Abstract: Purpose Uncertainty about the severity of the A(H1N1) pandemia persists. Information about disease severity can be obtained by investigating intensive care unit (ICU) admissions, especially when historical comparisons can be made with cases of community-acquired pneumonia (CAP). Methods This prospective observational study was conducted in 155 ICUs contributing to the GiViTI national database. To assess the impact on ICU workload, the occupancy rate during the epidemic phase was compared with influenza periods in previous years. A logistic regression model was developed to assess the prognostic importance of A(H1N1) influenza. Results The characteristics of the 319 A(H1N1) cases were similar to those reported in other studies, confirming the young age of patients (mean 43 years) and the higher prevalence among pregnant women and obese people. At the epidemic’s peak (October–December 2009) the occupancy rate did not significantly differ from the same period of the previous year, and was significantly lower than the 2009 seasonal influenza outbreak (January–March 2009). Compared with CAP of other origin (3,678 patients), A(H1N1) pneumonia was associated with a lower risk of death. However, after adjusting for confounding this was no longer the case (OR 0.88; 95% CI 0.59–1.31; p = 0.52). Conclusion This study confirmed the specific features of critically ill A(H1N1) patients (i.e., young age, pregnancy, obesity). The pandemic did not increase ICU workload compared with other periods. A(H1N1) pneumonia did not have a higher risk of death than CAP of different origin among patients admitted to the ICU. Abstract: We evaluated the systemic and cerebral effects induced by an increase to 100% of the inspired oxygen fraction (FiO2) on 20 comatose patients with head injury (9 patients) and SAH (11 patients). Brain tissue oxygen tension (PtiO2) was measured through a Clark electrode inserted in penumbra-like areas. We performed 55 hyperoxia tests by increasing FiO2 from 35 ± 8% to 100% in one second and calculating the PtiO2 index as: PtiO2 variation from baseline at 1 minute/arterial oxygen tension (PaO2) variation from baseline at 1 minute x 100. One hundred percent FiO2 caused an increase of both arterial (from 139 ± 28 to 396 ± 77 mmHg) and cerebral (from 22.6 ± 14 to 65.4 ± 60 mmHg) oxygenation after 1 minute. The range of the PtiO2 response was not uniform and two groups were identified. The change was small, 0.8 mmHg/min/100 mmHg PaO2(±0.7; range 0–2) when mean PtiO2 was 19.7 ± 13.1 mmHg, while a stronger response, 8 mmHg/min/100 mmHg PaO2 (±5; range 3–18) (p < 0.01) was found when mean PtiO2 was 31.7 ± 14.3 mmHg. Since O2 diffusion should follow the gas diffusion law, the increase in diffusion distance due to a reduction of capillary density in focal lesions may explain this relationship.

Abstract: Arterial hyperoxia improves oxygen tension measured into the cerebral tissue (ptiO2). The extent of this improvement in amelio- rating O2 delivery to the cerebral tissue, when cerebral blood flow (CBF) is reduced, is still unclear. The present experiment was developed to investigate the effect of arterial hyperoxia at normal or reduced CBF (baseline, CBF = 50–60%, and CBF = 20–30% of the baseline). CBF reduction was achieved in 7 pigs by saline infusion in a lateral ventricle. PtiO2 was measured by Licox equipment. Arterovenous oxygen difference (AVDO2) was calculated as the difference between arterial oxygen content and superior sagittal sinus oxygen content. Hyperoxia was induced by increasing inspired oxygen fraction to 100%. PtiO2 moved respectively from 27.95(± 10.15) to 45.98 (±15.31), from 14.77 (±3.58) to 30.71 (±12.2), and from 3.45 (±2.89) to 11.1 (±12.6) mmHg at normal CBF, after the first reduction and after the second reduction. O2 supply showed only a negligible increase. AVDO2 decreased during the phases of intact and moderate CBF impairment, while it did not change during the phase of severe CBF impairment. In conclusion: an increase of ptiO2 does not necessarily correspond to an improvement of brain oxygen delivery. The small increase in oxygen delivery due to hyperoxia may cause a slight improvement in the balance between O2 delivery and consumption during mild CBF reduction, but such improvement is negligible when severe CBF reduction occurs.

Abstract: Interest in tissue oxygen (PtiO2) monitoring is increasing. However the exact interactions between ptiO2, systemic and cerebral variables are a matter of debate. Particularly, the relationship between ptiO2, cerebral oxygen supply and consumption needs to be clarified. We designed a model to achieve progressive Cerebral Blood Flow (CBF) reduction through 3 steps: 1. baseline, 2. CBF between 50–60% of the baseline, 3. CBF < 30% of the baseline. In 7 pigs, under general anaesthesia, Cerebral Perfusion Pressure (CPP) and CBF were reduced through the infusion of saline in a lateral ventricle. PtiO2 and CBF were monitored respectively through a Clark electrode (Licox, GMS) and laser doppler (Peri-Flux). Blood from superior sagittal sinus and from an arterial line was simultaneously drawn to calculate the artero-venous difference of oxygen (AVD02). Brain oxygen supply was calculated by multiplying relative CBF change and arterial oxygen content. Pti02 reflected CBF reductions, as it was 27.95 (±10.15) mmHg during the first stage of intact CBF, declined to 14.77 (±3.58) mmHg during the first CBF reduction, declined to 3.45 (±2.89) mmHg during the second CBF reduction and finally fell to 0 mmHg when CBF was completely abolished. CBF changes were also followed by a decline in 02 supply and a parallel increase in AVD02. Conclusion: this model allows stable and reproducible steps of progressive CBF reduction in which ptiO2 changes can be studied together with oxygen supply and consumption.

110. [Cranial trauma and multiple trauma: from the street to the operating room]

Abstract: Brain injury occurs with a range of severity: even less severe cases should be carefully observed since they may deteriorate. By definition severe head injury has a Glasgow Coma Scale score of 8 or less; comatose patients are defined as cases who do not obey commands, do not open their eyes and do not speak. Very often (50% of case in our series) brain injury is associated with relevant extracranial injuries that may add to the severity of cases and may worsen outcome. The conceptual framework for treating head injury is based on the evidence that after the impact, the initial damage may be exacerbated by insults capable of further disturbing cerebral metabolism, leading to a final damage defined as secondary damage. Secondary damage represents the final end of many pathways that can be studied at the biochemical level and are centered in a calcium influx into the neuronal cell. Most probably there is a genetic susceptibility to secondary damage leading to a range of cellular dysfunctions for any given level of insult. The management of traumatic brain injury is aimed at interrupting the chain of events leading to secondary brain damage and from this perspective the fact that damage may develop over time can be seen as a window of opportunity for timely treatment. The milestone of treatment is the removal of surgical masses. This surgical treatment can be performed only in a brain that is properly perfused and once coagulation is preserved. Therefore the organization of treatment from rescue to neuro-traumatological centers should provide appropriate restoration of the volume and a normal oxygen delivery to the brain and to the overall organism.

Abstract: Monitoring cerebral oxygenation has been one of the main fields of interest in neurointensive care during the past few years. In fact it is strongly believed that restoring adequate cerebral oxygenation is the premise to maintaining the viability and restoring the function of the damaged CNS. Global monitoring provides an indirect estimation of adequacy of substrates supply to the brain. Local measurement of brain oxygen tension (ptiO2) is possible through a Clark electrode implanted into the cerebral parenchyma. The paper describes the physical basis of the monitoring, the pathophysiology of ptiO2 and its clinical use.

Abstract: Objective: (1) To describe the pattern of intracranial pressure (ICP) and cerebral perfusion pressure (CPP) in a group of severe head-injured patients, (2) to quantify complications of ICP monitoring, and (3) to describe a management protocol and its results. Design: Prospective observational study. Setting: General intensive care unit in a teaching hospital. Patients: 138 comatose patients, selected according to the following criteria: age > 16 years, coma [Glasgow Coma Scale (GCS) K 8] with at least one pupil reactive after resuscitation, digital recording of intracranial and arterial pressure, and jugular saturation measurements. Measurements and results: Median GCS was 5, and 62 patients had significant extracranial injuries; 71 had intracranial hematomas, which were urgently evacuated. Mean ICP was 20.5 mm Hg (SD 8.34), mean CPP was 71.86 mm Hg (SD 11.22); cerebral extraction of oxygen averaged 29 %. Medical therapy was used to control ICP in 130 cases; 93 patients required hyperventilation. Vasopressors were infused in 16 cases; in 14 cases a barbiturate infusion was started. In 6 patients all pharmacological treatments failed and surgical decompression was done. The only complication of ICP monitoring was meningitis in 3 patients. Outcome at 6 months was a good recovery and moderate disability for 82 patients (59.4 %), severe disability and vegetative status for 37 (26.8 %), and 19 patients died (13.7 %). The severity of intracranial hypertension was related to poorer results at 6 months. Conclusions: Intracranial hypertension is very frequent in severe head injury but can be reasonably well controlled by combined surgical and medical therapy.

114. [Neurosheet: an electronic sheet for neurosurgical intensive care]

Abstract: A growing number of monitoring and diagnostic instruments has considerably increased the amount of information available in the intensive care unit. A data collection system which can be useful in highlighting and processing the most relevant data for the care of head-injured patients (Neurosheet) has been set up. The purpose of this study is to describe Neurosheet and to analyse the results achieved through its use.Neurosurgical Intensive Care Unit.Neurosheet consists of a data sheet which follows the course of some "key" variables in head-injured patients who undergo ICP monitoring for a period of more than 48 hours.Through a questionnaire distributed to the medical staff, the learning period and the opinion of the staff concerning the clinical usefulness of this instrument has been evaluated. The amount of time needed to compile Neurosheet has been measured and some criteria indicating its completeness identified.It has been found that Neurosheet can be learned quickly and is rather easy to use. Data resulting from the study of the learning curve and from the answers to the questionnaire, suggest that its learning period is quite short (10 to 20 minutes). Among the 28 eligible patients, Neurosheet was used for 24 (86%). The data collected show 1,912 hours of monitoring, with an average of 112.8 (+/- 55) hours per patient. Neurosheet was completed in 83% of the cases.The aim of Neurosheet is to provide an answer to the overload of information using computerised systems able to simplify and support the daily clinical work.

115. Complications and Safety Associated with ICP Monitoring: A Study of 542 Patients

Abstract: In our institution ICP was monitored in patients with GCS ≤ 8 and abnormal CT scan: 362 severely head injured and 180 subarachnoid hemorrhage. Mean duration of monitoring was 103.6 hours (SD 74.96). Among 542 patients, 440 showed at least one episode of ICP above the threshold of 20mm Hg. Among 362 head injured patients only 71 (19.3%) had an ICP lower than 20mm Hg. In the remaining 289 (81.7%) at least one episode ≥20mm Hg was measured. In 13 cases (2.2%) a ventricular infection has been diagnosed. In 1 case an intraparenchimal hemorrhage related to the presence of the catheter was detected. Elevated risk of HICP and low incidence of complications have been shown in this series.

Abstract: A simplified model of intracranial dynamics is used to reproduce the intracranial pressure (ICP) time pattern in 20 patients with severe brain damage during PVI tests. A comparison of model responses and clinical tracings was achieved by minimizing a least square criterion function and adjusting just 5 parameters. These are: the CSF outflow resistance, the intracranial elastance coefficient, the autoregulation gain and time constant, and the basal values of arteriolar compliance. Based on the value of the autoregulation gain, the patients were classified into two groups: those with damaged autoregulation (8 out of 12) and those with preserved autoregulation (12 out of 20). Finally, analysis of the correlation between parameter estimates provided suggestions on the combination of parameter changes which may have the greater impact on ICP in the individual cases. Once these parameters have been identified, they may become possible targets for therapeutic interventions.

Physiol. 82(4): 1270–1282, 1997.—The time pattern of intracranial pressure (ICP) during pressure-volume index (PVI) tests was analyzed in 20 patients with severe acute brain damage by means of a simple mathematical model. In most cases, a satisfactory fitting between model response and patient data was achieved by adjusting only four parameters: the cerebrospinal fluid (CSF) outflow resistance, the intracranial elastance coefficient, and the gain and time constant of cerebral autoregulation. The correlation between the parameter estimates was also analyzed to elucidate the main mechanisms responsible for ICP changes in each patient. Starting from information on the estimated parameter values and their correlation, the patients were classified into two main classes: those with weak autoregulation (8 of 20 patients) and those with strong autoregulation (12 of 20 patients). In the first group of patients, ICP mainly reflects CSF circulation and passive cerebral blood volume changes. In the second group, ICP exhibits paradoxical responses attributable to active changes in cerebral blood volume. Moreover, in two patients of the second group, the time constant of autoregulation is significantly increased (>40 s). The correlation between the parameter estimates was significantly different in the two groups of patients, suggesting the existence of different mechanisms responsible for ICP changes. Moreover, analysis of the correlation between the parameter estimates might give information on the directions of parameter changes that have a greater impact on ICP.

Abstract: Serum levels of alpha 1-antichymotrypsin (alpha 1-ACT), macroglobulin, alpha 1-antitrypsin, ceruloplasmin, acid glycoprotein, transferrin, and C-reactive protein were measured in patients with probable late onset dementia of Alzheimer type (I-AD), patients with vascular dementia (VD), healthy elderly (HC), and patients with chronic or acute inflammation. Three blood samples were taken at 10-15 day intervals. Serum alpha 1-ACT levels from first and second serum samples were elevated in one out of 11 I-AD patients. Serpin serum levels from the third sample were increased in four out of 11 I-AD patients. None of these patients showed increased levels of other acute phase proteins measured in the three serum samples. VD patients did not show abnormal serum levels of alpha 1-ACT. Serum levels of alpha 1-ACT along with those of other acute phase proteins were altered in patients with inflammation. This investigation showed that in I-AD patients with increased alpha 1-ACT and no concomitant elevation of other acute phase proteins, serum levels of alpha 1-ACT varied with time. Based on our results, serum alpha 1-ACT does not appear to be a useful biomarker for clinical diagnosis of probable I-AD, but it might be associated with the clinical history of the disease.

120. Effects of hyperoxia on brain tissue oxygen tension in cerebral focal lesions

Abstract: We evaluated the systemic and cerebral effects induced by an increase to 100% of the inspired oxygen fraction (FiO2) on 20 comatose patients with head injury (9 patients) and SAH (11 patients). Brain tissue oxygen tension (PtiO2) was measured through a Clark electrode inserted in penumbra-like areas. We performed 55 hyperoxia tests by increasing FiO2 from 35 +/- 8% to 100% in one second and calculating the PtiO2 index as: PtiO2 variation from baseline at 1 minute/arterial oxygen tension (PaO2) variation from baseline at 1 minute x 100. One hundred percent FiO2 caused an increase of both arterial (from 139 +/- 28 to 396 +/- 77 mmHg) and cerebral (from 22.6 +/- 14 to 65.4 +/- 60 mmHg) oxygenation after 1 minute. The range of the PtiO2 response was not uniform and two groups were identified. The change was small, 0.8 mmHg/min/100 mmHg PaO2 (+/- 0.7; range 0-2) when mean PtiO2 was 19.7 +/- 13.1 mmHg, while a stronger response, 8 mmHg/min/100 mmHg PaO2 (+/- 5; range 3-18) (p0.01) was found when mean PtiO2 was 31.7 +/- 14.3 mmHg. Since O2 diffusion should follow the gas diffusion law, the increase in diffusion distance due to a reduction of capillary density in focal lesions may explain this relationship.

Abstract: We investigated the changes in brain oxygen tension (ptiO2) after ventilation with pure O2 in order to (1) clarify the pathophysiology of O2 exchange in the cerebral microcirculation; and (2) investigate the relationship between brain O2 tension, O2 delivery, and consumption in steady-state conditions during stepwise cerebral blood flow (CBF) reductions. A swine model was developed to reduce CBF in three stable steps: (1) baseline (CBF 100%), (2) CBF of 50-60% of baseline, and (3) CBF of30% of baseline. CBF was reduced by infusing saline into the left lateral ventricle through a catheter connected with an infusion pump. At each step, hyperoxia was tested by increasing the inspired oxygen fraction up to 100%, PtiO2 reflected the CBF reductions, since it was respectively 27.95 (+/-10.15), 14.77 (+/-3.58), and 3.45 (+/-2.89) mm Hg during the three CBF steps. Hyperoxia was followed by an increase in ptiO2, although the increase was significantly lower when hyperoxia was applied during progressive ischemia. O2 supply to the brain did not change during hyperoxia. Arteriovenous oxygen difference (AVDO2) decreased during the phases of intact CBF and moderate impairment, but not during the phase of severe CBF reduction. In conclusion, ptiO2 reductions closely reflect the imbalance between oxygen delivery and demand; this implies a link between low ptiO2 and defective O2 supply due to impaired CBF. However, this relation is not necessarily reciprocal, since manipulating brain oxygen tension does not always influence brain oxygen delivery, as in the case of ventilation with pure oxygen.

127. Cerebral veno-arterial pCO2 difference as an estimator of uncompensated cerebral hypoperfusion

Abstract: The aim of the present study was to assess the veno-arterial difference in pCO2 (delta pCO2) as an indicator of ischemia compared to the arteriovenous O2 difference (AVDO2). Staircase cerebral blood flow (CBF) reductions were obtained in seven domestic pigs by inducing intracranial hypertension: CBF 100%, 50-60% of baseline, 20-30% of baseline. ICP, MAP, CPP and CBF (Laser-Doppler method) were continuously recorded. The superior sagittal sinus was punctured to determine AVDO2 and delta pCO2. AVDO2 was 5.9 (+/- 1.78, range 3.3-7.4), 7.01 (+/- 1.31, range 5-8.9) and 8.17 (+/- 1.51, range 6.0-11.3) ml/100 ml in the three CBF steps (p = 0.001). CBF impairment was accompanied by the following increases in delta pCO2: from 10 (+/- 4, range 4-15) mmHg to 14.5 (+/- 4.11, range 10-27) mmHg, and to 31.2 (+/- 9.0, range 17-39) mmHg (p0.001). When CBF declines AVDO2 increases, indicating greater extraction of O2 to satisfy the aerobic metabolism. However, this mechanism can no longer compensate once a critical CBF threshold is reached. delta pCO2 rises slowly during moderate CBF reduction because of defective washout; the rise is impressive during marked CBF impairment when anaerobic metabolism takes place with proton buffering in CO2 and H2O. Therefore, when the brain's ability to compensate for low blood flow is exceeded, CO2 production outweighs O2 extraction.

Abstract: Subarachnoid hemorrhage has cerebral and systemic consequences as well. The main purpose of admitting a patient in the Intensive Care setting is to provide protection, mainly by stabilizing fundamental physiological parameters. There are both systemic parameters and cerebral parameters to be controlled continuously, and cerebral parameters may consider the brain as a whole (global cerebral parameters as intracranial pressure) or may provide information on specific areas. Cerebral protection may be achieved only when multiparametric monitoring is instituted, since only the combination of many physiologic parameters provides information capable of identifying, and hopefully of treating, deleterious derangements.

Abstract: Excitatory Amino Acids (EAAs) release has been considered to be neurotoxic in traumatic brain injury patients. Microdialysis samples of extracellular space (ECS) and high glutamate concentrations in cerebrospinal fluid (CSF) following Traumatic Brain Injury (TBI) have been documented. The objective of this study was to determine the correlation between EAA release in ECS and CSF in focal and diffuse injury. Head injury patients (GCSor = 8, n = 16) admitted to Medical College of Virginia Hospital were instrumented for microdialysis collection of ECS samples. CSF samples were collected through the external ventricular drainage catheter at four hour intervals for the first four days following injury. As a control group, CSF was collected from normal pressure hydrocephalus patients (n = 6). Elevated glutamate levels were observed in both CSF and ECS following head injury. The average glutamate concentration in CSF (3.20 +/- 3.62 mumol/l) was significantly increased from control levels (1.13 +/- 0.49 mumol/l, p0.05). Comparison of CSF and extracellular fluid (ECF) samples showed that the glutamate concentrations were maximal on the first and second days and gradually decreased on days 3 and 4. On days 4, the level of the glutamate had remained elevated above the normal level.

Abstract: A simplified model of intracranial dynamics is used to reproduce the intracranial pressure (ICP) time pattern in 20 patients with severe brain damage during PVI tests. A comparison of model responses and clinical tracings was achieved by minimizing a least square criterion function and adjusting just 5 parameters. These are: the CSF outflow resistance, the intracranial elastance coefficient, the autoregulation gain and time constant, and the basal values of arteriolar compliance. Based on the value of the autoregulation gain, the patients were classified into two groups: those with damaged autoregulation (8 out of 12) and those with preserved autoregulation (12 out of 20). Finally, analysis of the correlation between parameter estimates provided suggestions on the combination of a parameter changes which may have the greater impact on ICP in the individual cases. Once these parameters have been identified, they may become possible targets for therapeutic interventions.

Abstract: The International Severe Acute Respiratory and Emerging Infection Consortium (ISARIC) COVID-19 dataset is one of the largest international databases of prospectively collected clinical data on people hospitalized with COVID-19. This dataset was compiled during the COVID-19 pandemic by a network of hospitals that collect data using the ISARIC-World Health Organization Clinical Characterization Protocol and data tools. The database includes data from more than 705,000 patients, collected in more than 60 countries and 1,500 centres worldwide. Patient data are available from acute hospital admissions with COVID-19 and outpatient follow-ups. The data include signs and symptoms, pre-existing comorbidities, vital signs, chronic and acute treatments, complications, dates of hospitalization and discharge, mortality, viral strains, vaccination status, and other data.
