The common method of closing conjunctival incisions is by suturing, which is associated with several disadvantages. It requires skill to apply and does not always provide a watertight closure, which is required in some operations (e.g., glaucoma filtration). The purpose of the present study was to evaluate laser soldering as an alternative method for closing conjunctival incisions. Conjunctival incisions of 20 ex vivo porcine eyes were laser soldered using a temperature-controlled fiberoptic laser system and an albumin mixed with indocyanine green as a solder. The control group consisted of five repaired incisions by a 10-0 nylon running suture. The leak pressure of the repaired incisions was measured. The mean leak pressure in the laser-soldered group was 132 mm Hg compared to 4 mm Hg in the sutured group. There was no statistically significant difference in both the incision's length and distance from the limbus between the groups, before and after the procedure, indicating that there was no severe thermal damage. These preliminary results clearly demonstrate that laser soldering may be a useful method for achieving an immediate watertight conjunctival wound closure. This procedure is faster and easier to apply than suturing.

Background and Objective: In today's age of advancing surgical technology, there is a need for better and simpler methods of tissue bonding. The use of lasers for tissue welding or soldering is one of these sutureless methods. In 30 years of laser tissue bonding (LTB) research, published reports of cylindrical organ anastomosis were limited to small diameters. The tension resisted by the anastomosis, which is caused by the intraluminal pressure, is also proportional to the organ diameter. Therefore the anastomosis of large diameter organs requires significantly stronger mechanical strength. The aim of this study is to demonstrate such an anastomosis., Materials and Methods: In vitro anastomosis of porcine small bowel was performed by either LTB or sutures. Anastomosis in the laser group (number of samples, n = 15) included two main stages of soldering. The bowel edges were approximated over a solid albumin stent and heated with a temperature controlled GaAs laser system to 75 degrees C. This was followed by spreading liquid albumin on the anastomotic line and heating by the same system again to 75 degrees C. The control group (n = 5) was sutured anastomosis. All anastomoses were assessed by burst pressure measurement., Results: The burst pressure of the laser group attained 170+/-40 mmHg which was significantly higher than that of the sutured group 83+/-37 mmHg (P < 0.001)., Conclusion: This report describes the in vitro LTB anastomosis of a large diameter cylindrical organ. The immediate bond strength, as estimated by burst pressure measurements, was double compared to sutured anastomosis.

Several studies have examined the use of room air for the resuscitation of newborns., Objective: To assess the effects of room air resuscitation versus 100% oxygen resuscitation on mortality at 1 week and 1 month in asphyxiated newborn infants., Study Design: Systematic review and meta-analysis of seven randomized and quasi-randomised controlled trials comparing room air and 100% oxygen resuscitation of newborn infants., Results: Compared to the 100% oxygen resuscitation group, neonates in the room air resuscitation group had a lower mortality both in the first week of life (odds ratio 0.70, 95% CI 0.50, 0.98) and at 1 month and beyond (odds ratio 0.63, 95% CI 0.42, 0.94). The incidence of severe hypoxic ischemic encephalopathy (Stage II and Stage III) was similar between the two groups., Conclusion: This meta-analysis supports the hypothesis that room air is superior to 100% oxygen as the initial choice for resuscitating clinically depressed newborns as it may result in a lower mortality rate. However, adequately powered studies of long-term neurodevelopmental outcomes are not yet available.

Objectives: There has been growing interest in the use of serum B-type natriuretic peptide (BNP) and the N-terminal segment of its pro-hormone (NT-proBNP) as biomarkers for cardiac disease. The aim of this review is to summarize the current state of knowledge regarding BNP and NT-proBNP measurement in the pediatric population., Design and Methods: A computerized literature search on the National Library of Medicine was done and all articles including BNP and pediatrics were selected and discussed., Results: The data from several studies suggest that the measurement of BNP may be useful in diagnosing and managing pediatric heart failure, congenital heart disease, cardiac transplantation and patients on chemotherapy. There are difficulties in establishing appropriate reference ranges in children., Conclusion: There is insufficient evidence for the routine use of BNP or NT-proBNP. Further research to clearly define the clinical utility in the pediatric age group is eagerly anticipated.

Objective: To describe the changes in oxygen saturation (SpO2) in healthy infants during the first 10 minutes of life., Study Design: In this observational study, infants > or = 35 weeks gestation at birth who did not require supplemental oxygen had continuous recordings taken of the preductal SpO2 over the first 10 minutes of life., Results: A total of 115 infants were analyzed. On average, infants delivered by cesarean delivery had a 3% lower SpO2 than infants delivered by vaginal delivery (95% confidence interval [CI] = -5.8 to -0.7; P = .01). Infants born by cesarean delivery also took longer (risk ratio, 1.79) to reach a stable SpO2 > or = 85% (95% CI = 1.02 to 3.14; P = .04). At 5 minutes of age, median SpO2 values (interquartile range) were 87% (80% to 95%) for infants delivered vaginally and 81% (75% to 83%) for those delivered through cesarean section. The median SpO2 did not reach 90% until 8 minutes of age in either group., Conclusions: The process of transitioning to a normal postnatal oxygen saturation requires more than 5 minutes in healthy newborns breathing room air.

We demonstrate a new optical imaging technique based on a directional detector that measures the intensity of light waves that propagate only in a narrow angular window around a specific direction. Light waves that propagate in other directions do not significantly affect the detector output. The directional detector is obtained by illuminating the interrogated object with a high-coherence light source and measuring the interference between the light wave reflected from the object and a reference wave. By measuring the intensity of the interference pattern with an optical detector that has a finite width and moving the object by use of a rotation stage, one can obtain the angular directionality of the filter. The use of coherent detection in the directional detector makes it possible to increase the sensitivity of the system. The directional detector was analyzed theoretically and demonstrated experimentally for a Gaussian beam scattered from a conducting cylinder. The interference enabled us to theoretically increase the angular resolution by a factor of approximately 10 and experimentally by a factor of 8.5. A configuration for using a directional detector array to reconstruct a two-dimensional object is suggested. Since the directional detector makes it possible to reduce the effect of diffraction and scattering, reconstruction techniques based on nondiffracting sources, as implemented in x-ray tomography, may be used.

Male rats were chemically labyrinthectomized (n = 22) by intratympanic injections of sodium arsanilate, and control rats (n = 15) received intratympanic injections of isotonic saline. All rats were tested for labyrinthine integrity and then adjusted to a 23 h.d-1 water deprivation schedule. Both labyrinthectomized and control rats were exposed to a conditioned taste aversion (CTA) procedure or a control procedure. The CTA technique involved pairing a novel saccharin taste with subsequent intraperitoneal injection of ethanol (1.5 g.kg-1; 15% solution). The control CTA procedure paired a novel saccharin taste with injections of isotonic saline. Following two conditioning trials and 3 d of water only, saccharin preference ratios were obtained in two-bottle choice tests (saccharin vs. water) over 4 consecutive days. Control rats conditioned with ethanol exhibited a strong CTA (p < 0.01) relative to control rats injected with saline. Labyrinthectomized rats drinking saccharin followed by ethanol injections showed a strong CTA (p < 0.01) if conditioning occurred 29-30 d post-labyrinthectomy. However, CTA's were not apparent in labyrinthectomized rats conditioned with ethanol 19 d post-labyrinthectomy. Thus, ethanol-induced CTA formation varied across the post-labyrinthectomy time period.
