Abstract

Background: Patients with advanced cancer who progress on standard therapy are potential candidates for phase I clinical trials. Due to their aggressive disease and complex comorbid conditions, these patients often need inpatient admission. This study assessed the outcomes of such patients after they were discharged to hospice care. Patients and Methods: We performed a retrospective analysis of patients with solid tumor malignancies who were discharged to hospice care from the inpatient service. Results: One hundred thirty-three patients were included in the study cohort. All patients had metastatic disease and an Eastern Cooperative Oncology Group performance status ≥ 3. The median survival after discharge to hospice from an inpatient setting was 16 days, with a survival rate of 5% at 3 months after discharge. The median survival after the last cancer treatment was 46 days, with survival of 17% at 3 months, and 5% at 6 months. Patients with lactate dehydrogenase (LDH) >618 IU/L had a median post-discharge survival of 11 days versus 20 days for patients with LDH ≤ 618 IU/L. Conclusions: Patients with metastatic cancer participating in phase I trials who have poor performance status and require inpatient admission have a very short survival after discharge to hospice. Abstract

The date palm seed being one of the major forms of biomass produced from the date industry in UAE, its potential to be an appropriate precursor for the preparation of porous carbon utilizing KOH as an activating agent is assessed in the present work. The porous carbon is prepared at an activation temperature of 600 °C, impregnation ratio of 2, and activation duration of 1 hour, in an inert atmosphere using a conventional horizontal furnace. The resultant porous carbon has a Brunauer-Emmett-Teller surface area of 892 m2/g, pore volume of 0.45 cm3/g, and an average pore diameter of 1.97 nm. This porous carbon was used for adsorption studies at different initial concentrations (100-400 mg/l) and temperatures (30-50 °C). The adsorption isotherm parameters for the Langmuir and Freundlich models were determined using experimental adsorption data and it was found that both Langmuir and Freundlich isotherms described well the adsorption behavior of phenol on porous carbon. The mono layer adsorption capacity was observed to be 333 mg/g, which is highest for the reported date pam seed biomass-based porous carbon. From the data obtained, it was concluded that the removal of phenol from aqueous solution by porous carbon prepared from data palm seed is a low-cost process with an extremely high performance. Abstract

PURPOSE: Cancer pain management guidelines recommend initial treatment with intermediate-strength analgesics such as hydrocodone and subsequent escalation to stronger opioids such as morphine. There are no published studies on the process of opioid rotation (OR) from hydrocodone to strong opioids in cancer patients. Our aim was to determine the opioid rotation ratio (ORR) of hydrocodone to morphine equivalent daily dose (MEDD) in cancer outpatients. PATIENTS AND METHODS: We reviewed the records of consecutive patient visits at our supportive care center in 2011-2012 for OR from hydrocodone to stronger opioids. Data regarding demographics, Edmonton Symptom Assessment Scale (ESAS), and MEDD were collected from patients who returned for follow-up within 6 weeks. Linear regression analysis was used to estimate the ORR between hydrocodone and MEDD. Successful OR was defined as 2-point or 30% reduction in the pain score and continuation of the new opioid at follow-up. RESULTS: Overall, 170 patients underwent OR from hydrocodone to stronger opioid. The median age was 59 years, and 81% had advanced cancer. The median time between OR and follow-up was 21 days. We found 53% had a successful OR with significant improvement in the ESAS pain and symptom distress scores. In 100 patients with complete OR and no worsening of pain at follow-up, the median ORR from hydrocodone to MEDD was 1.5 (quintiles 1-3: 0.9-2). The ORR was associated with hydrocodone dose (r = -.52; p < .0001) and was lower in patients receiving ≥40 mg of hydrocodone per day (p < .0001). The median ORR of hydrocodone to morphine was 1.5 (n = 44) and hydrocodone to oxycodone was 0.9 (n = 24). 140. Enhanced Network Traffic Anomaly Detector.

Abstract

Network intrusion detection systems often rely on matching patterns that are learned from known attacks. While this method is reliable and rarely produces false alarms, it has the disadvantage that it cannot detect novel attacks. An alternative approach is to learn a model of normal traffic and report deviations, but these anomaly models are typically restricted to modeling IP addresses and ports. We describe an anomaly detection system which models all the fields of network, transport layer and payload of a packet at the byte level, by giving more weight to the most anomalous attributes. We investigated all the attributes and assigned weights to the attributes based on their anomalous behavior. We detect 144 of 185 attacks in the DARPA off-line intrusion detection evaluation data set [1] at 10 false alarms per day (total 100 false alarms), after training on one week of attack-free traffic.
