Abstract

Background Recent guidelines suggested a wider use of long-acting injectable antipsychotics (LAI) than previously, but naturalistic data on the consequences of LAI use in terms of discontinuation rates and associated factors are still sparse, making it hard for clinicians to be informed on plausible treatment courses. Objective Our objective was to assess, under real-world clinical circumstances, LAI discontinuation rates over a period of 12 months after a first prescription, reasons for discontinuation, and associated factors. Methods The STAR Network ‘Depot Study’ was a naturalistic, multicentre, observational prospective study that enrolled subjects initiating a LAI without restrictions on diagnosis, clinical severity or setting. Participants from 32 Italian centres were assessed at baseline and at 6 and 12 months of follow-up. Psychopathology, drug attitude and treatment adherence were measured using the Brief Psychiatric Rating Scale, the Drug Attitude Inventory and the Kemp scale, respectively. Results The study followed 394 participants for 12 months. The overall discontinuation rate at 12 months was 39.3% (95% confidence interval [CI] 34.4–44.3), with paliperidone LAI being the least discontinued LAI (33.9%; 95% CI 25.3–43.5) and olanzapine LAI the most discontinued (62.5%; 95% CI 35.4–84.8). The most frequent reason for discontinuation was onset of adverse events (32.9%; 95% CI 25.6–40.9) followed by participant refusal of the medication (20.6%; 95% CI 14.6–27.9). Medication adherence at baseline was negatively associated with discontinuation risk (hazard ratio [HR] 0.853; 95% CI 0.742–0.981; p = 0.026), whereas being prescribed olanzapine LAI was associated with increased discontinuation risk compared with being prescribed paliperidone LAI (HR 2.156; 95% CI 1.003–4.634; p = 0.049). Conclusions Clinicians should be aware that LAI discontinuation is a frequent occurrence. LAI choice should be carefully discussed with the patient, taking into account individual characteristics and possible obstacles related to the practicalities of each formulation. Abstract

The negative impacts of the COVID-19 pandemic have been exacerbated in Africa by hunger, poor health care, poor educational systems, poverty, and lack of potable water and sanitation. With the pandemic and a worrying global recession as a result of COVID-19, our ability to achieve the 17 United Nations sustainable development goals (SDGs) in the post-pandemic era has been questioned. There is concern that the economic stagnation caused by COVID-19 will not only push more populations below the poverty line but also limit international support to ensure progress toward achieving the SDGs in Africa. This article highlights how the COVID-19 pandemic could threaten the actualization of the SDGs in Africa. We assessed relevant published literature, observations, and current global trends. Our results suggest that although the improvement of healthcare systems has become a priority in Africa, there is a need to ensure that some SDGs are not sacrificed to achieve control of the pandemic. Despite the pandemic, African countries need to identify policies that will not compromise the implementation of the SDGs and/or jeopardize previously achieved SDG targets.

105. Germline Mutation Analysis in Sporadic Breast Cancer Cases With Clinical Correlations

Abstract

Demographics for breast cancers vary widely among nations. The frequency of germline mutations in breast cancers, which reflects the hereditary cases, has not been investigated adequately and accurately in highly-consanguineous Pakistani population. In the present discovery case series, germ-line mutations in twenty-seven breast cancer candidate genes were investigated in eighty-four sporadic breast cancer patients along with the clinical correlations. The germ-line variants were also assessed in two healthy gender-matched controls. The clinico-pathological features were evaluated by descriptive analysis and Pearson χ2 test (with significant p-value 40 years) and ethnicity. The breast cancer gene panel assay was carried out by BROCA, which is a genomic capture, massively parallel next generation sequencing assay on Illumina Hiseq2000 with 100bp read lengths. Copy number variations were determined by partially-mapped read algorithm. Once the mutation was identified, it was validated by Sanger sequencing. The ethnic analysis stratified on the basis of age showed that the frequency of breast cancer at young age (≤40 years) was higher in Sindhis (n = 12/19; 64%) in contrast to patients in other ethnic groups. Majority of the patients had stage III (38.1%), grade III (50%), tumor size 2–5 cm (54.8%), and invasive ductal carcinoma (81%). Overall, the analysis revealed germ-line mutations in 11.9% of the patients, which was not significantly associated with younger age or any particular ethnicity. The mutational spectrum was restricted to three genes: BRCA1, BRCA2, and TP53. The identified mutations consist of seven novel germ-line mutations, while three mutations have been reported previously. All the mutations are predicted to result in protein truncation. No mutations were identified in the remaining twenty-four candidate breast cancer genes. The present study provides the framework for the development of hereditary-based preventive and treatment strategies against breast cancers in Pakistani population.

Abstract

This study, based on the 'Servizi Territoriali Associati per la Ricerca' (STAR) Network Depot Study nationwide baseline data, explored whether individual symptoms severity and clusters might influence the prescription of paliperidone palmitate 1-month (PP1M) vs. aripiprazole monohydrate. The Brief Psychiatric Rating Scale (BPRS) was used to assess psychopathology and relevant symptoms clusters. Drug Attitude Inventory, 10 items, was used to test attitude towards medications. Adherence to treatments was rated according to the Kemp seven-point scale. We assessed for eligibility 451 individuals and, among them, we included 195 subjects (n = 117 who started PPM1 and n = 78 aripiprazole monohydrate). Individuals were comparable in terms of age, gender, treatment years, recent hospitalizations, previous long-acting injectable antipsychotic treatments, additional oral treatments, attitude toward drugs, medication adherence, and alcohol/substance-related comorbidities. Subjects starting PP1M presented higher BPRS overall (P = 0.009), positive (P = 0.015), and negative (P = 0.010) symptom scores compared to subjects starting aripiprazole monohydrate. Results were confirmed by appropriate regression models and propensity score matching analysis. No differences were found comparing the other BPRS subscale scores: affect, resistance, and activation. Clinicians may be more prone to prescribe PPM1, rather than aripiprazole monohydrate, to subjects showing higher overall symptom severity, including positive and negative symptoms. No additional clinical factors influenced prescribing attitudes in our sample.

Abstract

We investigate the strongly lensed (mu similar or equal to x10-100) Lyman continuum (LyC) galaxy, dubbed Sunburst, at z = 2.37, taking advantage of a new accurate model of the lens. A characterization of the intrinsic (delensed) properties of the system yields a size of similar or equal to 3 sq. kpc, a luminosity of M-UV = -20.3, and a stellar mass of M similar or equal to 10(9) M-circle dot; 16% of the ultraviolet light is located in a 3 Myr old gravitationally bound young massive star cluster (YMC), with an effective radius of similar to 8 pc (corresponding to 1 milliarcsec without lensing) and a dynamical mass of -107 M. (similar to the stellar mass) - from which LyC radiation is detected (lambda < 912 angstrom). The star formation rate and stellar mass surface densities for the YMC are Log(10)(Sigma(SFR)[M(circle dot)yr(-1) kpc(-2)]) similar or equal to 3.7 and Log(10)(Sigma(M)[M-circle dot pc(-2)]) similar or equal to 4.1, with sSFR > 330 Gyr(-1), consistent with the values observed in local young massive star clusters. The inferred outflowing gas velocity (>300 km s(-1)) exceeds the escape velocity of the cluster. The resulting relative escape fraction of the ionizing radiation emerging from the entire galaxy is higher than 6-12%, whilst it is greater than or similar to 46-93% if inferred from the YMC multiple line of sights. At least 12 additional unresolved star-forming knots with radii spanning the interval 3-20 pc (the majority of them likely gravitationally bound star clusters) are identified in the galaxy. A significant fraction (40-60%) of the ultraviolet light of the entire galaxy is located in such bound star clusters. In adopting a formation timescale of the star clusters of 20 Myr, a cluster formation efficiency Gamma greater than or similar to 30%. Abstract

PurposeDeletion of Glutathione S-Transferase Theta 1 (GSTT1) encoding gene is implicated in breast cancer susceptibility, clinical outcomes, and survival. Contradictory results have been reported in different studies. The present investigation based on a representative Pakistani population evaluated the GSTT1-absent genotype in breast cancer risk and prognosis.MethodsA prospective study comprising case-control analysis and case series analysis components was designed. Peripheral blood samples were collected from enrolled participants. After DNA extraction, GSTT1 genotyping was carried out by a multiplex PCR with β-globin as an amplification control. Association evaluation of GSTT1 genotypes with breast cancer risk, specific tumor characteristics, and survival were the primary endpoints.ResultsA total of 264 participants were enrolled in the molecular investigation (3 institutions). The study included 121 primary breast cancer patients as cases and 143 age-matched female subjects, with no history of any cancer, as controls. A significant genetic association between GSTT1-absent genotype and breast cancer susceptibility (p-value: 0.03; OR: 2.13; 95% CI: 1.08-4.29) was reported. The case-series analysis showed lack of association of GSTT1 genotypes with menopause (p-value: 0.86), tumor stage (p-value: 0.12), grade (p-value: 0.32), and size (p-value: 0.07). The survival analysis revealed that GSTT1-absent genotype cases had a statistically significant shorter overall survival (OS) than those with the GSTT1-present genotype cases (mean OS: 23 months vs 33 months). The HR (95% CI) for OS in patients carrying GSTT1-absent genotype was 8.13 (2.91-22.96) when compared with the GSTT1-present genotype.ConclusionsThe present study is the first report of an independent significant genetic association between GSTT1-absent genotype and breast cancer susceptibility in a Pakistani population. It is also the foremost report of the association of this genotype with OS in breast cancer cases. Upon further validation, GSTT1 variation may serve as a marker for devising better population-specific strategies. The information may have translational implications in the screening and treatment of breast cancers.

Abstract

We present a multifrequency analysis of the radio galaxy 3CR 196.1 (z =0.198), associated with the brightest galaxy of the cool-core cluster CIZAJ0815.4-0303. This nearby radio galaxy shows a hybrid radio morphology and an X-ray cavity, all signatures of a turbulent past activity, potentially due to merger events and active galactic nuclei (AGN) outbursts. We present results of the comparison between Chandra and Very Large Telescope Multi-Unit Spectroscopic Explorer data for the inner region of the galaxy cluster, on a scale of tens of kpc. We discovered H α + [N ii ] λ 6584 emission spatially associated with the X-ray cavity (at ∼10 kpc from the galaxy nucleus) instead of with its rim. This result differs from previous discoveries of ionized gas surrounding X-ray cavities in other radio galaxies harbored in galaxy clusters and could represent the first reported case of ionized gas filling an X-ray cavity, either due to different AGN outbursts or to the cooling of warm (104 < T ≤ 107 K) AGN outflows. We also found that the H α, [N ii ] λ λ 6548, 6584, and [S ii ] λ λ 6718, 6733 emission lines show an additional redward component, at ∼1000 km s−1 from rest frame, with no detection in H β or [O iii ] λ λ 4960, 5008. We believe the most likely explanation for this redward component is the presence of a background gas cloud as there appears to be a discrete difference of velocities between this component and the rest frame. Abstract

In carcinomas, dissemination of cancer cells via blood or lymph circulation constitutes an early event. E-cadherin is a transmembrane calcium dependent adhesion protein. Cellular de-differentiation and plasticity, underlying metastasis, is attributed to the loss of function ofE-cadherin(cdh1) gene. The loss of gene expression may arise from promoter hypermethylation, which has been reported in multiple cancers. In the present pilot project, sixty (60) blood samples were collected from the breast cancer patients at a tertiary care hospital in Karachi, Pakistan. DNA was isolated from the cells circulating in the peripheral blood of the participants. Promoter hypermethylation was investigated through sodium-bisulfite treatment of DNA followed by methylation-specific PCR. In 53.3% of the patients,E-cadheringene promoter hypermethylation was observed. Promoter hypermethylation ofE-cadherinhas been reported in DNA isolated from the tissue specimen. However, to the best of our knowledge this is the first report ofE-cadherinpromoter hypermethylation in cells isolated from the peripheral blood of breast cancer patients from a geographically specific population. The results have important implications in tumour staging and selection of treatment regimens.

Abstract

We present a detailed study of ionized outflows in a large sample of similar to 650 hard X-ray-detected active galactic neuclei (AGNs). Using optical spectroscopy from the BAT AGN Spectroscopic Survey (BASS), we are able to reveal the faint wings of the [OIII] emission lines associated with outflows covering, for the first time, an unexplored range of low AGN bolometric luminosity at low redshift (z similar to 0.05). We test if and how the incidence and velocity of ionized outflow is related to AGN physical parameters: black hole mass (M-BH), gas column density (N-H), Eddington ratio (lambda(Edd)), [OIII], X-ray, and bolometric luminosities. We find a higher occurrence of ionized outflows in type 1.9 (55 per cent) and type 1 AGNs (46 per cent) with respect to type 2 AGNs (24 per cent). While outflows in type 2 AGNs are evenly balanced between blue and red velocity offsets with respect to the [OIII] narrow component, they are almost exclusively blueshifted in type 1 and type 1.9 AGNs. We observe a significant dependence between the outflow occurrence and accretion rate, which becomes relevant at high Eddington ratios [log(lambda(Edd)) greater than or similar to -1.7]. We interpret such behaviour in the framework of covering factor-Eddington ratio dependence. We do not find strong trends of the outflow maximum velocity with AGN physical parameters, as an increase with bolometric luminosity can be only identified when including samples of AGNs at high luminosity and high redshift taken from literature.

Abstract

We discovered Bowen emission arising from a strongly lensed (i.e. with magnification factor mu > 20) source hosted in the Sunburst arc at z = 2.37. We claim this source is plausibly a transient stellar object and study the unique ultraviolet lines emerging from it. In particular, narrow (sigma(v) similar or equal to 40 km s(-1)) ionization lines of Fe fluoresce after being exposed to Ly alpha (1216 angstrom) radiation that pumps selectively their atomic levels. Data from VLT/MUSE, X-Shooter, and ESPRESSO observations (the latter placed at the focus of the four UTs) at increasing spectral resolution of R = 2500, 11 400, and 70 000, respectively, confirm such fluorescent lines are present since at least 3.3 yr (similar or equal to 1 yr rest frame). Additional Fe forbidden lines have been detected, while C and Si doublets probe an electron density n(e) greater than or similar to 10(6) cm(-3). Similarities with the spectral features observed in the circumstellar Weigelt blobs of Eta Carinae probing the circumstellar dense gas condensations in radiation-rich conditions are observed. We discuss the physical origin of the transient event, which remains unclear. We expect such transient events (including also supernova or impostors) will be easily recognized with ELTs thanks to high angular resolution provided by adaptive optics and large collecting area, especially in modest (mu <3) magnification regime.
