Objective To identify differences in quality of life (QoL) of patients with inflammatory bowel disease (IBD) between diagnosis (Crohn’s Disease and Ulcerative Colitis), gender (male and female), treatment condition (with and without surgery), and attachment styles (secure, preoccupied, and disconnected); to examine associations between QoL, sociodemographic, clinical, and positive psychological variables; to determine whether sociodemographic, clinical, and positive psychological variables predict QoL. Method The sample included 70 participants diagnosed with IBD (Mage = 43.37 years, SD = 12.81), of whom 71.4% were females and 67.1% had Crohn’s Disease. Positive psychological variables (meaning in life, positive body image, and attachment styles), sociodemographic (age, education, gender) and clinical variables (diagnosis, disease duration, surgery) were assessed as independent variables. QoL was the dependent variable, analyzed through four domains (physical, psychological, social, environment). Results Participants with a secure attachment style reported higher QoL (physical, psychological, and social) than participants with a preoccupied attachment style. Strong positive correlations were found between positive psychological variables and QoL. Body appreciation was a significant predictor of three QoL domains (physical, psychological, and environment). Meaning in life made a unique contribution to the social QoL regression model, and it was also a significant predictor of psychological QoL. Body acceptance by others was a significant predictor of physical QoL, whereas disease duration and education predicted environment QoL. Attachment styles did not predict any QoL domain. Conclusion The most significant predictors of QoL in patients with IBD were body appreciation and meaning in life. Body acceptance by others and body appreciation were the main predictors of physical QoL. Psychological interventions for patients who suffer from IBD should address body appreciation and meaning in life.

With the increasing number of women deprived of liberty worldwide, implementing specific strategies that should be applied to the support offered to these women are essential social measures. This study aims to analyze the supply of resources for the social reintegration of former inmates of the Brazilian prison system from a gender perspective. We propose to conduct a documentary analysis on governmental and non-governmental strategies aimed at this audience, with a gender perspective, through an analytical matrix for the 2020-2021 period. The results show several programs in the country aimed at the social reintegration of former prisoners; however, few have a gender perspective. The theme of social reintegration and the prison system was identified in 84 news items on government agencies' websites, 20 of which were federal and 64 state, in 11 international organizations operating in Brazil, and 12 NGOs. Only six had a gender profile. The challenge for the Brazilian penitentiary system is to introduce the citizenship and human dignity approach in the prison system, including an approach to the gender issue.

OBJETIVE: To estimate the prevalence of psychological, physical, and sexual violence perpetrated against women by their intimate partner (IP) in Quilombola communities located in Espírito Santo State, Brazil. METHODS: The data is from a population-based cross-sectional study of Quilombola women conducted from 2017 to 2018. In-person interviews collected information on women’s sociodemographic characteristics, behaviors, and their experience of violence perpetrated by their IP. The analysis used chi-square test and hierarchical logistic regression. RESULTS: 219 women (94.8% of the invited ones) agreed to participate in the study. 59.0% (95%CI: 5.25–65.5) reported psychological violence; 41% (95%CI: 34.5–47.5) physical violence; and 8.2% (95%CI: 4.6–11.8) sexual violence. Psychological violence was associated with having three or more sexual partners in life, when compared to those who had up to two partners (p = 0,009), and previous violence involving other people outside of family increased the chance of suffering psychological violence by an IP more than nine times (p ≤ 0.001). Regarding physical violence, the association with use of barrier contraception (p = 0.031) and having a partner with other sexual partners (p = 0.024) were protective factors for IP violence. Having 3 or more sexual partners in the last 12 months (p = 0.006), partner using illicit drugs (p = 0,006), and alcoholism in the family (p = 0,001), increased the chance of suffer physical violence by the partner. Sexual violence perpetrated by the IP was associated with miscarriage (p = 0.016), partner using drugs (p = 0.020), and gynecological symptoms (p = 0.045). CONCLUSIONS: These results showed the high frequency of intimate partner violence in Quilombola women and highlight the importance of reducing social and race inequities for interrupting the culture of violence against women.

Wind energy is a renewable source, cheap and requires little maintenance, so its implementation is being expanded, including in Brazil. Currently, there is a large investment in the deployment of wind power in Pernambuco, in 2016 two wind complexes were installed in the state. However, there is controversy regarding an environmentally friendly energy that can impact fauna and flora, noise, glove reduction, temperature change and local climate change over the years. Thus, the need for interviewing residents of wind farm areas, comparing reports with capture data on the APAC website, comparing NDVI images after wind turbines installation was visualized. Based on the capture results, NDVI images and resident reports, it can be observed that there was a reduction in rainfall rates, however, this period corresponds to a prolonged drought in the northeastern semiarid, thus, a towers installation and analysis. may have aggravated a situation in the region. KEYWORDS: Wind Energy, Rainfall, Climate Change, NDVI. PALAVRAS-CHAVE: Energia Eólica, Precipitação, Alteração do Clima, NDVI.

As in any other sector, the construction industry needs to improve its processes and create differentiations in the face of its competitors, in order to increase profits and minimize losses. Because of this, top management seeks solutions to make their company stand out in the market. Traditional capital budgeting evaluation models, such as the Discounted Cash Flow (DCF) method, do not consider the value of the uncertainties and the flexibility that may exist in the project. With this in mind, this paper analyzes, through the Real Options approach, a way to minimize the risks of a residential project by determining the optimal time to expand it. To this end, we will use data from a residential development located in the western zone of the city of Rio de Janeiro, which will be submitted to a model based on Real Options guidelines. The results obtained in the three scenarios analyzed in the numerical model indicate that when the sales velocity is constant, it is best to defer the expansion as long as possible since this postpones the investment costs and increases the revenues.

We aimed to identify the main challenges in the gynecological care and management of patients with intellectual disability by gynecologists. Gynecologists answered to online questionnaires about situations in which the professional reports their confidence in caring for patients with intellectual disability. Were studied 143 responses, 75% women. Among them, 53.8% felt apprehensive to conduct a routine gynecological screening, 60% were confident in advising about contraceptive services and examining the genitals. The greatest difficulty was indicating drugs to suppress menstruation and managing patients with severe intellectual disability. The female doctors were more prepared in the care of disability patients and recent certified gynecologists were more insecure. The main difficulty was time for examinations (39.2%) and the main solution was to improve medical training (64.2%). The main challenges of the gynecologist included the amount of time needed to provide clinical services, the lack of knowledge and skill; and ethical concerns.

Gastric duplication cyst (GDC) is a rare congenital abnormality and the development of malignant transformation in these lesions is even rarer, with only few reported cases worldwide to date. We hereby report an additional case of cancer arising from a GDC in a 54-year-old male. The patient's chief complaints were abdominal pain and significant weight loss. Computed tomography and endoscopy ultrasonography (EUS) revealed a nodular formation with a cystic component, localized in the great gastric curvature and invading the spleen and left adrenal gland. The biopsy from EUS was inconclusive. After exploratory laparotomy, the patient was submitted to an en-bloc resection with partial gastrectomy, splenectomy and left adrenalectomy. Histopathologic examination revealed a cystic mass non-communicating with the gastric wall. Immunohistochemistry staining showed a moderately differentiated pancreatobiliary adenocarcinoma within a duplication cyst with lymphovascular and perineural invasion. The patient was proposed to adjuvant systemic treatment, however, after few months he developed metachronous metastasis. To our knowledge, this is the first case of adenocarcinoma with pancreatobiliary differentiation arising from a gastric duplication cyst.

Asthma is an inflammatory lung disease that affects more women than men in adulthood. Clinical evidence shows that hormonal fluctuation during the menstrual cycle and menopause are related to increased asthma severity in women. Considering that life expectancy has increased and that most women now undergo menopause, strategies to prevent the worsening of asthma symptoms are particularly important. A recent study from our group showed that re-exposure of ovariectomised allergic mice to antigen (ovalbumin) leads to an exacerbation of lung inflammation that is similar to clinical conditions. However, little is known about the role of probiotics in the prevention of asthma exacerbations during the menstrual cycle or menopause. Thus, our objective was to evaluate the effects of supplementation with kefir, a popular fermented dairy beverage, as a preventive strategy for modulating allergic disease. The results show that the preventive kefir administration decreases the influx of inflammatory cells in the airways and exacerbates the production of mucus and the interleukin 13 cytokine. Additionally, kefir changes macrophage polarisation by decreasing the number of M2 macrophages, as shown by RT-PCR assay. Thus, kefir is a functional food that potentially prevents allergic airway inflammation exacerbations in ovariectomised mice.

Aims To evaluate prescription of prophylactic treatment before and after consultation in a neurology headache clinic and to determine predictors for prophylactic treatment and clinical improvement. Methods Clinical records of consecutive patients assessed in a neurologic headache clinic in Portugal and diagnosed with acute or chronic migraine and/or tension-type headache were assessed. Prescription of prophylaxis before and after the first visit to the clinic were compared. Logistic regression was used to evaluate predictors of the need for therapeutic intervention and clinical improvement. Results Among 409 patients (86.8% women; mean age 41.6 years), 315 (77%) had indication for prophylaxis, and 70 (22%) of these patients were already on prophylactic treatment. Among the 265 patients with information for follow-up, prophylactic treatment was added in 178 (67.2%), and there was a significant change in the number of treated patients between the first and second visits. Ongoing treatment was switched or the dose increased in 21 patients. Multivariate logistic regression revealed that women (odds ratio [OR] = 2.09, 95% confidence interval [CI] 1.1 to 3.97] and patients with medication overuse headache (MOH) (OR = 6.97, 95% CI 1.60 to 30.39) were more likely to need therapeutic intervention, whereas patients referred from the emergency room were less likely to need it (OR = 0.44, 95% CI 0.22 to 0.89). Of the 265 patients, 185 (69.8%) had improved at a follow-up. Having prophylactic treatment at the time of the second visit was associated with improvement (OR = 2.39, 95% CI 1.23 to 4.63; P = .01). Conclusion Women and medication overuse headache patients were more likely to need therapeutic intervention. However, only a minority of patients with treatment indication were treated before their first visit to the headache clinic. Prophylaxis prescription was associated with clinical improvement at follow-up.

Body appreciation is a key characteristic, and the most common operationalization, of positive body image. Positive body image is often measured using the Body Appreciation Scale-2 (BAS-2; Tylka & Wood-Barcalow, 2015a), which has demonstrated good psychometric properties in adult samples. The purpose of this study is to extend the validation of the BAS-2 to late adulthood facilitating future research with this population. A total of 202 male and female participants (aged 65–91 years) completed the Portuguese version of the BAS-2, along with other body image-related instruments and measures of self-esteem and intuitive eating. Confirmatory factor analysis supported the one-factor model and demonstrated sex invariance. There were no significant sex differences in BAS-2 scores. The BAS-2 had satisfactory convergent, discriminant, and incremental validity. Internal consistency and test–retest stability estimates were adequate. The BAS-2 will likely meet the needs of researchers interested in exploring body appreciation among elderly people.

The authors wish to acknowledge the financial support of the CoordenacAo de Aperfeicoamento de Pessoal de Nivel Superior (CAPES) for their financial support., Preterm birth (PTB) is a phenomenon that brings risks and challenges for the survival of the newborn child. Despite many advances in research, not all the causes of PTB are already clear. It is understood that PTB risk is multi-factorial and can also be associated with socioeconomic factors. Thereby, this article seeks to use unsupervised learning techniques to stratify PTB risk in Brazil using only socioeconomic data. Through the use of datasets made publicly available by the Federal Government of Brazil, a new dataset was generated with municipality-level socioeconomic data and a PTB occurrence rate. This dataset was processed using various unsupervised learning techniques, such as k-means, principal component analysis (PCA), and density-based spatial clustering of applications with noise (DBSCAN). After validation, four clusters with high levels of PTB occurrence were discovered, as well as three with low levels. The clusters with high PTB were comprised mostly of municipalities with lower levels of education, worse quality of public services—such as basic sanitation and garbage collection—and a less white population. The regional distribution of the clusters was also observed, with clusters of high PTB located mostly in the North and Northeast regions of Brazil. The results indicate a positive influence of the quality of life and the offer of public services on the reduction in PTB risk., Coordenacao de Aperfeicoamento de Pessoal de Nivel Superior (CAPES)

Introduction and objectives: Chronic obstructive pulmonary disease (COPD) is a major public health problem. The aim of this study was to ascertain the prevalence of COPD and whether such prevalence was positively or negatively associated with COPD admissions, using all the data of a regional health care system. Materials and methods: We designed a descriptive cross-sectional study which included all subjects aged over 45 years, diagnosed with COPD in primary care in 2013. We also calculated the number of such patients who had a record of hospital admissions due to this disease. COPD prevalence and incidence of admissions were calculated. Poisson regression models were then used to analyse the association between cases with diagnosis of COPD and admissions due to COPD, by sex, adjusting for socio-demographic variables and distance to hospital. Sensitivity subanalyses were performed by reference to the respective municipal rurality indices. Results: Median municipal prevalence of COPD was 5.29% in men and 2.19% in women. Among patients with COPD, 28.22% of men and 16.00% of women had at least one hospital admission. The relative risk of admission per unit of the standardised prevalence ratio was 0.37 (95% CI 0.34–0.41) for men and 0.39 (95% CI 0.34–0.45) for women. Conclusions: There is a significant negative association between COPD prevalence and hospital admissions due to this disease. The proportion of admissions is lower in municipalities lying furthest from hospitals. There is considerable municipal variability in terms of COPD prevalence and proportion of admissions. In-depth attention should be given to disease-management training programmes. This study aims to describe the epidemiological profile of patients co-infected with HTLV-1 and Strongyloides stercoralis, from the Center for Tropical Medicine (NTM) located at the Federal University of Pará (UFPA). A quantitative, retrospective and cross-sectional analysis was performed, using information from patients with HTLV-positive serology, confirmed by the Polymerase Chain Reaction (PCR) for HTLV-1 or HTLV-2, in addition to parasitological tests confirmed by the Hoffman method. Of the 4.679 patients examined, 538 positive samples were detected, 432 for HTLV-1 (80%) and 106 (20%) for HTLV-2. In the parasitological examination, 14 samples positive for S. stercoralis and 81 negative were obtained from a total of 95 patients who took their material for analysis, were it was possible to determine a 14,7% of coinfection prevalence’s between the two infectious agents. There was a predominance of HTLV-1 positive female patients over male patients. This can occur due to the ease of transmission through sexual intercourse and also to the hormonal effects of women, making them more susceptible to infection. From that, it was found that many patients with HTLV-1/2 may also have co-infection with S. Stercoralis, especially in the state of Pará.

Background Casbene synthase (CS) is responsible for the first committed step in the biosynthesis of phorbol esters (PE) in the Euphorbiaceae. PE are abundant in the seeds of the biofuel crop Jatropha curcas and its toxicity precludes the use of the protein-rich cake obtained after oil extraction as an animal feed and the toxicity of the fumes derived from burning PE containing biofuel is also a matter of concern. This toxicity is a major hindrance to exploit the potential of this crop as a source of raw material to produce biodiesel. For this reason, the current research on J. curcas is mainly focused on the understanding of the biosynthesis and site of synthesis of PE, as an avenue for the development of genotypes unable to synthesize PE in its seeds. Results Here, we present targeted proteomics assays (SRM and PRM) to detect and quantify CS in leaves, endosperm, and roots of two J. curcas genotypes with contrasting levels of PE. These assays were based on the use of reference isotopic labeled synthetic peptides (ILSP) predicted from 12 gene models of CS from the J. curcas genome. Conclusion Our targeted proteomics methods were able to detect and quantify, for the first time, CS gene products and demonstrate the distribution of CS isoforms only in roots from J. curcas genotypes with a high and low concentration of PE. These methods can be expanded to monitor CS, at the protein level, in different tissues and genotypes of J. curcas.

As a mini city with a range of activities, a university campus can be used as a simulation and living lab. Smartness is a quality of contributing to sustainable development and resilience, and results achieved in a smart campus can be extrapolated to real and larger city as well. Indicators and targets not only represent important tools for smart cities planning but also for civil society participation and engagement. This paper reports the experience of incorporating data science and IoT in university activities, through a continuous and smart monitoring system. The results related to energy efficiency are presented, in order to share the positive and negative aspects of indicators measurement and control, improving decision making addressed to university campus sustainability and smartness.

Genetic testing is currently the leading edge of clinical care when it comes to diagnostics. However, many questions remain unanswered even when employing next-generation sequencing techniques due to our inability to decode genetic variations and our limited repertoire of available diagnoses. Accordingly, diagnostic yields for current genomic screenings are50% and fail to provide the whole picture, leaving the remaining patients without a definitive diagnosis. Human phenotypic/disease expression is explained by alterations not only at the genome, but also at the transcriptome, proteome and metabolome levels. These "higher" complexity levels represent at wealth of information, and diagnostic screenings tests at these levels have been shown to significantly improve diagnostic yields in specific populations compared to conventional diagnostic workup or gold standards in use (7-30% increase in diagnostic yields, depending on the population, approach and gold standard being compared against). However, these are not yet routinely available to clinicians. Due to their dynamic and modifiable nature, tapping into data from different omics will improve our understanding of the pathophysiological bases underlying (many yet to characterize) human disorders. We herein review how alterations at these levels (e.g. post-transcriptional and post-translational) may be pathogenic, how such tests may be implemented and in which situations they are of significant utility.

Cross-sectional studies suggest a correlation between alterations in dream content reports and executive dysfunction tests in Parkinson's disease (PD), but this has not been assessed in longitudinal studies. Our objective was to assess the predictive value of dream content for progression of cognitive dysfunction in PD. We prospectively addressed all consecutive, non-demented patients with PD attending an outpatient clinic during a 1-year period. Dream reports were collected at baseline by means of a dream diary and analysed according to the Hall and Van de Castle system. Patients were assessed at baseline for rapid eye movement sleep behaviour disorder, motor stage, mood disorder and psychosis. The Montreal Cognitive Assessment (MoCA) was applied at baseline and 4 years later. Linear regression analysis was used to the test the relation between each dream index (predictors), demographic and other motor and non-motor variables (covariates), and change in MoCA scores (dependent variable). In all, 58 patients were assessed at both time points and 23 reported at least one dream (range 1-27, total 148). Aggression, physical activities, and negatively toned content predominated in dream reports. The MoCA scores decreased significantly from baseline to follow-up. In the multivariate model, negative emotion index was the strongest predictor of cognitive decline. We found a significant positive association between negative emotions in dreams at baseline and subsequent reduction in MoCA scores. These findings suggest that some dream content in patients with PD could be considered a predictor of cognitive decline, independent of other factors known to influence either dream content or cognitive deterioration.

This article aims to address omni-channel communication between provider and consumer in tourism services, more specifically between travel agencies and their private consumers. The omni-channel approach, which aims to integrate online and offline communication, is driven by the synergistic management of channels and consumer contact points, enabling the consumer to enjoy a more complete and optimized cross-channel shopping experience, which translates into a consistent and continuous shopping experience across multiple channels, be it communication or distribution. The objectives of this study are: i) to identify customary omni-channel communication practices between agencies and their private consumers; ii) to understand the omni-channel communication attributes that private consumers value most in their relationship with travel agencies; and iii) to suggest service procedures to the travel agencies in order to make their service more effective. We used participant observation in travel agencies with different market positioning and interviews with their leaders, in order to identify their service practices. Subsequently, interviews were conducted with travel agencies’ private consumers to determine which aspects of interpersonal communication were most valued by them. The results suggest that interpersonal communication is the most effective in creating a greater proximity and a better relationship with the consumer (being privileged by both travel agencies and consumers), while nonverbal communication validates the trust that consumers can place in the agent and in the travel agency. However, it should be noted that while agencies are aware of some procedures they can take to make their service more personalized and in accordance with their positioning (and consumer demand), they have not yet put them into practice.

Objective To assess the predictive value of polysomnographic (PSG) data in the prospective assessment of cognitive, motor, daytime and nighttime sleep dysfunction in Parkinson's Disease (PD) patients. Methods PD patients were assessed at baseline with video-PSG and with cognitive (MoCA), Sleep (SCOPA-Sleep Nighttime and Daytime scores) and Motor (UPDRSIII) function scales at both baseline and four years later. Linear regression analysis was used to assess the relation between PSG variables at baseline and change in symptoms scores. Results We included a total of 25 patients, 12 with rapid eye movement (REM) sleep behavior disorder (RBD) (in 8 PSG was inconclusive, due to lack of REM sleep). MoCA scores decreased significantly at follow-up, while SCOPA-Sleep Daytime and SCOPA-Sleep Nighttime and UPDRSIII did not vary. Lower N3 percentage at baseline was significantly associated with MoCA decrease. Higher Periodic Limb Movements in Sleep index (PLMS) and the presence of RBD were significantly associated with SCOPA daytime score increase. Higher global severity of RBD, tonic RSWA and total number of motor events during REM sleep were associated with SCOPA Nighttime score increase. Conclusions The present work suggests that PSG data could be useful for predicting PD cognitive and sleep dysfunction progression. Reduced SWS could predict deterioration of cognitive function, while baseline PLMS could be useful to predict worsening of daytime sleep dysfunction. Severity of RBD could be used for estimating nighttime sleep symptoms progression.

Our objectives were to assess the prevalence of REM sleep behaviour disorder in patients with Essential Tremor, using video-polysomnography and to compare REM sleep behaviour disorder features in essential tremor with those of patients with alpha-synucleinopathies. Forty-nine patients with essential tremor were screened with the REM Sleep Behaviour Disorder Screening Questionnaire. Patients scoring positive and those with spontaneous complaints of REM sleep behaviour disorder (n = 6) underwent video-polysomnography. The clinical features of essential tremor were compared between patients with and without REM sleep behaviour disorder. Video-polysomnography data were compared between patients who had essential tremor and Parkinson's disease with REM sleep behaviour disorder and those with idiopathic REM sleep behaviour disorder. Fourteen patients (23.5%) screened positive for REM sleep behaviour disorder, confirmed by video-polysomnography in five (11.6%). All patients with essential tremor and REM sleep behaviour disorder had rest tremor, compared with 13 (34.2%) in the group with essential tremor but without REM sleep behaviour disorder (p = .009). In video-polysomnography, patients with essential tremor and REM sleep behaviour disorder were similar to patients with Parkinson's disease with REM sleep behaviour disorder and presented worse sleep dysfunction and lower severity of REM sleep behaviour disorder compared to those with idiopathic REM sleep behaviour disorder. We found a high prevalence of REM sleep behaviour disorder in patients with essential tremor, associated with a predominance of rest tremor. Polysomnography data from patients with essential tremor and REM sleep behaviour disorder were similar to those in patients with Parkinson's disease. This suggests a relation between this subgroup of patients with essential tremor and the alpha-synucleinopathies.

Percutaneous approach for valvular heart disease is accepted as a safe and feasible strategy to treat patients considered at high surgical risk with aortic stenosis and mitral regurgitation. Additionally, the growing experience led to the increasing use of transcatheter aortic valve implantation in several indications, such as in pure aortic regurgitation. The authors present the case of a 72-year-old woman with prohibitive surgical risk due to several comorbidities, including a chronic arterial dissection from the right carotid artery to the femoral arteries bilaterally that presented with signs and symptoms of heart failure, having a transthoracic echocardiography that showed severe functional mitral and aortic regurgitation. Combined percutaneous intervention for multivalvular disease in this clinical scenario is challenging, and the reported experience is still scarce and limited to inoperable patients.
