Omental infarction is caused by vascular obstruction with resulting tissue ischemia, representing a rare cause of abdominal pain. It has been described as a rare complication of gastric bypass. It is important to recognize omental infarction and its possible complications as The management is usually conservative with surgery deferred to specific cases. We present the case of a 56-year-old male with a history of gastric adenocarcinoma who underwent esophagogastrectomy with Roux-n-y reconstruction and 3 months later presented with severe persistent abdominal pain, due to a path proven giant omental infarction. Patient later was complicated with a colonic fistula to the omentum.

A modular electromagnetic railgun accelerator facility named “RAFTAR” (i.e., Railgun Accelerator Facility for Technology and Research) has been commissioned and its performance has been characterized for high velocity impact testing on materials in a single-shot mode. In the first tests, RAFTAR demonstrated an acceleration of more than 1000 m/s for an 8 g solid aluminum-7075 armature projectile. The current fed was 220 kA, having a muzzle time of about 1.75 ms. It is a single pulse breech-fed rectangular bore (14 × 13 mm2) railgun, and its 1.15 m long barrel assembly consists of two parallel copper bars with an inter-gap of 13 mm that are encased within 50 mm thick high strength reinforced fiberglass sheets (Garolite G10-FR4) and bolted from both the sides. RAFTAR is powered by two capacitor bank modules that have a maximum stored energy of 160 kJ each (containing eight 178 μF/15 kV capacitors), two high power ignitron switches, and a pulse shaping inductor. To obtain consistent acceleration of the armature inside the barrel, reversal of driving current is prevented, and its pulse duration is stretched by tactical integration of the crowbar switch and bitter coil inductor in the circuit. Armature projectile velocity measurement in-bore and outside in free space was performed by the time-of-flight technique using indigenously made miniature B-dot sensors and a novel shorting-foil arrangement, respectively. The time resolved measurement of the in-bore armature evidenced a velocity-skin-effect in the high acceleration phase. There is good agreement between the experimentally measured and theoretically predicted efficiency, confirming the optimal choice of operating parameters. The conclusion summarizes important experimental findings and analyzes the underlying causes that limit the performance of railguns.

Data security is critical in wireless sensor networks (WSNs) because communication signals are highly available due to data transmission in free space. Attacks ranging from passive eavesdropping to active snooping are more common on these networks. This paper proposes secure data transfer using data encryption based on the improved Rivest–Shamir–Adleman (RSA) with Diffie–Hellman (DH) key exchange algorithm (IRSA-DH). For this purpose, the adaptive distance-based agglomerative hierarchical (ADAH)-based clustering method is used. Then the cluster head (CH) is selected using the improved weight-based rain optimization (IWRO) to improve the network’s lifespan. This study aims to design a secure group communication method for WSNs. In order to generate and distribute the key to the group, the RSA and DH and key exchange algorithm had been hybridized with the Key Management Center (KMC). For safe communication between users, the key exchange technique is investigated. The performance measures such as throughput, packet loss ratio (PLR), packet delivery ratio (PDR), latency, energy consumption, end-to-end delay (EED) and network lifetime are analyzed and compared with the existing approaches.

Coronavirus disease 2019 (COVID-19) has claimed the lives of millions of people worldwide since it first emerged. The impact of the COVID-19 pandemic on public health and the global economy has highlighted the medical need for the development of broadly acting interventions against emerging viral threats. Galidesivir is a broad-spectrum antiviral compound with demonstrated in vitro and in vivo efficacy against several RNA viruses of public health concern, including those causing yellow fever, Ebola, Marburg, and Rift Valley fever. In vitro studies have shown that the antiviral activity of galidesivir also extends to coronaviruses. Herein, we describe the efficacy of galidesivir in the Syrian golden hamster model of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. Treatment with galidesivir reduced lung pathology in infected animals compared with untreated controls when treatment was initiated 24 h prior to infection. These results add to the evidence of the applicability of galidesivir as a potential medical intervention for a range of acute viral illnesses, including coronaviruses.

We show that a unified framework based on an $SU(2)_H$ horizontal symmetry which generates a naturally large neutrino transition magnetic moment and explains the XENON1T electron recoil excess also predicts a positive shift in the muon anomalous magnetic moment. This shift is of the right magnitude to be consistent with the Brookhaven measurement as well as the recent Fermilab measurement of the muon $g-2$. A relatively light neutral scalar from a Higgs doublet with mass near 100 GeV contributes to muon $g-2$, while its charged partner induces the neutrino magnetic moment. In contrast to other multi-scalar theories, in the model presented here there is no freedom to control the sign and strength of the muon $g-2$ contribution. As a part of our ongoing discovery efforts exploring azasugar as agents for treating various unmet medical needs, we prepared analogs of azasugar as potential anti-hepatitis C virus (HCV) agents. Herein we describe the synthesis of novel 2'β-C-Me 9-deazanucleoside azasugar analogs.

Background The long-term use of calcium hydroxide, or Ca(OH)2, on dentin has been established in the literature. However, scarce data is available on dentin wettability with Ca(OH)2. Aim The present study was conducted to assess the outcomes of Ca(OH)2 use on the wettability of dentin following two and four weeks of using sealers of bioceramic (BC) and Tubli-Seal™ in the root canal. Methods In this study, 168 specimens were divided into 12 groups of 14 specimens each and were numbered from G1 to G12. G1-G6 groups were tested with Tubli-Seal. Sterile water irrigation was reserved for G1 for two weeks (14 days) followed by two minutes treatment with 6% sodium hypochlorite (NaOCl) chemical irrigation and 17% EDTA (10 ml). G2 and G3 were subjected to Ca(OH)2 (0.1 ml of UltraCal) for two weeks followed by 10ml sterile water irrigation for two minutes with chemical irrigation. G4 to G6 were given similar treatment for four weeks. G7-G12 were assessed for BC sealer similarly to G1-G6. Two and four weeks of incubation with sterile water or Ca(OH)2 was done in 100% humidity at 37°C. SEM and EDX were done to evaluate the surface morphology of G1 and G6, and results were formed. Results Significantly smaller contact angles were seen for Tubli-Seal (G1 to G6) compared to BC (G7 to G12) with p0.05. Application of water irrigants and Ca(OH)2 (in G2, G5, and G11) showed a smaller (p0.05, statistically significant) contact angle compared to the use of chemical irrigation agents and Ca(OH)2 (in G3, G6, and G12) except for G8 and G9. Based on EDX and SEM, compared to the use of chemical irrigation with Ca(OH)2, higher remnants of Ca(OH)2 were seen in the water irrigation, whereas, no remnant was seen with chemical irrigants. Conclusion Better dentin wettability is seen with Tubli-seal compared to BC sealer. A smaller contact angle between root canal sealers and dentin is seen in the remaining calcium hydroxide samples. Also, calcium hydroxide can be removed from the polished dentin surface with two minutes of irrigation with 17% EDTA and 6% NaOCl.

The principal reason behind the genesis of Roll On – Roll Off vessel design was to increase the pace of loading/unloading operations by eliminating the requirement of cargo-handling equipment thus enabling the wheeled cargo to be driven to its stowage location. The practice being followed today leads to a higher dependency of port operations, associated to any vessel, on ballast water to counter the trim/list generated due to unplanned and imbalanced stowage of cargo. Though advancements in field of ship automation have led to a development of ballast water management system, still, optimization of port operations, namely loading/unloading, is missing due to operator’s consideration of usage of ballast as the only natural resort. The earlier research focus at speeding up the ballasting process, whereas the need of the hour is to counter the generation of ballast, because time is a pre-requisite of all ballast operation which cannot be overlooked. The study involves the generation of an Eco-Friendly Decision Support System which counters the moment of similar cargo against each other in a three dimensionally modeled ship simulation, thus, reducing requirement of ballast. The result of the study indicates that vessel’s intact statical stability could be maintained without performing ballasting operations, thus enabling cargo stowage at port to save time, as well as the surrounding marine ecosystem. The flexible design of Decision Support System makes its application possible for various other vessel-types, while at the same time opening a plethora of opportunities to inculcate Artificial intelligence, Big-Data analysis and whereas other advancements, thus, shortening the gap between automation and optimization.

BackgroundMaternal mental disorders are considered a leading complication of childbirth and a common contributor to maternal death. In addition to undermining maternal welfare, untreated postpartum psychopathology can result in child emotional and physical neglect, and associated significant pediatric health costs. Some women may experience a traumatic childbirth and develop posttraumatic stress disorder (PTSD) symptoms following delivery (CB-PTSD). Although women are routinely screened for postpartum depression in the U.S., there is no recommended protocol to inform the identification of women who are likely to experience CB-PTSD. Advancements in computational methods of free text has shown promise in informing diagnosis of psychiatric conditions. Although the language in narratives of stressful events has been associated with post-trauma outcomes, whether the narratives of childbirth processed via machine learning can be useful for CB-PTSD screening is unknown.ObjectiveThis study examined the utility of written narrative accounts of personal childbirth experience for the identification of women with provisional CB-PTSD. To this end, we developed a model based on natural language processing (NLP) and machine learning (ML) algorithms to identify CB-PTSD via classification of birth narratives.Study DesignA total of 1,127 eligible postpartum women who enrolled in a study survey during the COVID-19 era provided short written childbirth narrative accounts in which they were instructed to focus on the most distressing aspects of their childbirth experience. They also completed a PTSD symptom screen to determine provisional CB-PTSD. After exclusion criteria were applied, data from 995 participants was analyzed. An ML-based Sentence-Transformer NLP model was used to represent narratives as vectors that served as inputs for a neural network ML model developed in this study to identify participants with provisional CB-PTSD.ResultsThe ML model derived from NLP of childbirth narratives achieved good performance: AUC 0.75, F1-score 0.76, sensitivity 0.8, and specificity 0.70. Moreover, women with provisional CB-PTSD generated longer narratives (t-test results: t=2.30, p=0.02) and used more negative emotional expressions (Wilcoxon test: ‘sadness’: p=8.90e-04, W=31,017; ‘anger’: p=1.32e-02, W=35,005.50) and death-related words (Wilcoxon test: p=3.48e-05, W=34,538) in describing their childbirth experience than those with no CB-PTSD.ConclusionsThis study provides proof of concept that personal childbirth narrative accounts generated in the early postpartum period and analyzed via advanced computational methods can detect with relatively high accuracy women who are likely to endorse CB-PTSD and those at low risk. This suggests that birth narratives could be promising for informing low-cost, non-invasive tools for maternal mental health screening, and more research that utilizes ML to predict early signs of maternal psychiatric morbidity is warranted.

Tm3+ doped oxyfluoride glass-ceramics (GCs) containing NaLaF4 nanocrystals (NCs) have been obtained by spark plasma sintering (SPS). First, the precursor glasses were melted and then milled and sieved to a suitable particle size. Glass powder pellets were sintered by spark plasma sintering under vacuum conditions. The SPS processing parameters (temperature, pressure, and holding time) were optimized to obtain transparent glass-ceramics. The times of SPS processing are considerably shorter compared with those for the preparation of these GCs by conventional thermal treatment. All glass-ceramics contain nanocrystals of the β- NaLaF4 phase with an average crystal size of 20 nm, but the more highly doped samples (2Tm3+ and 0.5Tm3+/2Yb3+) show evidence of the presence of another phase corresponding with α-NaLaF4. The luminescence properties of the near infrared (NIR) emissions of Tm3+ for different concentrations reveal the presence of concentration quenching of the 3H4 and 3F4 levels. The analysis of the decay from the 3H4 level with increasing concentration is consistent with a dipole-dipole quenching process assisted by energy migration, whereas the self-quenching of the 3F4→3H6 emission can be attributed to fast diffusion. Energy transfer between Yb3+ and Tm3+ ions is confirmed by the NIR and upconverted (UC) emissions after Yb3+ excitation at 975 nm. No UC emission is observed under 791 nm excitation of Tm3+ ions. Previously, we demonstrated that supplementation of resuscitation fluids with the Kv7 voltage-activated potassium channel inhibitor linopirdine reduces fluid resuscitation requirements and stabilizes hemodynamics in various rat models of hemorrhagic shock. To further evaluate the therapeutic potential of linopirdine, we tested the effects of linopirdine-supplemented resuscitation fluids in a rat model of ischemia-reperfusion injury-induced acute respiratory distress syndrome (ARDS). Ventilated rats underwent unilateral lung ischemia from t=0–75 min, followed by lung reperfusion and fluid resuscitation to a mean arterial blood pressure of 60 mmHg with normal saline (NS, n=9) or NS supplemented with 50 μg/ml linopridine (NS-L), n=7) until t=360 min. As compared with NS, fluid resuscitation with NS-L stabilized blood pressure and reduced fluid requirements by 40 % (p360 min with NS-L. Hereditary angioedema (HAE) is a rare and potentially life-threatening disease that affects an estimated 1 in 50 000 individuals worldwide. Until recently, prophylactic HAE treatment options were limited to injectables, a burdensome administration route that has driven the need for an oral treatment. A substantial body of evidence has shown that potent and selective plasma kallikrein inhibitors that block the generation of bradykinin represent a promising approach for the treatment of HAE. Berotralstat (BCX7353, discovered by BioCryst Pharmaceuticals using a structure-guided drug design strategy) is a synthetic plasma kallikrein inhibitor that is potent and highly selective over other structurally related serine proteases. This once-daily, small-molecule drug is the first orally bioavailable prophylactic treatment for HAE attacks, having successfully completed a Phase III clinical trial (meeting its primary end point) and recently receiving the U.S. Food and Drug Administration's approval for the prophylactic treatment of HAE attacks in patients 12 years and older.

Background: The coronavirus pandemic is one of the deadliest pandemic in history. The authorities worldwide responded to this pandemic by implementing business closures, travel restrictions, lockdowns and quarantines. Aims and Objectives: To determine the type of cases and compare the patterns of deaths which came for autopsy to Department of Forensic Medicine, Government Medical College, Kottayam during the Pre lockdown period in 2019 and COVID-19 Lockdown Period in 2020. Materials and Methods: This retrospective study was conducted in the Department of Forensic Medicine, Govt. Medical College, Kottayam. The study data was collected from postmortem reports of Department of Forensic Medicine, Govt. Medical College, Kottayam. Pre-lockdown period was taken as from 21 March to 31 May 2019.There were total 424 cases in 2019,whereas in the Lockdown period taken as from 21 March 2020 to 31 May 2020,no: of cases reduced to 270. All these cases were grouped according to cause of death, manner of death and modalities of natural and unnatural deaths. Details of cases were entered utilizing a proforma. Results: There were total 424 cases in the pre-lockdown period in the year 2019, whereas in the lockdown period in 2020, number of cases declined to 270. Among the manner of deaths, accidents accounted for most of the deaths in the year 2019, i.e. 44.6% deaths due to accidents, whereas in the year 2020, deaths due to natural diseases topped the list, 46.3%. These differences in the manner of deaths were found statistically significant. Conclusion: Two months into lockdown, statistically significant differences were noted in patterns of deaths. Further study should be conducted in the near future, when more data will become available. Providing early warning in patterns of mortality within the community could prove to be an invaluable public health asset. Disseminated extramedullary plasmacytoma (EMP) is an unusual entity that has nonspecific imaging features at MRI. Nevertheless, knowledge of its imaging features and unusual locations might help radiologists to suspect it in the appropriate clinical scenario. Some noncontiguous EMPs might mimic lymphoma and MRI would be useful as a problem-solving tool in EMP and would help in treatment planning. We report an unusual cardiac mass in cardiac MRI, a detailed work-up of which led to the diagnosis of multifocal EMP involving the heart, ribs, pancreas, retroperitoneum, and soft tissues of the thigh, rarely reported in the literature.

Regional climatic implications of aerosol black carbon (BC), which has a wide variety of anthropogenic sources in large abundance, are well recognized over South Asia. Significant uncertainties remain in its quantification due to a lack of sufficient information on the microphysical properties (its concentration, size, and mixing state with other aerosol components) that determine the absorption potential of BC. In particular, the information on the mixing state of BC is extremely sparse over this region. In this study, the first observations of the size distribution and mixing state of individual refractory black carbon (rBC) particles in the South Asian outflow to the south-eastern Arabian Sea and the northern and equatorial Indian Ocean regions are presented based on measurements using a single particle soot photometer (SP2) aboard the Integrated Campaign for Aerosols, gases, and Radiation Budget (ICARB-2018) ship during winter 2018 (16 January to 13 February). The results revealed significant spatial heterogeneity of BC characteristics. The highest rBC mass concentrations (∼938±293 ng m−3) with the highest relative coating thickness (RCT; the ratio of BC core to its coating diameters) of ∼2.16±0.19 are found over the south-east Arabian Sea (SEAS) region, which is in the proximity of the continental outflow. As we move to farther oceanic regions, though the mass concentrations decreased by nearly half (∼546±80 ng m−3), BC still remained thickly coated (RCT∼2.05±0.07). The air over the remote equatorial Indian Ocean, which received considerable marine air masses compared to the other regions, showed the lowest rBC mass concentrations (∼206±114 ng m−3) with a moderately thick coating (RCT∼1.73±0.16). Even over oceanic regions far from the landmass, regions that received the outflow from the more industrialized east coast/the Bay of Bengal had a thicker coating (∼104 nm) compared to regions that received outflow from the west coast and/or peninsular India (∼86 nm). Although different regions of the ocean depicted contrasting concentrations and mixing state parameters due to the varied extent and nature of the continental outflow as well as the atmospheric lifetime of air masses, the modal parameters of rBC mass–size distributions (mean mass median diameters ∼ 0.19–0.20 µm) were similar over all regions. The mean fraction of BC-containing particles (FBC) varied in the range of 0.08–0.12 (suggesting significant amounts of non-BC particles), whereas the bulk mixing ratio of coating mass to rBC mass was highest (8.31±2.40) over the outflow regions compared to the remote ocean (4.24±1.45), highlighting the role of outflow in providing condensable material for coatings on rBC. These parameters, along with the information on the size-resolved mixing state of BC cores, throw light on the role of sources and secondary processing of their complex mixtures for coatings on BC under highly polluted conditions. Examination of the non-refractory sub-micrometre aerosol chemical composition obtained using the aerosol chemical speciation monitor (ACSM) suggested that the overall aerosol system was sulfate-dominated over the far-oceanic regions. In contrast, organics were equally prominent adjacent to the coastal landmass. An association between the BC mixing state and aerosol chemical composition suggested that sulfate was the probable dominant coating material on rBC cores.

Access to randomized clinical trial (RCT) protocols is necessary for the interpretation and reproducibility of the study results, but protocol availability has been lacking. We determined the prevalence of protocol availability for all published cancer RCTs in January 2020. We found that only 36.1% (48/133) of RCTs had an accessible protocol and only 11.3% of RCTs (15/133) had a publicly accessible protocol that was not behind a paywall. Only 18.0% (24/133) of RCTs were published in conjunction with the protocol on the journal website. In conclusion, few cancer RCTs have an accessible research protocol. Journals should require publication of RCT protocols along with manuscripts to improve research transparency.
