK-sensitive K562 erythroleukemia target cells in vitro, and NK cell degranulation and cytolysis were measured via CD107a expression and Live/Dead near-IR dye, respectively. Results: Compared to WT STXBP2, the patient’s STXBP2 mutation caused significantly decreased NK cell cytolysis and associated degranulation in vitro. Conclusion: These findings add weight to the hypothesis that some severe cases of COVID-19 may be accompanied by sHLH and hyperinflammation, especially in the setting of heterozygous pHLH genetic mutations. This has implications both diagnostically and therapeutically for severe COVID-19.

117. Large-scale brain organization during facial emotion processing as a function of early life trauma among adolescent girls

Abstract

Background: A wealth of research has investigated the impact of early life trauma exposure on functional brain activation during facial emotion processing and has often demonstrated amygdala hyperactivity and weakened connectivity between amygdala and medial PFC (mPFC). There have been notably limited investigations linking these previous node-specific findings into larger-scale network models of brain organization. Method: To address these gaps, we applied graph theoretical analyses to fMRI data collected during a facial emotion processing task among 88 adolescent girls (n=59 exposed to direct physical or sexual assault; n=29 healthy controls), aged 11–17, during fMRI. Large-scale organization indices of modularity, assortativity, and global efficiency were calculated for stimulus-specific functional connectivity using an 883 region-of-interest parcellation. Results: Among the entire sample, more severe early life trauma was associated with more modular and assortative, but less globally efficient, network organization across all stimulus categories. Among the assaulted girls, severity of early life trauma and PTSD diagnoses were both simultaneously related to increased modular brain organization. We also found that more modularized network organization was related both to amygdala hyperactivation and weakened connectivity between amygdala and medial PFC. Conclusions: These results demonstrate that early life trauma is associated with enhanced brain organization during facial emotion processing and that this pattern of brain organization might explain the commonly observed association between childhood trauma and amygdala hyperactivity and weakened connectivity with mPFC. Implications of these results for neurocircuitry models are discussed.

124. 'What Is Africa to Me?': Language, Ideology and 'African American.'

Abstract

A study examined the history of racial labelling of Black Americans, from the perspective of their changing material condition and opinions concerning use of the term "African American." Using the paradigm that language is representative of a social construction of reality drawn from linguistics and sociology, use of the terms "African,""colored,""Negro,""Black," and "African American" is chronicled, focusing on societal forces and the relationship of Blacks to American society. Survey methodology and results are then discussed. The survey was conducted in 1989 in Atlanta (Georgia), Chicago (Illinois), Cincinnati (Ohio), Detroit (Michigan), and Philadelphia (Pennsylvania). Of 667 respondents, including both African and European Americans, 512 responded to the question "Do you think the term 'African American' should replace the term 'Black' as the name for Black people in the United States?" Results show just over one-third to one-half support a shift to "African American." Three broad explanations were offered by Blacks for approving the change: identification of dual heritage; inadequacy of the "color" label; aesthetic quality of the newer term. Three explanations were given for disapproval: Blacks are not African; syllabic density; no need for change. Demographic differences in responses were also revealed. Contains 35 references. (MSE)

Abstract

OBJECTIVES Despite poor healthcare transition outcomes among young adults with pediatric rheumatic diseases, adoption of transition best practices is low. We sought to understand how structured transition processes were operationalized within pediatric rheumatology practices and what factors were perceived to enable adaptations during a global pandemic. METHODS We conducted a mixed methods study of team leaders' experiences during an interim analysis of a pilot project to implement transition policy discussions at sites in the Childhood Arthritis and Rheumatology Research Alliance Transition Learning Collaborative. We combined quantitative assessments of organizational readiness for change (9 sites) and semi-structured interviews of team leaders (8 sites) using determinants in the Exploration, Preparation, Implementation, Sustainment Framework. RESULTS Engagement of nursing and institutional improvement efforts facilitated decisions to implement transition policies. Workflows incorporating educational processes by non-physicians were perceived to be critical for success. When the pandemic disrupted contact with non-physicians, capacity for automation using electronic medical record (EMR)-based tools was an important facilitator, but few sites could access these tools. Sites without EMR-based tools did not progress despite reporting high organizational readiness to implement change at the clinic level. Lastly, educational processes were often superseded by acute issues, such that youth with greater medical/psychosocial complexity may not receive the intervention. CONCLUSIONS We generated several considerations to guide implementation of transition processes within pediatric rheumatology from the perspectives of team leaders. Careful assessment of institutional and nursing support is advisable before conducting complex transition interventions. Ideally, new strategies would ensure interventions reach youth with high complexity.

129. Time to cancer treatment and reproductive outcomes after fertility preservation among adolescent and young adult women with cancer

Abstract

Fertility preservation (FP) may be underused after cancer diagnosis because of uncertainty around delays to cancer treatment and subsequent reproductive success.Women aged 15 to 39 years diagnosed with cancer between 2004 and 2015 were identified from the North Carolina Central Cancer Registry. Use of assisted reproductive technology (ART) after cancer diagnosis between 2004 and 2018 (including FP) was assessed through linkage to the Society for Assisted Reproductive Technology. Linear regression was used to examine time to cancer treatment among women who did (n = 95) or did not (n = 469) use FP. Modified Poisson regression was used to estimate risk ratios (RRs) and 95% CIs for pregnancy and birth based on timing of ART initiation relative to cancer treatment (n = 18 initiated before treatment for FP vs n = 26 initiated after treatment without FP).The median time to cancer treatment was 9 to 33 days longer among women who used FP compared with women who did not, matched on clinical factors. Women who initiated ART before cancer treatment may be more likely to have a live birth given pregnancy compared with women who initiated ART after cancer treatment (age-adjusted RR, 1.47; 95% CI, 0.98-2.23), though this may be affected by the more frequent use of gestational carriers in the former group (47% vs 20% of transfer cycles, respectively).FP delayed gonadotoxic cancer treatment by up to 4.5 weeks, a delay that would not be expected to alter prognosis for many women. Further study of the use of gestational carriers in cancer populations is warranted to better understand its effect on reproductive outcomes.

131. Low Risk of Lymphoma in Pediatric Patients Treated for Inflammatory Bowel Disease

Abstract

Despite the effectiveness of immune suppressing therapies in treating pediatric inflammatory bowel diseases (IBD), concerns of lymphoma may limit their use. We used a large administrative claims database to evaluate the risk of lymphoma in pediatric IBD and conducted a case series analysis of medication exposure in children diagnosed with lymphoma.We analyzed administrative claims from the 2007-2018 IQVIA database and identified pediatric (≤18 years) patients with Crohn's disease (CD), or ulcerative colitis (UC) using ICD-9/10 codes and pharmacy claims. Lymphoma cases were identified by diagnosis codes and confirmed by independent, claim-by-claim review by a pediatric oncologist and gastroenterologist. We calculated incidence rates (IR) for lymphoma among patients with and without pharmacy claims for treatment followed by treatment description among those who developed lymphoma during follow-up.A total of 10,777 pediatric IBD patients received ≥1 IBD therapy (median age 15 years (12,17), 45% female and 61% diagnosed with CD) during 28,292 patient-years of follow up. Among treated patients, five lymphoma cases were identified (IR 17.7/100,000 patient-years; 95% CI 6.5, 39.2). Of these, 4 four were treated with a thiopurine prior to lymphoma diagnosis and none received anti-TNF monotherapy.The overall lymphoma incidence was low among our cohort of treated pediatric IBD patients. We observed no cases of lymphoma among patients prescribed anti-TNF monotherapy. These findings reinforce the relative safety of anti-TNF monotherapy for the treatment of pediatric IBD.

Abstract

In this pilot study, we evaluated the feasibility of implementing the Needs Assessment & Service Bridge (NA-SB)- an intervention to address the pervasive unmet needs of adolescents and young adults (AYAs) during cancer treatment. We conducted a mixed methods single-arm feasibility pilot study of NA-SB at the North Carolina Basnight Cancer Hospital. Eligible participants were AYAs ages 18–39 in active cancer treatment. After receiving NA-SB, participants completed a postintervention survey assessing their perceptions of NA-SB. We interviewed participating providers to assess their implementation experiences. On average, AYA participants (n = 26) rated NA-SB's feasibility as 4.5/5, its acceptability as 4.5/5, and its appropriateness as 4.4/5. 77% of participants agreed or strongly agreed that their needs were met in the study period. This pilot study generated preliminary evidence to establish NA-SB's feasibility as well as proof of concept for the intervention as a viable approach for identifying and addressing AYAs' unmet needs. 137. Gateway Lunar Habitat Modules as the Basis for a Modular Mars Transit Habitat

Abstract

This paper provides a summary of the results from a recent concept study of various configurations for a Mars Transit Habitat. The designs considered are composed of modules based on published contractor concepts proposed for the lunar Gateway through NASA’s NextSTEP program. Using these Gateway concepts as a starting point for the design of a Mars Transit Habitat has potential advantages. Both Gateway and Mars Transit Habitats will have similar requirements for long-term operations in deep space, autonomous and remote operations when the crew is not onboard, and similar requirements for transferring crew to and from a planetary surface—the Moon and Mars respectively. The contractor designs for Gateway were traded against a monolithic transit habitat previously proposed by NASA’s Mars Integration Group. In addition, these concepts were considered for a “shakedown” mission for the transit habitat hardware in cislunar space to build confidence in new systems, including the advanced environmental control and life support systems needed for Mars missions. The results presented include overall vehicle configurations, mass, and volume estimates for the selected design concepts. Two concepts using large expandable modules are identified as leading candidates for a Mars Transit Habitat and the remaining elements are identified as representative of the habitable pressure vessels needed for safe haven configurations, logistics modules, surface habitats, rovers, and descent and ascent crew cabins in the overall Mars Architecture.

Abstract

The purpose of this study was to use meta-analytic thinking to evaluate the results of a three-week professional development on mathematics teachers’ technological pedagogical content knowledge (TPACK). The study aims to elucidate the necessity of technology professional development activities to support mathematics teaching and learning in urban schools. This study utilizes mean difference confidence intervals as measures of the effectiveness of a professional development intervention in an urban school district in the U.S. This article presents a practical application of meta-analytic thinking to better contextualize the results through direct comparisons to similar studies. The results of this study suggest that the professional development increased mathematics teachers’ perceptions of their pedagogical knowledge, technological knowledge, pedagogical content knowledge, and technological content knowledge. The study results indicate that despite smaller overall effect sizes the results observed in this urban intervention were not statistically significantly different from most prior research in this area. Because of the chosen research approach, the research results have practical as well as empirical implications for the development and delivery of professional development in urban schools. This article contributes to the literature by providing a contextualized assessment of PD effects and by placing these effects in a broader scholarly context.
