112. Capsules with evolving brittleness to resist the preparation of self-healing concrete

Abstract

Capsules for self-healing concrete have to possess multifunctional properties and it would be an enormous advantage in the valorization process when they could also be mixed in. Therefore, we aimed to develop capsules with evolving brittleness. Capsules with high initial flexibility were prepared by adding a plasticizer to an ethyl cellulose matrix. During hardening of the concrete, the plasticizing agent should leach out to the moist environment yielding more brittle capsules which break upon crack appearance. The tested capsules could easily be mixed in during concrete production. However, incompatibility issues between the capsule wall and the inner polymeric healing agent appeared. Moreover, the capsules became insufficiently brittle and the bond strength to the cementitious matrix was too weak. Consequently, multilayer capsules were tested. These capsules had a high impact resistance to endure concrete mixing and were able to break upon crack formation., Las cápsulas para la auto-reparación del hormigón tienen que poseer propiedades multifuncionales. Una enorme ventaja en el proceso para su valorización se obtendría si aquellas pudieran resistir con éxito el mezclado. Por lo tanto, nos propusimos desarrollar cápsulas cuya fragilidad evoluciona. Cápsulas con una alta flexibilidad inicial se prepararon mediante la adición de un plastificante a una matriz de etil celulosa. Durante el endurecimiento del hormigón, el agente plastificante debe filtrarse hacia el medio ambiente húmedo produciendo cápsulas más frágiles que se rompen con el surgimiento de fisuras. Las cápsulas pudieron ser fácilmente mezcladas durante la producción de hormigón. Sin embargo, aparecieron problemas de incompatibilidad entre la pared de la cápsula y el agente de curación polimérico interior. Por otra parte, las cápsulas se comportaron insuficientemente frágiles y con una baja adherencia hacia la matriz cementicia. En consecuencia, se probaron las cápsulas multicapa. Estas cápsulas tenían una alta resistencia al impacto para sobrevivir el mezclado del hormigón y fueron capaces de romperse luego de la formación de fisuras.

Le développement de nouveaux traitements ciblés dans les carcinomes bronchiques non à petites cellules (CBNPC) dépend d’une meilleure compréhension des bases moléculaires de la cancérogénèse, de la connaissance du rôle des aberrations moléculaires dans la progression de la maladie et du développement de plateformes de biologie en capacité d’identifier des nouveaux biomarqueurs. Dans cet article, nous passons en revue les techniques utilisées en routine dans les plateformes de biologie moléculaire des cancers ainsi que les nouvelles techniques en développement. Ces nouveaux biomarqueurs des CBNPC sont mis à disposition des cliniciens et des biologistes de façon parallèle au développement de médicaments ciblés. Les nouvelles anomalies moléculaires de l’EGFRexon 20, de HER2, MET, RET, BRAF, ROS1 et NTRKsont développées ainsi que les essais thérapeutiques des médicaments ciblés les plus innovants.

Abstract

BACKGROUND AND AIM: Few well-designed randomized controlled trials (RCT) regarding the impact of community pharmacist interventions on pharmacotherapeutic monitoring of patients with Chronic Obstructive Pulmonary Disease [COPD) have been conducted. We assessed the effectiveness of a pharmaceutical care program for patients with COPD. METHODS: The PHARMACOP-trial was a single-blind 3-month RCT, conducted in 170 community pharmacies in Belgium, enrolling patients prescribed daily COPD medication, aged > or = 50 years, and with a smoking history > or = 10 pack-years. A computer-generated randomization sequence allocated patients to intervention (n = 371), receiving protocol-defined pharmacist care, or control group (n = 363), receiving usual pharmacist care 11:1 ratio, stratified by center). Interventions, focusing on inhalation technique and adherence to maintenance therapy, were carried out at start of the trial and at one month follow-up. Primary outcomes were inhalation technique and medication adherence. Secondary outcomes were exacerbation rate, dyspnea, COPD specific and generic health status and smoking behavior. RESULTS: From December 2010 to April 2011, 734 patients were enrolled. 42 patients (5.7%) were lost to follow-up. At the end of the trial, inhalation score (Mean estimated difference [delta], 13.5%; 95% Confidence Interval [CI], 10.8-16.1; P < .0001] and medication adherence [(delta, 8.51%; 95% CI, 4.63-12.4; P < .0001) were significantly higher in the intervention group compared to the control group. In the intervention group, a significantly lower hospitalization rate was observed (9 vs 35 hospitalizations; Rate Ratio, 0.28; 95% CI, 0.12-0.64; P = .003). No other significant between-group differences were observed. CONCLUSION: The PHARMACOP-trial demonstrates that pragmatic pharmacist care programs improve both inhalation technique and medication adherence in patients with COPD and could reduce hospitalization rates. The protocolled intervention used in this trial was specifically designed for and evaluated in (Belgian) community pharmacies. This may facilitate future implementation in the Belgian context.

Abstract

Immune checkpoint inhibitors (ICIs) have been incorporated in the treatment strategy of advanced non-small cell lung cancer (NSCLC) in first- and second-line setting improving the prognosis of these patients. However, the treatment landscape has been also drastically overturned with the advent of targeted therapies in oncogenic-addicted advanced NSCLC patients. Despite ICIs represent an active and new treatment option for a wide range of advanced NSCLC patients, the efficacy and the optimal place of ICI in the treatment strategy algorithm of oncogenic-addicted tumors remains still controversial, as only a minority of trials with ICI enrol oncogenic-addicted NSCLC patients previously treated with standard therapy. Therefore, there are still several open questions about ICI in oncogenic-driven NSCLC, such as the efficacy and toxicities, which need to be addressed before considering treatment with ICI as a standard approach in this population. Abstract

Immune checkpoint inhibitors (ICIs) have been incorporated in the treatment strategy of advanced non-small cell lung cancer (NSCLC). Beyond the already approved indications in first- and second-line setting of advanced NSCLC, new data has recently emerged demonstrating its efficacy in locally advanced disease as maintenance after chemo-radiotherapy and currently several trials are also exploring its efficacy in earlier stages of the disease to evaluate whether these results could be extrapolated to the adjuvant setting. With the advent of all these new therapies, their potential in other thoracic malignancies such as mesothelioma and small-cell lung cancer are also being evaluated with encouraging preliminary data that endorses their short-term incorporation as new therapeutic options in these thoracic malignancies. However, despite all these new evidence, there are still several open questions that remain to be solved like the use of immune agents in special subpopulations such as elderly or fragile patients or the case of patients with brain metastases or autoimmune disorders. In addition some other open questions remain with regards ICIs activity in patients receiving corticosteroid or antibiotics, the potential use in oncogenic addicted tumours, as well as the safety of retreatment after the onset of immune-related adverse events (ir-AE) or the optimal dose schedule or time on treatment for ICIs administration. Herein, we propose to address all these questions, reviewing most recent evidence available in order to give readers some practical advises and guidance on how to deal with these challenges when treating NSCLC patients with immunotherapy. Intraperitoneal (IP) chemotherapy is an effective way of treating peritoneal carcinomatosis of colorectal origin after complete cytoreduction. Although IP therapy has been already performed for many years, no standardized treatment design has been developed in terms of schedule, residence time, drug, or carrier solution. Because of the fast clearance of the conventional intravenous (IV) drug delivery systems used for IP therapy, a lot of research is performed to optimize IP drug delivery and extend the residence time of the cytotoxic agent in the peritoneal cavity. This paper reviews the recent advances made in drug delivery systems for IP chemotherapy, discussing the use of microparticles, nanoparticles, liposomes, micelles, implants, and injectable depots for IP delivery.

Abstract

Tablets are conventionally produced via consecutive batch process steps. Recent introduction of continuous process equipment is gaining industrial importance in pharmaceutics. Transition to continuous production requires improved understanding of all operations, necessitating the development of mechanistic models of multi‐phase systems which in the end allow process control. This contribution focuses on continuous fluidized bed drying of pharmaceutical wet granules. A stepwise approach is used in model development, starting with the drying behaviour of single granules. Experiments to determine the drying characteristics were conducted in a continuous fluidized bed dryer at several gas temperatures and velocities. The drying process was found to consist of two subsequent phases: a fast drop in moisture content followed by a slower evaporation. A mechanistic model for single granules [1] yielded good description of the data, and was calibrated and validated. The conceptual model consists of a liquid evaporation model (fast part) and a wet core ‐ dry crust model describing the decrease of the wet core diameter and thus the moisture content (slower part). Resulting distinction between weakly and strongly bonded water in the granules yields deeper knowledge of the process. The validated model can now be used for optimization of the continuous drying process.

Abstract

The aim of this study is to propose a strategy to implement a PAT system in the blending step of pharmaceutical production processes. It was examined whether Raman spectroscopy can be used as PAT tool for the in-line and real-time endpoint monitoring and understanding of a powder blending process. A screening design was used to identify and understand the significant effects of two process variables (blending speed and loading of the blender) and of a formulation variable (concentration of active pharmaceutical ingredient (API): diltiazem hydrochloride) upon the required blending time (response variable). Interactions between the variables were investigated as well. A Soft Independent Modelling of Class Analogy (SIMCA) model was developed to determine the homogeneity of the blends in-line and real-time using Raman spectroscopy in combination with a fiber optical immersion probe. One blending experiment was monitored using Raman and NIR spectroscopy simultaneously. This was done to verify whether two independent monitoring tools can confirm each other's endpoint conclusions. The analysis of the experimental design results showed that the measured endpoints were excessively rounded due to the large measurement intervals relative to the first blending times. This resulted in effects and critical effects which cannot be interpreted properly. To be able to study the effects properly, the ratio between the blending times and the measurement intervals should be sufficiently high. In this study, it anyway was demonstrated that Raman spectroscopy is a suitable PAT tool for the endpoint control of a powder blending process. Raman spectroscopy not only allowed in-line and real-time monitoring of the blend homogeneity, but also helped to understand the process better in combination with experimental design. 133. Leptomeningeal carcinomatosis in non-small cell lung cancer patients: A continuing challenge in the personalized treatment era.

Abstract

Leptomeningeal metastasis is a fatal manifestation seen in advanced cancer patients. Its incidence is increasing, reaching 3.8% in molecularly unselected non-small cell lung cancer patients and up to 5% and 9% in ALK-rearranged and EGFR-mutant lung cancer patients, respectively. The prognosis remains poor despite systemic treatment, intrathecal chemotherapy, radiation therapy and personalized treatments in molecularly selected patients. However, new therapies with improved cerebral-spinal fluid penetration have been developed for subgroups of molecular selected patients indicating they could be promising therapeutic options for managing leptomeningeal disease. Systemic chemotherapy, which may be combined with intrathecal chemotherapy, remains standard treatment for lung cancer patients with leptomeningeal disease and a good-risk profile. We summarize evidence reported in the literature for managing this complication in lung cancer patients. 134. Thymic malignancies: Moving forward with new systemic treatments.

Thymic neoplasms are rare malignant tumours, for which the mainstay of treatment is surgical resection. Platinum-based chemotherapy remains the principal treatment in metastatic tumours, with no standard second-line option. Many genes implicated in tumour onset, growth and metastases have been demonstrated to be therapeutic targets in thymic malignancies. Other current efforts to improve outcomes are based on a better understanding of the stromal compartment and tumour microenvironment, facilitating novel therapeutic approaches such as angiogenesis inhibition and immunotherapy. This review seeks to explore the present cutting edge for systemic treatment of advanced thymic neoplasms, examining novel agents under clinical investigation such as cytotoxic therapies, targeted therapies and immunotherapy.
