To address the clinical need for readily available small diameter vascular grafts, biomimetic tubular scaffolds were developed for rapid in situ blood vessel regeneration. The tubular scaffolds were designed to have an inner layer that is porous, interconnected, and with a nanofibrous architecture, which provided an excellent microenvironment for host cell invasion and proliferation. Through the synthesis of poly(spirolactic-co-lactic acid) (PSLA), a highly functional polymer with a norbornene substituting a methyl group in poly(l-lactic acid) (PLLA), we were able to covalently attach biomolecules onto the polymer backbone via thiol-ene click chemistry to impart desirable functionalities to the tubular scaffolds. Specifically, heparin was conjugated on the scaffolds in order to prevent thrombosis when implanted in situ. By controlling the amount of covalently attached heparin we were able to modulate the physical properties of the tubular scaffold, resulting in tunable wettability and degradation rate while retaining the porous and nanofibrous morphology. The scaffolds were successfully tested as rat abdominal aortic replacements. Patency and viability were confirmed through dynamic ultrasound and histological analysis of the regenerated tissue. The harvested tissue showed excellent vascular cellular infiltration, proliferation, and migration with laminar cellular arrangement. Furthermore, we achieved both complete reendothelialization of the vessel lumen and native-like media extracellular matrix. No signs of aneurysm or hyperplasia were observed after 3 months of vessel replacement. Taken together, we have developed an effective vascular graft able to generate small diameter blood vessels that can function in a rat model.

El objetivo de este trabajo de investigación fue realizar el control óptimo de la composición de dióxido de carbono en la columna de absorción empacada del Laboratorio de Operaciones y Procesos Unitarios de la Facultad de Ingeniería Química mediante un modelo de red neuronal artificial. Un modelo de red neuronal artificial con una capa de entrada, una capa escondida y una capa de salida fue desarrollado, entrenado con el algoritmo de retropropagación y validado experimentalmente para un proceso de absorción con reacción química. Asimismo, siguiendo el mismo procedimiento fue implementado el modelo del neurocontrolador (modelo inverso de la red neuronal artificial). El trabajo experimental fue realizado en una columna de absorción de vidrio pyrex de 1.15 metros de altura y 8.5 centímetros de diámetro interno, empacada con anillos Raschig de vidrio de 20 milímetros y opera con un rango en el flujo de gas de 12 a 16 L/min y un rango en el flujo de líquido de 0.8 a 2.8 L/min. La variable controlada fue la concentración de dióxido de carbono a la salida de la columna de absorción en un rango de 4 a 8 %V y la variable manipulada fue la velocidad de flujo de entrada de la solución de hidróxido de sodio 0.11M en un rango de 0.8 a 2.8 L/min. Para contrastar la hipótesis el neurocontrolador fue probado experimentalmente estableciéndose diferentes valores de referencia (set points) y el control óptimo de la columna de absorción demostró ser realizable suministrando una adecuada acción de control para alcanzar los valores de referencia deseados

El objetivo de la presente investigación es evaluar las propiedades psicométricas de la Escala de Autoeficacia Percibida Específica de Situaciones Académicas (EAPESA). La muestra estuvo conformada por 308 adolescentes (157 varones) de nivel secundaria de una Institución Educativa Estatal de Lima-Perú entre los 13 y 18 años (Medad = 15.22; DEedad = 1.126). Se realizó un análisis factorial confirmatorio con el programa Mplus y se plantearon dos hipótesis estructurales: el modelo original (M1) y una estructura unidimensional en que no se considera el ítem 9 (M2). Además, las puntuaciones se correlacionaron con una medida de afecto positivo y negativo. Los resultados indican M2 es el modelo más plausible en la muestra de adolescentes, la fiabilidad del constructo y de las puntuaciones está en el rango de lo aceptable, y que las asociaciones encontradas con el afecto positivo y negativo son teóricamente coherentes. Se concluye que el EAPESA es un instrumento que se puede utilizar para fines de investigación y descripción de grupos.

Objectives Early initiation of antiretroviral therapy (ART) during acute HIV infection is associated with favourable clinical and epidemiological outcomes. Barriers to prompt treatment initiation limit the benefits of universal access to ART in Mexico. We sought to create an algorithm for the immediate detection and treatment of patients with acute HIV infection. Methods A nationwide cohort of patients with acute HIV infection was created in 2015. In order to identify cases and treat them promptly at our centre, an interdisciplinary group coordinated through an instant-messaging tool using smart phones was established. When a probable case was detected, a discussion was initiated to confirm the diagnosis and facilitate the administrative processes to initiate ART as soon as possible. We compared time to ART initiation with that in a comparison group of patients with chronic HIV infection enrolled during the same period (May 2015 to February 2017) through routine care, using survival analysis estimators and log-rank tests. Results We recruited 29 patients with acute HIV infection. The median time to ART initiation was 2 days in these patients, in contrast to 21 days for patients with chronic infection. There were no significant differences in the percentages of patients engaged in care, on treatment or virologically suppressed at 1 year of follow-up. Conclusions Implementing immediate ART initiation programmes is feasible in Mexico, in spite of the substantial administrative barriers that exist in the country. More extensive replication of this model in other centres and in patients with chronic infection is warranted to evaluate its effect on the continuum of care.

Introduction Given the high prevalence of erectile dysfunction in male population between 40 and 70 years old and the effect of radical prostatectomy on this domain, it is important to perform a baseline study. Material and methods Prior radical prostatectomy, erectile function has been assessed prospectively in 112 prostate cancer patients using the erectile function (EF) domain of the International Index of Erectile Function (EF-IIEF), Erectile Hardness Score (EHS) and a penile doppler ultrasound (PDUS). Comorbidities and Charlson index were collected. The EORTC QLQ C-30 and PR-25 tests were administered. Results According to EF-IIEF questionnaire, 50.9% of patients showed normal EF and EHS grade 3–4 erection was achieved in the 75.9%. PDUS was normal only in 28.6% of patients and 51.8% showed arterial insufficiency. We found a significant association (p Conclusions In order to predict EF recovery after surgery, global assessment is required. Solely self-administered tests are not enough. In this baseline study, PDUS can play an important role.

Resumen Introduccion La disfuncion erectil peneana tiene una prevalencia alta entre los 40-70 anos, por lo que es importante su valoracion basal antes de la prostatectomia radical. Material y metodos Se ha evaluado la funcion erectil (FE) de 112 pacientes con cancer de prostata previamente a la prostatectomia radical, mediante el dominio de la FE del Indice Internacional de Funcion Erectil (IIEF), el test Erectile Hardness Score (EHS) y una ecografia doppler de pene (EDP). Se recogieron comorbilidades, el indice de Charlson y se administro el test de calidad de vida de la EORTC QLQ C -30 y PR-25. Resultados Un 50,9% de la poblacion tenian una FE normal usando el cuestionario IIEF y un 75,9% conseguian una ereccion grado 3-4 en el EHS. Unicamente el 28,6% presentaban una EDP normal, y el 51,8% mostraron insuficiencia arterial. Encontramos una asociacion significativa (p Conclusiones Es importante una valoracion global de la FE para poder valorar las expectativas de recuperacion de dicha funcion tras la cirugia y no reducirla unicamente a test autoadministrados, pudiendo jugar un papel importante la EDP.

Background The role of dietary patterns in the prevention of unipolar depression has been analyzed in several epidemiological studies. The primary aims of this study are to determine the effectiveness of an extra-olive oil-enriched Mediterranean diet in reducing the recurrence of depression and improving the symptoms of this condition. Methods Multicenter, two-arm, parallel-group clinical trial. Arm 1, extra-virgin olive oil Mediterranean diet; Arm 2, control group without nutritional intervention. Dieticians are in charge of the nutritional intervention and regular contact with the participants. Contacts are made through our web platform (https://predidep.es/participantes/) or by phone. Recurrence of depression is assessed by psychiatrists and clinical psychologists through clinical evaluations (semi-structured clinical interviews: Spanish SCID-I). Depressive symptoms are assessed with the Beck Depression Inventory. Information on quality of life, level of physical activity, dietary habits, and blood, urine and stool samples are collected after the subject has agreed to participate in the study and once a year. Discussion To the best of our knowledge, the PREDI-DEP trial is the first ongoing randomized clinical trial designed to assess the role of the Mediterranean diet in the prevention of recurrent depression. It could be a cost-effective approach to avoid recurrence and improve the quality of life of these patients. Trial registration The study has been prospectively registered in the U.S. National Library of Medicine (https://clinicaltrials.gov) with NCT number: NCT03081065. Mechanically tunable hydrogels are attractive platforms for 3D cell culture, as hydrogel stiffness plays an important role in cell behavior. Traditionally, hydrogel stiffness has been controlled through altering either the polymer concentration or the stoichiometry between crosslinker reactive groups. Here, an alternative strategy based upon tuning the hydrophilicity of an elastin-like protein (ELP) is presented. ELPs undergo a phase transition that leads to protein aggregation at increasing temperatures. It is hypothesized that increasing this transition temperature through bioconjugation with azide-containing molecules of increasing hydrophilicity will allow direct control of the resulting gel stiffness by making the crosslinking groups more accessible. These azide-modified ELPs are crosslinked into hydrogels with bicyclononyne-modified hyaluronic acid (HA-BCN) using bioorthogonal, click chemistry, resulting in hydrogels with tunable storage moduli (100-1000 Pa). Human mesenchymal stromal cells (hMSCs), human umbilical vein endothelial cells (HUVECs), and human neural progenitor cells (hNPCs) are all observed to alter their cell morphology when encapsulated within hydrogels of varying stiffness. Taken together, the use of protein hydrophilicity as a lever to tune hydrogel mechanical properties is demonstrated. These hydrogels have tunable moduli over a stiffness range relevant to soft tissues, support the viability of encapsulated cells, and modify cell spreading as a consequence of gel stiffness.

Hydatidosis is an uncommon zoonotic infection in the Philippines with only a few reported cases. It frequently presents as benign liver cysts with variable symptoms often related to mass effect.We present a 49-year-old male with a significant travel history from East and Central Asia, surgically treated twice as a benign liver cyst and now presenting with recurrent multiple intraabdominal cysts. He subsequently underwent surgical excision of multiple hydatid cysts with prolonged albendazole treatment. The patient presently remains disease-free as of most recent follow-up.The disease is caused by the tapeworm Echinococcus granulosus and transmission happens when humans acting as intermediate host ingest food contaminated with the parasite eggs, resulting to the liver being the most common location. Frequently, it manifests and is treated as incidental hepatic cysts for the disease is characterized to have a long asymptomatic period. Symptoms are variable and may range from having abdominal pain, increasing abdominal girth, vomiting, fever or myalgia. Ultrasound or CT scan reveals single or multiple cysts however, biopsy of the cyst wall with demonstration of the larval form (protoscoleces) is diagnostic.This case highlights the value of history-taking and having a high-index of suspicion particularly for rare cases.

Pulsars in the Galactic centre promise to enable unparalleled tests of gravity theories and black hole physics and to serve as probes of the stellar formation history and evolution and the interstellar medium in the complex central region of the Milky Way. The community has surveyed the innermost region of the galaxy for decades without detecting a population of pulsars, which is puzzling. A strong scattering of the pulsed signals in this particular direction has been argued to be a potential reason for the non-detections. Scattering has a strong inverse dependence on observing frequency, therefore an effective way to alleviate its effect is to use higher frequencies in a survey for pulsars in the Galactic centre, in particular, close to the supermassive black hole Sagittarius A*. We present the first pulsar survey at short millimetre wavelengths, using several frequency bands between 84 and 156 GHz (3.57-1.92 mm), targeted to the Galactic centre. The observations were made with the Institut de Radioastronomie Millimetrique (IRAM) 30m Telescope in 28 epochs between 2016 December and 2018 May. This survey is the first that is essentially unaffected by scattering and therefore unbiased in population coverage, including fast-spinning pulsars that might be out of reach of lower-frequency Galactic centre surveys. We discovered no new pulsars and relate this result mainly to the decreased flux density of pulsars at high frequencies, combined with our current sensitivity. However, we demonstrate that surveys at these extremely high radio frequencies are capable of discovering new pulsars, analyse their sensitivity limits with respect to a simulated Galactic centre pulsar population, and discuss the main challenges and possible improvements for similar surveys in the future., 14 pages, 6 figures, 4 tables. Introduction: White matter hyperintensities (WMH) are a radiographic marker for cerebral small vessel disease (CSVD). Conditions altering cerebral venous outflow such as elevated central venous pressure and right atrial pressure in individuals with cardiac valvular disease have been implicated in the development of WMH. Hypothesis: We hypothesize that increased right-heart chamber size in individuals without significant cardiac valvular disease is associated with worse WMH. Methods: A retrospective chart review of adults with a brain MRI and a 2-dimensional transthoracic echocardiogram (TTE) was performed. Worst burden of WMH by way of Fazekas score, either periventricular or deep white matter, served as the primary outcome. Statistical analysis was performed using a multivariate ordinal logistic regression model. Results: A total of 132 individuals were included. Right atrial area (OR 0.93, 95% CI 0.87 to 1.00, p = 0.0041), right ventricular internal diameter (OR 0.48, 95%CI 0.27 to 0.83, p = 0.008) and left atrial area (OR 0.93, 95%CI 0.88 to 0.98, p = 0.007) was identified as being significant. Cardiac functional markers were not significant, including tricuspid annular plane systolic excursion (OR 0.99, 95%CI 0.93 to 1.05, p = 0.670), right ventricular ejection fraction (OR 0.99, 95%CI 0.96 to 1.02, p = 0.670) and left ventricular ejection fraction (OR 0.99, 95%CI 0.96 to 1.02, p = 0.567). Analysis of isolated DWM or PVWM Fazekas scores did not find significant predictors in relation to cardiac structure or function. Conclusions: Through non-invasive cardiac imaging, we identified that cardiac structural abnormalities as opposed to functional abnormalities were associated with worse WMH. Mechanistically this may result from altered intracerebral arteriovenous coupling or a shared pathophysiologic pathway between WMH and coronary microvascular disease.

Objective Identify abnormal cardiac chamber size and hemodynamic parameters on transthoracic echocardiogram (TTE) as predictors of advancing cerebral small vessel disease (CSVD) on brain magnetic resonance imaging (MRI). Materials and methods A retrospective chart review of adults with a brain MRI and a 2-dimensional TTE was performed. WMH measured by the Fazekas score served as the primary outcome. We fit multivariate ordinal logistic regression models to the Fazekas score with the individual predictors of the TTE measurements and adjusted for potential confounders. Results 132 individuals were included. Cardiac functional markers were not significant, including tricuspid annular plane systolic excursion (p = 0.818), right ventricular ejection fraction (p = 0.818) and left ventricular ejection fraction (p = 0.673). Cardiac structural markers included right atrial area (p = 0.247), right ventricular internal diameter (RVID, p = 0.020) and left atrial area (LAA, p = 0.041). RVID and LAA were identified as being predictors, although the direction of the association suggested that normal values resulted in more WMH. Analysis of isolated DWM or PVWM Fazekas scores were not associated with cardiac structure or function. Conclusions In our study, we found that normal LAA and RVID values were associated with an increased degree of WMH on MRI. This finding may represent earlier identification of WMH prior to TTE cardiac changes. Future studies are needed for more robust quantitative comparison as well as evaluation prospectively of the association between cardiac chamber sizes and development of WMH.

Background Coronavirus disease 2019 (COVID-19) was declared a pandemic by the WHO on 11 March 2020 and global surgical practice was compromised. This Commission aimed to document and reflect on the changes seen in the surgical environment during the pandemic, by reviewing colleagues’ experiences and published evidence. Methods In late 2020, BJS contacted colleagues across the global surgical community and asked them to describe how severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) had affected their practice. In addition to this, the Commission undertook a literature review on the impact of COVID-19 on surgery and perioperative care. A thematic analysis was performed to identify the issues most frequently encountered by the correspondents, as well as the solutions and ideas suggested to address them. Results BJS received communications for this Commission from leading clinicians and academics across a variety of surgical specialties in every inhabited continent. The responses from all over the world provided insights into multiple facets of surgical practice from a governmental level to individual clinical practice and training. Conclusion The COVID-19 pandemic has uncovered a variety of problems in healthcare systems, including negative impacts on surgical practice. Global surgical multidisciplinary teams are working collaboratively to address research questions about the future of surgery in the post-COVID-19 era. The COVID-19 pandemic is severely damaging surgical training. The establishment of a multidisciplinary ethics committee should be encouraged at all surgical oncology centres. Innovative leadership and collaboration is vital in the post-COVID-19 era.

Introduction Pulmonary hypertension (PH) is defined as mean pulmonary arterial pressure (mPAP) ≥25 mmHg at rest, measured by right heart catheterization (RHC). Purpose To describe classical and myocardial deformation echocardiographic parameters in patients with established PH and to identify prognostic variables Methods We prospectively enrolled 76 patients with mPAP ≥25 mmHg undergoing RHC between 2017 and 2018. All subjects underwent transthoracic echocardiography (TTE) according to the latest ASE/EACVI guidelines the same day of the RHC. Strain analysis was carried out by speckle-tracking echocardiography (QLAB 10.7, Philips). Clinical events during the follow-up were: acute heart failure hospitalization, cardiac transplant and all-cause mortality. Results Mean age was 59±12, 43.4% were women and 49 patients (64.5%) belonged to group 2 of PH. The median follow-up was 288 (ICR 92–534) days. Total number of events was 42 (55.3%, 9 deaths). Variables associated to events are shown in Table 1. All classic LV and RV systolic function and strain parameters were associated with a worse prognosis, being free-wall RV longitudinal strain (RVLS) the only one that remained as a prognostic factor in mutivariate analysis. Other variables associated with a worse prognosis were PCP>15 mmHg and NT-proBNP>1800, the latter being independent predictor of events. The attached figure shows event-free survival curves for the global population divided according to whether or not they belong to group II PH. Conclusions Our data highlight the prognostic value of free-wall RVLS and NT-proBNP in patients with established PH. Background/Introduction Previous studies using conventional echocardiographic measurements have reported subclinical left diastolic dysfunction in patients with Marfan syndrome (MFS). Certain speckle-tracking echocardiography (STE) derived parameters have shown good correlation with invasive measurements of LV relaxation. Purpose To evaluate diastolic function in Marfan patients using STE-derived parameters along with traditional diastolic measurements and compare them with healthy controls. Methods We consecutively included 127 MFS patients with normal LV ejection fraction and no previous cardiovascular surgery or significant valvular regurgitation. We also studied 38 healthy controls (age and sex-matched). We performed LV and LA strain analysis using 2D STE (QLAB 10, Philips). Echocardiographic parameters recommended to assess diastolic function (according to ASE/EACVI 2016 guidelines) were also considered. Results Reported use of BB/ARB was 28/39% in MFS patients. According to current guidelines, all participants had normal diastolic function. MFS patients showed lower TDI e' velocities (septal and lateral) and higher average E/e' ratio compared to controls, but still within normal range. Regarding STE-derived measurements, LVGLS, EDLSR and PALS were significantly reduced in MFS. E/EDLSR ratio was higher among MFS patients, reflecting impaired LV relaxation. Interestingly, E/LVGLS was not different between both groups. Conclusion STE-derived parameters showed impaired LV relaxation in MFS patients compared to controls. Our results suggest that STE derived parameters can be markers of early diastolic dysfunction and provide a better insight into Marfan-related cardiomyopathy. Resumen La personalidad es una característica relevante para predecir la conducta académica del estudiante, y su asociación con el agotamiento emocional académico aún es un tópico reciente. El propósito de la presente investigación fue analizar la capacidad predictiva de los cinco grandes factores de personalidad sobre el agotamiento emocional académico en universitarios, en una muestra de 453 estudiantes (60.9% mujeres) entre 17 y 40 años de edad, quienes fueron evaluados con la Escala de Cansancio Emocional y el Big Five Inventory-15p. Fue implementado un análisis de regresión múltiple para agotamiento emocional académico con los rasgos de personalidad como predictores. Los resultados indican que los rasgos de personalidad explican significativamente el agotamiento emocional (R2 > .20), y el neuroticismo, extraversión y responsabilidad predicen el agotamiento emocional académico. En conclusión, la capacidad predictiva de los rasgos de personalidad sobre el agotamiento emocional académico es significativa. Se discuten las consecuencias prácticas y las limitaciones del estudio. Abstract Personality is a relevant feature to predict the student’s academic behavior, and its association with academic emotional exhaustion is still a recent topic. The aim was determine the predictive ability of personality on academic emotional exhaustion in 453 college students (60.9% women) between ages 17 and 40 years old, who were assessed with the Emotional Exhaustion Scale and the Big Five Inventory-15p. Was implemented a regression multiple analysis on emotional exhaustion, with the personality traits as predictors. The findings shows that the personality traits explain significantly the emotional exhaustion (R2> .20), and the neuroticism, extraversion and consciousness predict the emotional exhaustion. In conclusion, the predictive ability of personality on academic emotional exhaustion is significant. The practical consequences and limitations are discussed. Resumo A personalidade é uma característica relevante para prever o comportamento acadêmico do aluno, e sua associação com a exaustão emocional acadêmica ainda é um tópico recente. O objetivo da presente investigação foi analisar a capacidade preditiva dos cinco fatores de personalidade na exaustão emocional acadêmica de universitários em uma amostra de 453 estudantes (60,9% mulheres) entre 17 e 40 anos, avaliados com a Escala de Exaustão Emocional e o Big Five Inventory-15p. Uma análise de regressão múltipla para exaustão emocional acadêmica foi implementada, com traços de personalidade como preditores. Os resultados indicam que os traços de personalidade explicam significativamente a exaustão emocional (R2 > .20), e o neuroticismo, a extroversão e a responsabilidade predizem a exaustão emocional acadêmica. Concluindo, a capacidade preditiva dos traços de personalidade no esgotamento emocional acadêmico é significativa. As conseqüências práticas e limitações do estudo são discutidas.

The mental health care of medical students is a complex process influenced by various innate or personal characteristics of the student, stressors related to medical training, social, economic, demographic and many other factors. Therefore, it is important to identify both the prevalence and the main risk factors that can trigger mental disorders in medical students, such as stress, burnout, anxiety, depression, suicidal ideation, suicide, among others. Specifically in these students, the prevalence of depression ranges between 2.9% and 38.2% respectively; likewise, exhaustion ("burnout") prevails in 28-45% of them. Most students do not seek the necessary support to solve such problems, which leads to serious consequences, such as the abuse of substances harmful to health, in addition to environments, situations or actions that lead to mental destabilization. The following paper is a review that emphasizes the prevalence and risk factors that can trigger these mental disorders, in order to make an early detection and timely intervention, such as attending psychiatric therapy and/or receiving support from the educational system itself.

The sulfur–iodine thermochemical water-splitting cycle is a promising route proposed for hydrogen production. The decomposition temperature remains a challenge in the process. Catalysts, such as Pd supported on Al2O3, are being considered to decrease reaction temperatures. However, little is known regarding the kinetic behavior of such systems. In this work, zinc sulfate thermal decomposition was studied through non-isothermal thermogravimetric analysis to understand the effect of a catalyst within the sulfur–iodine reaction system context. The findings of this analysis were also related to a thermodynamic assessment. It was observed that the presence of Pd/Al2O3 modified the reaction mechanism, possibly with some intermediate reactions that were suppressed or remarkably accelerated. The proposed model suggests that zinc sulfate transformation occurred in two sequential stages without the Pd-based material. Activation energy values of 238 and 368 kJ·mol−1 were calculated. In the presence of Pd/Al2O3, an activation energy value of 204 kJ·mol−1 was calculated, which is lower than observed previously.

Flaxseed (Linum usitatissimum L.) and yerba mate (Ilex paraguariensis) are particularly interesting as functional ingredients due to their high content of essential omega 3-fatty acids and antioxidant properties, respectively. The objective of this work was to formulate a corn snack enriched with flaxseed flour and yerba mate extract and evaluate its physicochemical and sensory properties. Flaxseed flour, added at three levels 1, 3 and 5%, increased the water activity, moisture content and cohesiveness, and decreased the hardness and plastic deformation of the samples. The flour also changed the superficial colour and made the microstructure more compact. On the contrary, yerba mate extract (1 and 3%) decreased the water activity, moisture content and cohesiveness, counteracting the effect of flaxseed flour. Yerba mate increased the plastic deformation and the antioxidant activity of the snacks. The colour was not modified and the microstructure was more fragmented in the presence of yerba mate. Sensory evaluation suggested that flaxseed flour and yerba mate affected the flavour but did not modify the colour and crunchiness of the product.

A.; Wright, Melvin; Yamaguchi, Paul; Yu, Chen-Yu; Zeballos, Milagros; Zhang, Shuo; Ziurys, Lucy; Event Horizon Telescope Collaboration, 3C 279 is an archetypal blazar with a prominent radio jet that show broadband flux density variability across the entire electromagnetic spectrum. We use an ultra-high angular resolution technique - global Very Long Baseline Interferometry (VLBI) at 1.3mm (230 GHz) - to resolve the innermost jet of 3C 279 in order to study its fine-scale morphology close to the jet base where highly variable-ray emission is thought to originate, according to various models. The source was observed during four days in April 2017 with the Event Horizon Telescope at 230 GHz, including the phased Atacama Large Millimeter/submillimeter Array, at an angular resolution of ∼20 μas (at a redshift of z = 0:536 this corresponds to ∼0:13 pc ∼ 1700 Schwarzschild radii with a black hole mass MBH = 8 × 108 M⊙). Imaging and model-fitting techniques were applied to the data to parameterize the fine-scale source structure and its variation.We find a multicomponent inner jet morphology with the northernmost component elongated perpendicular to the direction of the jet, as imaged at longer wavelengths. The elongated nuclear structure is consistent on all four observing days and across diffierent imaging methods and model-fitting techniques, and therefore appears robust. Owing to its compactness and brightness, we associate the northern nuclear structure as the VLBI "core". This morphology can be interpreted as either a broad resolved jet base or a spatially bent jet.We also find significant day-to-day variations in the closure phases, which appear most pronounced on the triangles with the longest baselines. Our analysis shows that this variation is related to a systematic change of the source structure. Two inner jet components move non-radially at apparent speeds of ∼15 c and ∼20 c (∼1:3 and ∼1:7 μas day-1, respectively), which more strongly supports the scenario of traveling shocks or instabilities in a bent, possibly rotating jet. The observed apparent speeds are also coincident with the 3C 279 large-scale jet kinematics observed at longer (cm) wavelengths, suggesting no significant jet acceleration between the 1.3mm core and the outer jet. The intrinsic brightness temperature of the jet components are ≤1010 K, a magnitude or more lower than typical values seen at ≥7mm wavelengths. The low brightness temperature and morphological complexity suggest that the core region of 3C 279 becomes optically thin at short (mm) wavelengths. © J.-Y. Context. NIKA2 is a dual-band millimetre continuum camera of 2 900 kinetic inductance detectors, operating at 150 and 260 GHz, installed at the IRAM 30-m telescope in Spain. Open to the scientific community since October 2017, NIKA2 will provide key observations for the next decade to address a wide range of open questions in astrophysics and cosmology. Aims. Our aim is to present the calibration method and the performance assessment of NIKA2 after one year of observation. Methods. We used a large data set acquired between January 2017 and February 2018 including observations of primary and secondary calibrators and faint sources that span the whole range of observing elevations and atmospheric conditions encountered by the IRAM 30-m telescope. This allowed us to test the stability of the performance parameters against time evolution and observing conditions. We describe a standard calibration method, referred to as the "Baseline"method, to translate raw data into flux density measurements. This includes the determination of the detector positions in the sky, the selection of the detectors, the measurement of the beam pattern, the estimation of the atmospheric opacity, the calibration of absolute flux density scale, the flat fielding, and the photometry. We assessed the robustness of the performance results using the Baseline method against systematic effects by comparing results using alternative methods. Results. We report an instantaneous field of view of 6.5′ in diameter, filled with an average fraction of 84%, and 90% of valid detectors at 150 and 260 GHz, respectively. The beam pattern is characterised by a FWHM of 17.6″ ± 0.1″ and 11.1″ ± 0.2″, and a main-beam efficiency of 47%±3%, and 64%±3% at 150 and 260 GHz, respectively. The point-source rms calibration uncertainties are about 3% at 150 GHz and 6% at 260 GHz. This demonstrates the accuracy of the methods that we deployed to correct for atmospheric attenuation. The absolute calibration uncertainties are of 5%, and the systematic calibration uncertainties evaluated at the IRAM 30-m reference Winter observing conditions are below 1% in both channels. The noise equivalent flux density at 150 and 260 GHz are of 9 ± 1 mJy s1/2 and 30 ± 3 mJy s1/2. This state-of-the-art performance confers NIKA2 with mapping speeds of 1388 ± 174 and 111 ± 11 arcmin2 mJy-2 h-1 at 150 and 260 GHz. Conclusions. With these unique capabilities of fast dual-band mapping at high (better that 18″) angular resolution, NIKA2 is providing an unprecedented view of the millimetre Universe., With funding from the Spanish government through the "María de Maeztu Unit of Excellence" accreditation (MDM-2017-0737)

Resumen Antecedentes: Los estresores cotidianos son eventos de bajo impacto emocional pero con alta probabilidad de ocurrencia, y en la adolescencia pueden provenir de la escuela, amigos y la familia; por ello es necesario contar con instrumentos para su evaluación. Objetivo: Identificar las evidencias de validez de la estructura interna e invarianza de medición del Children’s Hassles Scale en adolescentes de Lima, Perú. Método: La muestra estuvo conformada por 524 adolescentes entre los 14 y 18 años. La estructura interna se evaluó mediante el análisis factorial confirmatorio y la invarianza de medición mediante un procedimiento en que se agregan restricciones progresivamente, finalmente, la confiabilidad se obtuvo con el coeficiente omega. Resultados: La estructura factorial original no se replica en la muestra peruana, por el contrario, una estructura parsimoniosa de dos factores oblicuos orientados a factores estresantes en el colegio y la familia es la que presenta indicadores robustos e invarianza de medición entre varones y mujeres. Conclusión: Se puede utilizar el instrumento para fines de investigación en el contexto peruano, el cual carece de instrumentos para la medición de estresores cotidianos en adolescentes. Abstract Background: Daily stressors are events of low emotional impact but with high probability of occurrence, which in adolescence can come from school, friends and family. Therefore, it is necessary to have instruments for their evaluation. Aim: To determine the validity of the internal structure and measurement invariance of the Children’s Hassles Scale in adolescents from Lima, Peru. Method: The sample consisted of 524 adolescents between 14 and 18 years old. The internal structure was evaluated by confirmatory factor analysis, and the measurement invariance was assessed using a procedure in which restrictions were added progressively. Finally, reliability was obtained through coefficient omega. Results: The original factor structure is not replicated in the Peruvian sample. On the contrary, a parsimonious structure of two oblique factors oriented to stressors in school and family presents robust indicators and measurement invariance among males and females. Conclusion: The instrument can be used for research purposes in the Peruvian setting, which lacks instruments for measuring daily stressors in adolescents.

La muestra fue de 255 adolescentes entre los 13 y 19 anos (Medad 15.494, DEedad=1.192) de una Institucion Educativa publica del distrito de Ventanilla (Peru). Los instrumentos utilizados fueron el CERQ18 y el Inventario de Depresion Estado/Rasgo. Se plantearon cuatro hipotesis estructurales que fueron evaluados mediante un analisis factorial confirmatorio, incluyendo analisis bifactor. Los resultados indican que el modelo evolucionista representadas por las estrategias automaticas (autoculpa, rumiacion, catastrofizacion) y elaborativas (aceptacion, focalizacion en los planes, reinterpretacion positiva, focalizacion positiva y poner en perspectiva), presentan indicadores satisfactorios, favoreciendo su interpretacion, y mostrando relaciones significativas con afecto positivo y negativo. Se concluye que la estructura interna del CERQ-18 en adolescentes es respaldada por el modelo evolucionista. EnglishThe present study aimed to identify the evidence of validity of the internal structure and in relation to other variables of the Cognitive Emotion Regulation Questionnaire (CERQ-18) in peruvian adolescents. The sample consisted of 255 adolescents between the ages of 13 and 19 (M=15.494, DE=1.192) from a public school of the district of Ventanilla (Peru). The instruments used were the CERQ-18 and the State / Trait Depression Inventory. Four structural hypotheses were proposed, which were evaluated by confirmatory factor analysis, including bifactor analysis. The results indicate that the evolutionary model represented by automatic (self-blame, rumination, catastrophization) and deliberate strategies (acceptance, refocus on planning, positive reappraisal, positive refocusing and putting into perspective), present satisfactory indicators, favoring their interpretation, and showing significant relationships with positive and negative affect. It is concluded that the internal structure of CERQ-18 in adolescents is supported by the evolutionary model.

There is increasing evidence of an association between long-term environmental exposure to pesticides and neurodegenerative disorders; however, the relationship with epilepsy has not been addressed thus far. This study was aimed at determining the prevalence and risk of developing epilepsy among people from South-East Spain living in areas of high vs. low exposure to pesticides based on agronomic data. The study population consisted of 4007 subjects with a diagnosis of epilepsy and 580,077 control subjects adjusted for age, sex and geographical area. Data were collected from hospital records of the Spanish health care system (basic minimum dataset) between the years 1998 and 2010. The prevalence of epilepsy was significantly higher in areas of greater pesticide use relative to areas of lesser use. Overall, an increased risk of epilepsy was observed in the population living in areas of high vs. low use of pesticides (OR: 1.65; p 0.001). Although this study was exploratory in nature, the results suggest that environmental exposure to pesticides might increase the risk of having epilepsy.

133. Tratamiento endoscópico en pacientes con estenosis subglótica

Objetivos: Determinar cuáles son los tipos de estenosis subglótica que pueden ser manejados por vía endoscópica. Diseño: Estudio observacional, descriptivo, tipo serie de casos. Métodos: La medición principal es el éxito/fracaso con el manejo endoscópico. Se tomaron 83 pacientes con estenosis subglótica, con manejo endoscópico; la información fue completa en 65 casos. La edad media fue de 22,3 años, 53 hombres y 12 mujeres; 40 tuvieron estenosis simples, 25 complejas. Resultados: No hubo asociación manifiesta entre el éxito y el procedimiento endoscópico, la edad y la localización de la estenosis. Hubo diferencias significativas entre la medición principal (éxito/fracaso) y las variables de porcentaje de estenosis (61,1% vs. 81,7%), longitud de la estenosis (1,1 cm vs. 2,2 cm), sitio comprometido (simples-éxito = 80%, complejas-éxito = 25%) y consistencia (blanda-éxito = 86%, dura-éxito = 44%). Conclusiones: Los procedimientos endoscópicos tienden a ser exitosos en estenosis simples, blandas, con porcentajes de estenosis ≤ 62% y longitud de la estenosis ≤ 1,1 cm.

Thousands of new asteroids are discovered every year and the rate of discovery is by far larger than the determination rate of their physical properties. In 2015 a group of researchers and students of several Mexican institutions have established an observational program to study asteroids photometrically. The program, named Mexican Asteroid Photometry Campaign, is aiming to derive rotation periods of asteroids based on optical photometric observations. Since then four campaigns have been carried out. The results obtained throughout these campaigns, as well as future work, are presented.

This paper explores the feasibility of using the Emotiv Cortex Application Programming Interface (API) service to obtain raw sensor signals from the Emotiv Insight NeuroHeadset for the control of a robotic arm. A data acquisition software based on Python was developed to use the service through data streaming in a Websocket. Raw EEG and motion sensor signals from the headset were processed for event detection. Detected events were then used to trigger control signals to a robotic arm controlled by a Raspberry Pi 3 using the IoT MQTT protocol. The results obtained indicate that the proposed approach and the algorithms developed were adequate translated into valid commands for effective development of a Head-Computer Interface that enables complete interaction with a robotic arm.

Resumen: El objetivo del Consenso Mexicano para el Tratamiento de la Hepatitis C fue el de desarrollar un documento como guía en la práctica clínica con aplicabilidad en México. Se tomó en cuenta la opinión de expertos en el tema con especialidad en: gastroenterología, infectología y hepatología. Se realizó una revisión de la bibliografía en MEDLINE, EMBASE y CENTRAL mediante palabras claves referentes al tratamiento de la hepatitis C. Posteriormente se evaluó la calidad de la evidencia mediante el sistema GRADE y se redactaron enunciados, los cuales fueron sometidos a voto mediante un sistema modificado Delphi, y posteriormente se realizó revisión y corrección de los enunciados por un panel de 34 votantes. Finalmente se clasificó el nivel de acuerdo para cada oración. Esta guía busca dar recomendaciones con énfasis en los nuevos antivirales de acción directa y de esta manera facilitar su uso en la práctica clínica. Cada caso debe ser individualizado según sus comorbilidades y el manejo de estos pacientes siempre debe ser multidisciplinario. Finally, the level of agreement was classified for each statement. The present guidelines provide recommendations with an emphasis on the new direct-acting antivirals, to facilitate their use in clinical practice. Each case must be individualized according to the comorbidities involved and patient management must always be multidisciplinary. Finally, the level of agreement was classified for each statement. The present guidelines provide recommendations with an emphasis on the new direct-acting antivirals, to facilitate their use in clinical practice. Each case must be individualized according to the comorbidities involved and patient management must always be multidisciplinary.

Background Evolution of left and right ventricular (LV and RV) function after heart transplantation (HT) has not been well described. Our objective was to evaluate the normal evolution of echocardiographic parameters of both ventricles and to explore if there is a link between the decrease of strain values and acute rejection (AR) or coronary allograft vasculopathy (CAV) Methods We followed 29 HT recipients with serial echocardiograms performed between 2011 and 2018, with a median follow-up of 5 years. LV global longitudinal strain (LV GLS) was analyzed by speckle tracking in 12 LV segments in 4 and 2 chamber views, and RV free wall longitudinal strain (RV free Wall LS) was measured in 4 chamber view. Acute rejection was diagnosed by EMB following our HT protocol. We take into consideration only moderate or severe rejection episodes (grade ≥2R).The presence of CAV was studied by coronariography or IVUS one year post-HT. Results As shown in the table below, LVEF was preserved from the begining of the follow up while LV GLS reached the normality in the 6th month, and both remained in normal ranges untill the 5th year. Regarding RV function, TAPSE was impaired in the early post-HT period and increased progressively and reached normality 1 year after HT. RV lateral wall LS rose during follow-up as well, reaching normal values 6 months after HT. Nevertheless, we noticed an impairment in this parameter at 5 years (−20.1±2.7, p=0.001), although it remained within normal ranges compared to guidelines reference parameters. We did not find any correlation between any parameter evaluated and the presence of AR or CAV at five years of follow-up. Background Elastic properties of the thoracic aorta in patients with Marfan Syndrome (MS) have already been evaluated with classic echocardiographic parameters. In the latest years the use of Speckle-Tracking (STE) ecocardiography has been widely extended. Our aim is to describe and provide new parameters of aortic deformation measured by STE in patients with MS. Methods 95 unoperated adult patients with MS and 32 healthy controls were prospectively enrolled. We measured classic parameters of the aortic root using 2D echocardiography. We calculated the posterior aortic wall systolic excursion at the sinuses of Valsalva and ascending aorta using M Mode in TDI colour; with ST 2D ecocardiography we measured the aortic strain at the sinuses of Valsalva (SV) and the anterior and posterior aortic wall displacement at the SV. Aortic distensibility was calculated using the formula: 1000 * (Ds − Dd)/Dd * 1/(Ps − Pd) in mmHg–1 (Ds: systolic and Dd: diastolic diameters, Ps systolic and Pd diastolic blood pressure). Aortic stiffness index was calculated as Ln((Ps/Pd)/(Ds-Dd)/Dd)). Results As shown in the table bellow, patients with MS had lower aortic strain, aortic anterior and posterior wall displacement and impaired aortic distensibility and stiffness index compared to healthy controls. We found a strong negative linear correlation between aortic root diameter at the SV and aortic root strain (r=−0.56, figure 1). Dispersion plot Conclusions Our results suggest that aortic deformation and displacement obtained by STE echocardiography is impaired in MS, showing a reduced distensibility and an increased stiffness of the aortic wall, with a strong negative correlation between aortic root dilation and aortic strain. All these parameters may be useful as additional tools for the diagnosis and follow-up of Marfan patients, and could be useful to to improve the echocardiographic evaluation of the aortic root.

Background Left Ventricular systolic disfunction has already been described in Marfan Syndrome (MS) in patients without valvular dysfunction using 2D and 3D speckle tracking echocardiography (STE). This dysfunction has been related to a more severe causal genetic mutation, which suggest the presence of a primary cardiomiopathy in these patients. Right ventricular function has been less studied so far. We sought to evaluate biventricular function in our cohort of MS patients with 2D-STE. Methods 95 unoperated adult patients with MS and 32 healthy controls were prospectively enrolled. Patients with more than mild mitral or aortic regurgitation were excluded. Using STE we obtanied left ventricular global longitudinal strain (LVGLS) from the average of 16 segments from 4,2 and 3-chamber views and RVGLS values were obtained from the average of 6 segments from the apical 4-chamber view. We also measured classic parameters of systolic biventricular function (LVEF and TAPSE). Results Compared to controls, patients with MFS had significantly lower LVGLS and RVGLS (table 1). Values obtanied for LVGLS in MS patients were at the lower limit of normality stablished in the latest cuantification guidelines, while RVGLS and RV free wall LS were slightly above the limit of normality. LVEF and TAPSE were also slightly diminished in MS patients, though the differences found were clinically not relevant. Background/Introduction Previous studies using conventional echocardiographic measurements have reported subclinical left diastolic dysfunction in patients with Marfan syndrome (MFS). Left atrial strain (LAS) has been shown to be an accurate predictor of left ventricular diastolic dysfunction. However, there is no evidence regarding the use of LAS in MFS. Purpose To assess feasibility of LAS and compare LAS derived measurements along with traditional diastolic parameters in MFS patients vs healthy controls. Methods 46 MFS patients (normal LV ejection fraction, no previous cardiovascular surgery, no significant valvular regurgitation) vs. 20 healthy controls (age and sex-matched). We performed LAS analysis using 2D speckle-tracking (QLAB 10, Philips). LA strain was determined as the average value of the longitudinal deformation (7 segments) in the apical 4-chamber view (RR gating). Results LAS analysis was feasible in 40 MFS patients (87%). All participants had normal diastolic function according to current guidelines (ASE/EACVI 2016). MFS patients showed lower TDI e' velocities and higher average E/e' ratio, but still within normal range. Similarly, LVEF was normal but slightly reduced in MFS patients. Bioethanol is being promoted in Mexico to be used in a blend with gasoline. On the north of the Yucatan peninsula, bioethanol could be produced from sweet sorghum, as it can grow efficiently on this land; it can be harvested 2–3 times in a year and possesses a better agronomical stability than sugarcane with low nitrogen requirements and high productivity. In this work , the potential environmental impacts and energy efficiency of bioethanol production from sweet sorghum were evaluated using life-cycle assessment. Four scenarios were evaluated: scenario PI considered only bioethanol production from the stalk juice; scenarios PII and PIII added cogeneration from the dry-stalk biomass in single and combined cycle, respectively. Scenario PIV considered bioethanol production from both stalk juice and dry-stalk biomass. Scenario PI demanded more fossil energy than what was generated as bioethanol, while scenarios PII and PIII were fossil energy independent. Scenario PIII showed the higher net energy ratio (1.89) and a better environmental performance in all CML-IA baseline impact categories. In terms of global warming potential, the scenario PIII showed a mitigation potential of 16% with respect to the fossil reference. In the categories where the sweet sorghum scenarios represented larger emissions than the fossil reference, it was due mainly to the use of fertilizers and the conventional energy consumption in the various processing steps of the biomass. Scenario PIV showed the highest energy demand and worst environmental performance due to large demands of energy and chemicals in the bagasse pretreatment step.

Resumen Introduccion Los microRNA (miR) son RNA de pequeno tamano involucrados en la regulacion de numerosos procesos biologicos. Debido a su estabilidad y a su capacidad para detectarse en fluidos, se han convertido en objeto de estudio como posibles biomarcadores de diferentes patologias. Objetivo Seleccionar un numero reducido de miR candidatos a ser biomarcadores de aneurisma de aorta abdominal (AAA). Material y metodos Se determino la expresion de 179 miR en el plasma de 7 pacientes y 7 voluntarios sanos, mediante paneles predisenados, basados en la tecnologia de RT-qPCR. Resultados Hemos seleccionado 10 miR disregulados en pacientes como buenos candidatos para su estudio como biomarcadores debido a su implicacion en diversos procesos biologicos relacionados con el desarrollo y evolucion de AAA. Conclusiones Aunque son necesarios estudios adicionales y de validacion, algunos de estos miR podrian ser biomarcadores no invasivos de AAA.

A national energy strategy in Mexico to 2028, derived from the recent energy reform, projects a considerable shift in the national power generation mix leading to changes on its environmental profile. Therefore, this paper aims to compare the potential environmental impacts of the electricity generation under previous, current and projected energy mixes in Mexico at a national and regional scale considering the Yucatan state as the regional case study. These were estimated with a cradle-to-grave Life Cycle Assessment (LCA) following the CML–IA 2000 V2.05 methodology and including the baseline environmental impact categories. Results indicate that the environmental impacts per generated kWh of electricity have decreased in Mexico from 2007 to 2013 and would keep decreasing at a faster rate until 2028 according to current projections. In Yucatan, the environmental impacts increased from 2007 to 2013 due to an increased diesel consumption, but would decrease by 2028 reaching lower levels than in 2007. From 2007 to 2013 almost all the environmental impacts in Yucatan were lower than the country average due to a higher contribution of natural gas in the power generation mix, while in 2028 they would be similar. Overall, there is a significant difference between the national and regional estimations of the potential environmental impacts due to differences in their power generation mix. Furthermore, the potential introduction of shifts derived from the energy reform in Mexico will have a strong positive influence on the environmental impacts from the electricity generation.

Depression in adolescents is a multicausal phenomenon and a predictor of social problems and other disorders. Thus, it is considered a public health problem. This investigation aims to systematically review the characteristics of the scientific articles about depression in Peruvian adolescents published in Peruvian and non-Peruvian journals. The search was performed in the databases of SciELO, Latindex, Redalyc, Scopus, PubMed, and through Google, using a combination of keywords related to the construct, the sample and its origin. The unit of analysis consisted of 21 articles. The reviewed articles showed inconsistency in the reporting and bibliographic support of the research designs, sampling procedures, psychometric properties of the instruments, instruments used to assess the construct which were not evaluated in our context, and cut-off points obtained in other countries’ samples. Therefore, we can state that the reviewed articles do not fully comply with the publication standards proposed by the American Psychological Association. Por ello, es considerada un problema de salud pública. El objetivo de la presente investigación es realizar una revisión sistemática de las características de los artículos científicos publicados en revistas peruanas y no peruanas sobre depresión en adolescentes peruanos. La búsqueda se realizó en las bases de datos Scielo, Latindex, Redalyc, Scopus, PubMed y a través de Google, utilizando combinación de palabras clave relacionadas al constructo, la muestra y su procedencia. La unidad de análisis estuvo conformada por 21 artículos. En los estudios destaca la inconsistencia en el reporte y sustento bibliográfico del diseño de investigación, del procedimiento de muestreo y de las propiedades psicométricas de los instrumentos; también, de la utilización de instrumentos que no han sido evaluados en nuestro contexto y de puntos de corte obtenidos en muestras de otros países. Debido a ello, se puede afirmar que los estudios no cumplen en su totalidad con los estándares de publicación propuestos por la American Psychological Association.
