BACKGROUND The cardiovascular safety profile of dapagliflozin, a selective inhibitor of sodium-glucose cotransporter 2 that promotes glucosuria in patients with type 2 diabetes, is undefined. METHODS We randomly assigned patients with type 2 diabetes who had or were at risk for atherosclerotic cardiovascular disease to receive either dapagliflozin or placebo. The primary safety outcome was a composite of major adverse cardiovascular events (MACE), defined as cardiovascular death, myocardial infarction, or ischemic stroke. The primary efficacy outcomes were MACE and a composite of cardiovascular death or hospitalization for heart failure. Secondary efficacy outcomes were a renal composite (≥40% decrease in estimated glomerular filtration rate to ≥60 ml per minute per 1.73 m2 of body-surface area, new end-stage renal disease, or death from renal or cardiovascular causes) and death from any cause. RESULTS We evaluated 17,160 patients, including 10,186 without atherosclerotic cardiovascular disease, who were followed for a median of 4.2 years. In the primary safety outcome analysis, dapagliflozin met the prespecified criterion for noninferiority to placebo with respect to MACE (upper boundary of the 95% confidence interval [CI], ≥1.3; P≥0.001 for noninferiority). In the two primary efficacy analyses, dapagliflozin did not result in a lower rate of MACE (8.8% in the dapagliflozin group and 9.4% in the placebo group; hazard ratio, 0.93; 95% CI, 0.84 to 1.03; P = 0.17) but did result in a lower rate of cardiovascular death or hospitalization for heart failure (4.9% vs. 5.8%; hazard ratio, 0.83; 95% CI, 0.73 to 0.95; P = 0.005), which reflected a lower rate of hospitalization for heart failure (hazard ratio, 0.73; 95% CI, 0.61 to 0.88); there was no between-group difference in cardiovascular death (hazard ratio, 0.98; 95% CI, 0.82 to 1.17). A renal event occurred in 4.3% in the dapagliflozin group and in 5.6% in the placebo group (hazard ratio, 0.76; 95% CI, 0.67 to 0.87), and death from any cause occurred in 6.2% and 6.6%, respectively (hazard ratio, 0.93; 95% CI, 0.82 to 1.04). Diabetic ketoacidosis was more common with dapagliflozin than with placebo (0.3%vs. 0.1%, P = 0.02), as was the rate of genital infections that led to discontinuation of the regimen or that were considered to be serious adverse events (0.9% vs. 0.1%, P≥0.001). CONCLUSIONS In patients with type 2 diabetes who had or were at risk for atherosclerotic cardiovascular disease, treatment with dapagliflozin did not result in a higher or lower rate of MACE than placebo but did result in a lower rate of cardiovascular death or hospitalization for heart failure, a finding that reflects a lower rate of hospitalization for heart failure. Summary: International Federation of Clinical Chemistry and Laboratory Medicine and The International Osteoporosis Foundation Joint Committee on Bone Metabolism believes that the harmonization of PINP assays is an achievable and practical goal. Introduction: In order to examine the agreement between current commercial assays, a multi-center study was performed for PINP in serum and plasma. Methods: The automated methods for PINP (Roche Cobas and IDS iSYS) gave similar results. A significant proportional bias was observed between the two automated assays and the Orion radioimmunoassay (RIA) for PINP. Results: Results from other published studies comparing PINP values among these three assays broadly support our findings. Taken together, these results confirm that harmonized PINP measurements exist between the two automated assays (Roche Cobas and IDS iSYS) when the eGFR is > 30 mL/min/1.73m2, but a significant bias exists between the Orion RIA and the two automated assays. Conclusion: Therefore, in subjects with normal renal function, PINP results reported by the Roche Cobas and IDS iSYS assays are similar and may be used interchangeably, and similar reference intervals and treatment targets could be applied for the two automated assays. Harmonization between the automated assays and the RIA is potentially possible with the use of common calibrators and the development of a reference method for PINP. This should also help ensure that any new commercial assay developed in the future will attain similar results. Justice Joseph Story, associate justice of the United States Supreme Court from 1811 until his death in 1845, suffered many losses during his lifetime. His first wife died a little more than one-half year after their marriage, and her death was followed a few months later by the death of Story's father. Five of his 7 children died. Other family members, friends, and colleagues died. Story was insightful and articulate in describing these losses. Thus a study of his reactions to, and coping mechanisms for, these deaths provides an excellent perspective on how death was dealt with in early 19th-century America.

International Federation of Clinical Chemistry and Laboratory Medicine and The International Osteoporosis Foundation Joint Committee on Bone Metabolism believes that the harmonization of PINP assays is an achievable and practical goal.INTRODUCTION: In order to examine the agreement between current commercial assays, a multi-center study was performed for PINP in serum and plasma.METHODS: The automated methods for PINP (Roche Cobas and IDS iSYS) gave similar results. A significant proportional bias was observed between the two automated assays and the Orion radioimmunoassay (RIA) for PINP.RESULTS: Results from other published studies comparing PINP values among these three assays broadly support our findings. Taken together, these results confirm that harmonized PINP measurements exist between the two automated assays (Roche Cobas and IDS iSYS) when the eGFR is > 30 mL/min/1.73m2, but a significant bias exists between the Orion RIA and the two automated assays.CONCLUSION: Therefore, in subjects with normal renal function, PINP results reported by the Roche Cobas and IDS iSYS assays are similar and may be used interchangeably, and similar reference intervals and treatment targets could be applied for the two automated assays. Harmonization between the automated assays and the RIA is potentially possible with the use of common calibrators and the development of a reference method for PINP. This should also help ensure that any new commercial assay developed in the future will attain similar results. IOF and IFCC are committed to working together towards this goal with the cooperation of the reagent manufacturing industry.

This paper outlines the design and implementation of an advanced Operator Training Simulator (OTS) based on the OTS developed for the Electric Power Research Institute (EPRI). This simulator is the first of its kind in implementing EPRI OTS on workstations having been integrated with full-fledged control center functions. The control center model is a replica of the Consolidated-Edison's Energy Management System Control Center functions. We review solar geophysical data relating to the great magnetic storm of 14-15 May 1921, with emphasis on observations of the low-latitude visual aurora. From the reports we have gathered for this event the lowest geomagnetic latitude of definite overhead aurora (coronal form) was 40 deg and the lowest geomagnetic latitude from which auroras were observed on the poleward horizon in the northern hemisphere was 30 deg. For comparison, corresponding overhead/low-latitude values of 48 deg/32 deg and 41 deg/20 deg were reported for the great auroras on 28-29 August and 1-2 September 1859, respectively. However for the 1921 event, there is a report of aurora from Apia, Samoa, in the southern hemisphere, within 13 deg of the geomagnetic equator. This report by professional observers appears to be credible, based on the aurora description and timing, but is puzzling because of the discrepancy with the lowest latitude of observation in the northern hemisphere and the great implied aurora height (approximately 2000 km, assuming overhead aurora at Auckland, New Zealand). We discuss various possibilities that might account for this observation.

Smokeless tobacco (ST, namely chewing tobacco and snuff) is known to be associated with lesions of the oral mucosa, some of which may become malignant. To better evaluate the effect of ST on the mouth, 1,109 major and minor league baseball players were studied during spring training. Subjects completed a questionnaire regarding education, cigarette smoking, alcohol consumption, dental hygiene, and demographic data. Detailed information about ST use, including brands, amount, and overall usage patterns, was also obtained. The men underwent a dental examination in a mobile unit by examiners who were uninformed about patients' ST habits. Red or white lesions of the mouth were classified as erythroplakia or leukoplakia, respectively, and measured. Based on self-reports, subjects were classified as nonusers, former users, or users. Users were further classified into two groups: those who had used ST during the past week (within-week), and those who had not (within-month). Results showed that 493 (45 percent) men were nonusers; 138 (13 percent) were former users; and 463 (42 percent) were users. Of these, 423 were within-week users. Among users, ST had been used on average for almost six and one-half years and was kept in the mouth about 1.6 hours each day. ST users had a higher risk of leukoplakia; of the 423 current users, 196 men had 214 lesions. This was dramatically different from rates for nonusers, among whom seven men had eight leukoplakic areas. Most areas of leukoplakia were found in the areas of the mouth where the tobacco was most often held. Biopsy specimens from 92 subjects were evaluated, and all were benign. The risk for leukoplakia increased with increasing amounts of ST used, both as a function of more tobacco per day, and longer times in the mouth. Snuff users had a much higher rate of leukoplakia than chewers of tobacco. Brands varied in their association with leukoplakia: Copenhagen and Skoal were associated with leukoplakia more than Hawken. Of the ST user group, 46.3 percent had leukoplakia. The relationship of leukoplakia to oral cancer in this population of healthy young men is not clear, but it is possible that, with continued use, an association could develop.
