Dysphagia and upper airway obstruction may be post-acute complication of COVID-19. The studied case showed that in people newly infected with SARS-CoV-2, the two mentioned clinical complications probably occur due to the strong immune response and cytokine storm in the throat which can be prevented from oropharynx obstruction by using corticosteroids and antihistamines.

Fast and relatively accurate determination of the fragment size distribution of a muck-pile is still a challenge in mining operations and the existing measurement methods are inefficient. In this research, a new algorithm to determine fragment size distribution due to blasting was presented, using the image processing technique. In the newly proposed approach, delineating of the fragmented rock particles, as the main core of processing, was carried out, using a convolutional neural network. Two networks were defined and trained by 150 laboratory and 150 field data images. Also, 30 laboratory and 30 field data images were applied to carry out the validation visually, and by using F1-scores. For the two laboratory and field networks and results obtained by Split-Desktop software automatic edge detection on the same images, the F1-scores are equal to (0.98, 0.74) and (0.99, 0.85) respectively. Also, for determination of fragment size distribution by laboratory data network and Split-Desktop software automatic edge detection on the same images, the Root Mean Square Error (RMSE) for F30 and F80 are equal to (0.36, 1.20) and (0.31, 1.24) respectively. These indicate better performance of the proposed approach for both rock edge detection and fragment size distribution over Split-Desktop software automatic edge detection.

This paper introduces a novel four-parameter discrete bivariate distribution, termed the bivariate discretized Fréchet–Weibull distribution (BDFWD), with marginals derived from the discretized Fréchet–Weibull distribution. Several statistical and reliability properties are thoroughly examined, including the joint cumulative distribution function, joint probability mass function, joint survival function, bivariate hazard rate function, and bivariate reversed hazard rate function, all presented in straightforward forms. Additionally, properties such as moments and their related concepts, the stress–strength model, total positivity of order 2, positive quadrant dependence, and the median are examined. The BDFWD is capable of modeling asymmetric dispersion data across various forms of hazard rate shapes and kurtosis. Following the introduction of the mathematical and statistical frameworks of the BDFWD, the maximum likelihood estimation approach is employed to estimate the model parameters. A simulation study is also conducted to investigate the behavior of the generated estimators. To demonstrate the capability and flexibility of the BDFWD, three distinct datasets are analyzed from various fields, including football score records, recurrence times to infection for kidney dialysis patients, and student marks from two internal examination statistical papers. The study confirms that the BDFWD outperforms competitive distributions in terms of efficiency across various discrete data applications.

Forecasting can be utilized to predict future trends in physiological demands, which can be beneficial for developing effective interventions. This study implemented forecasting models to predict fatigue level progression when performing exoskeleton (EXO)-assisted tasks. Specifically, perceived and muscle activity data were utilized from nine recruited participants who performed 45° trunk flexion tasks intermittently with and without assistance until they reached medium-high exertion in the low-back region. Two forecasting algorithms, Autoregressive Integrated Moving Average (ARIMA) and Facebook Prophet, were implemented using perceived fatigue levels alone, and with external features of low-back muscle activity. Findings showed that univariate models without external features performed better with the Prophet model having the lowest mean (SD) of root mean squared error (RMSE) across participants of 0.62 (0.24) and 0.67 (0.29) with and without EXO-assisted tasks, respectively. Temporal effects of BSIE on delaying fatigue progression were then evaluated by forecasting back fatigue up to 20 trials. The slope of fatigue progression for 20 trials without assistance was ~48–52% higher vs. with assistance. Median benefits of 54% and 43% were observed for ARIMA (with external features) and Prophet algorithms, respectively. This study demonstrates some potential applications for forecasting models for workforce health monitoring, intervention assessment, and injury prevention.

- Abstract
This paper reports initial results from a usability study conducted in the formative and user-centered design phase of a larger project to translate an existing, science-focused educational technology for neurotypical middle school students into a new, social-reasoning-focused educational technology for students on the autism spectrum. Participants in our study included both adolescents on the autism spectrum and typically developing adolescents, who were asked to complete the Betty's Brain educational-technology-based science activity as well as a social-reasoning movie question-answering activity. Results include qualitative observations of general student engagement and challenges as well as quantitative measures of performance and eye gaze, including key differences observed across our two sample groups, with the goal of informing the design and adaptation of future technology-based interventions. Our findings suggest specific considerations for designing educational technologies for adolescents on the autism spectrum, including: (1) finding ways to help students follow instructional/tutorial portions of new technologies, especially when lengthy instructions and/or complex interfaces are involved; (2) proactively anticipating and finding ways to mitigate potential student episodes of frustration / dysregulation while using the technology; and (3) capitalizing on features of the technology found to be engaging/motivating for students. - Abstract
Modern social intelligence includes the ability to watch videos and answer questions about social and theory-of-mind-related content, e.g., for a scene in "Harry Potter," "Is the father really upset about the boys flying the car?" Social visual question answering (social VQA) is emerging as a valuable methodology for studying social reasoning in both humans (e.g., children with autism) and AI agents. However, this problem space spans enormous variations in both videos and questions. We discuss methods for creating and characterizing social VQA datasets, including: (1) crowdsourcing versus in-house authoring, including sample comparisons of two new datasets that we created (TinySocial-Crowd and TinySocial-InHouse) and the previously existing Social-IQ dataset; (2) a new rubric for characterizing the difficulty and content of a given video; and (3) a new rubric for characterizing question types. We close by describing how having well-characterized social VQA datasets will enhance the explainability of AI agents and can also inform assessments and educational interventions for people.

Condensed Matter - Mesoscale and Nanoscale Physics - Abstract
Armchair phosphorene nanoribbons (APNRs) are known to be semiconductors with an indirect bandgap. Here, we propose to introduce new states in the gap of APNRs by creating a periodic structure of vacancies (antidots). Based on the tight-binding model, we show that a periodic array of vacancies or nanopores leads to the formation of an impurity band inside the gap region. We first present an analytical expression for the dispersion relation of an impurity band induced by hybridization of bound states associated with each single vacancy defect. Then, we increase the size of vacancy defects to include a bunch of atoms and theoretically investigate the effect of nanopores size and their spacing on electronic band structure, carrier transmission function, and thermoelectric properties. Our analysis of the power generation rate and thermoelectric efficiency of these structures reveals that an ANPR can be used as a superb thermoelectric power generation module.

Background: Small colony variants (SCVs) are biotypes of bacteria that have a size of approximately one-tenth or less of the wild types and has distinct characteristics comparing to the wild type strains. Clinical SCVs are usually associated with persistent infection and require a long-term treatment program with antibiotics. In Saudi Arabia, there are few studies about SCVs Escherichia coli for this reason, this study is aimed to investigate the ability of gentamicin to mutate E. coli ATCC 25922 to produce small SCVs and investigate the genotypes and phenotypes changes and stress tolerance comparing to clinical SCVs E. coli and normal clinical E. coli Isolated from blood samples. Methods: In this investigation, four clinical blood samples were collected ted from patients and the cultivation and isolation were carried out in KFMC between December 2019 and February 2021. The identification of positive blood culture samples was done using phoenix MD. Non-SCV E. coli ATCC25922 were mutated to SCV using exposure to increasing gradual concentrations of gentamicin at 100-generation intervals. Biochemical features and susceptibility to standard antibiotics using automated Phoenix MD 50 and. The survival assays were done using several stresses including heat shock, low pH, high osmotic pressure, and oxidative pressure. Virulence genes screening included the detection of genes that encoded to α-haemolysin, CS12 fimbriae, F17-like fimbrial adhesion, P-related fimbriae, yersiniabactin siderophore system, P-fimbriae, aerobactin, iron-regulated genes using PCR and gel electrophoresis. Results: The data from the mutating E. coli ATCC 25922 small colony test with gentamicin revealed that the first emergence of the multidrug resistance (MDR) SCV E. coli strain occurred at generation number 250, corresponding to a gentamicin concentration of 57 g/ml. Pathogenicity islands detection revealed that all tested E. coli strains have PAI IV 536 genes on their chromosomes furthermore, mutated SCV E. coli ATCC 25922 acquired PAII CFT073 and PAI IV 536. Survival tests showed no significant differences changes in tolerance of mutated SCVs comparing to parental strain. Conclusion: The present work concluded that gentamicin sub-MIC concentration gradual exposure can induce mutation responsible for SCV formation and evolving of MDR E. coli strains. The mutated SCVs evolved high-level aminoglycoside resistance for gentamicin and resistance to amikacin, it also developed resistance to 2 cephalosporin antibiotics cefuroxime, and cephalothin and a resistance to aztreonam.

Background: Urinary tract infection (UTI) is one of the most prevalent bacterial diseases worldwide. Escherichia coli is a well-known etiological agent of UTI. The emergence and spread of metallo-beta-lactamase (MBL)-producing E. coli is a serious threat to public health. This study aimed to investigate the antibiotic resistance pattern and prevalence of MBL-producing E. coli isolated from UTI. Methods: From January 2020 to June 2021, 1200 urine specimens were collected from patients suspected of having UTI. Antibiotic susceptibility testing was carried out by the disk diffusion method. The prevalence of MBL (blaVIM, blaIMP, blaSPM, and blaNDM) genes was determined by the polymerase chain reaction (PCR) method. Results: The highest susceptibility was observed against amikacin (96%) and gentamicin (95%). The isolates were mostly resistant against ampicillin (72%) and cephalothin (60%). All carbapenem-resistant isolates were MBL-positive. Based on the results of PCR, 75% of the isolates were blaNDM-positive. Conclusion: Resistance to some antibiotics, such as ampicillin and cephalothin, was high, and their prescription must be restricted. The prevalence of MBL-producing isolates was not high; however, due to the high level of resistance against other antibiotics, continuous monitoring of MBL-producing isolates is highly essential.

Objectives The capacity to manage financial affairs is a multidimensional construct and includes a wide range of judgment, perceptual, and pragmatic activities, ranging from basic to higher abilities. The decline of cognitive function and the ability to manage financial affairs in the elderly can cause problems in them, their families, and society. The present study aims to design and develop a tool to measure the financial capacity of older adults in Iran. Methods & Materials This pilot study was conducted using Waltz’s four-step method: Defining objectives, selecting a conceptual mod, blueprinting, and construction of measure (by application of procedures, item sets, and scoring rules). To check the validity of the tool, its face validity and content validity were evaluated. Results The initial draft included nine main areas, 27 sub-areas, and 65 items. The questionnaire was completed through a semi-structured interview by an expert. The items in each area were scored as “yes,” “no,” and “not applicable”. By developing instructions, acceptable, unacceptable, and non-applicable answers for each item were explained. The content validity ratio and content validity index for the overall scale were 0.57 and 0.83, respectively. Conclusion The designed tool can help clinicians in Iran to make more consistent judgments and more accurately identify older adults who are no longer able to manage their financial affairs.

Buildings account for a significant amount of energy consumption and greenhouse gas emissions worldwide. Electricity and fossil fuels are currently the primary sources of energy used for cooling and heating buildings, depending on the climate and location. Both energy sources are responsible for significant greenhouse gas emissions. In contrast, plants and vegetation absorb carbon dioxide and, thus, improve the quality of air. This effect indirectly influences climate change to lower energy demands and produce additional emissions due to rising energy consumption trends. Plants also reduce the ambient temperature by providing shade on roof surfaces. Hence, the large-scale deployment of green roofs reduces energy consumption, emissions, and costs. However, green roofs also impact the overall weight of a building and require additional construction costs. Therefore, the contribution of green roofs to the various structural and thermal performances of buildings varies for extensive intensive or semi-intensive systems. These interactions warranted a lifecycle analysis to optimize the extent of green roof applications. This approach highlighted sustainability performance measures, including energy, emissions, water, and waste. The presented study addressed a lifecycle analysis of green roof deployment during a hot summer in a Mediterranean climate zone. This climate applies to many areas that benefit from warming temperatures without extreme needs for cooling or heating. The emphasis on comparing two towns within the same climate zone facilitated a more detail-oriented approach to the lifecycle analysis. The results illustrated the energy consumption and associated release of greenhouse gas emissions related to structural and roofing materials and thermal operations throughout the service life of a building. The conclusions assessed the challenges and opportunities of green roof applications on new and existing buildings.

Objective Researchers have called the new century the century of communication and expansion of new businesses, in which users communicate with each other and share their products through social media platforms. In shared businesses, economic activities have different definitions. Respecting ethics and maintaining moral values is one of the essential issues in new businesses. Shared businesses have been welcomed by customers due to their non-face-to-face nature and virtual sales of products, and their ability to offer diverse and cheap products through sharing on the Internet. This research was conducted to investigate the effect of ethical leadership, responsibility, and fair behaviors on attracting and retaining customers in shared businesses. Methodology The research is applied in terms of purpose. Its statistical population included all consumers of goods, services, and ideas in shared businesses, who used sharing platforms in the past year. A researcher-made questionnaire was used to collect the required data. The reliability of the questionnaire was confirmed using Cronbach's alpha coefficient. It stood at 0.83. Due to the large size of the community, the sample size was determined to be 384 people through Cochran's formula for indeterminate communities. The questionnaire was randomly distributed among 384 customers of shared businesses in Tehran. To test the main hypotheses of the research, the structural equation model was used in the Lisrel software. To measure the compatibility of the proposed model and the data, model fitting was done. Also, the average, the standard deviation, and other required statistics were used for descriptive statistics. Findings Among the total of nine proposed hypotheses, three main hypotheses and six sub-hypotheses were confirmed. Also, the mediating role of satisfaction and purchase intention variables was proved. Conclusion According to the results, the customers of shared businesses buy the products of shared businesses in person and without physically observing the goods. These customers bear more risk compared to traditional business customers. Further studies about the ethical components of attracting and retaining customers in new shared businesses are needed. Ethical responsibility for respecting the rights of customers is a binding principle in shared businesses. Ethical behaviors in practice reflect the ethical and governance values of that company. Making ethical decisions when sharing products to attract and retain customers is more than a choice. The results of the research conducted in Tehran investigating the components of ethical leadership, responsibility, and fair behavior in attracting and retaining customers in shared businesses showed that ethical leadership has a positive effect on attracting and retaining customers in shared businesses. It seems that the nature of moral behavior can be seen in virtual environments as well.

Objective The aim of this study was to emphasize on the interaction of spatial and temporal gait parameters and analyse the gait asymmetry in the patients with lumbar disc herniation (LDH) before and after microdiscectomy. Methods This was a prospective, observational study conducted on 59 cases of LDH planned for lumbar microdiscectomy, and healthy control group with 54 participants for analysis was performed prior to surgery and 15 days after surgery. The spatiotemporal gait parameters were measured using a “Win-Track” gait analysis platform system. All the participants walked barefoot for 10 times with their normal walking speed in the same day. The 3 flawless walking data were recorded and the arithmetic means were computed. The gait symmetry index was used to calculate the walking asymmetry. The pain intensity of the patients was recorded shortly before performing the analysis by a visual analogue scale. Results In the postoperative assessment LDH patients had significantly shorter temporal parameters, longer spatial parameters, faster walking speed, and more cadence than the preoperative assessment (p < 0.05). There were improvements in the asymmetry values of the postoperative gait parameters compared to the preoperative values, but these differences were not significant (p > 0.05). In addition, there was a significant difference in all parameters in terms of gait asymmetry between the postoperative assessment and the healthy controls (p < 0.05). Conclusion These results can guide the patient-specific evaluating and implementation of gait rehabilitation programs, and design protocols before or after surgery in the LDH patients.

ABSTRACT Dementia poses a significant societal and health challenge in the 21st century, with many hospitalized patients experiencing dementia without a documented diagnosis. Objective: To evaluate the prevalence of dementia and its associated risk factors among older patients admitted to hospitals. Methods: The study included older patients (≥ 60 years) admitted to medical departments of a general hospital in three major Iranian cities. Researchers utilized the Activities of Daily Living-Instrumental Activities of Daily Living (ADL-IADL) scale, the Geriatric Depression Scale (GDS), the Mini-Cog test, the 4 A’s test (4AT), and the Abbreviated Mental Test Score (AMTS). Among the 420 recruited older inpatients, 228 (54.3%) were female. Results: The mean age of participants was 71.39 years (standard deviation ±7.95), with 30.7% diagnosed with major neurocognitive disorder (dementia). The likelihood of dementia exhibited statistically significant correlations with gender, age, number of children, and occupation. Conclusions: Screening older individuals for cognitive impairment upon hospital admission holds the potential to prevent adverse outcomes and enhance the quality of treatment for patients concurrently dealing with dementia.
