Interventricular septal dissection is a rare Complication of the interventricular septum. It may result from an aneurysm of the sinuses of Valsalva, bacterial endocarditis, trauma, cardiac surgery, Myocardial infarction, endomyocardial biopsy, or a congenital myocardial developmental anomaly. Postmyocardial infarction ventricular septal rupture (VSR) is a rare complication (1/1000), and ventricular septal dissection is an even less common complication with only five case reports previously described, But unruptured post Myocardial Infarction (MI) dissection is even rarer with only one reported study before. In this case report we describe an unruptured post-MI Interventricular septal dissection following anterior wall MI.

This study aimed to eliminate the bitter taste of Moringa peregrina press cake (MPC) as a byproduct of oil extraction (by employing safe and conventional methods) and evaluating its potential for formulating value-added food products. The characteristics (nutritional and anti-nutritional properties, monosaccharide composition, in vitro starch and protein digestibility, antioxidant capacity, and in vitro α-glucosidase, and α-amylase inhibitory activity) of raw, debittered (soaked in distilled water and boiled), and roasted (after debitterization) MPC flours were determined. Debitterization significantly increased total protein, fiber, arabinose, xylose, antioxidant activity, in vitro protein digestibility, and α-amylase inhibitory activity, whereas it decreased total starch, resistant starch, starch digestibility, ash, glucose, phytic acid, tannin, and oxalate contents. Fiber content, protein digestibility, α-amylase inhibitory activity, and antioxidant activity were further increased as the result of roasting. MPC and its products could inhibit α-amylase activity, with the highest inhibition belonging to roasted debittered samples. The current study is the first to report on the comprehensive nutritional and bio- and physicochemical aspects of Moringa peregrina press cake and the effect of treatments on improving its sensorial, nutritional, and health-promoting properties. Therefore, these results indicate the potential of treated MPC as a novel natural functional ingredient for various food formulations.

One of the most important challenges of smart grids is the congestion of transmission lines. A flexible smart grid with demand-side resources can be a suitable solution to manage transmission lines congestion. This paper proposes a multi-objective model with the aim of congestion management through generation rescheduling considering cost and emission purposes in a flexible smart grid. An inconvenience cost for consumers is defined to model the consumers' unsatisfactory as a consequence of participating in demand response programs (DRPs). Furthermore, a smart grid flexibility index (SGFI) has been presented to show the available flexibility of smart grid as a result of DRPs and gas turbine generators as fast response resources. The DRPs could increase the flexibility of the smart grids due to their impact on flattening the load curve, but this may cause some inconveniences for consumers. On the other hand, participation of consumers in DRPs and the power output gas turbine are associated with uncertainty. In this paper, the uncertainty of consumer's participation in the DRPs has been modeled by Fuzzy-Markov. The proposed multi-objective particle swarm optimization (MOPSO) has been implemented on the IEEE 30-bus system. The results show that the total operation cost including the generation cost, DRP cost, inconvenience cost of consumers, and pollution is reduced. This article addresses the optimal energy management and operation of networked microgrids considering different types of dispatchable units like fuelcell and microturbine and nondispatchable units such as wind turbine and solar units. To change the just-consuming role of vehicles into an active role with the ability of making profit, the vehicle-to-grid technology (V2G) is deployed here. Due to the complex and nonlinear structure of the problem, an effective optimization energy management framework based on the bat algorithm (with a modification) and unscented transform is devised to find the most optimal operation point of the devices from the economic point of view. Due to the high uncertainties injected by electric vehicles pattern behavior in addition to the renewable sources output power variations, the unscented transform is proposed to make the analysis more realistic. The simulation results on an IEEE networked microgrid test system advocate the high capability and proper performance of the proposed method. The results show that the total system operation cost is 53897.004$ and 53711.704$ in the 1st and 2nd scenarios, respectively. Moreover, it is seen that considering uncertainty in the problem has added 0.586% and 0.762% to the cost function value in the first and second scenarios, compared to the deterministic framework.

Background: Coronary Artery Disease (CAD) as a complex process will be the most common cause of death in the world by 2020. One of the relatively new factors associated with CAD is the plasma level of melatonin. This study aimed to determine the effect of plasma melatonin level on the occurrence and severity of CAD.Methods: This cross-sectional study was conducted from August to December 2018 in Kerman, Iran. Eighty-seven adolescents with suspected CAD were selected via the convenience sampling method. Severity of CAD was evaluated by a cardiologist for each patient using Gensini score. The anxiety, depression, and sleep disturbance of participants were examined by HADS and PSIQ questionnaires, respectively. The blood sample of patients was taken at 3:30 a.m. and it was immediately transferred to the laboratory for serum separation. A two-part model was used for data analysis using STATA software.Results:The mean age (±SD) of the participants was 54.0 (±10.83) years. Less than half of the patients experienced anxiety and depression symptoms during last month (33% and 42%, respectively). Results showed that more than half of the patients (n=51, 57.5%) were diagnosed as CAD patients. According to multivariate regression models, melatonin (AOR=0.96, 95% CI: 0.94, 0.98) and depression (β: 0.79, 95% CI: 0.06, 1.52) were determined as predictors for CAD occurrence and severity, respectively.Conclusion: Melatonin as a protective factor has an effect on the occurrence and severity of CAD, but the existence of some diseases like mental disorders can lead to a decrease in the plasma concentration of melatonin. By treating depression and improving melatonin synthesis and secretion cycle, the occurrence and severity of CAD may be decreased.

Background:Cardiovascular diseases are still among the most important causes of death in different countries. There are several risk factors for the onset of this disease. The rating of these risk factors is very important for informing the community and planning for the future.Methods: Linear regression is one of the classic statistical methods that has many applications in medical sciences. When dealing with fuzzy data, it is not possible to use linear regression. The use of angiography to estimate the extent of congestion is associated with an estimate of more or less stenosis and the increase of atherosclerosis. Hence, this variable has been considered as a fuzzy variable. Fuzzy linear regression (FLR) was used to rank and compare the known risk factors for coronary artery occlusion.Results:After analyzing the data by least squares FLR, the most important risk factors included Family history, history of diabetes, age, history of hypercholesterolemia, history of cigarette smoking, Body Mass Index and history of hypertension respectively.Conclusion: When it is not possible to calculate the response variable or one of the independent variables examined in the model accurately, FLR, or to be more precise, regression in fuzzy environment can be a good alternative for conventional regression analysis.

In this study, sodium caseinate (NaCas), soy protein isolate (SPI), and whey protein isolate (WPI) were used as structural materials for the delivery of rutin, naringenin, curcumin, hesperidin, and catechin. For each polyphenol, the protein solution was brought to alkaline pH, and then the polyphenol and trehalose (as a cryo-protectant) were added. The mixtures were later acidified, and the co-precipitated products were lyophilized. Regardless of the type of protein used, the co-precipitation method exhibited relatively high entrapment efficiency and loading capacity for all five polyphenols. Several structural changes were seen in the scanning electron micrographs of all polyphenol–protein co-precipitates. This included a significant decrease in the crystallinity of the polyphenols, which was confirmed by X-ray diffraction analysis, where amorphous structures of rutin, naringenin, curcumin, hesperidin, and catechin were revealed after the treatment. Both the dispersibility and solubility of the lyophilized powders in water were improved dramatically (in some cases, >10-fold) after the treatment, with further improvements observed in these properties for the powders containing trehalose. Depending on the chemical structure and hydrophobicity of the tested polyphenols, there were differences observed in the degree and extent of the effect of the protein on different properties of the polyphenols. Overall, the findings of this study demonstrated that NaCas, WPI, and SPI can be used for the development of an efficient delivery system for hydrophobic polyphenols, which in turn can be incorporated into various functional foods or used as supplements in the nutraceutical industry.

Long-chain omega-3 fatty acids esterified in lysophosphatidylcholine (LPC-omega-3) are the most bioavailable omega-3 fatty acid form and are considered important for brain health. Lysophosphatidylcholine is a hydrolyzed phospholipid that is generated from the action of either phospholipase PLA1 or PLA2. There are two types of LPC; 1-LPC (where the omega-3 fatty acid at the sn-2 position is acylated) and 2-LPC (where the omega-3 fatty acid at the sn-1 position is acylated). The 2-LPC type is more highly bioavailable to the brain than the 1-LPC type. Given the biological and health aspects of LPC types, it is important to understand the structure, properties, extraction, quantification, functional role, and effect of the processing of LPC. This review examines various aspects involved in the extraction, characterization, and quantification of LPC. Further, the effects of processing methods on LPC and the potential biological roles of LPC in health and wellbeing are discussed. DHA-rich-LysoPLs, including LPC, can be enzymatically produced using lipases and phospholipases from wide microbial strains, and the highest yields were obtained by Lipozyme RM-IM®, Lipozyme TL-IM®, and Novozym 435®. Terrestrial-based phospholipids generally contain lower levels of long-chain omega-3 PUFAs, and therefore, they are considered less effective in providing the same health benefits as marine-based LPC. Processing (e.g., thermal, fermentation, and freezing) reduces the PL in fish. LPC containing omega-3 PUFA, mainly DHA (C22:6 omega-3) and eicosapentaenoic acid EPA (C20:5 omega-3) play important role in brain development and neuronal cell growth. Additionally, they have been implicated in supporting treatment programs for depression and Alzheimer’s. These activities appear to be facilitated by the acute function of a major facilitator superfamily domain-containing protein 2 (Mfsd2a), expressed in BBB endothelium, as a chief transporter for LPC-DHA uptake to the brain. LPC-based delivery systems also provide the opportunity to improve the properties of some bioactive compounds during storage and absorption. Overall, LPCs have great potential for improving brain health, but their safety and potentially negative effects should also be taken into consideration.

Caffeine, as one of the most consumed bioactive compounds globally, has gained considerable attention during the last years. Considering the bitter taste and adverse effects of high levels of caffeine consumption, it is crucial to apply a strategy for masking the caffeine's bitter taste and facilitating its programmable deliverance within a long time. Other operational parameters such as food processing parameters, exposure to sunlight and oxygen, and gastrointestinal digestion could also degrade the phenolic compounds in general and caffeine in special. To overcome these challenges, various nano/micro-platforms have been fabricated, including lipid-based (e.g., nanoliposomal vehicles; nanoemulsions, double emulsions, Pickering emulsions; microemulsions; niosomal vehicles; solid lipid nanoparticles and nanostructured lipid carriers), as well as biopolymeric (e.g., nanoparticles; hydrogels, organogels, oleogels; nanofibers and nanotubes; protein-polysaccharide nanocomplexes, conjugates; cyclodextrin inclusion complexes) and inorganic (e.g., gold and silica nanoparticles) nano/micro-structures. In this review, the findings on various caffeine-loaded nano/micro-carriers and their potential applications in functional food products/supplements will be discussed. Also, the controlled release and bioavailability of encapsulated caffeine will be given, and finally, the toxicity and safety of encapsulated caffeine will be presented. [ABSTRACT FROM AUTHOR]

Background: Patients' hesitation to seek medical care has seriously compromised the management of acute coronary syndrome during coronavirus disease 2019 (COVID-19) outbreak. In the present study, we have reported the rate of discharged against medical advice (DAMA) among patients referred to chest pain unit (CPU) of a tertiary cardiovascular center and compared their clinical outcomes with whom admitted or managed conservatively. Methods: Patients with modified HEART risk score ≥4 referred to the CPU were included in the present study. Population requiring CPU admission due to positive serial troponin were divided into admitted and DAMA groups. Patients with negative serial troponin were managed as outpatient (conservative treatment group). 30-day major adverse cardiac events (MACE) and all-cause mortality were compared between the study groups. Results: A modified HEART risk score (≥4) was calculated for 440 of 5490 patients visited our CPU. One hundred and one (22.9%), 80 (18.1%), and 249 (56.5%) patients were categorized as DAMA, admitted, and conservative treatment groups, respectively. Myocardial infarction was significantly higher in the DAMA versus admitted group (2 vs. 0; P ≤ 0.001). MACE and all-cause mortality were significantly higher in the DAMA group than that in the conservative treatment group (5 vs. 2; P = 0.02 and 4 vs. 2; P = 0.055, respectively). Conclusion: In the present study, we have demonstrated a considerable rate of DAMA in patients with high modified HEART risk score referring to CPU during the COVID-19 outbreak. Importantly, the DAMA group experienced a higher incidence rate of 30-day MACE and all-cause mortality compared to patients who were admitted or managed conservatively.

Abstract High impact–low probability (HILP) incidents, such as hurricanes, usually and gravely damage electric distribution networks. A resilient distribution network must have an ability to recover itself with a fast restoration methodology against the effects of HILP events. Due to improve the electric distribution network resiliency against HILP incidents, this paper suggests a probabilistic–proactive distribution network operation model based upon the chaos theory (P‐PDNOMC(. Here, a resilience index based upon operational cost and load shedding cost is employed to decrease the effects of a hurricane as an HILP incident. P‐PDNOMC is formulated as a framework that consists of a novel hurricane modelling and operation scheduling in normal and emergency conditions with uncertainties considering. It should be noted that a prediction approach based on the chaos theory and the least‐squares support vector machine (LS‐SVM) is also designed to consider the provisional and spatial behaviour of the hurricane. Furthermore, this paper proposes a novel optimization framework for damaged lines determination based upon the P‐PDNOM C (DLsP‐PDNOM C) by considering multi‐zone and multi‐period damaged equipment budget constraints. Here, the load shedding cost is applied to present the efficiency of the proposed model. The numerical results indicate the resiliency enhancement of the electric distribution network in the face of HILP incidents.
