Abstract

Background: Studies have shown associations between mortality and long-term exposure to particulate matter air pollution. Few cohort studies have estimated the effects of the elemental composition of particulate matter on mortality. Objectives: Our aim was to study the association between natural-cause mortality and long-term exposure to elemental components of particulate matter. Methods: Mortality and confounder data from 19 European cohort studies were used. Residential exposure to eight a priori-selected components of particulate matter ( PM) was characterized following a strictly standardized protocol. Annual average concentrations of copper, iron, potassium, nickel, sulfur, silicon, vanadium, and zinc within PM size fractions <= 2.5 mu m (PM2.5) and <= 10 mu m (PM10) were estimated using land-use regression models. Cohort-specific statistical analyses of the associations between mortality and air pollution were conducted using Cox proportional hazards models using a common protocol followed by meta-analysis. Results: The total study population consisted of 291,816 participants, of whom 25,466 died from a natural cause during follow-up (average time of follow-up, 14.3 years). Hazard ratios were positive for almost all elements and statistically significant for PM2.5 sulfur (1.14; 95% CI: 1.06, 1.23 per 200ng/m(3)). In a two-pollutant model, the association with PM2.5 sulfur was robust to adjustment for PM2.5 mass, whereas the association with PM2.5 mass was reduced. Conclusions: Long-term exposure to PM2.5 sulfur was associated with natural-cause mortality. This association was robust to adjustment for other pollutants and PM2.5.

Abstract

There is increasing awareness amongst the general public about exposure to atmospheric pollution while travelling in urban areas especially when taking active travelling modes such as walking and cycling. This study presents a comparative investigation of ultrafine particles (UFP), PM10, PM2.5, PM1 exposure levels associated with four transport modes (i.e., walking, cycling, car, and subway) in the city of Milan measured by means of portable instruments. Significant differences in particle exposure between transport modes were found. The subway mode was characterized by the highest PM mass concentrations: PM10, PM2.5, PM1 subway levels were respectively about 2-4-3 times higher than those of the car and open air active modes (i.e. cycling and walking). Conversely, these latter modes displayed the highest UFP levels about 2 to 3 times higher than the subway and car modes, highlighting the influence of direct traffic emissions. The car mode (closed windows, air conditioning and air recirculation on) reported the lowest PM and UFP concentration levels. In particular, the open-air/car average concentration ratio varied from about 2 for UFP up to 4 for PM1 and 6 for PM10 and PM2.5, showing differences that increase with increasing particle size. This work points out that active mode travelling in Milan city centre in summertime results in higher exposure levels than the car mode. Walkers' and cyclists' exposure levels is expected to be even higher during wintertime, due to the higher ambient PM and UFP concentration. Intervent ions intended to re-design the urban mobility should therefore include dedicated routes in order to limit their exposure to PM and UFP by increasing their distance from road traffic. Abstract

BACKGROUND Various retrospective studies have reported on the increase of mortality risk due to higher diurnal temperature range (DTR). This study projects the effect of DTR on future mortality across 445 communities in 20 countries and regions. METHODS DTR-related mortality risk was estimated on the basis of the historical daily time-series of mortality and weather factors from Jan 1, 1985, to Dec 31, 2015, with data for 445 communities across 20 countries and regions, from the Multi-Country Multi-City Collaborative Research Network. We obtained daily projected temperature series associated with four climate change scenarios, using the four representative concentration pathways (RCPs) described by the Intergovernmental Panel on Climate Change, from the lowest to the highest emission scenarios (RCP 2.6, RCP 4.5, RCP 6.0, and RCP 8.5). Excess deaths attributable to the DTR during the current (1985-2015) and future (2020-99) periods were projected using daily DTR series under the four scenarios. Future excess deaths were calculated on the basis of assumptions that warmer long-term average temperatures affect or do not affect the DTR-related mortality risk. FINDINGS The time-series analyses results showed that DTR was associated with excess mortality. Under the unmitigated climate change scenario (RCP 8.5), the future average DTR is projected to increase in most countries and regions (by -0·4 to 1·6°C), particularly in the USA, south-central Europe, Mexico, and South Africa. The excess deaths currently attributable to DTR were estimated to be 0·2-7·4%. Furthermore, the DTR-related mortality risk increased as the long-term average temperature increased; in the linear mixed model with the assumption of an interactive effect with long-term average temperature, we estimated 0·05% additional DTR mortality risk per 1°C increase in average temperature. Based on the interaction with long-term average temperature, the DTR-related excess deaths are projected to increase in all countries or regions by 1·4-10·3% in 2090-99. INTERPRETATION This study suggests that globally, DTR-related excess mortality might increase under climate change, and this increasing pattern is likely to vary between countries and regions. Considering climatic changes, our findings could contribute to public health interventions aimed at reducing the impact of DTR on human health. FUNDING Korea Ministry of Environment.

Abstract

Background: Heterogeneity in temperature-mortality relationships across locations may partly result from differences in the demographic structure of populations and their cause-specific vulnerabilities. Here we conduct the largest epidemiological study to date on the association between ambient temperature and mortality by age and cause using data from 532 cities in 33 countries., Methods: We collected daily temperature and mortality data from each country. Mortality data was provided as daily death counts within age groups from all, cardiovascular, respiratory, or noncardiorespiratory causes. We first fit quasi-Poisson regression models to estimate location-specific associations for each age-by-cause group. For each cause, we then pooled location-specific results in a dose-response multivariate meta-regression model that enabled us to estimate overall temperature-mortality curves at any age. The age analysis was limited to adults., Results: We observed high temperature effects on mortality from both cardiovascular and respiratory causes compared to noncardiorespiratory causes, with the highest cold-related risks from cardiovascular causes and the highest heat-related risks from respiratory causes. Risks generally increased with age, a pattern most consistent for cold and for nonrespiratory causes. For every cause group, risks at both temperature extremes were strongest at the oldest age (age 85 years). 140. Excess mortality during the warm summer of 2015 in Switzerland.

Abstract

Question Under Study: In Switzerland, summer 2015 was the second warmest summer for 150 years (after summer 2003). For summer 2003, a 6.9% excess mortality was estimated for Switzerland, which corresponded to 975 extra deaths. The impact of the heat in summer 2015 in Switzerland has not so far been evaluated., Methods: Daily age group-, gender- and region-specific all-cause excess mortality during summer (June-August) 2015 was estimated, based on predictions derived from quasi-Poisson regression models fitted to the daily mortality data for the 10 previous years. Estimates of excess mortality were derived for 1 June to 31 August, at national and regional level, as well as by month and for specific heat episodes identified in summer 2015 by use of seven different definitions., Results: 804 excess deaths (5.4%, 95% confidence interval [CI] 3.0‒7.9%) were estimated for summer 2015 compared with previous summers, with the highest percentage obtained for July (11.6%, 95% CI 3.7‒19.4%). Seventy-seven percent of deaths occurred in people aged 75 years and older. Ticino (10.3%, 95% CI -1.8‒22.4%), Northwestern Switzerland (9.5%, 95% CI 2.7‒16.3%) and Espace Mittelland (8.9%, 95% CI 3.7‒14.1%) showed highest excess mortality during this three-month period, whereas fewer deaths than expected (-3.3%, 95% CI -9.2‒2.6%) were observed in Eastern Switzerland, the coldest region. The largest excess estimate of 23.7% was obtained during days when both maximum apparent and minimum night-time temperature reached extreme values (+32 and +20 °C, respectively), with 31.0% extra deaths for periods of three days or more., Conclusions: Heat during summer 2015 was associated with an increase in mortality in the warmer regions of Switzerland and it mainly affected older people. Estimates for 2015 were only a little lower compared to those of summer 2003, indicating that mitigation measures to prevent heat-related mortality in Switzerland have not become noticeably effective in the last 10 years.

Abstract

Advances in microsensor technologies for air pollution monitoring encourage a growing use of portable sensors. This study aims at testing their performance in the development of exposure surfaces for nitrogen dioxide (NO2) and ozone (O3). In Montreal, Canada, a data-collection campaign was conducted across three seasons in 2014 for 76 sites spanning the range of land uses and built environments of the city; each site was visited from 6 to 12 times, for 20 min, using NO2 and O3 sensors manufactured by Aeroqual. Land-use regression models were developed, achieving R(2) values of 0.86 for NO2 and 0.92 for O3 when adjusted for regional meteorology to control for the fact that all of the locations were not monitored at the same time. A total of two exposure surfaces were then developed for NO2 and O3 as averages over spring, summer, and fall. Validation against the fixed-station data and previous campaigns suggests that Aeroqual sensors tend to overestimate the highest NO2 and O3 concentrations, thus increasing the range of values across the city. However, the sensors suggest a good performance with respect to capturing the spatial variability in NO2 and O3 and are very convenient to use, having great potential for capturing temporal variability.

Abstract

There is a lack of studies assessing the exposure-response relationship between transportation noise and annoyance in North America. Our aims were to investigate the prevalence of noise annoyance induced by road traffic, trains and airplanes in relation to distance to transportation noise sources, and to total environmental noise levels in Montreal, Canada; annoyance was assessed as noise-induced disturbance. A telephone-based survey among 4336 persons aged >18 years was conducted. Exposure to total environmental noise (A-weighted outdoor noise levels-LAeq24h and day-evening-night equivalent noise levels-Lden) for each study participant was determined using a statistical noise model (land use regression-LUR) that is based on actual outdoor noise measurements. The proportion of the population annoyed by road traffic, airplane and train noise was 20.1%, 13.0% and 6.1%, respectively. As the distance to major roads, railways and the Montreal International Airport increased, the percentage of people disturbed and highly disturbed due to the corresponding traffic noise significantly decreased. When applying the statistical noise model we found a relationship between noise levels and disturbance from road traffic and total environmental noise, with Prevalence Proportion Ratios (PPR) for highly disturbed people of 1.10 (95% CI: 1.07-1.13) and 1.04 (1.02-1.06) per 1 dB(A) Lden, respectively. Our study provides the first comprehensive information on the relationship between transportation noise levels and disturbance in a Canadian city. LUR models are still in development and further studies on transportation noise induced annoyance are consequently needed, especially for sources other than road traffic.

Abstract

We simulated commuter routes and long-term exposure to traffic-related air pollution during commute in a representative population sample in Basel (Switzerland), and evaluated three air pollution models with different spatial resolution for estimating commute exposures to nitrogen dioxide (NO2) as a marker of long-term exposure to traffic-related air pollution. Our approach includes spatially and temporally resolved data on actual commuter routes, travel modes and three air pollution models. Annual mean NO2 commuter exposures were similar between models. However, we found more within-city and within-subject variability in annual mean (±SD) NO2 commuter exposure with a high resolution dispersion model (40 ± 7 µg m(-3), range: 21-61) than with a dispersion model with a lower resolution (39 ± 5 µg m(-3); range: 24-51), and a land use regression model (41 ± 5 µg m(-3); range: 24-54). Highest median cumulative exposures were calculated along motorized transport and bicycle routes, and the lowest for walking. For estimating commuter exposure within a city and being interested also in small-scale variability between roads, a model with a high resolution is recommended. For larger scale epidemiological health assessment studies, models with a coarser spatial resolution are likely sufficient, especially when study areas include suburban and rural areas.
