Abstract

Sulfhydryls were determined in the serum of 61 patients affected with malignant hemolymphopathies. The mean values were under normal, except for the group of acute leukemias. A particularly marked difference was found between primary acute leukemias and acutizations of chronic myeloid leukemias. In the former, normal mean values and, in most cases, normal initial values were observed, whereas in the latter no cases with normal initial values were found: here, in addition, the lowest mean values were present. In these cases a relation was observed between lowering of sulfhydryl value and degree of the leukemic hyperplasy. In Hodgkin's disease the lowest sulfhydryl values were found to be consistent with a shorter survival.

Abstract

A study on steroid hormone metabolism in 24 cases of blood dyscrasias has been performed through chemical determination of urinary 17-ketosteroids and 17-hydroxycorticosteroids and chromatographic analysis of the former.Corticoids appeared to be preferentially decreased in acute leukemia, chronic myelogenous leukemia and polycythemia, while in chronic lymphatic leukemia and in Waldenstrom's disease 17-ketosteroids were affected as well.The chromatographic analysis of 17-ketosteroids showed that fractions IV and V, corresponding to androsterone and etiocholanolone, are decreased in almost all cases. High and bifid peaks III are constantly found in polycythemia and haemophilia; changes of peak III of a minor importance are occasionally found in all types of leukemia.The significance of the data observed is discussed from the point of view of steroid metabolism and a hypothesis is presented to explain the mechanism of the changes occurring in pattern of the various chromatographic fractions of 17-ketosteroids.

Abstract

Inferior venacavography was used for revealing enlarged retro- or paracaval lymph nodes, some of which are not visualized by lymphography as they lay above L 2. As criteria for involvement of lymph nodes the dislocation or narrowing of the vena cava and the reflux of contrast medium in the lumbar veins were studied. Venacavography was carried out in 48 consecutive patients with lymphomas: 23 Hodgkin's disease and 25 non-Hodgkin's lymphoma. Signs of enlargement of paracaval lymph nodes were seen in 52% of patients with Hodgkin's disease and in 80% of the cases of non-Hodgkin's lymphoma. A fairly good agreement was found to between lymphograms and cavograms. Cavography proved to be an easy, inexpensive and minimally invasive technique that can be repeated several times without inconveniencing the patients too much, thus enabling restaging of patients after therapy; moreover, it produced fairly good urograms as the contrast medium is promptly eliminated through the kidney, thus eliminating the need for intravenous pyelography.

Abstract

The role of in vivo administration of lymphoblastoid alpha-interferon (IFN-alpha) on the immune status of 12 patients with hairy cell leukemia (HCL) was studied. In most cases an increase in T3(CD3), T4(CD4), and T8(CD8) lymphocyte subsets was documented at the same time as hairy cells disappeared from the circulation. This led in most cases to an improved T4/T8 ratio. The effect of treatment with IFN-alpha was particularly evident on the natural killer cell compartment, which is often functionally depressed in HCL at diagnosis. After therapy, a progressive increase in the cytotoxic activity was observed in most patients. This was more evident 6-9 months after commencing treatment. These findings suggest that, in addition to the known clinicohematological effects, IFN-alpha can improve both the T and natural killer compartments in patients with HCL.

Abstract

This is a review of current treatment for hairy cell leukemia (HCL). Data for this analysis were obtained from the Italian HCL Registry, as well as from other published series. We have given space to the impact of interferon and pentostatin on the management of this disease. Other issues are also discussed, such as the relevance of achieving a complete remission with respect to overall and relapse-free survival. We include a final section on recommendations which may prove useful in designing an appropriate therapeutic strategy.

Abstract

Hodgkin's disease (HD) after the age of 65 years is uncommon and there are no published data on chemotherapy regimens devised for elderly HD patients.From 1990 to 1993, 25 elderly HD patients were treated with the CVP/CEB regimen: chlorambucil 6 mg/sqm p.o. days 1 through 7, vinblastine 6 mg/sqm i.v. on day 1, procarbazine 100 mg/sqm p.o. days 1 through 7, prednisone 30 mg/sqm p.o. days 1 through 7, cyclophosphamide 500 mg./sqm i.v. day 15, etoposide 70 mg/sqm i.v. day 15, bleomycin 10 mg/sqm i.v. day 15. Each course was repeated every 4 weeks. Stage I and II patients were treated with 3 courses followed by involved field radiotherapy, while more advanced stage patients received 6 courses and radiotherapy was limited to bulky areas. The results of the CVP/CEB regimen are retrospectively compared to those of 74 elderly patients treated between 1982 and 1989 and subdivided into the following 2 groups: 32 patients treated according to the same therapy used at that time in younger patients, and 42 patients given alternative low aggressivity or palliative treatment.CVP/CEB is a well-tolerated regimen, with only 1 (4%) toxic death and 2 (8%) protocol violations/interruptions. The CVP/CEB complete remission rate (73%) compares favorably with our previous groups of patients, mainly because of the lower toxic death rate. However, the CVP/CEB relapse-free survival rate is lower than that of patients treated with more aggressive conventional regimens (47% vs. 77%, p0.02). The CVP/CEB overall survival and event-free survival rates are 55% and 32%, respectively, and they are not statistically different from those of patients treated before 1990.CVP/CEB is a well-tolerated low toxicity regimen with a high CR rate. The relapse rate is high and event-free survival is comparable to that of patients treated conventionally. Our results suggest the need for individualized treatment criteria for older patients with HD.

Abstract

Mantle cell lymphoma (MCL) is a separate histological and clinical entity recently recognized in the new revised European-American Lymphoma Classification. Little information exists regarding its therapy. We report the results of a retrospective study of 27 patients affected by MCL evaluating the clinical characteristics and the results of different therapeutical options used during the period of observation.From 1983 to 1993, we observed 27 patients affected by MCL according to the criteria proposed by European Lymphoma Task Force in a revision of 55 cases classified as NHL E according to Working Formulation (WF) criteria. We analyzed the clinical characteristics, the prognostic factors and the O.S. of these patients.The clinical characteristics of our patients (pts) are similar to those observed in other series: male prevalence, median age 62 years, B symptoms in 9 cases, P.S.2 in 11 cases, 3 pts were in stage I and II, 4 in stage III, 20 in stage IV; 18 pts had a bone marrow involvement, 13 pts had spleen enlargement and 14 had extranodal localization; 8 pts had bulky tumor and 5 had LDH above normal. The CR rate was 51.8%, the median O.S. was 43 months, and DFS was 18 months; the pts without bulky disease and with localized disease had a better CR rate. The inclusion of an anthracycline in the regimen did not affect the results.Our results were not divergent from those present in literature. The mantle cell lymphoma is an incurable and highly aggressive disease. Autologous bone marrow transplantation as support of high dose chemotherapy or allogenic bone marrow transplantation may be a chance for some patients, but not for the majority of patients, which are older than 65 years. Studies of a larger series and different therapeutical approaches, i.e. using biological modifiers in association or as maintenance after chemotherapy are essential.
