107. Informing the standardising of care for prolonged stay patients in the ICU: A scoping review of quality improvement tools

ObjectivesTo inform design of quality improvement tools specific to patients with prolonged intensive care unit stay, we determined characteristics (format/content), development, implementation and outcomes of published multi-component quality improvement tools used in the intenisve care unit irrespective of length of stay.Research MethodologyScoping review searching electronic databases, trial registries and grey literature (January 2000 to January 2022).ResultsWe screened 58,378 citations, identifying 96 studies. All tools were designed for use commencing at intensive care unit admission except three tools implemented at 3, 5 or 14 days. We identified 32 studies of locally developed checklists, 28 goal setting/structured communication templates, 23 care bundles and 9 studies of mixed format tools. Most (43 %) tools were designed for use during rounds, fewer tools were designed for use throughout the ICU day (27 %) or stay (9 %). Most studies (55 %) reported process objectives i.e., improving communication, care standardisation, or rounding efficiency. Most common clinical processes quality improvement tools were used to standardise were sedation (62, 65 %), ventilation and weaning (55, 57 %) and analgesia management (58, 60 %). 44 studies reported the effect of the tool on patient outcomes. Of these, only two identified a negative effect; increased length of stay and increased days with pain and delirium.ConclusionAlthough we identified numerous quality improvement tools for use in the intensive care unit, few were designed to specifically address actionable processes of care relevant to the unique needs of prolonged stay patients. Tools that address these needs are urgently required.

109. Exploring the effects of health shocks on anti-poverty interventions: Experience of poor beneficiary households in Bangladesh

How and whether health shocks impact poverty reduction interventions remains a largely unexplored topic to which not much attention has been paid. This study explored whether and how health shocks affect anti-poverty interventions targeted to extremely poor households using data from 8 focus group discussions and 12 case studies. Those in extremely poor households mostly experienced episodes of chronic disease that incurred greater health-care costs, largely financed by the out-of-pocket payment system. The majority of those from poor households met health-care costs by selling their means of livelihood, borrowing cash, and marketing physical assets. This study argues that livelihood support alone is likely to be insufficient to reduce poverty. Health needs, subsequently, should be prioritized while designing an anti-poverty program.

A scoping review of published evidence on the influence of critical care/high dependency (HDU) environments on patient, family and staff outcomes. Objectives 1. To identify outcomes, measures, and timing of measures used in studies of critical care environmental design 2. To describe the effect of various ICU design characteristics on these outcomes. ELIGIBILITY We will apply the following PICOST (population, intervention, comparator, outcome, study design, and other limits) eligibility criteria: Population a. patients and family members admitted to critical care or a HDU environment b. healthcare personnel working in a critical care or a high dependency environment Intervention New or modified designs of critical care or HDU environments/aspects of environment including patient rooms, central areas, family waiting areas, and universal support services. METHODS We will conduct this review according to the Preferred Reporting Items for Systematic Review and Meta-Analyses Protocol (PRISMA-P) guidelines and complete a PRISMA-P checklist. Data Sources and Search Strategy We will iteratively determine our search strategy with guidance from an experienced information specialist. Search strategies will utilize a combination of controlled vocabulary and keyword combinations. We will adjust vocabulary and syntax across databases. We will remove animal-only studies and opinion pieces (e.g., editorials, letters). As we are including a range of study designs, we will not apply study-specific filters. Prior to search execution, a second information specialist will use the Peer Review for Electronic Search Strategies (PRESS) template to review and approve the search strategy. We will limit our search to published literature. We will search the following electronic databases (Jan 2007 to Jul 2018): Medline, CINAHL, Embase, Web of Science, Cochrane Library, PROSPERO, and the Joanna Briggs Institute. Study Selection and Data Extraction We will use a pre-designed screening tool listing inclusion and exclusion criteria, and pilot test screening with two reviewers to establish >90 % agreement. Two reviewers will examine independently examine study titles and abstracts. We will obtain full text articles considered potentially relevant by either reviewer and examine for eligibility. Disagreements will be resolved through discussion and referred to a third author for arbitration if necessary. We will manage the screening process using Endnote; all decisions resulting from full-text review will be recorded in an Excel file. One reviewer will extract study data using a standardized form developed a priori; a second reviewer will check all data extraction for accuracy. We will extract data on study characteristics (e.g., year published, country(ies) study was performed, study design; aspects of ICU/HDU innovative/modified design studied, comparator design (if applicable), participant characteristics, outcomes and measures used, study results including qualitative themes if application. We will not assess risk of bias or study quality. Data Synthesis We will summarize study (including types of outcomes reported and measures used), ICU/HDU design/environment and participant characteristics using descriptive analyses. We will synthesize quantitative and qualitative outcomes narratively.

For patients requiring an artificial airway (either an endotracheal or tracheostomy tube), establishing effective communication is particularly challenging. The inability to communicate is one of the key stressors for patients with an artificial airway. Core outcome sets aim to standardise outcome reporting by identifying outcomes perceived fundamental for measurement in trials of a specific interest area. Our aim is to establish a core outcome measurement set (COMS) for studies of interventions designed to enable both non-vocal and vocal communication in adults requiring an artificial airway with or without mechanical ventilator support in a critical care environment including intensive care units (ICU), specialised weaning centres and step-down units. The purpose of this scoping review is to identify the outcomes and measures tools used in treatments designed to enable communication in adults with breathing tubes.

112. Outcomes reported in studies of interventions to promote secretion clearance in patients with neuromuscular disorders: a scoping review

Patients with neuromuscular disorders often develop respiratory muscle weakness. These neuromuscular disorders include congenital diseases such as the muscular dystrophies, and non-congenital diseases such as multiple sclerosis, and amyotrophic lateral sclerosis. High spinal cord injuries can also result in respiratory muscle weakness. Irrespective of the cause, these patients experience weakened cough strength, impeding adequate secretion clearance and leading to increased risk of respiratory infection, a common cause of death. To reduce the risk of respiratory infection, patients with neuromuscular disorders are often supported with interventions to augment secretion clearance. Methods used to augment secretion clearance can be categorised into proximal (‘cough augmentation’) or distal techniques (‘sputum mobilisation’). Proximal techniques include assisted inspiration (e.g., intermittent positive pressure breathing, air stacking, glossopharyngeal breathing, lung volume recruitment), assisted expiration (e.g., manual assisted cough) and assisted inspiration and expiration (e.g., mechanical insufflation-exsufflation). Distal techniques include high frequency chest wall oscillation or compression, intrapulmonary percussive ventilation, and chest wall strapping. Use of secretion clearance techniques in patients with a neuromuscular disorder is varied in terms of choice of intervention, technique used, and availability of equipment between sites and countries. A frequently cited reason for this practice variation is the lack of evidence for the efficacy of these techniques or guidelines based on consensus opinion. Assessing the comparative efficacy of secretion clearance techniques is currently challenging due to the lack of a consensus in outcomes and measurement instruments. A key stakeholder informed core outcome set would assist with homogeneity in the outcomes reported in future trials. A core outcome set is an agreed set of outcomes which should be measured and reported when undertaking an investigation into a particular area. This set is agreed via a series of consensus building activities by a panel of experts including end users. As the first step to informing the development of a core outcome set, our objective is to conduct a scoping review to describe the outcomes reported in studies recruiting patients with neuromuscular disorders that result in respiratory weakness requiring airway clearance techniques. This core outcome set will inform studies of interventions to improve secretion clearance. Our secondary objectives are to describe (1) the instruments used to measure the outcomes identified; (2) the analysis metric used to evaluate the outcome (3); the method of aggregation for each outcome; and (4) the time horizon for measurement of each outcome. We have chosen a scoping review approach to chart outcomes and measures using various airway clearance techniques Scoping reviews are the advised methodology for this broad scope of work.

Aim and objectives: To conduct a diagnostic evaluation of physical restraint practice using the integrated‐Promoting Action on Research Implementation in Health Services (i‐PARIHS) framework. Background: Evidence indicates that physical restraints are associated with adverse physical, emotional and psychological sequelae and do not consistently prevent intensive care unit (ICU) patient‐initiated device removal. Nevertheless, physical restraints continue to be used extensively in ICUs both in Canada and internationally. Implementation science frameworks have not been previously used to diagnose, develop and guide the implementation of restraint minimisation interventions. Design: A prospective observational study of restrained patients in a 20‐bed, academic ICU in Toronto, Canada. Methods: Data collection methods included patient observation, electronic medical record review, and verbal check with the point‐of‐care nurses. Data were collected pertaining to framework domains of unit culture (restraint application/removal), evaluation capacity (documentation) and leadership (rounds discussion). The reporting of this study followed the STROBE guidelines. Results: A total of 102 restrained patients, 67 (66%) male and mean age 58 years (SD 1.92), were observed. All observed devices were wrist restraints. Restraint application and removal time was verified in 83 and 57 of 102 patients respectively. At application, 96.4% were mechanically ventilated and 71% sedated/unarousable. Nurses confirmed 71% were prophylactically restrained; 7.2% received restraint alternatives. Restraint removal occurred after interprofessional team rounds (87%), during daytime (79%) and following extubation (52.6%). Of the 923 discrete patient observation of physical restraint use, 691 (75%) were not documented. Of the 30 daytime interprofessional team rounds reviewed, physical restraint was discussed at 3 (10%). Conclusion: In this single‐centre study, a culture of prophylactic physical restraint was observed. Future facilitation of restraint minimisation warrants theoretically informed implementation strategies including leadership involvement to advance interprofessional collaboration. Relevance to clinical practice: The findings draw attention to the importance of a preliminary diagnostic study of the context prior to designing, and implementing, a physical restraint minimisation intervention. Nutritional status differs between infants and young children living in slum and non-slum conditions-infants and young children living in City Corporation slums are likely to have worse nutritional status compared to those from non-slums. Furthermore, families in slums tend to engage female labor in cash-earning activities as a survival strategy; hence, a higher percentage of mothers stay at work. However, little is known about feeding practices for infants and young children in families with working mothers in slums. This study aims to understand the factors that determine feeding practices for infants and young children living in families with working mothers in Dhaka slums.This study adopted a qualitative approach. Sixteen In-depth Interviews, five Key Informant Interviews, and Focused Group Discussions were conducted with family members, community leaders, and program staff. Method triangulation and thematic analyses were conducted.Feeding practices for infants and young children in families with working mothers are broadly determined by mothers' occupation, basis civic facilities, and limited family buying capacity. Although mothers have good nutritional knowledge, they negotiate between work and feeding their infants and young children. Household composition, access to cooking facilities, and poverty level were also found to be significant determining factors.The results suggest a trade-off between mothers' work and childcare. The absence of alternative care support in homes and/or work places along with societal factors outweighs full benefits of project interventions. Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score < 19, ICU stay > 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat.

- Abstract
A holistic learning environment is one that nurtures all aspects of students' learning. The environment is safe, supportive, and provides opportunities to help students deal with nonacademic as well as academic factors that impact their learning. Creation of such an environment requires the establishment of a supportive learning community. For a variety of reasons, establishing such a learning community of first-year medical students can be challenging. This communication presents one approach to meeting this challenge in a medical school Human Physiology course. Steps were taken at the beginning of the course to create the community, and activities designed to reinforce these efforts were incorporated into the course as it progressed. Two pilot studies were conducted to test the hypothesis that providing students with a participatory music experience may help to promote a holistic learning environment by helping them restore a sense of balance to their emotional well-being as well as reinforce a sense of community in the classroom. Student response to these activities indicated that these efforts provided emotional support during stressful periods during the quarter, helped promote a feeling of safety within the environment, and reenergized the class during long class sessions. This project illustrates that each instructor, within the confines of his/her own classroom, can make a significant contribution to achieving a holistic learning environment for his/her students. 'The Crown' has been described as a 'term of art' in constitutional law. This is more than misleading, obscuring the pivotal legal position of the king, which in modern times has been conveniently ignored by lawyers and politicians alike. -- This work examines the legal processes by which a king is made, tracing those processes from the earliest times to the present day. It concludes that the king is made by the selection and recognition by the people, his taking of the Oath of Governance, and his subsequent anointing. (The religious aspects of the making of the king, though of considerable legal significance, are not examined herein, because of space constraints.) -- The Oath of Governance is conventionally called the 'Coronation Oath'-which terminology, while correctly categorising the Oath by reference to the occasion on which it is usually taken, has led by subliminal implication to an erroneous conclusion by many modern commentators that the Oath is merely ceremonial. -- This work highlights the legal implications of the king's Oath of Governance throughout history, particularly in times of political unrest, and concludes that the Oath legally :- conveys power from the people to the person about to become king (the willingness of the people so to confer the power having been evidenced in their collective recognition of that person); - bestows all the prerogatives of the office of king upon that person; - enshrines the manner in which those prerogatives are to be exercised by the king in his people(s)' governance; and that therefore the Oath of Governance is the foundation of the British Constitution. -- All power and prerogative lie with the king, who as a result of his Oath of Governance is sworn to maintain the peace and protection of his people(s), and the king can not, in conscience or law, either do, or allow, anything that is in opposition to the terms of that Oath.

Rationale Ventilator Assisted Individuals (VAIs) frequently remain in intensive care units (ICUs) for a prolonged period once clinically stable due to a lack of transition options. These VAIs occupy ICU beds and resources that patients with more acute needs could better utilize. Moreover, VAIs experience improved outcomes and quality of life in long-term and community-based environments. Objective To better understand the perspectives of healthcare providers (HCPs) working in an Ontario ICU regarding barriers and facilitators to referral and transition of VAIs from the ICU to a long-term setting. Methods We conducted semi-structured interviews with ten healthcare providers involved in VAI transitions. Main Results Perceived barriers included long wait times for long-term care settings, insufficient bed availability at discharge locations, medical complexity of patients, long waitlists, and a lack of transparency of waitlists. Facilitators included strong partnerships and trusting relationships between referring and discharge locations, a centralized referral system, and utilization of community partnerships across care sectors. Conclusions Insufficient resourcing of long-term care is a key barrier to transitioning VAIs from ICU to long-term settings; strong partnerships across care sectors are a facilitator. System-level approaches, such as a single-streamlined referral system, are needed to address key barriers to timely transition. [ABSTRACT FROM AUTHOR]
